| 1 | Current trends of liver cirrhosis in Mexico: Similitudes and differences with other world regions显示文摘AIM To investigate the main current etiologies of cirrhosis in Mexico.METHODS We performed a cross-sectional retrospective multicenter study that included eight hospitals in different areas of Mexico. These hospitals provide health care to people of diverse social classes. The inclusion criteria were a histological, clinical, biochemical, endoscopic, or imaging diagnosis of liver cirrhosis. Data were obtained during a 5-year period(January 2012-December 2017). RESULTS A total of 1210 patients were included. The mean age was 62.5 years(SD = 12.1), and the percentages of men and women were similar(52.0% vs 48.0%). The most frequent causes of liver cirrhosis were hepatitis C virus(HCV)(36.2%), alcoholic liver disease(ALD)(31.2%), and nonalcoholic steatohepatitis(23.2%), and the least frequent were hepatitis B virus(1.1%), autoimmune disorders(7.3%), and other conditions(1.0%).CONCLUSION HCV and ALD are the most frequent causes of cirrhosis in Mexico. However, we note that non-alcoholic fatty liver disease(NAFLD) as an etiology of cirrhosis increased by 100% compared with the rate noted previously. | 16 | Antimicrobial lipids:Emerging effector molecules of innate host defense显示文摘The antimicrobial properties of host derived lipids have become increasingly recognized and evidence is mounting that antimicrobial lipids(AMLs),like antimicrobial peptides,are effector molecules of the innate immune system and are regulated by its conserved pathways.This review,with primary focus on the human body,provides some background on the biochemistry of lipids,summarizes their biological functions,expands on their antimicrobial properties and site-specific composition,presents modes of synergism with antimicrobial peptides,and highlights the more recent reports on the regulation of AML production as well as bacterial resistance mechanisms.Based on extant data a concept of innate epithelial defense is proposed where epithelial cells,in response to microbial products and proinflammatory cytokines and through activation of conserved innate signaling pathways,increase their lipid uptake and up-regulate transcription of enzymes involved in lipid biosynthesis,and induce transcription of antimicrobial peptides as well as cytokines and chemokines.The subsequently secreted antimicrobial peptides and lipids then attack and eliminate the invader,assisted by or in synergism with other antimicrobial molecules delivered by other defense cells that have been recruited to the site of infection,in most of the cases.This review invites reconsideration ofthe interpretation of cholesteryl ester accumulation in macrophage lipid droplets in response to infection as a solely proinflammatory event,and proposes a direct antimicrobial role of lipid droplet-associated cholesteryl esters.Finally,for the interested,but new-to-the-field investigator some starting points for the characterization of AMLs are provided.Before it is possible to utilize AMLs for anti-infectious therapeutic and prophylactic approaches,we need to better understand pathogen responses to these lipids and their role in the pathogenesis of chronic infectious disease. | Edith Porter Daniel C Ma Sandy Alvarez Kym F Faull | 2015 | World Journal of Immunology2015,5,2: | 0 |
| 17 | Role of capsule endoscopy in inflammatory bowel disease:Anything new?显示文摘Capsule endoscopy(CE)is a recently developed diagnostic method for diseases of the small bowel that is non-invasive,safe,and highly tolerable.Its role in patients with inflammatory bowel disease has been widely validated in suspected and established Crohn’s disease(CD)due to its ability to assess superficial lesions not detected by cross-sectional imaging and proximal lesions of the small bowel not evaluable by ileocolonoscopy.Because CE is a highly sensitive but less specific technique,differential diagnoses that can simulate CD must be considered,and its interpretation should be supported by other clinical and laboratory indicators.The use of validated scoring systems to characterize and estimate lesion severity(Lewis score,Capsule Endoscopy Crohn’s Disease Activity Index),as well as the standardization of the language used to define the lesions(Delphi Consensus),have reduced the interobserver variability in CE reading observed in clinical practice,allowing for the optimization of diagnoses and clinical management strategies.The appearance of the panenteric CE,the incorporation of artificial intelligence,magnetically-guided capsules,and tissue biopsies are elements that contribute to CE being a promising,unique diagnostic tool in digestive tract diseases. | Edith Pérez de Arce Rodrigo Quera Paulina Núñez F Raúl Araya | 2021 | Artificial Intelligence in Gastrointestinal Endoscopy2021,2,4: | 0 |
