1化疗所致恶心呕吐对复发性卵巢癌患者预后的影响:纳入1213例患者数据的meta分析结果显示文摘背景与目的 本研究旨在研究化疗所致恶心呕吐(chemotherapy-induced nausea and vomiting,CINV)对化疗药物剂量减少、中止以及患者生存获益的潜在影响。方法 本研究基于德国东北妇科肿瘤学会(North-Eastern German Society of Gynecological Oncology,NOGGO)开展的3项Ⅱ/Ⅲ期临床试验中的1213例复发性卵巢癌患者的原始数据,进行了个体病例数据的meta分析。通过Logistic回归和Cox回归分析,对年龄、ECOG评分、化疗周期数、复发次数、并发症以及不同研究等因素进行校正后,评估相应的比值比和风险比(hazard ratio,HR)。结果 大多数(58.1%)患者发生了恶心,且近三分之一(31.0%)的患者出现呕吐。CINV的发生与FIGO分期、分级、组织学以及复发次数无关;必须的化疗药物减量和中止化疗与恶心、呕吐的发生亦无相关性(P=0.88,P=0.39;P=0.25,P=0.54)。发生Ⅲ/Ⅳ级恶心呕吐患者的无进展生存期更短(P=0.02,Ⅲ/Ⅳ级恶心的HR=1.58,95%CI:1.14–2.20;P=0.02,Ⅲ/Ⅳ级呕吐的HR=1.67,95%CI:1.15–2.42)。发生Ⅲ/Ⅳ级恶心和呕吐的患者生存期较短(P <0.001,Ⅲ/Ⅳ级恶心的HR=2.35,95%CI:1.64–3.37;P <0.001,Ⅲ/Ⅳ级呕吐的HR=1.67,95%CI:1.15–2.42)。结论 CINV与复发性卵巢癌患者较差的预后显著相关,而与必须的化疗药物减量和中止无相关性。本研究强调CINV防治是早期最佳支持治疗的重要环节。Hannah Woopen R Richter R Chekerov G Inci S Alavi J P Grabowski J Sehouli 尹如铁(译) 2022癌症2022,41,6:2
