1First-generation EGFR tyrosine kinase inhibitor therapy in 106 patients with compound EGFR-mutated lung cancer: a single institution’s clinical practice experience显示文摘Background:The antitumour efficacy of tyrosine kinase inhibitors(TKIs)in lung cancer patients with compound epidermal growth factor receptor(EGFR)mutations has not been resolved.Our study summarizes a single institutional experience of first-generation TKI therapy for lung cancers with compound EGFR mutations.Methods:A total of 106 consecutive patients with tumours bearing compound EGFR mutations were identified between January 2012 and May 2016;all patients received first-generation TKI therapy.Deletions in exon 19 and the L858R point mutation in exon 21 were considered common mutations;T790M was considered separately because of its association with TKIs resistances.Any other mutation was defined as a rare mutation.Patients were divided as follows:double common mutations(group A);common plus T790M mutations(group B);common plus rare muta-tions(group C);double rare mutations(group D);and rare plus T790M mutations(group E).A separate group of 115 consecutive patients with a single common mutation was created for comparative analysis(group F).Results:The frequency of patients with compound EGFR was 2.9%(114/3925)and their response rate to first-genera-tion TKIs was 50.9%,which was not significantly different from group F(67.0%,P=0.088).The progression-free survival(PFS)of the 106 patients receiving TKI therapy was worse than that of group F(median,9.1 vs.13.0 months,respec-tively;P<0.001).The PFS of the compound mutation group was shorter than that of the single common mutation group(median,10.1 months in group A,P=0.240;9.1 months in group B,P<0.001;9.6 months in group C,P=0.010;6.5 months in group D,P=0.048;5.4 months in group E,P=0.017).Patients with a co-occurring mutation in exon 20(excluding T790M)exhibited significantly worse PFS than the patients with other compound mutations or with a single common mutation(median,6.5 vs.9.1 vs.13.0 months,respectively,P=0.002).Conclusions:There was significant heterogeneity among the compound EGFR mutations and their response to first-generation TKIs.Individualized treatment in clinical practice should be considered for each case.Xiangyang Yu Xuewen Zhang Zichen Zhang Yongbin Lin Yingsheng Wen Yongqiang Chen Weidong Wang Lanjun Zhang 2018Cancer Communications2018,38,1:5

5Few-shot Named Entity Recognition with Joint Token and Sentence Awareness显示文摘Few-shot learning has been proposed and rapidly emerging as a viable means for completing various tasks.Recently,few-shot models have been used for Named Entity Recognition(NER).Prototypical network shows high efficiency on few-shot NER.However,existing prototypical methods only consider the similarity of tokens in query sets and support sets and ignore the semantic similarity among the sentences which contain these entities.We present a novel model,Few-shot Named Entity Recognition with Joint Token and Sentence Awareness(JTSA),to address the issue.The sentence awareness is introduced to probe the semantic similarity among the sentences.The Token awareness is used to explore the similarity of the tokens.To further improve the robustness and results of the model,we adopt the joint learning scheme on the few-shot NER.Experimental results demonstrate that our model outperforms state-of-the-art models on two standard Fewshot NER datasets.Wen Wen Yongbin Liu Qiang Lin Chunping Ouyang 2023Data Intelligence2023,5,3:1

12Taurocholic acid inhibits the response to interferon-αtherapy in patients with HBeAg-positive chronic hepatitis B by impairing CD8^(+)T and NK cell function显示文摘Pegylated interferon-alpha (PegIFNα) therapy has limited effectiveness in hepatitis B e-antigen (HBeAg)-positive chronic hepatitis B (CHB) patients. However, the mechanism underlying this failure is poorly understood. We aimed to investigate the influence of bile acids (BAs), especially taurocholic acid (TCA), on the response to PegIFNα therapy in CHB patients. Here, we used mass spectrometry to determine serum BA profiles in 110 patients with chronic HBV infection and 20 healthy controls (HCs). We found that serum BAs, especially TCA, were significantly elevated in HBeAg-positive CHB patients compared with those in HCs and patients in other phases of chronic HBV infection. Moreover, serum BAs, particularly TCA, inhibited the response to PegIFNα therapy in HBeAg-positive CHB patients. Mechanistically, the expression levels of IFN-γ, TNF-α, granzyme B, and perforin were measured using flow cytometry to assess the effector functions of immune cells in patients with low or high BA levels. We found that BAs reduced the number and proportion and impaired the effector functions of CD3^(+)CD8^(+) T cells and natural killer (NK) cells in HBeAg-positive CHB patients. TCA in particular reduced the frequency and impaired the effector functions of CD3^(+)CD8^(+) T and NK cells in vitro and in vivo and inhibited the immunoregulatory activity of IFN-α in vitro. Thus, our results show that BAs, especially TCA, inhibit the response to PegIFNα therapy by impairing the effector functions of CD3^(+)CD8^(+) T and NK cells in HBeAg-positive CHB patients. 18Chinese speaker-recognition based on ARMA model显示文摘A Chinese speaker recognition system, which only use speech material of nasal initials and is text-independent, is presented in this paper. According to the properties of speaker’s fixed nasal cavity and stable pharynx cavity when Chinese nasal initials is spoken and a few Chinese nasal initials (the total number of them is only 101 which consists of 53 Tn- and 48 n-), the spectrum parameters of zero and pole point coefficients of all Chinese nasal initials can be gotten by using ARMA model. 19Real-world effectiveness and safety of goserelin 10.8-mg depot in Chinese patients with localized or locally advanced prostate cancer显示文摘Objective:Real-word data on long-acting luteinizing hormone-releasing hormone(LHRH)agonists in Chinese patients with prostate cancer are limited.This study aimed to determine the real-world effectiveness and safety of the LHRH agonist,goserelin,particularly the long-acting 10.8-mg depot formulation,and the follow-up patterns among Chinese prostate cancer patients.Methods:This was a multicenter,prospective,observational study in hormone treatment-na?ve patients with localized or locally advanced prostate cancer who were prescribed goserelin 10.8-mg depot every 12 weeks or 3.6-mg depot every 4 weeks with or without an anti-androgen.The patients had follow-up evaluations for 26 weeks.The primary outcome was the effectiveness of goserelin in reducing serum testosterone and prostate-specific antigen(PSA)levels.The secondary outcomes included testosterone and PSA levels,attainment of chemical castration(serum testosterone<50 ng/d L),and goserelin safety.The exploratory outcome was the monitoring pattern for serum testosterone and PSA.All analyses were descriptive.Results:Between September 2017 and December 2019,a total of 294 eligible patients received≥1 dose of goserelin;287 patients(97.6%)were treated with goserelin 10.8-mg depot.At week 24±2,the changes from baseline[standard deviation(95%confidence interval)]in serum testosterone(n=99)and PSA(n=131)were-401.0 ng/d L[308.4 ng/d L(-462.5,-339.5 ng/d L)]and-35.4 ng/m L[104.4 ng/m L(-53.5,-17.4 ng/m L)],respectively.Of 112 evaluable patients,100(90.2%)achieved a serum testosterone level<50 ng/d L.Treatment-emergent adverse events(TEAEs)and severe TEAEs occurred in 37.1%and 10.2%of patients,respectively.The mean testing frequency(standard deviation)was 1.6(1.5)for testosterone and 2.2(1.6)for PSA.Conclusions:Goserelin 10.8-mg depot effectively achieved and maintained castration and was well-tolerated in Chinese patients with localized and locally advanced prostate cancer.Nanhui Chen Zengjun Wang Ming Chen Qi Ma Yi He Yujie Wang Xin Li Mingxing Qiu Lei Shi Shaoxing Zhu Qun Xie Xiuheng Liu Benkang Shi Guowen Lin Weizhong Yang Yongbin Liao Haibin Zhang Shusheng Wang Jiexian Li Shaogang Wang Lijun Dong Hui Chen Jiaju Lu YongyiCheng Xiaoping Zhang Lulin Ma Liqun Zhou He Wang Shen Li Dingwei Ye 2023Cancer Biology & Medicine2023,20,12:0
