Effects of Saccharomycesboulardiion fecal short-chain fatty acids and microflora in patients on long-term total enteral nutrition显示文摘AIM: To assess the effects of Sb on fecal flora and shortchain fatty acids (SCFA) in patients on long-term TEN.METHODS: Ten patients (3 females, 7 males, 59±5.5 years),on TEN for a median of 13 mo (1-125), and 15 healthy volunteers (4 females, 11 males, 32±2.0 years) received Sb (0.5 g bid PO) for 6 d. Two stool samples were taken before, on the last 2 d and 9-10 d after treatment, for SCFA measurement and for culture and bacterial identification.Values (mean±SE) were compared using sign tests and ANOVA.RESULTS: Fecal butyrate levels were lower in patients(10.1±2.9 mmol/kg) than in controls (19.2±3.9, P= 0.02).Treatment with Sb increased total fecal SCFA levels in patients (150.2±27.2 vs 107.5±18.2 mmol/kg, P = 0.02)but not in controls (129.0±28.6 vs 113.0±15.2 mmol/kg,NS). At the end of treatment with Sb, patients had higher fecal butyrate (16.0±4.4 vs 10.1 [2.9] mmol/kg, P = 0.004).Total SCFAs remained high 9 d after treatment was discontinued. Before the treatment, the anaerobe to aerobe ratio was lower in patients compared to controls (2.4±2.3 vs69.8±1.8, P = 0.003). There were no significant changes in the fecal flora of TEN patients.CONCLUSION: Sb-induced increase of fecal SCFA concentrations (especially butyrate) may explain the preventive effects of this yeast on TEN-induced diarrhea. | Stéphane M Schneider Fernand Girard-Pipau Jér(o|^)me Filippi Xavier Hébuterne Dominique Moyse Gustavo Calle Hinojosa Anne Pompei Patrick Rampal | 2005 | World Journal of Gastroenterology2005,11,39: | 26
Understanding plant drought resistance in a Mediterranean coastal sand dune ecosystem:differences between native and exotic invasive species显示文摘Aims Mediterranean coastal dunes are habitats of great conservation interest, with a distinctive and rich flora. In the last century,Acacia spp.,native from Australia, have been introduced in Portugal, with the objective of stabilizing sand dunes, and since have become dominant in numerous sand dune habitats. This invasion process led to the reduction of native plant species richness, changed soil characteristics and modified habitat’s microclimatic char-acteristics.The aim of this research was to typify and compare,in Mediterranean sand dune ecosystems, the ecophysiological responses to drought of Helichrysum italicum and Corema album, two native species, and Acacia longifolia,an exotic invasive spe-cies.We addressed the following specific objectives:(i)to com-pare water relations and water use efficiencies,(ii)to evaluate water stress,(iii)to assess water use strategies and water sources used by plants and(iv)to evaluate the morphological adaptations at leaf and phyllode level. Methods In order to obtain an integrative view of ecophysiological patterns, water relations and performance measuring methods have been applied:predawn(ψPD)and midday(ψMD)water potential,chloro-phyll a fluorescence, oxygen isotopic composition of xylem, rain and groundwater(δ18O)and leaf carbon isotopic discrimination(Δ13C).The leaf characteristics of the three species,as well as the histochemistry of non-glandular trichome cell walls, were also studied to identify morpho-traits related to drought resistance.Important Findings The results support our initial hypothesis:although A.longifolia clearly possesses a degree of resistance to water stress, such ability is provided by a different water strategy, when compared to native species.Natives relied on morphological adaptations to restrict water loss, whereas the invasive species adjusted the water uptake as a way to balance their limited ability of restricting water loss.We corroborate that woody native species(i)have a conservative water-saving strategy and minor seasonal variations relative to invasive species,(ii)use enriched water sources during drought periods, indicating different water sources and root systems com-paring with invasive species and(iii)present drought leaf morpho-functional adaptations related with limiting water loss.Comparing the physiological performance of invasive and native species can offer causal explanations for the relative success of alien plant invasions on sand dunes ecosystems. | Cristina Antunes Ana Júlia Pereira Patrícia Fernandes Margarida Ramos Lia Ascensão Otília Correia Cristina Máguas | 2018 | Journal of Plant Ecology2018,11,1: | 4
Rice Drying,Storage and Processing:Effects of Post-Harvest Operations on Grain Quality显示文摘Various post-harvest processes of rice are commonly employed, especially during the off-season,to ensure its consumption feasibility, which often affect the grain quality.Different forms of drying, storage and processing of rice are evaluated to identify their effects on grain quality.Microwave drying has emerged as an alternative to the widely-used intermittent-drying and fixed-bed-dryer methods of drying paddy rice.Control of drying-air temperatures(between 40℃ and 60℃)according to the rice variety can improve quality, especially for exotic varieties.Keeping stored grain in hygroscopic balance, with water content between 11%to 15%,at temperatures between 16℃ and 20℃ and with intergranular relative humidity near 60%,allows 12 months of storage in a controlled environment without significant deterioration.Other innovations, notably the application of artificial refrigeration to grain stored in bulk in vertical cylindrical silos and the use of impermeable packaging for storage, ensure the conservation of grain mass. The different stages and equipments used to obtain polished, brown and parboiled rice result in significant changes in the nutritional value of rice because of the removal of the outermost layers of the grains.Polishing reduces the nutritional value and physical homogeneity of rice.Brown rice retains more bioactive compounds and nutrients because it does not lose the outer layer of the grains in the polishing processes.Parboiled rice, although less nutritious than brown rice, has better grain integrity and milling yield and less loss of nutrients than white rice. | Amanda MÜLLER Marcela Trojahn NUNES Vanessa MALDANER Paulo Carteri CORADI Rosana Santos de MORAES Samuel MARTENS Andressa Fernandes LEAL Vladison Fogliato PEREIRA Cristielle König MARIN | 2022 | Rice science2022,29,1: | 3
Giant hepatic extra-gastrointestinal stromal tumor treated with cytoreductive surgery and adjuvant systemic therapy:A case report and review of literature显示文摘BACKGROUND Primary extra-gastrointestinal stromal tumors(E-GIST)of the liver are rare. The clinical presentation may range from asymptomatic to bleeding or manifestations of mass effect.Oncologic surgery followed by adjuvant therapy with imatinib is the standard of care. However, under specific circumstances,a cytoreductive approach may represent a therapeutic option. We describe herein the case of an 84-year-old woman who presented with a tender, protruding epigastric mass.Abdominal computed tomography scan revealed a large, heterogeneous mass located across segments III,IV,V,and VIII of the liver. The initial approach was transarterial embolization of the tumor, which elicited no appreciable response.Considering the large size and central location of the tumor and the advanced age of the patient,non-anatomic complete resection was indicated.Due to substantial intraoperative bleeding and hemodynamic instability, only a near-complete resection could be achieved.Histopathology and immunohistochemical staining confirmed the diagnosis of primary E-GIST of the liver.Considering the risk/benefit ratio for therapeutic options, debulking surgery may represent a strategy to control pain and prolong survival.CASE SUMMARY Here,we present a case report of a patient diagnosed with E-GIST primary of the liver, which was indicated a cytoreductive surgery and adjuvant therapy with imatinib.CONCLUSION E-GIST primary of the liver is a rare conditional, the treatment is with systemic therapy and total resection surgery. However,a cytoreductive surgery will be necessary when a complete resection is no possible. | Michel Ribeiro Fernandes Caroline Lorenzoni Almeida Ghezzi Tomaz J M Grezzana-Filho Flávia Heinz Feier Ian Leipnitz Aljamir Duarte Chedid Carlos Thadeu Schmidt Cerski Marcio Fernandes Chedid Cléber RositoPinto Kruel | 2021 | World Journal of Gastrointestinal Surgery2021,13,3: | 3
Obstructive Sleep Apnea Hypopnea Syndrome and Pulmonary Embolism: Warfarin Versus Positive Airway Pressure as Determinant显示文摘To the Editor:Xie et al.analyzed warfarin doses in pulmonary embolism (PE) and its adverse effects in patients with obstructive sleep apnea hypopnea syndrome (OSAHS).The key findings were that OSAHS patients required a significantly higher dose of warfarin than their non-OSAHS counterparts (4.73 mg vs.3.61 mg,P < 0.001) and that PE recurrence was higher in OSAHS than in non-OSAHS groups after withdrawal of warfarin (21.43% vs.6.78%,P =0.047).They conclude that OSAHS patients may present with hypercoagulation and relatively high risk of recurrence of PE after cessation of 6-month warfarin treatment. We have read this with great interest.[1] | Filipa Ramalho Fernandes Dulce Apolinario Antonio M Esquinas | 2016 | Chinese Medical Journal2016,,2: | 3
Quality of life, work productivity impairment and healthcare resources in inflammatory bowel diseases in Brazil显示文摘BACKGROUND Inflammatory bowel diseases(IBD)have been associated with a low quality of life(QoL)and a negative impact on work productivity compared to the general population.Information about disease control,patient-reported outcomes(PROs),treatment patterns and use of healthcare resources is relevant to optimizing IBD management.AIM To describe QoL and work productivity and activity impairment(WPAI),treatment patterns and use of healthcare resources among IBD patients in Brazil.METHODS A multicenter cross-sectional study included adult outpatients who were previously diagnosed with moderate to severe Crohn’s disease(CD)or ulcerative colitis(UC).At enrolment, active CD and UC were defined as having a Harvey Bradshaw Index≥8 or a CD Activity Index≥220 or calprotectin>200μg/g or previous colonoscopy results suggestive of inadequate control(per investigator criteria)and a 9-point partial Mayo score≥5,respectively.The PRO assessment included the QoL questionnaires SF-36 and EQ-5D-5L,the Inflammatory Bowel Disease Questionnaire(IBDQ),and the WPAI questionnaire.Information about healthcare resources and treatment during the previous 3 years was collected from medical records.Chi-square,Fisher’s exact and Student’s t-/Mann-Whitney U tests were used to compare PROs,treatment patterns and the use of healthcare resources by disease activity(α=0.05).RESULTS Of the 407 patients in this study(CD/UC:64.9%/35.1%,mean age 42.9/45.9 years,54.2%/56.6%female,38.3%/37.1%employed),44.7%/25.2%presented moderate-to-severe CD/UC activity, respectively,at baseline.Expressed in median values for CD/UC,respectively,the SF-36 physical component was 46.6/44.7 and the mental component was 45.2/44.2,the EQ-visual analog scale score was 80.0/70.0,and the IBDQ overall score was 164.0/165.0.Moderate to severe activity, female gender, being unemployed,a lower educational level and lower income were associated with lower QoL(P<0.05).Median work productivity impairment was 20%and 5%for CD and UC patients, respectively, and activity impairment was 30%,the latter being higher among patients with moderate to severe disease activity compared to patients with mild or no disease activity(75.0%vs 10.0%,P<0.001).For CD/UC patients,respectively,25.4%/2.8%had at least one surgery,38.3%/19.6%were hospitalized, and 70.7%/77.6%changed IBD treatment at least once during the last 3 years. The most common treatments at baseline were biologics(75.3%)and immunosuppressants(70.9%)for CD patients and 5-ASA compounds(77.5%)for UC patients.CONCLUSION Moderate to severe IBD activity, especially among CD patients,is associated with a substantial impact on QoL,work productivity impairment and an increased number of IBD surgeries and hospitalizations in Brazil. | Rogerio Serafim Parra Julio MF Chebli Heda MBS Amarante Cristina Flores Jose ML Parente Odery Ramos Milene Fernandes Jose JR Rocha Marley R Feitosa Omar Feres Antonio S Scotton Rodrigo B Nones Murilo M Lima Cyrla Zaltman Carolina D Goncalves Isabella M Guimaraes Genoile O Santana Ligia Y Sassaki Rogerio S Hossne Mauro Bafutto Roberto LK Junior Mikaell AG Faria Sender J Miszputen Tarcia NF Gomes Wilson R Catapani Anderson A Faria Stella CS Souza Rosana F Caratin Juliana T Senra Maria LA Ferrari | 2019 | World Journal of Gastroenterology2019,25,38: | 2
CITED2 and the modulation of the hypoxic response in cancer显示文摘CITED2(CBP/p300-interacting transactivator with Glu/Asp-rich C-terminal domain,2)is a ubiquitously expressed protein exhibiting a high affinity for the CH1 domain of the transcriptional co-activators CBP/p300,for which it competes with hypoxia-inducible factors(HIFs).CITED2 is particularly efficient in the inhibition of HIF-1α-dependent transcription in different contexts, ranging from organ development and metabolic homeostasis to tissue regeneration and immunity, being also potentially involved in various other physiological processes. In addition,CITED2 plays an important role in inhibiting HIF in some diseases, including kidney and heart diseases and type 2-diabetes.In the particular case of cancer,CITED2 either functions by promoting or suppressing cancer development depending on the context and type of tumors. For instance,CITED2 overexpression promotes breast and prostate cancers,as well as acute myeloid leukemia, while its expression is downregulated to sustain colorectal cancer and hepatocellular carcinoma. In addition, the role of CITED2 in the maintenance of cancer stem cells reveals its potential as a target in non-small cell lung carcinoma and acute myeloid leukemia, for example. But besides the wide body of evidence linking both CITED2 and HIF signaling to carcinogenesis, little data is available regarding CITED2 role as a negative regulator of HIF-1αspecifically in cancer. Therefore, comprehensive studies exploring further the interactions of these two important mediators in cancer-specific models are sorely needed and this can potentially lead to the development of novel targeted therapies. | Mónica T Fernandes Sofia M Calado Leonardo Mendes-Silva JoséBraganca | 2020 | World Journal of Clinical Oncology2020,11,5: | 2
Streptococcus agalactiae:Identification methods, antimicrobial susceptibility, and resistance genes in pregnant women显示文摘BACKGROUND Group B Streptococcus(GBS)is a normal component of the gastrointestinal and genital microbiota in humans and can lead to important infections in newborns.AIM To compare GBS isolation and identification methods as well as to assess the antibiotic susceptibility and to identify resistance genes in GBS strains from pregnant women attended in healthcare services from the city of Vitória da Conquista,in Bahia State,Brazil.METHODS From January 2017 to February 2018,vaginorectal swabs were obtained from 186 participants and the samples were seeded onto chromogenic agar for GBS before and after inoculation in selective broth.Confirmatory identification using 3 CAMP and latex tests was performed in samples with GBS-suggestive colonies. Then, disk diffusion antibiograms were performed in GBS-positive samples, and the detection of the resistance genes ermB,ermTR,mefA,and linB in the clindamycin and/or erythromycin-resistant samples was carried out.RESULTS Thirty-two samples(17.2%)were GBS-positive.The culture in chromogenic agar after sample incubation in selective broth was the most sensitive method(96.9%)for GBS detection. All isolates were susceptible to penicillin, ampicillin, cefotaxime, and vancomycin.Clindamycin resistance was observed in 6 samples(18.8%),while 8 samples(25%)were erythromycin-resistant.All erythromycin and/or clindamycin-resistant GBS strains had negative D-tests.Two strains(25%)presented an M phenotype and 6 isolates(75%)presented a cMLSB phenotype. The ermB gene was identified in 4 samples(44.4%),the mefA gene was also found in 4 samples(44.4%),the ermTR gene was identified in 1 isolate(11.1%),and the linB gene was not found in any isolate.CONCLUSION This study evidenced that the screening for SGB can be performed by means of various methods, including chromogenic media, and that the chemoprophylaxis for pregnant women who cannot use penicillin must be susceptibility-guided. | Fabrícia Almeida Fernandes Santana Tais Viana Ledo de Oliveira Marcelo Barreto de Souza Filho Lucas Santana Coelho da Silva Breno Bittencourt de Brito Fabrício Freire de Melo Cláudio Lima Souza Lucas Miranda Marques Márcio Vasconcelos Oliveira | 2020 | World Journal of Clinical Cases2020,8,18: | 2

Energetic-protein supplementation in the last 60 days of gestation improves performance of beef cows grazing tropical pastures显示文摘Background: Nutrition is one of the most important factors that affect animal performance, and it therefore also impacts on financial results in beef systems. In this way, finding the best strategy for feeding supplements is of paramount importance. Aiming to evaluate the effect of supplement feeding strategies for beef cows in the last third of gestation, two experiments were conducted. In Experiment 1, 35 pregnant Nellore cows were assigned to a completely randomized design with four treatments: control, which received no supplement; supplementation for the last 30 d of gestation(30-d; 3.0 kg/d); supplementation for the last 60 d of gestation(60-d; 1.5 kg/d); or supplementation for the last 90 d of gestation(90-d; 1.0 kg/d). All supplemented treatments received the same total amount of supplement throughout the experiment: 90 kg(20% of crude protein). A second experiment(Experiment 2) was delineated to evaluate the effects of the amounts offered in Experiment 1 on intake and metabolism. Four multiparous pregnant Nellore cows were assigned to a 4 × 4 Latin square design, with periods of15 d each. Results: There was a linear effect of the number of days of supplementation on calving body weight(BW; P < 0.05)and a quadratic effect on BW change from parturition to d 31 post-calving(P < 0.05), with cows on the 60-d strategy losing less BW post-calving. No difference was found in offspring birth BW(P > 0.10). A significant linear effect on interval from parturition to conception(P < 0.05) was observed, with the highest calving to conception interval being observed in the 90-d strategy. The level of supplementation did not affect forage intake or neutral detergent fiber digestibility(P > 0.10). Nitrogen excreted through urine tended to increase linearly with the level of supplementation(P < 0.10).Conclusion: Providing 1.5 kg of supplement during the last 60 d of gestation improves cow performance after calving, reducing the magnitude of BW lost, and reduces the number of days from calving to re-conception in the following breeding season compared to the usually recommended period of supplementation of 90 d pre-partum. | Aline Gomes da Silva Mário Fonseca Paulino Edenio Detmann Henrique Jorge Fernandes Lincoln da Silva Amorim Román Enrique Maza Ortega Victor Valério de Carvalho Josilaine Aparecida da Costa Lima Felipe Henrique de Moura Mariana Benevides Monteiro Jéssika Almeida Bitencourt | 2018 | Journal of Animal Science and Biotechnology2018,9,1: | 2
