| 1 | Quality of life, work productivity impairment and healthcare resources in inflammatory bowel diseases in Brazil显示文摘BACKGROUND Inflammatory bowel diseases(IBD)have been associated with a low quality of life(QoL)and a negative impact on work productivity compared to the general population.Information about disease control,patient-reported outcomes(PROs),treatment patterns and use of healthcare resources is relevant to optimizing IBD management.AIM To describe QoL and work productivity and activity impairment(WPAI),treatment patterns and use of healthcare resources among IBD patients in Brazil.METHODS A multicenter cross-sectional study included adult outpatients who were previously diagnosed with moderate to severe Crohn’s disease(CD)or ulcerative colitis(UC).At enrolment,active CD and UC were defined as having a Harvey Bradshaw Index≥8 or a CD Activity Index≥220 or calprotectin>200μg/g or previous colonoscopy results suggestive of inadequate control(per investigator criteria)and a 9-point partial Mayo score≥5,respectively.The PRO assessment included the QoL questionnaires SF-36 and EQ-5D-5L,the Inflammatory Bowel Disease Questionnaire(IBDQ),and the WPAI questionnaire.Information about healthcare resources and treatment during the previous 3 years was collected from medical records.Chi-square,Fisher’s exact and Student’s t-/Mann-Whitney U tests were used to compare PROs,treatment patterns and the use of healthcare resources by disease activity(α=0.05).RESULTS Of the 407 patients in this study(CD/UC:64.9%/35.1%,mean age 42.9/45.9 years,54.2%/56.6%female,38.3%/37.1%employed),44.7%/25.2%presented moderate-to-severe CD/UC activity,respectively,at baseline.Expressed in median values for CD/UC,respectively,the SF-36 physical component was 46.6/44.7 and the mental component was 45.2/44.2,the EQ-visual analog scale score was 80.0/70.0,and the IBDQ overall score was 164.0/165.0.Moderate to severe activity,female gender,being unemployed,a lower educational level and lower income were associated with lower QoL(P<0.05).Median work productivity impairment was 20%and 5%for CD and UC patients,respectively,and activity impairment was 30%,the latter being higher among patients with moderate to severe disease activity compared to patients with mild or no disease activity(75.0%vs 10.0%,P<0.001).For CD/UC patients,respectively,25.4%/2.8%had at least one surgery,38.3%/19.6%were hospitalized,and 70.7%/77.6%changed IBD treatment at least once during the last 3 years.The most common treatments at baseline were biologics(75.3%)and immunosuppressants(70.9%)for CD patients and 5-ASA compounds(77.5%)for UC patients.CONCLUSION Moderate to severe IBD activity,especially among CD patients,is associated with a substantial impact on QoL,work productivity impairment and an increased number of IBD surgeries and hospitalizations in Brazil. | Rogerio Serafim Parra Julio MF Chebli Heda MBS Amarante Cristina Flores Jose ML Parente Odery Ramos Milene Fernandes Jose JR Rocha Marley R Feitosa Omar Feres Antonio S Scotton Rodrigo B Nones Murilo M Lima Cyrla Zaltman Carolina D Goncalves Isabella M Guimaraes Genoile O Santana Ligia Y Sassaki Rogerio S Hossne Mauro Bafutto Roberto LK Junior Mikaell AG Faria Sender J Miszputen Tarcia NF Gomes Wilson R Catapani Anderson A Faria Stella CS Souza Rosana F Caratin Juliana T Senra Maria LA Ferrari | 2019 | World Journal of Gastroenterology2019,25,38: | 2 |

| 3 | Leukocyte-technetium-99m uptake in Crohn's disease:Does it show subclinical disease?显示文摘AIM:To evaluate inflammatory activity in patients with Crohn's disease (CD) using technetium-99m-hexamethylpropyleneamine oxime (99mTc-HMPAO) granulocyte scintigraphy.METHODS: Twenty patients (7 male and 13 female) with CD and five healthy volunteers were selected for 99mTc-HMPAO granulocyte scintigraphy. The Crohn's Disease Activity Index (CDAI), blood tests and C-reactive protein (CRP) of each patient were performed 7 d before the scintigraphic images. The leukocytes were labeled according to the International Society of Radiolabeled Blood Elements (ISORBE) consensus protocol and the scintigraphic images, including single photon emission computed tomography, were obtained 30 min and 2 h after injection of the radiolabeled leukocytes.RESULTS:The labeling yield of the leukocytes with the lipophilic complex 99mTc-HMPAO was 55.0%±10%. Six of the 20 patients (30%) presented congruent results for the three parameters investigated (CDAI, Scintigraphic Index and CRP). On the other hand, 14 patients (70%) did not show congruent results. | 4 | Contributions on Selective Waste Collection and Recycling Cooperatives in Municipalities in the State of São Paulo,Brazil显示文摘The collection and proper disposal of MSW(Municipal Solid Waste)is one of the biggest challenges faced by Brazilian public managers.Dry recyclable waste represents about 33.6%of the gravimetric composition of MSW,with a tendency to increase in the next few years,due to the increasing consumption of processed products,with short life cycles and excess packaging.The inadequate management of these residues results in a series of social,environmental and economic problems.In recent years,there has been a great advance in Brazilian laws,assigning responsibilities and regulating waste management in the country,optimizing the use of infrastructure and human and financial resources.Scarce resources and insufficient infrastructure demand actions based on a lot of planning.One of the main limiting factors for the planning and development of public policies for the sector is the lack of reliable data and information on the generation and management of waste,that allow the creation of future scenarios and the definition of adequate strategies for minimization,valorization of waste and final disposal of waste that no longer can be processed.Thus,this work sought to analyze the current situation of selective waste collection and recycling cooperatives in 211 municipalities in the State of São Paulo,evaluating the issues and opportunities for the applicability of Law n.12,305/2010,Brazilian PNRS(in Portuguese)(National Policy of Solid Waste)in the management of this waste in small,medium and large cities.A bibliographic research was carried out on the panorama of solid waste management in Brazil and in the State of São Paulo,as well as a contextualization on selective waste collection and recycling cooperatives.Following that,data from 211 municipalities taking part in the survey Solid Waste Management—Municipalities of the State of São Paulo were analyzed,focusing on answers that correlated with the topics covered in this research.For this work,questions were used in order to obtain a basic overview of solid waste management in the municipalities,identifying the characteristics of selective waste collection and recycling cooperatives in the municipalities and to verify the similarity,benefits and difficulties of selective waste collection and waste cooperatives in the municipalities studied.After tabulating and analyzing the data,a matrix was prepared to compare the results obtained in relation to selective waste collection and recycling cooperatives and the guidelines of the PNRS(National Solid Waste Policy)in urban environmental management,which allowed the conclusion that all municipalities participating in the research have a good understanding of solid waste management practices,as well as seek to comply with the guidelines of the PNRS.However,the results of this matrix indicated that there are important points that need to be considered in the planning of actions for a better management of recyclable waste,such as the implementation of inter-municipal consortiums in the management of solid waste,investment in environmental education,stimulation to create recycling cooperatives and adoption of social inclusion regulations for informal waste collectors. | Clauciana Schmidt Bueno de Moraes Larissa Senden Carnicel Adriana Maria Nolasco Giulia Malaguti Braghini Marcolini Martires Cinthia Mara Vital Bonaretto Willian Leandro Henrique Pinto Bruna Ferrari Felipe Felipe JoséSasso Laís Araújo de Paula | 2022 | Journal of Environmental Science and Engineering(B)2022,11,5: | 0 |
