| 2 | Comparison of split-dosing vs non-split(morning) dosing regimen for assessment of quality of bowel preparation for colonoscopy显示文摘AIM: To compare(using the Ottawa Bowel Preparation Scale) the efficacy of split-dose vs morning administration of polyethylene glycol solution for colon cleansing in patients undergoing colonoscopy, and to assess the optimal preparation-to-colonoscopy interval.METHODS: Single-centre, prospective, randomized, investigator-blind stud in an academic tertiarycare centre. Two hundred patients requiring elective colonoscopy were assigned to receive one of the two preparation regimens(split vs morning) prior to colonoscopy. Main outcome measurements were bowel preparation quality and patient tolerability.RESULTS: Split-dose regimen resulted in better bowel preparation compared to morning regimen [Ottawascore mean 5.52(SD 1.23) vs 6.02(1.34); P = 0.017]. On subgroup analysis, for afternoon procedures, both the preparations were equally effective(P = 0.756). There was no difference in tolerability and compliance between the two regimens.CONCLUSION: Overall, previous evening- same morning split-dosing regimen results in better bowel cleansing for colonoscopy compared to morning preparation. | 4 | All ileo-cecal ulcers are not Crohn's:Changing perspectives of symptomatic ileocecal ulcers显示文摘AIM To investigated clinical,endoscopic and histopathological parameters of the patients presenting with ileocecal ulcers on colonoscopy.METHODS Consecutive symptomatic patients undergoing colonoscopy,and diagnosed to have ulcerations in the ileocecal(I/C) region,were enrolled.Biopsy was obtained and theirclinical presentation and outcome were recorded.RESULTS Out of 1632 colonoscopies,104 patients had ulcerations in the I/C region and were included in the study.Their median age was 44.5 years and 59% were males.The predominant presentation was lower GI bleed(55,53%),pain abdomen ± diarrhea(36,35%),fever(32,31%),and diarrhea alone(9,9%).On colonoscopy,terminal ileum was entered in 96(92%) cases.The distribution of ulcers was as follows:Ileum alone 40%(38/96),cecum alone 33%(32/96),and both ileum plus cecum 27%(26/96).The ulcers were multiple in 98% and in 34% there were additional ulcers elsewhere in colon.Based on clinical presentation and investigations,the etiology of ulcers was classified into infective causes(43%) and noninfective causes(57%).Fourteen patients(13%) were diagnosed to have Crohn's disease(CD).CONCLUSION Non-specific ileocecal ulcers are most common ulcers seen in ileo-cecal region.And if all infections are clubbed together then infection is the most common(> 40%) cause of ulcerations of the I/C region.Cecal involvement and fever are important clues to infective cause.On the contrary CD account for only 13% cases as a cause of ileo-cecalulcers.So all symptomatic patients with I/C ulcers on colonoscopy are not Crohn's. | Jay Toshniwal Romesh Chawlani Amit Thawrani Rajesh Sharma Anil Arora Hardik L Kotecha Mohan Goyal Vijendra Kirnake Pankaj Jain Pankaj Tyagi Naresh Bansal Munish Sachdeva Piyush Ranjan Mandhir Kumar Praveen Sharma Vikas Singla Rinkesh Bansal Vineet Shah Sunita Bhalla Ashish Kumar | 2017 | World Journal of Gastrointestinal Endoscopy2017，9，7： | 5 |
| 5 | Pricing and inventory decisions for non-instantaneous deteriorating items with price and promotional effort stochastic demand显示文摘This study investigates an inventory model for a non-instantaneous deteriorating item with partial backlogging wherein the demand is stochastic in nature and depends on price and promotional effort whereas the deterioration rate is time proportional.Under these settings,a mathematical model is developed with the objective to maximise the expected profit per unit time by determining the optimal price and the length of replenishment cycle.Some useful theoretical results are established to deduce the optimal replenishment schedule.An effective algorithmic procedure is developed to find the optimal solutions to the proposed model.The applicability of the proposed model is illustrated by numerical example.Sensitivity analysis of the optimal solution with respect to key parameters has been carried out and the implications are discussed.The study indicates that though promotional effort stimulates the market demand,it is beneficial in an economic sense when it applies in a conservative manner. | Hardik N.Soni Dipali N.Suthar | 2019 | Journal of Control and Decision2019，6，3： | 2 |
| 6 | Is myopia a protective factor against central serous chorioretinopathy?显示文摘AIM: To evaluate if any association exists between central serous chorioretinopathy(CSCR) and the refractive status of the eye.· METHODS: This retrospective, institutional, case control study included 499 patients, wherein 262 patients diagnosed as acute CSCR, were compared with an age and gender matched control group of 237 patients. All patients were evaluated with a detailed systemic and ocular history, objective and subjective refractions for both eyes and complete ocular examination by a retina specialist, at all visits. Optical coherence tomography confirmed the diagnosis of CSCR.·RESULTS: The mean age was found to be 40±7y in the study group(Group 1) compared to 38±10y in the control group(Group 2). Most common refractive status in the study group, was emmetropia seen in 191 patients(72.9%), followed by hypermetropia seen in 47 patients(17.9%) and astigmatism seen in 21 patients(8.0%). Only3 subjects(1.1%) had myopia, which was less than or equal to 1.0 D, compared to 70 subjects(29.5%) in the control group, suggesting a statistically significant lower incidence of CSCR among the myopic patients(P <0.0001). With respect to the systemic factors, 26(9.9%)patients were using systemic steroids in the study group(Group 1) compared to none in the control group(Group 2)suggesting a statistically significant association of CSCR with systemic steroid use(P <0.05).
