| 1 | How can colorectal neoplasms be treated during colonoscopy?显示文摘AIM: For many physicians who ordinarily treat patients with colonic diseases, colonoscopy is considered a prime study interest. Developments in colonoscopic equipment and methods have led to a large number of endoscopic diagnoses and treatment for colorectal neoplasms. The purpose of this investigation is to evaluate the efficacy and outcomes of endoscopic treatment for colorectal neoplastic lesions.METHODS: From September 1999 to May 2003, 11 447 consecutive colonoscopic examinations in 9 864 patients were gathered; totaling 5 502 endoscopic treatments for colorectal neoplasms. Macroscopic characteristics of the neoplasms were classified into protruded (n = 3 953), sessile (n = 1 402), lateral spreading tumor (n = 139) and depressed lesions (n = 8). Snare polypectomy was conducted in 3 984 lesions, hot forcep removal in 1 368 lesions, and endoscopic mucosal resection in 150 lesions.RESULTS: Histological diagnoses were 4 596 neoplastic lesions (4 376 adenomas and 220 adenocarcinomas) and 906 non-neoplastic lesions (891 hyperplastic and 15 inflammatory polyps). For the adenocarcinoma group, 31 instances involved submucosal invasion or resection margin, who received further operations, while 13 surgical specimens discovered no residual tumors. Three perforations and 96 bleedings were found following endoscopic treatment. | 2 | 沸石咪唑酯骨架结构材料ZIF-8催化Friedel-Crafts酰基化反应(英文)显示文摘A zeolite imidazolate framework,ZIF-8,was synthesized and characterized by dynamic laser light scattering,X-ray powder diffraction,scanning electron microscopy,transmission electron microscopy,thermogravimetric analysis,Fourier transform infrared,atomic absorption spectrophotometry,and nitrogen adsorption measurements.The ZIF-8 was highly crystalline and porous with a surface area of over 1600 m2 /g.Friedel-Crafts acylation of anisole and benzoyl chloride proceeded well in the presence of ZIF-8(2-6 mol%) without the need for an inert atmosphere.The reaction afforded a selectivity of 93%-95% to the p-isomer.The solid catalyst can be separated from the reaction mixture by simple centrifugation and reused without significant degradation in catalytic activity.There was no leaching of active acid species into the reaction solution. | Lien T. | 3 | DESIGN OF SLIT DAMS FOR CONTROLLING STONY DEBRIS FLOWS显示文摘A new method to a slit dam for controlling the stony debris flow has been derived based on the mass conservation law of the stony debris flow passing through a slit dam and the laboratory experiment results. This new method is then combined with three primary efficiency expressions: the dimensionless sediment outflow ratio, the sediment concentration ratio, and the sediment storage rate to develop a simple module, with which the height and the spacing of the posts, as well as the total spacing of slit dam are determined. Furthermore, these expressions can also be applied to check those slit dams that have already been constructed with their effectiveness against various magnitudes of the debris flow. | 4 | Heartburn and regurgitation have different impacts on life quality of patients with gastroesophageal reflux disease显示文摘AIM:To investigate the impact of heartburn and regurgitation on the quality of life among patients with gastroesophageal reflux disease(GERD).METHODS:Data from patients with GERD,who were diagnosed according to the Montreal definition,were collected between January 2009 and July 2010.The enrolled patients were assigned to a heartburn or a regurgitation group,and further assigned to an erosive esophagitis(EE)or a non-erosive reflux disease(NERD)subgroup,depending on the predominant symptoms and endoscopic findings,respectively.The general demographic data,the scores of the modified Chinese version of the GERDQ and the Short-form 36(SF-36)questionnaire scores of these groups of patients were compared.RESULTS:About 108 patients were classified in the heartburn group and 124 in the regurgitation group.The basic characteristics of the two groups were similar,except for male predominance in the regurgitation group.Patients in the heartburn group had more sleep interruptions(22.3%daily vs 4.8%daily,P=0.021),more eating or drinking problems(27.8%daily vs 9.7%daily,P=0.008),more work interferences(11.2%daily vs none,P=0.011),and lower SF-36 scores(57.68 vs64.69,P=0.042),than patients in the regurgitation group did.Individuals with NERD in the regurgitation group had more impaired daily activities than those with EE did.CONCLUSION:GERD patients with heartburn or regurgitation predominant had similar demographics,but those with heartburn predominant had more severely impaired daily activities and lower general health scores.The NERD cases had more severely impaired daily activity and lower scores than the EE ones did. | Shou-Wu Lee Han-Chung Lien Teng-Yu Lee Sheng-Shun Yang Hong-Jeh Yeh Chi-Sen Chang | 2014 | World Journal of Gastroenterology2014，20，34： | 8 |
| 5 | Anti-Helicobacter pylori therapy significantly reduces Helicobacter py/ori-Induced gastric mucosal damage in Mongolian gerbils显示文摘AIM: To investigate the effectiveness of 4 d' anti-Helicobacter pyloritherapy on the H pylori-infected Mongolian gerbils based on physiological and pathological changes.METHODS: We used 6-wk-old male gerbils orally inoculated with H pylori (ATCC43504, 2x108 CFU/mL).Seven weeks after H pylori inoculation, the animals of study group received 4 d' anti-H pylori triple therapy (H pylorieradicated group). Seven days later, all animals of the H pylori-eradicated and control groups (H pylori-infected& H pylori-uninfected groups) were sacrificed. | 7 | Eradication of Helicobacter pylori significantly reduced gastric damage in nonsteroidal anti-inflammatory drug-treated Mongolian gerbils显示文摘AIM: To examine the effect of eradication of Helicobacter pyloriprior to usage of NSAIDs, by investigating gastric inflammatory activity, myeloperoxidase (MPO) activity,prostaglandin (PG) E2 synthesis in H pylori-infected, and H pylori-eradicated gerbils followed by administration of indomethacin and rofecoxib.METHODS: Six-week-old male gerbils were orally inoculated with H pylori. Seven weeks later, anti-H pyloritriple therapy and vehicle were given to gerbils respectively and followed for 2 wk. | 8 | Impact of body mass index and gender on quality of life in patients with gastroesophageal reflux disease显示文摘AIM:To investigate the symptom presentation and quality of life in obese Chinese patients with gastroesophageal reflux disease(GERD).METHODS:Data from patients diagnosed with GERD according to the Montreal definition,were collected between January 2009 to March 2010.The enrolled patients were assigned to the normal [body mass index(BMI) < 25 kg/m 2 ],overweight(25-30 kg/m 2),and obese(BMI > 30 kg/m 2) groups.General demographic data,endoscopic findings,and quality of life of the three groups of patients were analyzed and compared.RESULTS:Among the 173 enrolled patients,102,56 and 15 patients were classified in the normal,overweight,and obese,respectively.There was significantly more erosive esophagitis(73.3% vs 64.3% vs 39.2%,P = 0.002),hiatal hernia(60% vs 33.9% vs 16.7%,P = 0.001),and males(73.3% vs 73.2% vs 32.4%,P = 0.001) in the obese cases.The severity and frequency of heartburn,not acid regurgitation,was positively correlated with BMI,with a significant association in men,but not in women.Obese patients were prone to have low quality of life scores,with obese women having the lowest scores for mental health.CONCLUSION:In patients with GERD,obese men had the most severe endoscopic and clinical presentation.Obese women had the poorest mental health. | Shou-Wu Lee Han-Chung Lien Chi-Sen Chang Yen-Chun Peng Chung-Wang Ko Ming-Chih Chou | 2012 | World Journal of Gastroenterology2012，18，36： | 5 |

| 10 | Deformity of duodenal bulb, gastric metaplasia of duodenal regenerating mucosa and recurrence of duodenal ulcer: A correlated study显示文摘AIM: To investigate the correlation among the presence and degree of gastric metaplasia of duodenal regenerating mucosa, the deformity of bulb and the recurrence of duodenal ulcer.METHODS: A total of 99 patients with duodenal ulcer were treated with H2-antagonist with or without antimicrobial therapy. All patients received follow-up endoscopic examinations 6 wk after treatment. When the ulcer(s) were noted to be healed, two biopsies were taken from the ulcer scar for histological study of gastric metaplasia, and 4 biopsies were taken from antrum for Helicobacter pylori(H pylori) study. Out of these cases,44 received further follow-up endoscopic examinations after 3, 6 and 12 mo respectively for studying the recurrence rate of duodenal ulcers. The correlation among ulcer recurrence, degree of gastric metaplasia of regenerating mucosa, bulbar deformity, and colonization of Hpylori in the stomach was then studied.RESULTS: The results showed that there was a strong correlation between the deformity of duodenal bulb and the degree of gastric metaplasia of regenerating duodenal mucosa. The recurrence rate of duodenal ulcer had a significant difference between patients with and without Hpyloricolonization in the stomach (P<0.001). The greater the degree of gastric metaplasia of duodenal regenerating mucosa, the higher the recurrence rate of duodenal ulcer (P= 0.021). The more deformed the duodenal bulb, the higher the incidence of recurrence of duodenal ulcer (P = 0.03).CONCLUSION: There is a correlation among deformity of duodenal bulb, gastric metaplasia of duodenal regenerating mucosa and recurrence of duodenal ulcer.A more severely deformed duodenal bulb is closely related to a greater extent of gastric metaplasia. | 16 | N-alkylamide profiling of Achillea ptarmica and Achillea millefolium extracts by liquid and gas chromatography–mass spectrometry显示文摘Achillea millefolium and Achillea ptarmica are both plants belonging to the Asteracea family and are traditionally used for their medicinal properties. It has already been shown that some N-alkylamides(NAAs)are responsible for these pharmacological actions. Therefore, in the present study, the NAA content of the two plants was analytically characterised. Different extracts were prepared from the roots, the leaves, the stems and the flowers. The structures of NAAs have been assigned in ethanolic extracts of Achillea millefolium and Achillea ptarmica using high performance liquid chromatography – electrospray ionisation – mass spectrometry(HPLC–ESI–MS) and gas chromatography – electron impact – mass spectrometry(GC–EI–MS). Using both analytical techniques, the structures of 14 and 15 NAAs have been assigned in Achillea ptarmica and Achillea millefolium, respectively. Structures of two new NAAs, previously never observed in Achillea ptarmica,were assigned: deca-2E,6Z,8E-trienoic acid 2-methylbutylamide(homospilanthol) or a related isomeric compound and deca-2E,4E-dienoic acid N-methyl isobutylamide. | 17 | Usefulness of the Hook knife in flexible endoscopic myotomy for Zenker's diverticulum显示文摘AIM To investigate the outcome of flexible endoscopic myotomy performed with the Hook knife in patients with symptomatic Zenker's diverticulum(ZD).METHODS All consecutive patients treated for ZD at our institution between 7/2012 and 12/2016 were included.The flexible endoscopic soft diverticuloscope-assisted technique with endoclips placement and Hook knife myotomy were performed in all patients.Here we report a retrospective review of prospectively collected data.Demographics,dysphagia score(Dakkak and Bennett),associated symptoms and adverse events were collected pre-procedure,at 2 and 6 mo post-procedure,and at the end of the follow-up period.Clinical success was defined as at least 1-point improvement in dysphagia score and a residual dysphagia score≤1,with no need for reintervention.Dysphagia scores were compared before treatment and at end-of-follow-up using the Wilcoxon test.RESULTS Twenty-four patients were included.Mean size of ZD was 3.0 cm(range 2-8 cm).Mean number of sessions was 1.17/patient(range 1-3 sessions).Overall clinical success was 91.7%.Two adverse events(8.3%) occurred,and both were managed conservatively.No bleeding or perforation was reported.Mild pain was reported by 9 patients(37.5%).Median hospital stay was 1 d(range 1-6).Median follow-up was 19.5 mo(range 6-53).Mean ± SD dysphagia score was 2.25±0.89 before treatment and decreased to 0.41 ± 0.92 at end-of-follow-up(P<0.001).Regurgitation and cough dropped from 91.7% and 50% to 12.5% and 0% at the end of follow-up,respectively.Recurrence was observed in 3 patients,and all 3 were symptom-free after one more session.CONCLUSION The Hook knife,used in the soft diverticuloscope-assisted technique setting,is efficient and safe for treatment of ZD. | Olivier Rouquette Armando Abergel Aurélien Mulliez Laurent Poincloux | 2017 | World Journal of Gastrointestinal Endoscopy2017，9，8： | 3 |
| 18 | Effects of glucocorticoids on the growth and chemosensitivity of carcinoma cells are heterogeneous and require high concentration of functional glucocorticoid receptors显示文摘AIM: To determine how glucocorticoids (GCs) may affect the growth and chemosensitivity of common carcinoma cells.METHODS: The effect of dexamethasone (DEX) on growth and chemosensitivity was assessed in 14 carcinoma cell lines. The function of GC receptors (GR) was assessed by MMTV reporter assay. Overexpression of GR was done by in vitro transfection and expression of a GR-expressing vector. Immunohistochemical stain of tissues and cells were done by PA1-511A, an anti-GR monoclonal antibody.RESULTS: DEX inhibited cell growth of four (MCF-7, MCF-7/MXR1, MCF-7/TPT300, and HeLa), increased cisplatin cytotoxicity of one (SiHa), and decreased cisplatin cytotoxicity of two (H460 and Hep3B) cell lines. The GR content of the seven cell lines affected by DEX was significantly higher than those of the seven cell lines unaffected by DEX(5.2±2.5×104 sites/cell vs 1.3±1.4×104 sites/cell, P= 0.005).Only two DEX-unresponsive cell lines (NPC-TW01 and NPCTW04) contained high GR amounts in the range (1.9-8.1×104sites/cell) of the seven DEX-responsive cell lines. The GR function of NPC-TW01 and NPC-TW04, however, was found to be impaired. The importance of high cellular amount of GR in mediating DEX susceptibility of the cells was further exemplified by GR dose-dependent drug resistance to cisplatin of AGS, a cell line with low GR content and was unaffected by DEX before transfection of GR-expressing vector. Immunohistochemical studies of human cancer tissues showed that 5 of the 45 (11.1%) breast cancer and 43 of the 85 (50.6%) non-small cell lung cancer had high GR contents at the ranges of the GC-responsive carcinoma cell lines.CONCLUSION: The growth and chemosensitivity of human carcinomas with high GR contents may be affected by GC. | 19 | DEBRIS FLOW CONTROL BY USING SLIT DAMS显示文摘Slit dams have been shown to be effective in trapping coarse sediment and retarding the debris flow. It is necessary to establish the controlling efficiency and the systematic procedures for designing slit dams, as well as reasonable sites for their location. Using the principle of mass conservation and laboratory experiments, this paper develops the sediment flow-out ratio and sediment concentration ratio to evaluate the effectiveness of the slit dam, and confirms that sediment concentration ratio increases with an increase in relative spacing and slit density. The sediment flow-out ratio depends on the sediment concentration ratio and sediment concentration before passing through a slit dam. For sediment concentration before passing through a slit dam, based upon Bagnold’s concept of the dispersive stress generated by grain collisions in flow and bed shear stress, a semi-empirical relationship is obtained. Compared with other researchers’ studies, this study is found to be in excellent conformity with theirs, when taking 0.1 as the empirical coefficient. Besides, height, spacing and location factors affecting a slit dam design for debris flow mitigation are established herein.
