| 2 | Criteria for the diagnosis and severity stratification of acute pancreatitis显示文摘Recent diagnostic and therapeutic progress for severe acute pancreatitis(SAP)remarkably decreased the casemortality rate.To further decrease the mortality rate of SAP,it is important to precisely evaluate the severity at an early stage,and initiate appropriate treatment as early as possible.Research Committee of Intractable Diseases of the Pancreas in Japan developed simpler criteria combining routinely available data with clinical signs.Severity can be evaluated by laboratory examinations or by clinical signs,reducing the defect values of the severity factors.Moreover,the severity criteria considered laboratory/clinical severity scores and contrastenhanced computed tomography(CE-CT)findings as independent risk factors.Thus,CE-CT scans are not necessarily required to evaluate the severity of acute pancreatitis.There was no fatal case in mild AP diagnosed by the CE-CT severity score,whereas case-mortality rate in those with SAP was 14.8%.Case-mortality of SAP that fulfilled both the laboratory/clinical and the CE-CT severity criteria was 30.8%.It is recommended,therefore,to perform CE-CT examination to clarify the prognosis in those patients who were diagnosed as SAP by laboratory/clinical severity criteria.Because the mortality rate of these patients with SAP is high,such patients should be transferred to advanced medical units. | Makoto Otsuki Kazunori Takeda Seiki Matsuno Yasuyuki Kihara Masaru Koizumi Masahiko Hirota Tetsuhide Ito Keisho Kataoka Motoji Kitagawa Kazuo Inui Yoshifumi Takeyama | 2013 | World Journal of Gastroenterology2013,19,35: | 24 |
| 3 | Activation of hypoxia-inducible factor 1 attenuates periapical inflammation and bone loss显示文摘Hypoxia(low oxygen level) is an important feature during infections and affects the host defence mechanisms. The host has evolved specific responses to address hypoxia, which are strongly dependent on the activation of hypoxia-inducible factor 1(HIF-1).Hypoxia interferes degradation of HIF-1 alpha subunit(HIF-1α), leading to stabilisation of HIF-1α, heterodimerization with HIF-1 beta subunit(HIF-1β) and subsequent activation of HIF-1 pathway. Apical periodontitis(periapical lesion) is a consequence of endodontic infection and ultimately results in destruction of tooth-supporting tissue, including alveolar bone. Thus far, the role of HIF-1 in periapical lesions has not been systematically examined. In the present study, we determined the role of HIF-1 in a wellcharacterised mouse periapical lesion model using two HIF-1α-activating strategies, dimethyloxalylglycine(DMOG) and adenovirusinduced constitutively active HIF-1α(CA-HIF1 A). Both DMOG and CA-HIF1 A attenuated periapical inflammation and tissue destruction. The attenuation in vivo was associated with downregulation of nuclear factor-κappa B(NF-κB) and osteoclastic gene expressions. These two agents also suppressed NF-κB activation and subsequent production of proinflammatory cytokines by macrophages. Furthermore, activation of HIF-1α by DMOG specifically suppressed lipopolysaccharide-stimulated macrophage differentiation into M1 cells, increasing the ratio of M2 macrophages against M1 cells. | 5 | Histological diagnosis of gastric submucosal tumors: A pilot study of endoscopic ultrasonography-guided fine-needle aspiration biopsy vs mucosal cutting biopsy显示文摘AIM: To compare the usefulness of endoscopic ultrasonography-guided fine-needle aspiration biopsy(EUS-FNAB) without cytology and mucosal cutting biopsy(MCB) in the histological diagnosis of gastric submucosal tumor(SMT).METHODS: We prospectively compared the diagnostic yield, feasibility, and safety of EUS-FNAB and those of MCB based on endoscopic submucosal dissection. The cases of 20 consecutive patients with gastric SMT ≥1 cm in diameter. who underwent both EUS-FNAB and MCB were investigated.RESULTS: The histological diagnoses were gastrointestinal stromal tumors(n = 7), leiomyoma(n =6), schwannoma(n = 2), aberrant pancreas(n = 2), and one case each of glomus tumor, metastatic hepatocellular carcinoma, and no-diagnosis. The tumors' mean size was 23.6 mm. Histological diagnosis was made in 65.0% of the EUS-FNABs and 60.0% of the MCBs, a nonsignificant difference. There were no significant differences in the diagnostic yield concerning the tumor location or tumor size between the two methods. However, diagnostic specimens were significantly more frequently obtained in lesions with intraluminal growth than in those with extraluminal growth by the MCB method(P = 0.01). All four SMTs with extraluminal growth were diagnosed only by EUSFNAB(P = 0.03). | 6 | Patterns of plant diversity at high altitudes on the Qinghai-Tibetan Plateau显示文摘To describe the biodiversity patterns of plants along an altitudinal gradient on the Qinghai-Tibetan Plateau and to clarify the bias in plant specimen records at high altitude.Methods We conducted a large-scale investigation of vegetation at a wide range of altitudes,focusing on a high-altitudinal range(3200-5200 m)at different locations on the Qinghai-Tibetan Plateau.We then compared the altitudinal distribution of plant species obtained from our field investigation with that in plant specimen records from published sources and an online database.Important Findings Our data provide evidence that altitude plays a large role in regulating species composition on the Qinghai-Tibetan Plateau.We could not,however,detect a clear relationship between altitude and species richness,although a weak monotonically increasing trend of richness was detected with increasing altitude.According to specimen records,most species have been sampled at a wide range of altitudes,and the average range of 145 species is>2000 m.Despite this wide range,more than half of the species we observed were at higher altitudes than the specimen records indicate.High-altitude areas have probably been so poorly sampled that only a small fraction of the resident species has been recorded.This study clearly shows the regional bias of specimen records in the Qinghai-Tibetan Plateau. | Ayako Shimono Huakun Zhou Haihua Shen Mitsuru Hirota Toshiyuki Ohtsuka Yanhong Tang | 2010 | Journal of Plant Ecology2010,3,1: | 6 |
| 7 | Altitudinal variation of ecosystem CO_(2) fluxes in an alpine grassland from 3600 to 4200 m显示文摘Aims Recent studies have recognized the alpine grasslands on the Qinghai-Tibetan plateau as a significant sink for atmospheric CO_(2).The carbon-sink strength may differ among grassland ecosystems at various altitudes because of contrasting biotic and physical environments.This study aims(i)to clarify the altitudinal pattern of ecosystem CO_(2) fluxes,including gross primary production(GPP),daytime ecosystem respiration(Redaytime)and net ecosystem production(NEP),during the period with peak above-ground biomass;and(ii)to elucidate the effects of biotic and abiotic factors on the altitudinal variation of ecosystem CO_(2) fluxes.Methods Ecosystem CO_(2) fluxes and abiotic and biotic environmental factors were measured in an alpine grassland at four altitudes from 3600 to 4200 m along a slope of the Qilian Mountains on the northwestern Qinghai-Tibetan Plateau during the growing season of 2007.We used a closed-chamber method combined with shade screens and an opaque cloth to measure several carbon fluxes,GPP,Redaytime and NEP,and factors,light-response curve for GPP and temperature sensitivity of Redaytime.Above-and below-ground biomasses and soil C and N contents at each measurement point were also measured.Important Findings(i)Altitudinal pattern of ecosystem CO_(2) fluxes:The maximum net ecosystem CO_(2) flux(NEPmax),i.e.the potential ecosystem CO_(2) sink strength,was markedly different among the four altitudes.NEPmax was higher at the highest and lowest sites,ap proximately7.460.9 and6.760.6 lmol CO_(2) m^(-2)s^(-1)(mean 6 standard error),respectively,but smaller at the intermediate altitude sites(3800 and 4000 m).The altitudinal pattern of maximum gross primary production was similar to that of NEPmax.The Redaytime,however,was significantly higher at the lowest altitude(3.460.3 lmol CO_(2) m^(-2)s^(-1))than at the other three altitudes.(ii)Altitudinal variation of vegetation biomass:The aboveground biomass was higher at the highest altitude(154627 g DW m2)than at the other altitudes,which we attribute mainly to the large biomass in cushion plants at the highest altitude.The small above-ground biomass at the lower altitudes was probably due to heavy grazing during the growing season.(iii)Features of ecosystem CO_(2) fluxes:Redaytime and GPP were positively correlated with above-ground biomass.The low ratio of Redaytime to GPP at either the measurement point or the site level suggests that CO_(2) uptake efficiency tends to be higher at higher altitudes,which indicates a high potential sink strength for atmospheric CO_(2) despite the low temperature at high altitudes.The results suggest that the effect of grazing intensity on ecosystem carbon dynamics,partly by decreasing vegetation biomass,should be clarified further. | Mitsuru Hirota Pengcheng Zhang Song Gu Mingyuan Du Ayako Shimono Haihua Shen Yingnian Li Yanhong Tang | 2009 | Journal of Plant Ecology2009,2,4: | 6 |

| 11 | Characterization of CO_(2) flux in three Kobresia meadows differing in dominant species显示文摘Aims Kobresia meadows,the dominant species of which differ in different habitats,cover a large area of alpine grassland on the QinghaiTibetan Plateau and act as potential CO_(2) sinks.Kobresia meadows with different dominant species may differ in carbon sink strength.We aimed to test the hypothesis and to clarify the differences in CO_(2) sink strength among three major Kobresia meadows on the plateau and the mechanisms underlying these differences.Methods We measured the net ecosystem exchange flux(NEE),ecosystem respiration flux(ER),aboveground biomass(AGB)and environmental variables in three Kobresia meadows,dominated by K.pygmaea,K.humilis,or K.tibetica,respectively,in Haibei,Qinghai.NEE and ER were measured by a closed-chamber method.Environmental variables,including photosynthetic photon flux density(PPFD),air and soil temperature and air and soil moisture,were monitored during the above flux measurements.Important findings The measured peak AGB increased with soil water content and was 365,402 and 434 g dry weight m2 for K.pygmaea,K.humilis and K.tibetica meadow,respectively.From the maximum ecosystem photosynthetic rate in relation to PPFD measured during the growing season,we estimated gross ecosystem photosynthetic potential(GEPmax)as 22.2,29.9 and 37.8 lmol CO_(2) m2 s1 for K.pygmaea,K.humilis and K.tibetica meadow,respectively.We estimated the respective gross primary production(GPP)values as 799,1063 and 1158 g C m^(-2) year^(-1) and ER as 722,914 and 1011 g C m^(-2) year^(-1).Average net ecosystem production(NEP)was estimated to be 76.9,149.4 and 147.6 g C m^(-2) year^(-1) in K.pygmaea,K.humilis and K.tibetica meadows,respectively.The results indicate that(i)the three meadows were CO_(2) sinks during the study period and(ii)Kobresia meadows dominated by different species can differ considerably in carbon sink strength even under the same climatic conditions,which suggests the importance of characterizing spatial heterogeneity of carbon dynamics in the future. | Pengcheng Zhang Mitsuru Hirota Haihua Shen Akinori Yamamoto Shigeru Mariko Yanhong Tang | 2009 | Journal of Plant Ecology2009,2,4: | 4 |

Curative treatments such as hepatectomy,liver transplantation, and radiofrequency ablation are applied in only 30%-60% of cases. Most remaining patients receive transarterial chemoembolization(TACE).Patients with intermediate-stage HCCs are regarded as good candidates for TACE. However, the intermediate stage includes non-homogeneous patients. | 14 | Diagnosis of small intramucosal signet ring cell carcinoma of the stomach by non-magnifying narrow-band imaging: A pilot study显示文摘AIM: To examine the efficacy of non-magnifying narrow-band imaging(NM-NBI) imaging for small signet ring cell carcinoma(SRC).METHODS: We retrospectively analyzed 14 consecutive small intramucosal SRCs that had been treated with endoscopic submucosal dissection(ESD) and 14 randomly selected whitish gastric ulcer scars(control). The strength and shape of the SRCs and whitish scars by NM-NBI and white-light imaging(WLI) were assessed with Image J(NIH, Bethesda).RESULTS: NM-NBI findings of SRC showed a clearly isolated whitish area amid the brown color of the surrounding normal mucosa. The NBI index, which indicates the potency of NBI for visualizing SRC, was significantly higher than the WLI index(P = 0.001), indicating SRC was more clearly identified by NM-NBI. Although the NBI index was not significantly different between SRCs and controls, the circle(C)-index, as an index of circularity of tumor shape, was significantly higher in SRCs(P = 0.001). According to the receiveroperating characteristic analysis, the resulting cut-off value of the circularity index(C-index) for SRC was 0.60(85.7% sensitivity, 85.7% specificity). | 18 | Pancreatic duct drainage using EUS-guided rendezvous technique for stenotic pancreaticojejunostomy显示文摘The patient was a 30-year-old female who had undergone excision of the extrahepatic bile duct and Rouxen-Y hepaticojejunostomy for congenital biliary dilatation at the age of 7.Thereafter,she suffered from recurrent acute pancreatitis due to pancreaticobiliary maljunction and received subtotal stomach-preserving pancreaticoduodenectomy.She developed a pancreatic fistula and an intra-abdominal abscess after the operation.These complications were improved by percutaneous abscess drainage and antibiotic therapy.How ever,upper abdominal discomfort and the elevation of serum pancreatic enzymes persisted due to stenosis from the pancreaticojejunostomy.Because we could not accomplish dilation of the stenosis by endoscopic retrograde cholangiopancreatography,we tried an endoscopic ultrasonography(EUS) guided rendezvous technique for pancreatic duct drainage.After transgastric puncture of the pancreatic duct using an EUS-fine needle aspiration needle,the guidewire was inserted into the pancreatic duct and finally reached to the jejunum through the stenotic anastomosis.We changed the echoendoscope to an oblique-viewing endoscope,then grasped the guidewire and withdrew it through the scope.The stenosis of the pancreaticojejunostomy was dilated up to 4 mm,and a pancreatic stent was put in place.Though the pancreatic stent was removed after three months,the patient remained symptomfree.Pancreatic duct drainage using an EUS-guided rendezvous technique was useful for the treatment of a stenotic pancreaticojejunostomy after pancreaticoduodenectomy. | Tetsuya Takikawa Atsushi Kanno Atsushi Masamune Shin Hamada Eriko Nakano Shin Miura Hiroyuki Ariga Jun Unno Kiyoshi Kume Kazuhiro Kikuta Morihisa Hirota Hiroshi Yoshida Yu Katayose Michiaki Unno Tooru Shimosegawa | 2013 | World Journal of Gastroenterology2013,19,31: | 3 |
