2美国临床肿瘤学会IV期非小细胞肺癌化疗的临床实践指南更新显示文摘本文旨在为IV期非小细胞肺癌患者的治疗提供更新版推荐。本文资料检索源自2002年以来公布的相关随机试验文献。此指南范围限于化疗与生物治疗。更新委员会对这些文献进行了总结并提供了推荐更新。162篇文献符合标准被纳入参考。本推荐基于可改善总生存期的治疗方法。仅改善无进展生存期的治疗方法推动了对毒性及生存质量的监测。对于体力状态评分为0分或1分患者的一线治疗,可推荐以铂类为基础的细胞毒性药物的两药联用。对铂类治疗有禁忌的患者,可采用非铂类细胞毒性两药联合。对于体力状态评分为2分的患者,单一细胞毒性药物即可。对于疾病进展或经过4个周期的治疗仍对治疗无反应的患者,应停止一线细胞毒性化疗。即使在6个周期后患者对治疗仍有反应,亦应停止两药细胞毒性化疗。对于伴有明确的表皮生长因子受体(epidermal growth factor receptor,EGFR)突变的患者,可推荐一线采用吉非替尼治疗;对于EGFR突变为阴性或不明确的患者,细胞毒性化疗更佳。除具有特定临床特征的患者外,可推荐贝伐单抗与卡铂-紫杉醇联用。对于通过免疫组化证实EGFR阳性的肿瘤患者,可推荐西妥昔单抗与顺铂-长春瑞滨联用。多西紫杉醇、厄洛替尼、吉非替尼或培美曲塞被推荐作为二线治疗。对于未曾接受过厄洛替尼或吉非替尼治疗的患者,可推荐厄洛替尼作为三线治疗。现有数据不足以推荐常规三线采用细胞毒性药物。已有的证据也不足以推荐常规应用分子标记物选择化疗。Christopher G. 5长白山原始阔叶红松林土壤有机质组分小尺度空间异质性显示文摘土壤有机质(SOM)对于维持生态系统生产力具有非常重要的意义,有机质的组成、空间分布和空间关联性是影响和控制诸多生态系统过程的重要因素。应用地统计学方法,对长白山原始阔叶红松林局部尺度内0—20 cm土壤有机质与活性有机质的空间异质性进行了研究,并通过交叉半方差分析探讨了二者之间的相关性。研究结果表明:(1)总体上来说,土壤有机碳(SOC)、全氮(TN)、颗粒态有机碳(POC)和颗粒态有机氮(PON)空间异质性较小;而土壤微生物量碳(MBC)、微生物量氮(MBN)和表层(0—10 cm)溶解性有机碳(DOC)的空间异质性较大;(2)SOC、TN、MBC、DOC、POC和PON随着深度的增加空间自相关性增加;而溶解性有机氮(DON)的空间自相关性随深度的增加变化不大;(3)SOC与TN在表层和下层(10—20 cm)均存在空间上的正相关关系;(4)SOC、TN在表层和下层分别与MBC、MBN、DOC、DON和POC呈空间上的正相关性,但是与PON之间的空间相关关系较差;(5)不同土层深度的土壤活性有机质之间的相关关系存在差异。在表层,除POC,PON外,其余土壤活性有机质组分在空间上两两相关;但是随着土壤深度的增加,活性有机质变量之间在空间上两两相关。研究结果表明土壤有机质组分在长白山原始阔叶红松林小尺度内存在不同空间异质性和空间关联性,这为人们更好的理解森林生态系统功能(如土壤养分循环)提供了重要的理论依据。景莎 田静 M. 7Heme oxygenase system in hepatic ischemia-reperfusion injury显示文摘Hepatic ischemia-reperfusion injury (IRI) limits access to transplantation. Heme oxygenase-1 (HO-1) is a powerful antioxidant enzyme which degrades free heme into biliverdin,free iron and carbon monoxide. HO-1 and its metabolites have the ability to modulate a wide variety of inflammatory disorders including hepatic IRI. Mechanisms of this protective effect include reduction of oxygen free radicals,alteration of macrophage and T cell phenotype. 9Targeting immunometabolism as an anti-inflammatory strategy显示文摘The growing field of immunometabolism has taught us how metabolic cellular reactions and processes not only provide a means to generate ATP and biosynthetic precursors,but are also a way of controlling immunity and inflammation.Metabolic reprogramming of immune cells is essential for both inflammatory as well as anti-inflammatory responses.Four anti-inflammatory therapies,DMF,Metformin,Methotrexate and Rapamycin all work by affecting metabolism and/or regulating or mimicking endogenous metabolites with anti-inflammatory effects.Evidence is emerging for the targeting of specific metabolic events as a strategy to limit inflammation in different contexts.Here we discuss these recent developments and speculate on the prospect of targeting immunometabolism in the effort to develop novel anti-inflammatory therapeutics.As accumulating evidence for roles of an intricate and elaborate network of metabolic processes,including lipid,amino acid and nucleotide metabolism provides key focal points for developing new therapies,we here turn our attention to glycolysis and the TCA cycle to provide examples of how metabolic intermediates and enzymes can provide potential novel therapeutic targets.Eva M.Palsson-McDermott Luke A.J.O'Neill 2020Cell Research2020,30,4:12

13Computed tomography overestimation of esophageal tumor length: Implications for radiotherapy planning显示文摘AIM:To assess the relationship between preoperative computed tomography(CT)and postoperative pathological measurements of esophageal tumor length and the prognostic significance of CT tumor length data.METHODS:A retrospective study was carried out in 56 patients who underwent curative esophagogastrectomy.Tumor lengths were measured on the immediate preoperative CT and on the post-operative resection specimens.Inter-and intra-observer variations in CT measurements were assessed.Survival data were collected.RESULTS:There was a weak correlation between CT and pathological tumor length(r=0.30,P=0.025).CT lengths were longer than pathological lengths in 68%(38/56)of patients with a mean difference of 1.67 cm(95%CI:1.18-2.97).The mean difference in measurements by two radiologists was 0.39 cm(95% CI:-0.59-1.44).The mean difference between repeat CT measured tumor length(intra-observer variation) were 0.04 cm(95%CI:-0.59-0.66)and 0.47 cm (95%CI:-0.53-1.47).When stratified,patients not receiving neoadjuvant chemotherapy showed a strong correlation between CT and pathological tumor length(r =0.69,P=0.0014,n=37)than patients that did(r= 0.13,P=0.43,n=19).Median survival with CT tumor length>5.6 cm was poorer than with smaller tumors,but the difference was not statistically significant.CONCLUSION:Esophageal tumor length assessed using CT does not reflect pathological tumor extent and should not be the only modality used for management decisions,particularly for planning radiotherapy.Karim Sillah Luke R Williams Hans-Ulrich Laasch Azeem Saleem Gillian Watkins Susan A Pritchard Patricia M Price Catharine M West Ian M Welch 2010World Journal of Gastrointestinal Oncology2010,2,4:10

15User-Level Device Drivers: Achieved Performance显示文摘Running device drivers as unprivileged user-level code, encapsulated into their own process, has often been proposed as a technique for increasing system robustness. However, in the past, systems based on user-level drivers have generally exhibited poor I/O performance. Consequently, user-level device drivers have never caught on to any significant degree. 17An Analysis of Social Vulnerability to Natural Hazards in Nepal Using a Modified Social Vulnerability Index显示文摘Social vulnerability influences the ability to prepare for, respond to, and recover from disasters. The identification of vulnerable populations and factors that contribute to their vulnerability are crucial for effective disaster risk reduction. Nepal exhibits multihazard risk and has experienced socioeconomic and political upheaval in recent decades, further increasing susceptibility to hazards.However, we still know little regarding social vulnerability in Nepal. Here, we investigate social vulnerability in Nepal by adapting Social Vulnerability Index(SoVI) methods to the Nepali context. Variables such as caste, and populations who cannot speak/understand Nepali were added to reflect the essence of the Nepali context. Using principal component analysis, 39 variables were reduced to seven factors that explained 63.02% of variance in the data.Factor scores were summarized to calculate final SoVI scores. The highest levels of social vulnerability are concentrated in the central and western Mountain, western Hill, and central and eastern Tarai regions of Nepal, while the least vulnerable areas are in the central and eastern Hill regions. 18特发性少精子症和无精子症与Pygo2基因蛋白编码区SNPs的相关性显示文摘男性不育常伴随精子数量减少。Pygo2基因在染色质重塑的伸长精细胞中表达,其功能受损会导致精子形成阻滞和精子生成减少而引发不育。文章旨在检测引起人特发性少精子症和无精子症的Pygo2基因突变。从77例正常生育力男性和195例特发性少精子症和无精子症患者静脉血提取DNA,采用聚合酶链式反应-测序方法对Pygo2基因3个蛋白质编码区进行测序对比,非同义单核苷酸多态性(Single nucleotide polymorphisms,SNPs)位点分别用SIFT、Polyphen-2和Mutation Taster软件进行诱发蛋白质结构和表型改变的检测和分析。结果表明,195例患者中,178例(30例轻度或中度少精子症,57例重度少精子症和91例无精子症)基因序列分析报告完好,无精子症中3例患者分别在2个位点(rs61758740,rs141722381)发生了非同义突变SNPs,重度少精子症中1例患者在位点rs61758741发生了非同义突变,3个突变位点在SNPs基因数据库都已有报道,轻度或中度少精子症患者以及正常生育力男性中不存在SNPs。rs61758740可使PYGO2蛋白第141位蛋氨酸(M)变为异亮氨酸(I),rs61758741使PYGO2蛋白第261位碱性赖氨酸(K)变为酸性谷氨酸(E),rs141722381使PYGO2蛋白第240位亲水侧链天冬酰胺(N)变为疏水侧链异亮氨酸(I)。软件分析表明,在所发现的3个SNP非同义突变位点中,rs141722381引起的单个氨基酸改变会导致PYGO2蛋白空间结构破坏和诱发相关疾病。因此,Pygo2基因蛋白质编码序列区SNPs可能是特发性少精子症和无精子症的诱发因素之一,导致男性不育。葛少钦 Jeanine Grifin 刘丽华 Kenneth I.Aston Luke Simon Timothy G.Jenkins Benjamin R.Emery Douglas T.Carrell 2013遗传2013,35,5:6
19Acoustic emission characterization of microcracking in laboratory-scale hydraulic fracturing tests显示文摘Understanding microcracking near coalesced fracture generation is critically important for hydrocarbon and geothermal reservoir characterization as well as damage evaluation in civil engineering structures.Dense and sometimes random microcracking near coalesced fracture formation alters the mechanical properties of the nearby virgin material. Individual microcrack characterization is also significant in quantifying the material changes near the fracture faces(i.e. damage). Acoustic emission(AE) monitoring and analysis provide unique information regarding the microcracking process temporally, and information concerning the source characterization of individual microcracks can be extracted. In this context,laboratory hydraulic fracture tests were carried out while monitoring the AEs from several piezoelectric transducers. In-depth post-processing of the AE event data was performed for the purpose of understanding the individual source mechanisms. Several source characterization techniques including moment tensor inversion, event parametric analysis, and volumetric deformation analysis were adopted.Post-test fracture characterization through coring, slicing and micro-computed tomographic imaging was performed to determine the coalesced fracture location and structure. Distinct differences in fracture characteristics were found spatially in relation to the openhole injection interval. Individual microcrack AE analysis showed substantial energy reduction emanating spatially from the injection interval. 20Treatment of malignant digestive tract obstruction by combined intraluminal stent installation and intra-arterial drug infusion显示文摘AIM To study the palliative treatment of malignant obstrution of digestive tract with placement of intraluminal stent combined with intra-arterial infusion of chemotherapeutic drugs.METHODS A total of 281 cases of digestive tract malignant obstruction were given per oral (esophagus,stomach, duodenum and jejunum), per anal (colon and rectum ) and percutaneous transhepatic ( biliary )installation of metallic stent. Among them, 205 cases received drug infusion by cannulation of tumor supplying artery with Seldingers technique.RESULTS Altogether 350 stents were installed in 281 cases, obstructive symptoms were relieved or ameliorated after installation. Occurrence of restenotic obstruction was 8 - 43 weeks among those with intraarterial drug infusion, which was later than 4 - 26 weeks in the group with only stent installation.
