1High-intensity focused ultrasound ablation:An effective bridging therapy for hepatocellular carcinoma patients显示文摘AIM:To analyze whether high-intensity focused ultrasound(HIFU) ablation is an effective bridging therapy for patients with hepatocellular carcinoma(HCC).METHODS:From January 2007 to December 2010,49 consecutive HCC patients were listed for liver transplantation(UCSF criteria).The median waiting time for transplantation was 9.5 mo.Twenty-nine patients received transarterial chemoembolization(TACE) as a bringing therapy and 16 patients received no treatment before transplantation.Five patients received HIFU ablation as a bridging therapy.Another five patients with the same tumor staging(within the UCSF criteria) who received HIFU ablation but not on the transplant list were included for comparison.Patients were comparable in terms of Child-Pugh and model for end-stage liver disease scores,tumor size and number,and cause of cirrhosis.RESULTS:The HIFU group and TACE group showed no difference in terms of tumor size and tumor number.One patient in the HIFU group and no patient in the TACE group had gross ascites.The median hospital stay was 1 d(range,1-21 d) in the TACE group and two days(range,1-9 d) in the HIFU group(P < 0.000).No HIFU-related complication occurred.In the HIFU group,nine patients(90%) had complete response and one patient(10%) had partial response to the treatment.In the TACE group,only one patient(3%) had response to the treatment while 14 patients(48%) had stable disease and 14 patients(48%) had progressive disease(P = 0.00).Seven patients in the TACE group and no patient in the HIFU group dropped out from the transplant waiting list(P = 0.559).CONCLUSION:HIFU ablation is safe and effective in the treatment of HCC for patients with advanced cirrhosis.It may reduce the drop-out rate of liver transplant candidate.Tan To Cheung Sheung Tat Fan See Ching Chan Kenneth SH Chok Ferdinand SK Chu Caroline R Jenkins Regina CL Lo James YY Fung Albert CY Chan William W Sharr Simon HY Tsang Wing Chiu Dai Ronnie TP Poon Chung Mau Lo 2013World Journal of Gastroenterology2013,19,20:23

3Neuroendocrine tumors of the gastrointestinal tract: Case reports and literature review显示文摘Neuroendocrine tumors(NET)previously called carcinoid tumors are neoplasms of enterochromaffin/neuroendocrine cell origin which display neurosecretory capacity that may result in the carcinoid syndrome.The annual incidence of patients with NET is 8.4 per 100000;yet many NET remain asymptomatic and clinically undetected.A majority of NET follows a benign course;however,some will display malignant characteristics.NET most commonly occur in the gastrointestinal tract(67%)and bronchopulmonary system(25%).Gastrointestinal NET occur within the stomach,small intestine,liver,and rectum.We report a retrospective study of 11 subjects:Eight with benign carcinoid tumors:duodenal bulb(n=2),terminal ileum(n=1),sigmoid colon(n=2),and rectum(n=3);three with malignant carcinoid:liver(n=1)and intra-abdominal site(n=2).The diagnosis,endoscopic images,outcome,treatment and review of the literature are presented.William J Salyers Kenneth J Vega Juan Carlos Munoz Bruce W Trotman Silvio S Tanev 2014World Journal of Gastrointestinal Oncology2014,6,8:8
4Modulation of graft vascular inflow guided by flowmetry and manometry in liver transplantation显示文摘BACKGROUND:Survival of the partial graft after living donor liver transplantation owes much to its tremendous regenerative ability.With excellent venous outflow capacity,a graft within a wide range of graft-to-standard-liver-volume ratios can cope with portal hypertension that is common in liver transplant recipients.However,when the ratio range is exceeded,modulation of graft vascular inflow becomes necessary for graft survival.The interplay between graft-to-standard-liver-volume ratio and portal pressure,in the presence of portosystemic shunt or otherwise,requires individualized modulation of graft portal and arterial inflows.Boosting of portal inflow by shunt ligation can be guided by transonic flowmetry,whereas muting of portal inflow by splenic artery ligation can be monitored by portal electronic manometry.METHOD:We describe four cases to illustrate the above.RESULTS:One patient had hepatic artery thrombosis resulting from splenic artery steal syndrome which was the sequela of small-for-size syndrome.Emergency splenic artery ligation and re-anastomosis of the hepatic artery successfully muted the portal inflow and boosted the hepatic arterial inflow.Another patient with portal vein thrombosis underwent thrombendvenectomy.Portal inflow was boosted with ligation of portosystemic shunt,which is often present in these patients with portal hypertension.The coexistence of splenic aneurysm and splenorenal shunt required ligation of both in the third patient.The fourth patient,with portal pressure and flow monitoring,avoided ligation of a coronary vein which became a main portal inflow after portal thrombendvenectomy.CONCLUSION:Management of graft inflow modulation guided selectively by transonic flowmetry or portal manometry was described.See Ching Chan Chung Mau Lo Kenneth SH Chok William W Sharr Tan To Cheung Simon HY Tsang Albert CY Chan Sheung Tat Fan 2011Hepatobiliary & Pancreatic Diseases International2011,10,6:5

6High-intensity focused ultrasound ablation as a bridging therapy for hepatocellular carcinoma patients awaiting liver transplantation显示文摘The scarcity of liver grafts in Asia leads to a significant dropout of patients from liver transplant waiting lists, particularly patients with hepatocellular carcinoma and a low model for end-stage liver disease score. In order to reduce dropping out, different bridging therapies are employed. We report the use of high-intensity focused ultrasound ablation as a bridging therapy for a patient with hepatocellular carcinoma of stage two and an extremely low platelet count (20×10 9 /L). The ablation was successful. Blood tests showed that his liver function was similar before and after the treatment. 7Intestinal stem cells and the colorectal cancer microenvironment显示文摘Colorectal cancer(CRC)remains a highly fatal condition in part due to its resilience to treatment and its propensity to spread beyond the site of primary occurrence.One possible avenue for cancer to escape eradication is via stem-like cancer cells that,through phenotypic heterogeneity,are more resilient than other tumor constituents and are key contributors to cancer growth and metastasis.These proliferative tumor cells are theorized to possess many properties akin to normal intestinal stem cells.Not only do these CRC'stem'cells demonstrate similar restorative ability,they also share many cell pathways and surface markers in common,as well as respond to the same key niche stimuli.With the improvement of techniques for epithelial stem cell identification,our understanding of CRC behavior is also evolving.Emerging evidence about cellular plasticity and epithelial mesenchymal transition are shedding light onto metastatic CRC processes and are also challenging fundamental concepts about unidirectional epithelial proliferation.This review aims to reappraise evidence supporting the existence and behavior of CRC stem cells,their relationship to normal stem cells,and their possible dependence on the stem cell niche.Bryan A Ong Kenneth J Vega Courtney W Houchen 2014World Journal of Gastroenterology2014,20,8:2

9Computation of flow through the oesophagogastric junction显示文摘Whilst methods exist to indirectly measure the effects of increased flow or gastro-oesophageal refluxing, they cannot quantitatively measure the amount of acid travelling back up into the oesophagus during reflux, nor can they indicate the flow rate through the oesophagogastric junction (OGJ). Since OGJ dysfunction affects flow it seems most appropriate to describe the geometry of the OGJ and its effect on the flow. A device known as the functional lumen imaging probe (FLIP) has been shown to reliably measure the geometry of and pressure changes in the OGJ. FLIP cannot directly measure flow but the data gathered from the probe can be used to model flow through the junction by using computational flow dynamics (CFD). CFD uses a set of equations known as the Navier-Stokes equations to predict flow patterns and is a technique widely used in engineering. These equations are complex and require appropriate assumptions to provide simplifications before useful data can be obtained. With the assumption that the cross-sectional areas obtained via FLIP are circular, the radii of these circles can be obtained. A cubic interpolation scheme can then be applied to give a high-resolution geometry for the OGJ. In the case of modelling a reflux scenario, it can be seen that at the narrowest section a jet of fluid squirts into the oesophagus at a higher velocity than the fluid surrounding it. This jet has a maximum velocity of almost 2 ms-1 that occurs where the OGJ is at its narrowest. This simple prediction of acid ‘squirting’ into the oesophagusillustrates how the use of numerical methods can be used to develop a better understanding of the OGJ. 10用音法评价正畸患者的咬合稳定性显示文摘目的:本研究拟通过音法检测正畸治疗前后患者的咬合稳定性,并通过对比主、客观两种检测方法,评价用主观法检查稳定性的可靠性。方法:选择2003年至2005年间在北京大学口腔医学院正畸科就诊的错畸形患者64例为初诊组;全口固定正畸矫治结束即刻患者15例为矫治结束即刻组。用主观听诊法及客观录音分析法评估患者音。对客观检查结果,通过卡方检验比较初诊组与矫治结束即刻组间咬合稳定性的差别,比较不同错类型的初诊患者间咬合稳定性的差别。以客观法为金标准计算主观法诊断的准确度,通过Kappa检验分析主客观方法间的一致性。结果:(1)43.75%(28/64)的初诊患者咬合不稳定,卡方检验显示不同类型的错畸形间咬合稳定性的差异没有统计学意义。(2)15例固定矫治结束即刻患者中40%(6/15)的患者咬合不稳定。(3)卡方检验发现初诊患者组与矫治结束即刻组咬合稳定性的差异没有统计学意义。(4)主观法音检查的准确度为81.01%(64/79)。(5)Kappa检验表明主客观两种方法存在较高一致性,Kappa值为0.488。结论:(1)正畸治疗前后均有部分患者咬合不稳定,不同类型错患者间咬合稳定性率差异没有统计学意义。(2)主、客观两种方法存在较高一致性。姜若萍 Kenneth W TYSON 2008北京大学学报(医学版)2008,40,1:2
11Retrohepatic vena cava deroofing in living donor liver transplantation for caudate hepatocellular carcinoma显示文摘The removal of tumor together with the native liver in living donor liver transplantation for hepatocellular carcinoma is challenged by a very close resection margin if the tumor abuts the inferior vena cava.This is in contrast to typical deceased donor liver transplantation where the entire retrohepatic inferior vena cava is included in total hepatectomy.Here we report a case of deroofing the retrohepatic vena cava in living donor liver transplantation for caudate hepatocellular carcinoma.In order to ensure clear resection margins,the anterior portion of the inferior vena cava was included.The right liver graft was inset into a Dacron vascular graft on the back table and the composite graft was then implanted to the recipient inferior vena cava.Using this technique,we observed the no-touch technique in tumor removal,hence minimizing the chance of positive resection margin as well as the chance of shedding of tumor cells during manipulation in operation.See Ching Chan William W Sharr Tan To Cheung Albert CY Chan Simon HY Tsang Kenneth SH Chok Kin Chung Leung Chung Mau Lo 2013Hepatobiliary & Pancreatic Diseases International2013,12,5:2
