2Perioperative management of distal pancreatectomy显示文摘Recent advances in surgical techniques and perioperative management have markedly reduced operative morbidity after distal pancreatectomy(DP).However,some questions remain regarding the protocol for the perioperative management of DP,in particular,with regard to the development of pancreatic fistula(PF).A review of DP was therefore conducted in order to standardize the management of patients for a favorable outcome.Overall,operative technique and perioperative management emerged as two critical factors contributing to favorable outcome in DP patients.As for the operative method,surgical and closure techniques exhibited differences in outcome.Laparoscopic DP generally yields more favorable perioperative outcomes compared to open DP,and is applicable for benign tumors and some ductal carcinomas of the pancreas.Robotic DP is also available for safe pancreatic surgery.En bloc celiac axis resection offers a high R0 resection rate and potentially allows for some local control in the case of advanced pancreatic cancer.Following resection,staple closure was not found to reduce the rate of PF when compared to hand-sewn closure.In addition,ultrasonic dissection devices,fibrin glue sealing,and staple closure with mesh reinforcement were shown to significantly reduce PF,although there was some bias in these studies.In perioperative management,both preoperative and postoperative treatment affected outcome.First,preoperative endoscopic pancreatic stenting may be an effective prophylactic measure against fistula development following DP in selected patients.Second,in postoperative management,a multifactorial approach including prophylactic antibiotics improved high surgical site infection rates following complex hepato-pancreatobiliary surgery.Furthermore,although conflicting results have been reported,somatostatin analogues should be administered selectively to patients considered to have a high risk for PF.Finally,careful drain management also facilitates a favorable outcome in patients with PF after DP.The results of the review indicate that laparoscopic DP coupled with perioperative management influences outcome in DP patients.Yasuhiro Fujino 2015World Journal of Gastroenterology2015,21,11:7
3The increase in surface CXCR4 expression on lung extravascular neutrophils and its effects on neutrophils during endotoxin-induced lung injury显示文摘Inflammatory stimuli,such as a microbes or lipopolysaccharides,induce a rapid release of neutrophils from the bone marrow and promote neutrophil migration into inflamed sites to promote host defense.However,an excess accumulation and retention of neutrophils in inflamed tissue can cause severe tissue injuries in the later stages of inflammation.Recent studies have reported that both CXCL12 levels in injured lungs and its receptor,CXCR4,on accumulated neutrophils in injured lungs,increased;furthermore,these studies showed that the CXCL12/CXCR4 signaling pathway participated in neutrophil accumulation in the later stages of lipopolysaccharide(LPS)-induced lung injury.However,the mechanisms underlying this increase in surface CXCR4 expression in neutrophils remain unclear.In this study,we found that surface CXCR4 expression increased in extravascular,but not intravascular,neutrophils in the lungs of LPS-induced lung injury model mice.Furthermore,ex vivo studies revealed that CXCL12 acted not only as a chemoattractant,but also as a suppressor of cell death for the lung neutrophils expressing CXCR4.Sulfatide,one of the native ligands for L-selectin,induced the increase of surface CXCR4 expression on isolated circulating neutrophils,suggesting that the activation of L-selectin may be involved in the increase in surface CXCR4.Our findings show that surface CXCR4 levels on neutrophils increase after extravasation into injured lungs,possibly through the activation of L-selectin.The CXCL12/CXCR4 signaling pathway plays an important role in the modulation of neutrophil activity during acute lung injury,not only by promoting chemotaxis but also by suppressing cell death.Mitsuhiro Yamada Hiroshi Kubo Seiichi Kobayashi Kota Ishizawa Mei He Takaya Suzuki Naoya Fujino Hiroyuki Kunishima Masamitsu Hatta Katsushi Nishimaki Tetsuji Aoyagi Kouichi Tokuda Miho Kitagawa Hisakazu Yano Hirokazu Tamamura Nobutaka Fujii Mitsuo Kaku 2011Cellular & Molecular Immunology2011,8,4:6
4Research and Implementations of Structural Monitoring for Bridges and Buildings in Japan显示文摘This paper provides a review on the development of structural monitoring in Japan, with an emphasis on the type, strategy, and utilization of monitoring systems. The review focuses on bridge and building structures using vibration-based techniques. Structural monitoring systems in Japan historically started with the objective of evaluating structural responses against extreme events. In the development of structural monitoring, monitoring systems and collected data were used to verify design assumptions, update speci cations, and facilitate the ef cacy of vibration control systems. Strategies and case studies on monitoring for the design veri cation of long-span bridges and tall buildings, the performance of seismic isolation systems in building and bridges, the veri cation of structural retro t, the veri cation of structural control systems (passive, semi-active, and active), structural assessment, and damage detec- tion are described. More recently, the application of monitoring systems has been extended to facilitate ef cient operation and effective maintenance through the rationalization of risk and asset management using monitoring data. 9Helicobacter pylori infection and gastropathy:A comparison between Indonesian and Japanese patients显示文摘AIM:To compare the effects of Helicobacter pylori(H pylori) infection on gastropathy between Indonesian and Japanese patients.METHODS:Biopsy specimens were obtained during upper gastrointestinal endoscopy from 167 subjects(125 Indonesians and 42 Japanese) with uninvestigated symptoms of dyspepsia.The specimens were analyzed for the presence of H pylori using urease analysis,histopathology,and cell culture.The grade and activity of gastritis was assessed using the updated Sydney system.RESULTS:The percentages of Indonesian and Japanese patients who were H pylori-positive at the antrum or body of the stomach were similar(68% and 59.5%,respectively;P=0.316).Of those who were H pylori-positive,more Japanese patients than Indonesian patients had high levels of polymorphonuclear cells(P=0.001),mononuclear cells(P=0.013),glandular atrophy(P=0.000),and intestinal metaplasia(P=0.011) in both the antrum and body of the stomach.CONCLUSION:The grade of gastritis and prevalence of mucosal atrophy and intestinal metaplasia were higher in Japanese patients.The difference between Indonesian and Japanese patients was signifi cant.Murdani Abdullah Hiroyuki Ohtsuka Abdul Aziz Rani Tadashi Sato Ari F Syam Masayuki A Fujino 2009World Journal of Gastroenterology2009,15,39:2

12Conservative reconstruction using stents as salvage therapy for disruption of esophago-gastric anastomosis显示文摘Esophagectomy with extended lymphadenectomy and gastric conduit reconstruction is a radical procedure for the treatment of esophageal cancer that is associated with a high morbidity rate.Gastric conduit necrosis is a fatal complication that occurs in 2%of patients.Conventionally,two-stage salvage surgery consisting of removal of the necrotic gastric conduit followed by reconstruction has been performed;however,this procedure has a high morbidity rate.We describe a61-year-old man who underwent minimally invasive esophagectomy complicated by slowly progressive gastric conduit necrosis associated with complete neck drainage and a stable overall condition.There was a 2 cm gap in the anastomosis.Because there was no evidence of residual gastric conduit necrosis,a removable,covered self-expanding metal stent(SEMS)was inserted to bridge the anastomosis.The stent was fixed to the patient's ear with silk thread through the lasso on its proximal end to prevent migration.Eight weeks after insertion,the stent was removed easily without any associated complications.The anastomotic defect was completely bridged with granulation tissue,showing progressive epithelialization without leakage or stenosis.The patient was discharged home in good general health.This is the first report of the successful conservative management of esophago-gastric conduit anastomosis disruption with SEMS placement.Taro Oshikiri Yoshinobu Yamamoto Ikuya Miki Masahiro Tsuda Tetsu Nakamura Yasuhiro Fujino Masahiro Tominaga Yoshihiro Kakeji 2015World Journal of Gastroenterology2015,21,28:2
