| 1 | Risk factors of infected pancreatic necrosis secondary to severe acute pancreatitis显示文摘BACKGROUND: Severe acute pancreatitis(SAP) remains a clinical challenge with considerable morbidity and mortality.An early identification of infected pancreatic necrosis(IPN), a life-threatening evolution secondary to SAP, is obliged for a more preferable prognosis. Thus, the present study was conducted to identify the risk factors of IPN secondary to SAP. METHODS: The clinical data of patients with SAP were retrospectively analyzed. Univariate and multivariate logistic regression analyses were sequentially performed to assess the associations between the variables and the development of IPN secondary to SAP. A receiver operating characteristic(ROC) curve was created for each of the qualified independent risk factors. RESULTS: Of the 115 eligible patients, 39(33.9%) progressed to IPN, and the overall in-hospital mortality was 11.3%(13/115).The early enteral nutrition(EEN)(P=0.0092, OR=0.264), maximum intra-abdominal pressure(IAP)(P=0.0398, OR=1.131)and maximum D-dimer level(P=0.0001, OR=1.006) in the first three consecutive days were independent risk factors associated with IPN secondary to SAP. The area under ROC curve(AUC) was 0.774 for the maximum D-dimer level in the first three consecutive days and the sensitivity was 90% and the specificity was 58% at a cut-off value of 933.5 μg/L; the AUC was 0.831 for the maximum IAP in the first three consecutive days and the sensitivity was 95% and specificity was 58%at a cut-off value of 13.5 mm Hg. | 3 | Cancer incidence and patient survival rates among the residents in the Pudong New Area of Shanghai between 2002 and 2006显示文摘With the growing threat of malignancy to health,it is necessary to analyze cancer incidence and patient survival rates among the residents in Pudong New Area of Shanghai to formulate better cancer prevention strategies.A total of 43,613 cancer patients diagnosed between 2002 and 2006 were recruited from the Pudong New Area Cancer Registry.The incidence,observed survival rate,and relative survival rate of patients grouped by sex,age,geographic area,and TNM stage were calculated using the Kaplan-Meier,life table,and Ederer II methods,respectively.Between 2002 and 2006,cancer incidence in Pudong New Area was 349.99 per 100,000 person-years,and the 10 most frequently diseased sites were the lung,stomach,colon and rectum,liver,breast,esophagus,pancreas,brain and central nervous system,thyroid,and bladder.For patients with cancers of the colon and rectum,breast,thyroid,brain and central nervous system,and bladder,the 5-year relative survival rate was greater than 40%,whereas patients with cancers of the liver and pancreas had a 5-year relative survival rate of less than 10%.The 1-year to 5-year survival rates for patients grouped by sex,age,geographic area,and TNM stage differed significantly(all P<0.001).Our results indicate that cancer incidence and patient survival in Pudong New Area vary by tumor type,sex,age,geographic area,and TNM stage. | Xiao-Pan Li Guang-Wen Cao Qiao Sun Chen Yang Bei Yan Mei-Yu Zhang Yi-Fei Fu Li-Ming Yang | 2013 | Chinese Journal of Cancer2013,32,9: | 27 |

| 5 | Notch signaling mediated by TGF-β/Smad pathway in concanavalin A-induced liver fibrosis in rats显示文摘AIM To explore the exact interaction between Notch and transforming growth factor(TGF)-β signaling in liver fibrosis. METHODS We established a rat model of liver fibrosis induced by concanavalin A. Peripheral blood mononuclear cells(PBMCs) were isolated from the modeled rats, and cultured with γ-secretase inhibitor DAPT and TGF-β inhibitor for 24 h. The m RNA levels of Notch and TGF-β signaling were detected by quantitative real-time polymerase chain reaction. Expression of Notch and TGF-β proteins was analyzed by western blotting.RESULTS Compared to control rats, Notch and TGF-β signaling was activated in PBMCs of model rats. Administration of DAPT and TGF-β inhibitor suppressed Notch and TGF-β signal transducer in PBMCs of model rats. DAPT reduced m RNA and protein expression of TGF-β signaling, such as TGF-β1 and Smad3. TGF-β inhibitor also downregulated Notch1, Hes1 and Hes5, and m RNA and protein expression of the Notch signaling pathway.CONCLUSION Notch and TGF-β signaling play a role in liver fibrosis. Moreover, the teeth have a complicated regulatory mechanism, special histologic origin, diverse structure, and important function in mastication, articulation, and aesthetics. These characteristics, to a certain extent, greatly complicate the research in tooth regeneration. Recently, new ideas for tooth and tissue regeneration have begun to appear with rapid developments in the theories and technologies in tissue engineering. Numerous types of stem cells have been isolated from dental tissue, such as dental pulp stem cells (DPSCs), stem cells isolated from human pulp of exfoliated deciduous teeth (SHED), periodontal ligament stem cells (PDLSCs), stem cells from apical papilla (SCAPs), and dental follicle cells (DFCs). All these cells can regenerate the tissue of tooth. | 8 | Comparative efficacy of various exercise interventions on cognitive function in patients with mild cognitive impairment or dementia:A systematic review and network meta-analysis显示文摘Background:Exercise is a promising nonpharmacological therapy for cognitive dysfunction,but it is unclear which type of exercise is most effective.The objective of this study was to compare and rank the effectiveness of various exercise interventions on cognitive function in patients with mild cognitive impairment(MCI)or dementia and to examine the effects of exercise on the symptoms relevant to cognitive impairment.Methods:We searched PubMed,Web of Science,Embase,Cochrane Central Register of Controlled Trials,SPORTDiscus,and PsycInfo through September 2019 and included randomized controlled trials that examined the effectiveness of exercise interventions in patients with MCI or dementia.Primary outcomes included global cognition,executive cognition,and memory cognition.Secondary outcomes included activities of daily living,neuropsychiatric symptoms,and quality of life.Pairwise analyses and network meta-analyses were performed using a random effects model.Results:A total of 73 articles from 71 trials with 5606 participants were included.All types of exercise were effective in increasing or maintaining global cognition,and resistance exercise had the highest probability of being the most effective intervention in slowing the decrease in global cognition(standard mean difference(SMD)=1.05,95%confidence interval(95%CI):0.56-1.54),executive function(SMD=0.85,95%CI:0.21-1.49),and memory function(SMD=0.32,95%CI:0.01-0.63)in patients with cognitive dysfunction.Subgroup analyses for patients with MCI revealed different effects,and multicomponent exercise was most likely to be the optimal exercise therapy for preventing the decline of global cognition(SMD=0.99,95%CI:0.44-1.54)and executive function(SMD=0.72,95%CI:0.06-1.38).However,only resistance exercise showed significant effects on memory function for patients with MCI(SMD=0.35,95%CI:0.01-0.69).Exercise interventions also showed various effects on the secondary outcomes.Conclusion:Resistance exercise has the highest probability of being the optimal exercise type for slowing cognitive declin e in patients withcognitive dysfunction,especially in patients with dementia.Multicomponent exercise tends to be most effective in protecting global cognition and executive function in patients with MCI. | Xiuxiu Huang Xiaoyan Zhao Bei Li Ying Cai Shifang Zhang Qiaoqin Wan Fang Yu | 2022 | Journal of Sport and Health Science2022,11,2: | 19 |

| 11 | Salidroside reduces tau hyperphosphorylation via up-regulating GSK-3β phosphorylation in a tau transgenic Drosophila model of Alzheimer’s disease显示文摘Background:Alzheimer’s disease(AD)is an age-related and progressive neurodegenerative disease that causes substantial public health care burdens.Intensive efforts have been made to find effective and safe treatment against AD.Salidroside(Sal)is the main effective component of Rhodiola rosea L.,which has several pharmacological activities.The objective of this study was to investigate the efficacy of Sal in the treatment of AD transgenic Drosophila and the associated mechanisms.Methods:We used tau transgenic Drosophila line(TAU)in which tau protein is expressed in the central nervous system and eyes by the Gal4/UAS system.After feeding flies with Sal,the lifespan and locomotor activity were recorded.We further examined the appearance of vacuoles in the mushroom body using immunohistochemistry,and detected the levels of total glycogen synthase kinase 3β(t-GSK-3β),phosphorylated GSK-3β(p-GSK-3β),t-tau and p-tau in the brain by western blot analysis.Results:Our results showed that the longevity was improved in salidroside-fed Drosophila groups as well as the locomotor activity.We also observed less vacuoles in the mushroom body,upregulated level of p-GSK-3βand downregulated p-tau following Sal treatment.Conclusion:Our data presented the evidence that Sal was capable of reducing the neurodegeneration in tau transgenic Drosophila and inhibiting neuronal loss.The neuroprotective effects of Sal were associated with its up-regulation of the p-GSK-3βand down-regulation of the p-tau. | Bei Zhang Qiongqiong Li Xingkun Chu Suya Sun Shengdi Chen | 2016 | Translational Neurodegeneration2016,5,1: | 14 |

Based on the characteristics of tidal currents, waves, and sediment in the Yangshan area, a two-dimensional numerical model of tidal currents, sediment transport, and sea bed deformation is developed. In the model, the effects of tidal currents and wind waves on sediment transport are considered. According to characteristics of the study area, unstructured grids are applied to fit the boundaries of the near-shore sea area. The results show that the calculated values are in good agreement with the measured data. | 15 | Factors predisposing to severe acute pancreatitis:evaluation and prevention显示文摘AIM: To analyze factors predisposing to the infections associated with severe acute pancreatitis (SAP) and to work out ways for its prevention.METHODS: Total 208 cases of SAP treated in this hospital from Jan. 1980 to Dec. 2001 were retrospectively analyzed.RESULTS: Statistical difference in the incidence of the aforementioned infections was found between the following pairs: between the groups of bloody or non-bloody ascites,paralytic ileus lasting shorter or longer than 5 days, Ranson scores lower or higher than 5, hematocrit lower or higher than 45 %, CT Balthazar scores lower or higher than 7 and between 1980.1-1992.6 or 1992.7-2001.12 admissions (χ2>3.84, P<0.05), while no statistical difference was established between the groups of biliogenic and nonbiliogenic pancreatitis, serum amylase <200 U/L and ≥200U/L, serum calcium <2 mmol/L and ≥ 2 mmol/L or groups of total parenteral nutrition shorter or longer than 7 days (χ2<3.84, P>0.05).CONCLUSION: Occurrence of infection in patients with SAP is closely related with bloody ascites, paralytic ileus ≥5 days, Ranson scores ≥5, hematocrit ≥45 % and CT Balthazar Scores ≥7, but not with pathogens, serum calcium and total parenteral nutrition (TPN). | 17 | Factors Associated with Thrombolysis Outcome in Ischemic Stroke Patients with Atrial Fibrillation显示文摘The outcome of early intravenous thrombolysis for ischemic stroke in patients with atrial fibrillation(AF)is worse than that without thrombosis. How to increase the efficacy of intravenous thrombolysis for AF-related ischemic stroke remains largely unknown. In this study, we investigated factors that influence the effect of intravenous thrombolysis in these patients. Our results showed that thrombolysis was independently associated with a favorable outcome(P \ 0.001) and did not influence the mortality of AF-related ischemic stroke, although it increased the risk of hemorrhage within 24 h after treatment. Risk factors for a poor outcome at admission were:heart failure(P = 0.045); high systolic pressure(P = 0.039); high blood glucose(P = 0.030); and a high National Institutes of Health Stroke Scale(NIHSS) score(P \ 0.001). Moreover, high systolic pressure at admission(P = 0.007), high blood glucose(P = 0.027), and a high NIHSS score(P \ 0.001) were independent risk factors for mortality at 3 months. Besides thrombolysis, a high NIHSS score(P = 0.006) and warfarin taken within 48 h before stroke onset(P = 0.032) were also independent risk factors for symptomatic hemorrhage within 24 h after treatment. | 19 | Entecavir add-on Peg-interferon therapy plays a positive role in reversing hepatic fibrosis in treatment-naive chronic hepatitis B patients: a prospective and randomized controlled trial显示文摘Background:The efficacy of entecavir(ETV)add-on peg-interferon therapy compared with ETV monotherapy in treatment-naive hepatitis B virus(HBV)patients remains controversial.We investigated whether adding peg-interferon to ongoing ETV treatment leads to a better curative effect or not.Methods:All patients have been recruited between August 2013 and January 2015 from the Shanghai Public Health Clinical Center and Zhongshan Hospital(China).Eligible HBV patients(n=144)were randomly divided(1:1)to receive either ETV monotherapy(n=70)or peg-interferon add-on therapy from week 26 to 52(«=74).Patients were followed-up for at least 2 years.Indexes including hepatitis B surface antigen(HBsAg)and hepatitis B e antigen(HBeAg)seroconversion rate,sustained virologic response,transient elastography value,and histological scores were evaluated every 3 months until the end of the study.The rate of patients with HBsAg loss was defined as the primary endpoint criteria.Results:At week 26,no patient achieved HBsAg seroconversion in either group.At week 52,one patient in the monotherapy group was HBsAg-negative but there was none in the combination therapy group.The monotherapy group showed significantly better liver function recovery results than the combination therapy group.At week 78,one patient in the combination group had HBsAg seroconverted.At week 104,only three patients in the combination therapy group were HBsAg-negative compared with one patient in monotherapy.The mean alanine aminotransferase and aspartate aminotransferase levels and transient elastography values decreased significantly compared with baseline.Both groups showed a favorable decrease in alpha-fetoprotein(monotherapy:4.5[2.8,7.1]vs.2.2[1.8,3.1]ng/mL,P<0.001;combination therapy:5.7[3.0,18.8]vs.3.2[2.0,4.3]ng/mL,P<0.001)and an improved result of liver biopsy examination scores.The combination group showed a better improvement in histology compared with the monotherapy group(mean transient elastography value 6.6[4.9,9.8]vs.7.8[5.4,11.1]kPa,P=0.028).But there was no significant difference in HBsAg conversion rate(1.8%[1/56]i^s.4.1%[3/73],P=0.809)and HBeAg conversion rate(12.5%[7/56]i/s.11.0%[8/73],P=0.787),as well as HBV-DNA,sustained virologic response(93.2%vs.98.5%,P=0.150)between the two groups.Conclusions:Both therapies supported liver function recovery and histology improvement.Combination therapy did not show better anti-viral efficacy in HBsAg or HBeAg seroconversion compared with monotherapy.However,combination therapy played a more positive role in reversing hepatic fibrosis compared with monotherapy.Trial registration:ClinicalTrials.gov:NCT02849132;http://gffzzd3f38a9638994581scuk596uk09uq69wo.ffgz.tsg.suse.edu.cn/ct2/show/NCT02849132. | Jing-Mao Yang Li-Ping Chen Ya-Jie Wang Bei Lyu Hong Zhao Zhi-Yin Shang Jun Li Zhen-Yu Fan Sheng-Di Wu Xiao Ming Xian Li Shao-Ping Huang Ji-Lin Cheng | 2020 | Chinese Medical Journal2020,,14: | 11 |
| 20 | Wetting Patterns and Nitrate Distributions in Layered-Textural Soils Under Drip Irrigation显示文摘Laboratory experiments were conducted in different sequence and thickness of the soil layers to investigate the effects of layered-textural soils on wetting patterns and water and nitrate distributions from a surface point source under various combinations of application rate and applied volume. Three layered soils, including a sandy-over-sandy loam (SL), a sandy loam-over-sandy (LS), and a sandy loam-sandy-sandy loam (LSL), and two uniform soils (a uniform sandy loam and a uniform sandy soil) were tested. In the experiments, the application rate was varied from 0.69 to 3.86 L h-1 and the applied volume from 5.7 to 12.1 L. The experimental results demonstrated that the wetting patterns and water and nitrate distributions were greatly affected by the sequence and thickness of soil layers as well as the application rate and volume applied. An interface existing in the layered soils, whether a fine-over-coarse or a coarse-over-fine, had a common feature of limiting downward water movement and of increasing horizontal water movement. For the fine-over-coarse layered soils of LS and LSL, water and nitrate were uniformly distributed at a given depth in the top layer soil. For a coarse-over-fine layered soil of SL, however, water accumulated in the sublayer soil underneath the interface and a zone of lower nitrate concentration was observed. The effect of application rate on water distribution pattern was dependent upon soil layering. A minor influence of application rate on water distribution for the fine-over-coarse layered soils (LS and LSL) than for the uniform soils was found. To obtain a greater wetted depth through selecting the emitters having a smaller application rate, which is a common method in the system design for a uniform soil, may not be necessarily applied for the layered soils. Measurements of nitrate distribution showed that nitrate accumulated toward the boundary of the wetted volume for both the uniform and the layered soils. This suggests the importance of optimal management of drip fertigation because nitrate is susceptible to the movement out of the root zone by mismanagement of fertigation.
