2Optimal timing and route of nutritional support after esophagectomy: A review of the literature显示文摘Some controversy surrounds the postoperative feeding regimen utilized in patients who undergo esophagectomy.Variation in practices during the perioperative period exists including the type of nutrition started,the delivery route,and its timing.Adequate nutrition is essential for this patient population as these patients often present with weight loss and have altered eating patterns after surgery,which can affect their ability to regain or maintain weight.Methods of feeding after an esophagectomy include total parenteral nutrition,nasoduodenal/nasojejunal tube feeding,jejunostomy tube feeding,and oral feeding.Recent evidence suggests that early oral feeding is associated with shorter LOS,faster return of bowel function,and improved quality of life.Enhanced recovery pathways after surgery pathways after esophagectomy with a component of early oral feeding also seem to be safe,feasible,and cost-effective,albeit with limited data.However,data on anastomotic leaks is mixed,and some studies suggest that the incidence of leaks may be higher with early oral feeding.This risk of anastomotic leak with early feeding may be heavily modulated by surgical approach.No definitive data is currently available to definitively answer this question,and further studies should look at how these early feeding regimens vary by surgical technique.This review aims to discuss the existing literature on the optimal route and timing of feeding after esophagectomy.Richard Zheng Courtney L Devin Michael J Pucci Adam C Berger Ernest L Rosato Francesco Palazzo 2019World Journal of Gastroenterology2019,25,31:11

6Spinal Mechanisms of Itch Transmission显示文摘Peripheral itch stimuli are transmitted by sensory neurons to the spinal cord dorsal horn, which then transmits the information to the brain. The molecular and cellular mechanisms within the dorsal horn for itch transmission have only been investigated and identified during the past ten years. This review covers the progress that has been made in identifying the peptide families in sensory neurons and the receptor families in dorsal horn neurons as putative itch transmitters, with a focus on gastrin-releasing peptide(GRP)—GRP receptor signaling. 7Gut microbiota profile in healthy Indonesians显示文摘BACKGROUND Recently, gut microbiota has been associated with various diseases other than intestinal disease. Thus, there has been rapid growth in the study of gut microbiota. Considering the numerous factors influencing gut microbiota such as age, diet, etc., area-based research is required. Indonesia has numerous different tribes and each of these tribes have different lifestyles. Hence, it is expected that each tribe has a specific gut microbiota. A deeper insight into the composition of gut microbiota can be used to determine the condition of gut microbiota in Indonesians and to consider which treatment may be suitable and effective to improve health status.AIM To investigate the gut microbiota of Indonesian subjects represented by Javanese and Balinese tribes by analyzing fecal samples.METHODS Fecal samples were collected from a total of 80 individuals with 20 in each of the young groups ranging from 25-45 years and the elderly group aged 70 years or more from two different regions, Yogyakarta and Bali. Fecal sample collection was performed at the end of the assessment period(day 14 ± 1 d) during which time the subjects were not allowed to consume probiotic or antibiotic products.The quantification of various Clostridium subgroups, Lactobacillus subgroups,Enterococcus, Streptococcus, Staphylococcus, Bacteroides fragilis group and Prevotella,Bifidobacterium and Atopobium cluster, Enterobacteriaceae and Pseudomonas was performed using the Yakult intestinal flora-scan(YIF-SCAN).RESULTS The bacterial population in younger subjects' feces was higher than that in the elderly population, with a total of approximately 10.0 – 10.6 log10 bacterial cells/g feces. The most abundant bacteria in all groups were Clostridium, followed by Prevotella, Atopobium, Bifidobacterium and Bacteroides. In the elderly, an increase in Enterobacteriaceae, Coliform and Escherichia coli was found. In terms of bacterial counts in Yogyakarta, total bacteria, Clostridium coccoides(C. coccoides) group,Bifidobacterium, Prevotella, Lactobacillus plantarum subgroup, and Streptococcus were significantly higher(P < 0.05) in younger than elderly subjects, while the Lactobacillus gasseri subgroup, Lactobacillus casei subgroup, and Lactobacillus reuteri subgroup counts were significantly lower(P < 0.05) in younger subjects. In Balinese subjects, total bacteria, C. coccoides group, Clostridium leptum subgroup,Bacteroides fragilis group, and Prevotella were significantly higher(P < 0.05) in younger compared to elderly individuals, while the Lactobacillus ruminis subgroup, and Enterobacteriaceae were significantly lower(P < 0.05) in younger subjects. The results also revealed that, besides the C. coccoides group and Clostridium leptum group being the most abundant gut microbiota in both Yogyakarta and Balinese people, the latter was indicated by a higher Clostridium perfringens count, which was almost 10 times that of Yogyakarta subjects. This may be a response to different lifestyles in the different tribes; however, this phenomenon requires further extensive study.CONCLUSION Bacterial populations were higher in younger than in elderly subjects. Most abundant bacterial groups were Clostridium, Prevotella, Atopobium, Bifidobacterium,and Bacteroides. 8Current update on molecular cytogenetics, diagnosis and management of gastrointestinal stromal tumors显示文摘Gastrointestinal stromal tumors(GISTs)are the most common mesenchymal tumors of the gastrointestinal(GI)tract and are thought to arise from precursors of the interstitial cells of Cajal.GISTs can arise anywhere in the GI tract,but most commonly originate from the stomach and small intestine.The majority of GISTs occur as a result of activating mutations in two receptor protein tyrosine kinases:KIT and/or platelet-derived growth factor receptor-α.Mutational analyses allow for predicting patient prognosis and treatment response.Clinical presentations can vary from no symptoms,typical in the case of small incidentally found tumors,to GI bleeding,abdominal discomfort,and ulcer-related symptoms when the tumor is enlarged.Imaging plays a critical role in the diagnosis and management of these tumors with multiphasic computed tomography serving as the imaging modality of choice.Magnetic resonance imaging and positron emission tomography-computed tomography can serve as imaging adjuncts in lesion characterization,especially with liver metastases,and subsequent staging and assessment for treatment response or recurrence.Surgical resection is the preferred management for small GISTs,while tyrosine kinase inhibitors−imatinib mesylate and sunitinib malate−serve as crucial molecular-targeted therapies for locally advanced and metastatic GISTs.This review article highlights the clinical presentation,pathology and molecular cytogenetics,imaging features,and current management of GISTs.Mindy X Wang Catherine Devine Nicole Segaran Dhakshinamoorthy Ganeshan 2021World Journal of Gastroenterology2021,27,41:5

10Nematode-Encoded RALF Peptide Mimics Facilitate Parasitism of Plants through the FERONIA Receptor Kinase显示文摘The molecular mechanism by which plants defend against plant root-knot nematodes(RKNs)is largely unknown.The plant receptor kinase FERONIA and its peptide ligands,rapid alkalinization factors(RALFs),regulate plant immune responses and cell expansion,which are two important factors for successful RKN parasitism.In this study,we found that mutation of FERONIA in Arabidopsis thaliana resulted in plants showing low susceptibility to the RKN Meloidogyne incognita.To identify the underlying mechanisms associated with this phenomenon,we identified 18 novel RALF-likes from multiple species of RKNs and showed that two RALF-likes(i.e.,MiRALF1 and MiRALF3)from M.incognita were expressed in the esophageal gland with high expression during the parasitic stages of nematode development.These nematode RALF-likes also possess the typical activities of plant RALFs and can directly bind to the extracellular domain of FERONIA to modulate specific steps of nematode parasitism-related immune responses and cell expansion.Genetically,both MiRALF1/3 and FERONIA are required for RKN parasitism in Arabidopsis and rice.Collectively,our study suggests that nematode-encoded RALFs facilitate parasitism via plant-encoded FERONIA and provides a novel paradigm for studying host-pathogen interactions.Xin Zhang Huan Peng Sirui Zhu Junjie Xing Xin Li Zhaozhong Zhu Jingyuan Zheng Long Wang Bingqian Wang Jia Chen Zhenhua Ming Ke Yao Jinzhuo Jian Sheng Luan Devin Coleman-Derr Hongdong Liao Yousong Peng Deliang Peng Feng Yu 2020Molecular Plant2020,13,10:3

12吡虫啉的不同施药方式对丽蚜小蜂的寄生效果评价显示文摘烟粉虱Q隐种是近几年新传入我国的入侵烟粉虱,主要危害蔬菜、棉花和花卉等经济作物,对多种化学农药都产生了很高的抗药性。为了了解吡虫啉不同施药方式对丽蚜小蜂控制烟粉虱效果的影响,本研究在实验室条件下,调查了经吡虫啉叶面处理和灌根处理两种不同施药方式处理,烟粉虱Q隐种的存活率、丽蚜小蜂的存活率和寄生率。结果表明:1、5、10、50 mg.L 1浓度吡虫啉灌根处理的丽蚜小蜂校正死亡率分别为21.8%、14.9%、19.3%、16.2%;1、5、50 mg.L 1浓度吡虫啉叶面处理的丽蚜小蜂校正死亡率分别为52.2%、81.8%、93.3%。相同浓度的吡虫啉处理中,叶面处理的丽蚜小蜂死亡率显著高于灌根处理;在吡虫啉和丽蚜小蜂协同作用中,用浓度5、10、50 mg.L 1的吡虫啉灌根处理时,烟粉虱的死亡率分别为41.5%、39.1%、45.9%,丽蚜小蜂寄生率分别为11.9%、15.9%、7.9%;用浓度5、50 mg.L 1的吡虫啉叶面处理时,烟粉虱的死亡率分别为12.6%、37.2%,丽蚜小蜂寄生率均为2%左右。灌根处理对寄生蜂的存活率影响较小,且有利于丽蚜小蜂对烟粉虱的寄生和取食;而叶面处理显著降低了寄生蜂的存活率及寄生率。饶琼 许勇华 张帆 罗晨 张宏宇 Greg J Devine Kevin Gorman 2012中国生物防治学报2012,28,4:3

15Features of primary pancreatic lymphoma:A bi-institutional review with an emphasis on typical and atypical imaging features显示文摘BACKGROUND Primary pancreatic lymphoma(PPL)is a rare neoplasm.Being able to distinguish it from other pancreatic malignancies such as pancreatic ductal adenocarcinoma(PDAC)is important for appropriate management.Unlike PDAC,PPL is highly sensitive to chemotherapy and usually does not require surgery.Therefore,being able to identify PPL preoperatively will not only direct physicians towards the correct avenue of treatment,it will also avoid unnecessary surgical intervention.AIM To evaluate the typical and atypical multi-phasic computed tomography(CT)imaging features of PPL.METHODS A retrospective review was conducted of the clinical,radiological,and pathological records of all subjects with pathologically proven PPL who presented to our institutions between January 2000 and December 2020.Institutional review board approval was obtained for this investigation.The collected data were analyzed for subject demographics,clinical presentation,laboratory values,CT imaging features,and the treatment received.Presence of all CT imaging findings including size,site,morphology and imaging characteristics of PPL such as the presence or absence of nodal,vascular and ductal involvement in these subjects were recorded.Only those subjects who had a pre-treatment multiphasic CT of the abdomen were included in the study.RESULTS Twenty-nine cases of PPL were diagnosed between January 2000 and December 2020(mean age 66 years;13 males/16 females).All twenty-nine subjects were symptomatic but only 4 of the 29 subjects(14%)had B symptoms.Obstructive jaundice occurred in 24%of subjects.Elevated lactate dehydrogenase was seen in 81%of cases,whereas elevated cancer antigen 19-9 levels were present in only 10%of cases for which levels were recorded.The vast majority(90%)of tumors involved the pancreatic head and uncinate process.Mean tumor size was 7.8 cm(range,4.0-13.8 cm).PPL presented homogenous hypoenhancement on CT in 72%of cases.Small volume peripancreatic lymphadenopathy was seen in 28%of subjects.Tumors demonstrated encasement of superior mesenteric vessels in 69%of cases but vascular stenosis or occlusion only manifested in 5 out of the twentynine individuals(17%).Mild pancreatic duct dilatation was also infrequent and seen in only 17%of cases,whereas common bile duct(CBD)dilation was seen in 41%of subjects.Necrosis occurred in 10%of cases.Size did not impact the prevalence of pancreatic and CBD dilation,necrosis,or mesenteric root infiltration(P=0.525,P=0.294,P=0.543,and P=0.097,respectively).Pancreatic atrophy was not present in any of the subjects.CONCLUSION PPL is an uncommon diagnosis best made preoperatively to avoid unnecessary surgery and ensure adequate treatment.In addition to the typical CT findings of PPL,such as homogeneous hypoenhancement,absence of vascular stenosis and occlusion despite encasement,and peripancreatic lymphadenopathy,this study highlighted many less typical findings,including small volume necrosis and pancreatic and bile duct dilation.Nicole Segaran Kumaresan Sandrasegaran Catherine Devine Mindy X Wang Chintan Shah Dhakshinamoorthy Ganeshan 2021World Journal of Clinical Oncology2021,12,9:2
