3Laparoscopic liver resection:Experience based guidelines显示文摘Laparoscopic liver resection(LLR) has been progressively developed along the past two decades. Despite initial skepticism, improved operative results made laparoscopic approach incorporated to surgical practice and operations increased in frequency and complexity. Evidence supporting LLR comes from case-series, comparative studies and meta-analysis. Despite lack of level 1 evidence, the body of literature is stronger and existing data confirms the safety, feasibility and benefits of laparoscopic approach when compared to open resection. Indications for LLR do not differ from those for open surgery. They include benign and malignant(both primary and metastatic) tumors and living donor liver harvesting. Currently, resection of lesions located on anterolateral segments and left lateral sectionectomy are performed systematically by laparoscopy in hepatobiliary specialized centers. Resection of lesions located on posterosuperior segments(1, 4a, 7, 8) and major liver resections were shown to be feasible but remain technically demanding procedures, which should be reserved to experienced surgeons. Hand-assisted and laparoscopy-assisted procedures appeared to increase the indications of minimally invasive liver surgery and are useful strategies applied to difficult and major resections. LLR proved to be safe for malignant lesions and offers some short-term advantages over open resection. Oncological results including resection margin status and long-term survival were not inferior to open resection. At present, surgical community expects high quality studies to base the already perceived better outcomes achieved by laparoscopy in major centers' practice. Continuous surgical training, as well as new technologies should augment the application of lap-aroscopic liver surgery. 4Radiofrequency ablation for treatment of hypersplenism: A feasible therapeutic option显示文摘We present a case of a patient with hypersplenism secondary to portal hypertension due to hepato-splenic schistosomiasis, which was accompanied by severe and refractory thrombocytopenia. We performed spleen ablation and measured the total spleen and ablated volumes with contrast-enhanced computed tomography and volumetry. No major complications occurred, thrombocytopenia was resolved, and platelet levels remained stable, which allowed for early treatment of the patient's underlying disease. Previous work has shown that splenic radiofrequency ablation is an attractive alternative treatment for hypersplenism induced by liver cirrhosis. We aimed to contribute to the currently sparse literature evaluating the role of radiofrequency ablation(RFA) in the management of hypersplenism. 5WJSC 6^(th) Anniversary Special Issues(2):Mesenchymal stem cells Progress of mesenchymal stem cell therapy for neural and retinal diseases显示文摘Complex circuitry and limited regenerative power make central nervous system(CNS)disorders the most challenging and difficult for functional repair.With elusive disease mechanisms,traditional surgical and medical interventions merely slow down the progression of the neurodegenerative diseases.However,the number of neurons still diminishes in many patients.Recently,stem cell therapy has been proposed as a viable option.Mesenchymal stem cells(MSCs),a widely-studied human adult stem cell population,have been discovered for more than 20 years.MSCs have been found all over the body and can be conveniently obtained from different accessible tissues:bone marrow,blood,and adipose and dental tissue.MSCs have high proliferative and differentiation abilities,providing an inexhaustible source of neurons and glia for cell replacement therapy.Moreover,MSCs also show neuroprotective effects without any genetic modification or reprogramming.In addition,the extraordinary immunomodulatory properties of MSCs enable autologous and heterologous transplantation.These qualities heighten the clinical applicability of MSCs when dealing with the pathologies of CNS disorders.Here,we summarize the latest progress of MSC experimental research as well as human clinical trials for neural and retinal diseases.This review article will focus on multiple sclerosis,spinal cord injury,autism,glaucoma,retinitis pigmentosa and age-related macular degeneration.Tsz Kin Ng Veronica R Fortino Daniel Pelaez Herman S Cheung 2014World Journal of Stem Cells2014,6,2:13

7Management of borderline and locally advanced pancreatic cancer:Where do we stand?显示文摘Many patients with pancreas cancer present with locally advanced pancreatic cancer(LAPC).The principle tools used for diagnosis and staging of LAPC include endoscopic ultrasound,axial imaging with computed tomography and magnetic resonance imaging,and diagnostic laparoscopy.The definition of resectability has historically been vague,as there is considerable debate and controversy as to the definition of LAPC.For the patient with LAPC,there is some level of involvement of the surrounding vascular structures,which include the superior mesenteric artery,celiac axis,hepatic artery,superior mesenteric vein,or portal vein.When feasible,most surgeons would recommend possible surgical resection for patients with borderline LAPC,with the goal of an R0 resection.For initially unresectable LAPC,neoadjuvant should be strongly considered.Specifically,these patients should be offered neoadjuvant therapy,and the tumor should be assessed for possible response and eventual resection.The efficacy of neoadjuvant therapy with this approach as a bridge to potential curative resection is broad,ranging from 3%-79%.The different modalities of neoadjuvant therapy include sin-gle or multi-agent chemotherapy combined with radiation,chemotherapy alone,and chemotherapy followed by chemotherapy with radiation.This review focuses on patients with LAPC and addresses recent advances and controversies in the field.Jin He Andrew J Page Matthew Weiss Christopher L Wolfgang Joseph M Herman Timothy M Pawlik 2014World Journal of Gastroenterology2014,20,9:11

11Species richness loss after nutrient addition as affected by N:C ratios and phytohormone GA_(3)contents in an alpine meadow community显示文摘Aims From the light-competition hypothesis,competition for light is asymmetric and the observed increases in plant-size variability with increasingly denser canopies are primarily due to competition for light.Greater plant height provides pre-emptive access to light and produces increased height differences among species.The question is what produces these differences in plant height or height growth response among species in response to fertilization.Methods In 2009,a field experiment of N,P and N+P enrichments at three levels each was initiated in an alpine meadow on the northeast Qinghai-Tibet Plateau.Effects of fertilization on species richness,aboveground net primary production(ANPP),relative light intensity and plant height of different plant functional groups were determined.Festuca ovina(grass),Kobresia humilis(sedge),Oxytropis ochrocephala(legume),Taraxacum lugubre(rosette forb)and Geranium pylzowianum(upright forb)were selected as exemplars of each of the indicated functional groups.The N:C ratios in aboveground biomass,gibberellic acid(GA_(3))concentrations in leaves,plant heights and height relative growth rate(RGR)of these exemplar species were analyzed in detail.Important Findings Species richness of grasses significantly increased with increasing N+P levels.Species richness of legumes and upright forbs decreased after N and N+P additions.P addition had no significant effect on species richness.The effects of N+P addition on species richness and ANPP were consistently stronger than those of the single N or P fertilization.Reductions in species richness caused by nutrient addition paralleled the increases in ANPP and decreases in light intensity under the canopies,indicating indirect effect of nutrient addition on species richness via ANPP-induced light competition.The exemplar species that responded most positively to fertilization in height and RGR also displayed stronger increases in their GA_(3) content and N:C ratios.GA_(3) concentrations and N:C ratios were positively correlated with height RGR when the data were pooled for all species.The tallest and the fastest-growing grass,F.ovina,had the largest increase in N:C ratios and the highest leaf GA_(3) concentrations after nutrient addition.These results indicated that differential responses of GA_(3) concentrations and N:C ratios to fertilization were related to the inequality in plant heights among species.Xiaomei Sun Kailiang Yu Herman HShugart Gang Wang 2016Journal of Plant Ecology2016,9,2:10
12Intrahepatic natural killer T cell populations are increased in human hepatic steatosis显示文摘AIM: To determine if natural killer T cell (NKT) populations are affected in nonalcoholic fatty liver disease (NAFLD). METHODS: Patients undergoing bariatric surgery underwent liver biopsy and blood sampling during surgery. The biopsy was assessed for steatosis and immunocyte infiltration. Intrahepatic lymphocytes (IHLs) were isolated from the remainder of the liver biopsy, and peripheral blood mononuclear cells (PBMCs) were isolated from the blood. Expression of surface proteins on both IHLs and PBMCs were quantified using flow cytometry. RESULTS: Twenty-seven subjects participated in thisstudy. Subjects with moderate or severe steatosis had a higher percentage of intrahepatic CD3+/CD56+ NKT cells (38.6%) than did patients with mild steatosis (24.1%, P = 0.05) or those without steatosis (21.5%, P = 0.03). Patients with moderate to severe steatosis also had a higher percentage of NKT cells in the blood (12.3%) as compared to patients with mild steatosis (2.5% P = 0.02) and those without steatosis (5.1%, P = 0.05). 13Pre-and postoperative systemic hemodynamic evaluation in patients subjected to esophagogastric devascularization plus splenectomy and distal splenorenal shunt:A comparative study in schistomomal portal hypertension显示文摘AIM: To investigate the systemic hemodynamic effects of two surgical procedures largely employed for treatment of schistosomal portal hypertension. METHODS: Thirty-six patients undergoing elective surgical treatment of portal hypertension due to hepatosplenic mansonic schistosomiasis were prospectively evaluated. All patients were subjected to preoperative pulmonary artery catheterization; 17 were submitted to esophagogastric devascularization and splenectomy (EGDS) and 19 to distal splenorenal shunt (DSRS). The systemic hemodynamic assessment was repeated 4 d after the surgical procedure. RESULTS: Preoperative evaluation revealed (mean ± SD) an increased cardiac index (4.78 ± 1.13 L/min per m2),associated with a reduction in systemic vascular resistance index (1457 ± 380.7 dynes.s/cm5.m2). The mean pulmonary artery pressure (18 ± 5.1 mmHg) as well as the right atrial pressure (7.9 ± 2.5 mmHg) were increased,while the pulmonary vascular resistance index (133 ± 62 dynes.s/cm5.m2) was decreased. Four days after EGDS,a significant reduction in cardiac index (3.80 ± 0.4 L/min per m2,P < 0.001) and increase in systemic vascular resistance index (1901.4 ± 330.2 dynes.s/cm5. m2,P < 0.001) toward normal levels were observed. There was also a significant reduction in pulmonary artery pressure (12.65 ± 4.7 mmHg,P < 0.001) and no significant changes in the pulmonary vascular resistance index (141.6 ± 102.9 dynes.s/cm5.m2). Four days after DSRS,a non-significant increase in cardiac index (5.2 ± 0.76 L/min per m2) and systemic vascular resistance index (1389 ± 311 dynes.s/cm5.m2) was observed. There was also a non-significant increase in pulmonary artery pressure (19.84 ± 5.2 mmHg),right cardiac work index (1.38 ± 0.4 kg.m/m2) and right ventricular systolic work index (16.3 ± 6.3 g.m/m2),without significant changes in the pulmonary vascular resistance index (139.7 ± 67.8 dynes.s/cm5.m2). CONCLUSION: The hyperdynamic circulatory state observed in mansonic schistosomiasis was corrected by EGDS,but was maintained in patients who underwent DSRS. Similarly,the elevated mean pulmonary artery pressure was corrected after EGDS and maintained after DSRS. 14Synthesis of Isotopically-Labeled Graphite Films by Cold-Wall Chemical Vapor Deposition and Electronic Properties of Graphene Obtained from Such Films显示文摘We report the synthesis of isotopically-labeled graphite films on nickel substrates by using cold-wall chemical vapor deposition(CVD).During the synthesis,carbon from^(12)C-and^(13)C-methane was deposited on,and dissolved in,a nickel foil at high temperature,and a uniform graphite film was segregated from the nickel surface by cooling the sample to room temperature.Scanning and transmission electron microscopy,micro-Raman spectroscopy,and X-ray diffraction prove the presence of a graphite film.Monolayer graphene films obtained from such isotopically-labeled graphite films by mechanical methods have electron mobility values greater than 5000 cm^(2)·V^(-1)·s^(-1)at low temperatures.Furthermore,such films exhibit the half-integer quantum Hall effect over a wide temperature range from 2 K to 200 K,implying that the graphite grown by this cold-wall CVD approach has a quality as high as highly oriented pyrolytic graphite(HOPG).The results from transport measurements indicate that^(13)C-labeling does not significantly affect the electrical transport properties of graphene.Weiwei Cai Richard D.Piner Yanwu Zhu Xuesong Li Zhenbing Tan Herman Carlo Floresca Changli Yang Li Lu M.J.Kim Rodney S.Ruoff 2009Nano Research2009,2,11:7

16男性不育和辅助生殖技术显示文摘多年来,男性不育的治疗和管理一直是以经验为基础的,而不是证据。尽管不是循证的,但是人类辅助生殖技术还是一直被广泛地运用于临床实践中。如果没有其它具体明确的治疗方法或者治疗失败了,那么辅助生殖技术就成为普遍的辅助治疗手段。根据一些有限的证据,一对女性生育力正常的夫妇,如果男方的前进性精子活力至少能够恢复到1×106的话,那么宫内人工授精就被视作最佳治疗方案。然而,如果3到6个周期的宫内人工授精后女方仍未怀孕,那么应该采取优化的体外受精技术。如果通过精液处理抑或从睾丸和附睾中手术获取精子后,前进性精子活力仍不足0.5×106的话,应该采用卵胞浆内单精子注射技术。尽管已仔细地研究了当前的辅助生殖技术疗效,但是在采用卵胞浆内单精子注射技术以来,大型的“宏观问题”还从未被研究过。而且,在接受体外受精技术或者卯胞浆内单精子注射技术之前,待受精者应该通过严格的筛选,而且他们应该被清楚地告知我们在男性不育的遗传性以及辅助生殖技术的安全性方面的知识的局限所在。Herman Tournaye 2012Asian Journal of Andrology2012,14,1:7
17Enamel microabrasion: An overview of clinical and scientific considerations显示文摘Superficial stains and irregularities of the enamelare generally what prompt patients to seek dental intervention to improve their smile. These stains or defects may be due to hypoplasia, amelogenesis imperfecta, mineralized white spots, or fluorosis, for which enamel microabrasion is primarily indicated. Enamel microabrasion involves the use of acidic and abrasive agents, such as with 37% phosphoric acid and pumice or 6% hydrochloric acid and silica, applied to the altered enamel surface with mechanical pressure from a rubber cup coupled to a rotatory mandrel of a lowrotation micromotor. If necessary, this treatment can be safely combined with bleaching for better esthetic results. Recent studies show that microabrasion is a conservative treatment when the enamel wear is minimal and clinically imperceptible. The most important factor contributing to the success of enamel microabrasion is the depth of the defect, as deeper, opaque stains, such as those resulting from hypoplasia, cannot be resolved with microabrasion, and require a restorative approach. Surface enamel alterations that result from microabrasion, such as roughness and microhardness, are easily restored by saliva. Clinical studies support the efficacy and longevity of this safe and minimally invasive treatment. 18Pre-recorded instructional audio vs. dispatchers' conversational assistance in telephone cardiopulmonary resuscitation: A randomized controlled simulation study显示文摘BACKGROUND:To assess the effectiveness of the telephone chest-compression-only cardiopulmonary resuscitation(CPR)guided by a pre-recorded instructional audio when compared with dispatcher-assisted resuscitation.METHODS:It was a prospective,blind,randomised controlled study involving 109 medical students without previous CPR training.In a standardized mannequin scenario,after the step of dispatcher-assisted cardiac arrest recognition,the participants performed compression-only resuscitation guided over the telephone by either:(1)the pre-recorded instructional audio(n=57);or(2)verbal dispatcher assistance(n=52).The simulation video records were reviewed to assess the CPR performance using a 13-item checklist.The interval from call reception to the first compression,total number and rate of compressions,total number and duration of pauses after the first compression were also recorded.RESULTS:There were no significant differences between the recording-assisted and dispatcher-assisted groups based on the overall performance score(5.6±2.2 vs.5.1±1.9,P>0.05)or individual criteria of the CPR performance checklist.The recording-assisted group demonstrated provided(170.2±48.0 vs.156.2±60.7).CONCLUSION:When provided by untrained persons in the simulated settings,the compression-only resuscitation guided by the pre-recorded instructional audio is no less efficient than dispatcher-assisted CPR.Future studies are warranted to further assess feasibility of using instructional audio aid as a potential alternative to dispatcher assistance.Alexei Birkun Maksim Glotov Herman Franklin Ndjamen Esther Alaiyex Esther Alaiye Temidara Adeleke Sergey Samarin 2018World Journal of Emergency Medicine2018,9,3:6
19三维超声心动图测定左心耳容量及其收缩功能的初步研究显示文摘目的 观察三维超声心动图测定左心耳容量的可行性。方法 对 10名患者行多平面经食管超声心动图 (TEE)检查。应用三维超声心动图采取圆盘总和方法 (disksummation)测定左心耳舒张末期容量 (EDV)和左心耳收缩末期容量 (ESV)。用二维超声心动图测定左心耳舒张末期面积 (EDA)和左心耳收缩末期面积(ESA)。上述左心耳容量和面积的测定均在二个独立的观察者之间以及同一观察者两次观察间重复进行。由三维超声心动图容量测定结果计算出左心耳的射血分数 (EFv) ,由二维超声心动图的面积测定结果计算出左心耳射血分数 (EFa)。结果 三维超声心动图测定左心耳EDV为 ( 19.5± 10 .9)ml,ESV为 ( 13 .5± 9.5 )ml ,EFv为0 .3 5± 0 .14。二维超声心动图测定左心耳EDA为 ( 8.7± 2 .2 )cm2 ,ESV为 ( 5 .9± 3 .5 )cm2 ,EFa为 0 .3 5± 0 .2 2。以上测量内容在观察者之间及观察者内的重复性检验中均未见显著性差异 ,EFv与EFa之间亦无显著性差异。但无论在观察者间或观察者内的重复性检验中EFa的范围和两测量数值之差均大于EFv的相应值。结论 三维超声心动图测量左心耳容量和评价其收缩功能是可行的 ,这种测定方法可能有助于改善二维超声心动图评价左心耳功能的准确性。李越 Otto Kamp Herman FJ Mannaerts Gabriel Valocik Cees A Visser 2000中华超声影像学杂志2000,9,5:6
20Taking silicon photonics modulators to a higher performance level: state-of-the-art and a review of new technologies显示文摘Optical links are moving to higher and higher transmission speeds while shrinking to shorter and shorter ranges where optical links are envisaged even at the chip scale.The scaling in data speed and span of the optical links demands modulators to be concurrently performant and cost-effective.Silicon photonics(SiPh),a photonic integrated circuit technology that leverages the fabrication sophistication of complementary metal-oxide-semiconductor technology,is well-positioned to deliver the performance,price,and manufacturing volume for the high-speed modulators of future optical communication links.SiPh has relied on the plasma dispersion effect,either in injection,depletion,or accumulation mode,to demonstrate efficient high-speed modulators.The high-speed plasma dispersion silicon modulators have been commercially deployed and have demonstrated excellent performance.Recent years have seen a paradigm shift where the integration of various electro-refractive and electro-absorptive materials has opened up additional routes toward performant SiPh modulators.These modulators are in the early years of their development.They promise to extend the performance beyond the limits set by the physical properties of silicon.The focus of our study is to provide a comprehensive review of contemporary(i.e.,plasma dispersion modulators)and new modulator implementations that involve the integration of novel materials with SiPh.Abdul Rahim Artur Hermans Benjamin Wohlfeil Despoina Petousi Bart Kuyken Dries Van Thourhout Roel Baets 2021Advanced Photonics2021,3,2:6
