| 1 | Anti program death-1/anti program death-ligand 1 in digestive cancers显示文摘Human tumors tend to activate the immune system regulatory checkpoints as a means of escaping immunosurveillance. For instance, interaction between program death-1(PD-1) and program death-ligand 1(PD-L1) will lead the activated T cell to a state of anergy. PD-L1 is upregulated on a wide range of cancer cells. Anti-PD-1 and anti-PD-L1 monoclonal antibodies(m Abs), called immune checkpoint inhibitors(ICIs), have consequently been designed to restore T cell activity. Accumulating data are in favor of an association between PD-L1 expression in tumors and response to treatment. A PD-L1 expression is present in 30% to 50% of digestive cancers. Multiple anti-PD-1(nivolumab, pembrolizumab) and anti-PD-L1 m Abs(MPDL3280A, Medi4736) are under evaluation in digestive cancers. Preliminary results in metastatic gastric cancer with pembrolizumab are highly promising and phase Ⅱ will start soon. In metastatic colorectal cancer(CRC), a phase Ⅲ trial of MPDL3280 A as maintenance therapy will shortly be initiated. Trials are also ongoing in metastatic CRC with high immune T cell infiltration(i.e., microsatellite instability). Major challenges are ahead in order to determine how, when and for which patients we should use these ICIs. New radiologic criteria to evaluate tumor response to ICIs are awaiting prospective validation. The optimal therapeutic sequence and association with cytotoxic chemotherapy needs to be established. | 2 | Rotational assisted endoscopic retrograde cholangiopancreatography in patients with reconstructive gastrointestinal surgical anatomy显示文摘AIM: To evaluate the success rates of performing therapy utilizing a rotational assisted enteroscopy device in endoscopic retrograde cholangiopancreatography(ERCP) in surgically altered anatomy patients. METHODS: Between June 1, 2009 and November 8, 2012, we performed 42 ERCPs with the use of rotational enteroscopy for patients with altered anatomy(39 with gastric bypass Roux-en-Y, 2 with Billroth Ⅱ gastrectomy, and 1 with hepaticojejunostomy associated with liver transplant). The indications for ERCP were: choledocholithiasis: 13 of 42(30.9%), biliary obstruction suggested on imaging: 20 of 42(47.6%), suspected sphincter of Oddi dysfunction: 4 of 42(9.5%), abnormal liver enzymes: 1 of 42(2.4%), ascending cholangitis: 2 of 42(4.8%), and bile leak: 2 of 42(4.8%). All procedures were completed with the Olympus SIF-Q180 enteroscope and the Endo-Ease Discovery SB overtube produced by Spirus Medical. RESULTS: Successful visualization of the major ampulla was accomplished in 32 of 42 procedures(76.2%). Cannulation of the bile duct was successful in 26 of 32 procedures reaching the major ampulla(81.3%). Successful therapeutic intervention was completed in 24 of 26 procedures in which the bileduct was cannulated(92.3%). The overall intention to treat success rate was 64.3%. In terms of cannulation success, the intention to treat success rate was 61.5%. Ten out of forty two patients(23.8%) required admission to the hospital after procedure for abdominal pain and nausea, and 3 of those 10 patients(7.1%) had a diagnosis of post-ERCP pancreatitis. | 13 | Adipose-derived stem cells improve grafted burn wound healing by promoting wound bed blood flow显示文摘Background:Researchers have explored the use of adipose-derived stem cells(ASCs)as a cellbased therapy to cover wounds in burn patients;however,underlying mechanistic aspects are not completely understood.We hypothesized that ASCs would improve post-burn wound healing after eschar excision and grafting by increasing wound blood flow via induction of angiogenesis-related pathways.Methods:To test the hypothesis,we used an ovine burn model.A 5 cm^(2) full thickness burn wound was induced on each side of the dorsum.After 24 hours,the burned skin was excised and a 2 cm^(2) patch of autologous donor skin was grafted.The wound sites were randomly allocated to either topical application of 7 million allogeneic ASCs or placebo treatment(phosphate-buffered saline[PBS]).Effects of ASCs culture media was also compared to those of PBS.Wound healing was assessed at one and two weeks following the application of ASCs.Allogeneic ASCs were isolated,cultured and characterized from non-injured healthy sheep.The identity of the ASCs was confirmed by flow cytometry analysis,differentiation into multiple lineages and gene expression via real-time polymerase chain reaction.Wound blood flow,epithelialization,graft size and take and the expression of vascular endothelial growth factor(VEGF)were determined via enzyme-linked immunosorbent assay and Western blot.Results:Treatment with ASCs accelerated the patch graft growth compared to the control(p<0.05).Topical application of ASCs significantly increased wound blood flow(p<0.05).Expression of VEGF was significantly higher in the wounds treated with ASCs compared to control(p<0.05).Conclusions:ASCs accelerated grafted skin growth possibly by increasing the blood flow via angiogenesis induced by a VEGF-dependent pathway. | Osamu Fujiwara Anesh Prasai Dannelys Perez-Bello Amina El Ayadi Irene Y.Petrov Rinat O.Esenaliev Yuriy Petrov David N.Herndon Celeste C.Finnerty Donald S.Prough Perenlei Enkhbaatar | 2020 | Burns & Trauma2020，8，1： | 1 |
