1Impact of preoperative chemoradiotherapy on survival in patients with resectable pancreatic cancer显示文摘AIM:To explore whether preoperative chemoradiation therapy improves survival of patients with pancreatic cancer undergoing resectional surgery. METHODS:Forty-seven patients with a malignant pancreatic tumor localized in the head or uncinate process of the pancreas underwent radical pancreaticoduodenectomy. Twenty-two received chemoradiation therapy (gemcitabine and radiation dose 50.4 Gy) before surgery (CRR) and 25 patients underwent surgery only (RO). The study was non-randomised. Patients were identified from a prospective database. RESULTS:The median survival time was 30.2 mo in the CRR group and 35.9 mo in the RO group. No statistically significant differences were found in subclasses according to lymph node involvement,TNM stages,tumor size,or perineural invasion. The one,three and five year survival rates were 81%,33% and 33%,respectively,in the CRR group and 72%,47% and 23%,respectively,in the RO group. In ductal adenocarcinoma,the median survival time was 27 mo in the CRR group and 20 mo in the RO group. No statistically significant differences were found in the above subclasses. The one,three and five year survival rates were 79%,21% and 21%,respectively,in the CRR group and 64%,50% and 14%,respectively,in the RO group. The overall hospital mortality rate was 2%. The morbidity rate was 45% in the CRR group and 32% (NS) in the RO group. 2Myofibroblastic cell activation and neovascularization predict native liver survival and development of esophageal varices in biliary atresia显示文摘AIM:To study the relation between collagen 1,α-smooth muscle actin(α-SMA)and CD34 expression and the most essential portoenterostomy(PE)outcomes.METHODS:Liver specimens were obtained at PE from33 biliary atresia(BA)patients for immunohistochemical analysis of collagen 1,α-SMA and CD34.Liver biopsies from 35 organ donors were used as controls.Expression patterns were related to clinical data including age at PE,serum total and conjugated bilirubin concentration at the time of PE and during follow-up,incidence of esophageal varices in follow-up upper gastrointestinal endoscopies,and native liver survival as well as to detailed histopathological findings.RESULTS:Collagen 1(16.4%vs 4.5%,P<0.0001),α-SMA(17.9%vs 4.6%,P<0.0001)and CD34(4.9%vs 3.8%,P=0.017)were markedly overexpressed in BA patients compared with controls.Patients who underwent liver transplantation by age of two years had significantly higher expression of collagen 1(18.6%vs 13.7%,P=0.024),α-SMA(20.4%vs 15.4%,P=0.009)and CD34(5.9%vs 4.0%,P=0.029)at PE compared with native liver survivors.CD34-positive microvessels were identified in the centrizonal region close to central vein in every BA patient.In majority of BA cases(56%)neovascularization was frequent as CD34-positive microvessels were observed in over half of the hepatic lobules.In controls,the CD34-positive microvessels were rare as they were completely absent in 40%and were found in less than 5%of the hepatic lobules in the rest.The difference between BA patients and controls was significant(P<0.0001).Patients who developed esophageal varices by two years had significantly higher expression of CD34 at PE compared with patients without varices(5.6%vs 4.0%,P=0.019).Expression ofα-SMA(r=0.758,P<0.0001)and collagen 1(r=0.474,P=0.016),and the amount of CD34-positive microvessels(r=0.356,P=0.047)were related to patient age at PE.CONCLUSION:Hepatic myofibroblastic cell activation,fibrogenesis and neovascularization are enhanced in BA,progress with increasing PE age and relate to native liver survival and development of esophageal varices.Janne S Suominen Hanna Lampela Pivi Heikkil Jouko Lohi Hannu Jalanko Mikko P Pakarinen 2014World Journal of Gastroenterology2014,20,12:6

4Use of a combination strategy to improve neuroprotection and neuroregeneration in a rat model of acute spinal cord injury显示文摘Spinal cord injury is a very common pathological event that has devastating functional consequences in patients. In recent years, several research groups are trying to find an effective therapy that could be applied in clinical practice. In this study, we analyzed the combination of different strategies as a potential therapy for spinal cord injury. Immunization with neural derived peptides(INDP), inhibition of glial scar formation(dipyridyl: DPY), as well as the use of biocompatible matrix(fibrin glue: FG) impregnated with bone marrow mesenchymal stem cells(MSCs) were combined and then its beneficial effects were evaluated in the induction of neuroprotection and neuroregeneration after acute SCI. Sprague-Dawley female rats were subjected to a moderate spinal cord injury and then randomly allocated into five groups: 1) phosphate buffered saline; 2) DPY; 3) INDP + DPY; 4) DPY+ FG; 5) INDP + DPY + FG + MSCs. In all rats, intervention was performed 72 hours after spinal cord injury. Locomotor and sensibility recovery was assessed in all rats. At 60 days after treatment, histological examinations of the spinal cord(hematoxylin-eosin and Bielschowsky staining) were performed. Our results showed that the combination therapy(DPY+ INDP + FG+ MSCs) was the best strategy to promote motor and sensibility recovery. In addition, significant increases in tissue preservation and axonal density were observed in the combination therapy group. Findings from this study suggest that the combination theapy(DPY+ INDP + FG + MSCs) exhibits potential effects on the protection and regeneration of neural tissue after acute spinal cord injury. 5Experimental Investigation of Immiscible Supercritical Carbon Dioxide Foam Rheology for Improved Oil Recovery显示文摘This paper presents the rheological behaviour of supercritical CO_2(s CO_2) foam at reservoir conditions of 1 500 psi and 80 °C. Different commercial surfactants were screened and utilized in order to generate a fairly stable CO_2 foam. Mixed surfactant system was also introduced to generate strong foam. Foam rheology was studied for some specific foam qualities using a high pressure high temperature(HPHT) foam loop rheometer. A typical shear thinning behaviour of the foam was observed and a significant increase in the foam viscosity was noticed with the increase of foam quality until 85%. A desired high apparent viscosity with coarse texture was found at 85% foam quality. Foam visualization above 85% showed an unstable foam due to extremely thin lamella which collapsed and totally disappeared in the loop rheometer. Below 5_2%, a non-homogenous and unstable foam was found having low viscosity with some liquid accumulation at the bottom of the circulation loop. 6Differential roles of EPS8 in carcinogenesis:Loss of protein expression in a subset of colorectal carcinoma and adenoma显示文摘AIM:To analyze the epidermal growth factor receptor pathway substrate 8(EPS8) expression status and role in colorectal carcinogenesis given that EPS8 has a conserved actin barbed-end capping function that is required for proper maturation in intestinal cells.METHODS:We studied 8 colon cancer cell lines and 58 colorectal tumors(19 adenomas and 39 carcinomas).We performed expression microarray analysis of colon cancer cell lines followed by loss of heterozygosity(LOH)analysis and immunohistochemistry for EPS8 expression in colon tumors.Subsequently,we performed mutation analysis by direct sequencing and methylation analysis by bisulfite sequencing and methylation-specific polymerase chain reaction assays.RESULTS:Expression microarray analysis of colon cancer cell lines showed overexpression of EPS8 transcript in all lines but RKO.Genome wide loss of heterozygosity(LOH) analysis of colon tumors,showed considerable LOH at the EPS8 gene locus.Immunohistochemically,EPS8 was constitutively expressed in normal colonic mucosa with a dot-like supranuclear localization with accentuation at the luminal surface supporting its proposed role in epithelial maturation.Nineteen colon tumors(4 adenoma,15 carcinoma) out of 51(37%) showed strikingly tumor specific EPS8 protein loss.Of the remaining tumors,5/51(2 adenoma,and 3 carcinoma,10%) showed marked overexpression,while 27/51 tumors(53%) showed retained expression.Mutation analysis revealed a missense mutation(c.794C>T,p.R265C) in exon 8 in RKO.The EPS8 promoter was also methylated in RKO,but there was no significant methylation in other cell lines or carcinoma specimens.CONCLUSION:The loss of EPS8 expression in colorectal adenomas and carcinomas suggests that down regulation of this gene contributes to the development of a subset of colorectal cancers,a finding which could have applications in diagnosis and treatment.Wael M Abdel-Rahman Salla Ruosaari Sakari Knuutila Pivi Peltomki 2012World Journal of Gastroenterology2012,18,29:5

8Aquatic exercise program-modulated oxidative stress markers in patients with Parkinson's disease显示文摘Parkinson's disease is a neurodegenerative disease.Oxidative stress,i.e.,the imbalance between the generation of reactive oxygen species and the antioxidant defense capacity of the body,plays an important role in the pathogenesis of this disease.Physical exercise can regulate oxidative stress.The purpose of this study was to analyze the short-and long-term effects of an aquatic exercise program on oxidative stress levels in patients with Parkinson's disease.The aquatic exercise program was carried out during 1 month with two sessions per week(1 hour/session).Blood samples were collected at four different time points:pre-intervention,immediately,48 hours,and 30 days after the first session of aquatic exercise program.Our results revealed that water-based programs modulated antioxidant enzyme activity,increased superoxide dismutase activity,reduced catalase activity,and increased the ratio of superoxide dismutase activity to catalase activity in patients with Parkinson's disease.Compared with pre-intervention and 48 hours after the first session of aquatic exercise program,superoxide dismutase activity was higher and catalase activity was lower immediately and 30 days after the first session.Our results demonstrated that aquatic exercise program could modulate oxidative stress,mainly by the effect of antioxidant enzyme activity.These results could better help understand the target of oxidative stress in Parkinson's disease.This study was approved by the Ethics Committee of Centro Universitário Metodista IPA(approval No.1.373.911)on August 9,2019 and registered with REBEC(registration number:RBR-6 NJ4 MK).Caroline Dani Isabel Teixeira Proenca Jessica Marinho Pamela Peccin Ivy Reichert Vital da Silva Simone Nique Vera Striebel Daniela Pochmann Viviane Rostirola Elsner 2020Neural Regeneration Research2020,15,11:4
9Methods, units and quality requirements for the analysis of haemoglobin A_(1c) in diabetes mellitus显示文摘The formation of glycohemoglobin, especially the hemoglobin A1c(Hb_(A1c)) fraction, occurs when glucose becomes coupled with the amino acid valine in the β-chain of Hb; this reaction is dependent on the plasma concentration of glucose. Since the early 1970 s it has been known that diabetics display higher values of Hb_(A1c) because they have elevated blood glucose concentrations. Thus Hb_(A1c) has acquired a very important role in the treatment and diagnosis of diabetes mellitus. After the introduction of the first quantitative measurement of Hb_(A1c), numerous methods for glycohemoglobin have been introduced with different assay principles: From a simple minicolumn technique to the very accurate automated highpressure chromatography and lastly to many automated immunochemical or enzymatic assays. In early days, the results of the quality control reports for Hb_(A1c) varied extensively between laboratories, therefore in United States and Canada working groups(WG) of the Diabetes Controls and Complications Trial(DCCT) were set up to standardize the Hb_(A1c) assays against the DCCT/National Glycohemoglobin Standardization Program reference method based on liquid chromatography. In the 1990 s, the International Federation of Clinical Chemistry and Laboratory Medicine(IFCC) appointed a new WG to plan a reference preparation and method for the Hb_(A1c) measurement. When the reference procedureswere established, in 2004 IFCC recommended that all manufacturers for equipment used in Hb_(A1c) assays should calibrate their methods to their proposals. This led to an improvement in the coefficient of variation(CV%) associated with the assay. In this review, we describe the glycation of Hb, methods, standardization of the Hb_(A1c) assays, analytical problems, problems with the units in which Hb_(A1c) values are expressed, reference values, quality control aspects, target requirements for Hb_(A1c), and the relationship of the plasma glucose values to Hb_(A1c) concentrations. We also note that the acceptance of the mmol/mol system for Hb_(A1c) as recommended by IFCC, i.e., the new unit and reference ranges, are becoming only slowly accepted outside of Europe where it seems that expressing Hb_(A1c) values either only in per cent units or with parallel reporting of percent and mmol/mol will continue. We believe that these issues should be resolved in the future and that it would avoid confusion if mmol/mol unit for Hb_(A1c) were to gain worldwide acceptance.Ilkka Penttil? Karri Penttil? 13Glial fibrillary acidic protein levels are associated with global histone H4 acetylation after spinal cord injury in rats显示文摘Emerging evidence has suggested global histone H4 acetylation status plays an important role in neural plasticity. For instance, the imbalance of this epigenetic marker has been hypothesized as a key factor for the development and progression of several neurological diseases. Likewise, astrocytic reactivity-a wellknown process that markedly influences the tissue remodeling after a central nervous system injury-is crucial for tissue remodeling after spinal cord injury(SCI). However, the linkage between the above-mentioned mechanisms after SCI remains poorly understood. We sought to investigate the relation between both glial fibrillary acidic protein(GFAP) and S100 calcium-binding protein B(S100B)(astrocytic reactivity classical markers) and global histone H4 acetylation levels. Sixty-one male Wistar rats(aged ~3 months) were divided into the following groups: sham; 6 hours post-SCI; 24 hours post-SCI; 48 hours post-SCI; 72 hours post-SCI; and 7 days post-SCI. The results suggested that GFAP, but not S100B was associated with global histone H4 acetylation levels. Moreover, global histone H4 acetylation levels exhibited a complex pattern after SCI, encompassing at least three clearly defined phases(first phase: no changes in the 6, 24 and 48 hours post-SCI groups; second phase: increased levels in the 72 hours post-SCI group; and a third phase: return to levels similar to control in the 7 days post-SCI group). Overall, these findings suggest global H4 acetylation levels exhibit distinct patterns of expression during the first week post-SCI, which may be associated with GFAP levels in the perilesional tissue.
