| 1 | Integrated approach to colorectal anastomotic leakage: Communication, infection and healing disturbances显示文摘Colorectal anastomotic leakage(CAL) remains a major complication after colorectal surgery. Despite all efforts during the last decades, the incidence of CAL has not decreased. In this review, we summarize the available strategies regarding prevention, prediction and intervention of CAL and categorize them into three categories: communication, infection and healing disturbances. These three major factors actively interact during the onset of CAL. We aim to provide an integrated approach to CAL based on its etiology. The intraoperative air leak test, intraoperative endoscopy, radiological examinations and stoma construction mainly aim to detect and to prevent communication between the intra- and extra-luminal content. Other strategies including postoperative drainage, antibiotics, and infectious-parameter evaluation are intended to detect and prevent anastomotic or peritoneal infection. Most currently available interventions for CAL focus on the control of communication and infection, while strategies targeting the healing disturbances such as lifestyle changes, oxygen therapy and evaluation of metabolic biomarkers still lack wide clinical application. This simplified categorization may contribute to an integrated understanding of CAL. We strongly believe that this integrated approach should be taken into consideration during clinical practice. | 2 | PM_(2.5) source apportionment in a French urban coastal site under steelworks emission influences using constrained non-negative matrix factorization receptor model显示文摘The constrained weighted-non-negative matrix factorization(CW-NMF) hybrid receptor model was applied to study the influence of steelmaking activities on PM_(2.5)(particulate matter with equivalent aerodynamic diameter less than 2.5 μm) composition in Dunkerque,Northern France. Semi-diurnal PM_(2.5)samples were collected using a high volume sampler in winter 2010 and spring 2011 and were analyzed for trace metals, water-soluble ions, and total carbon using inductively coupled plasma – atomic emission spectrometry(ICP-AES),ICP- mass spectrometry(ICP-MS), ionic chromatography and micro elemental carbon analyzer. The elemental composition shows that NO_3^-, SO_4^(2-), NH_4^+and total carbon are the main PM_(2.5)constituents. Trace metals data were interpreted using concentration roses and both influences of integrated steelworks and electric steel plant were evidenced. The distinction between the two sources is made possible by the use Zn/Fe and Zn/Mn diagnostic ratios. Moreover Rb/Cr, Pb/Cr and Cu/Cd combination ratio are proposed to distinguish the ISW-sintering stack from the ISW-fugitive emissions. The a priori knowledge on the influencing source was introduced in the CW-NMF to guide the calculation. Eleven source profiles with various contributions were identified: 8 are characteristics of coastal urban background site profiles and 3 are related to the steelmaking activities. Between them,secondary nitrates, secondary sulfates and combustion profiles give the highest contributions and account for 93% of the PM_(2.5)concentration. | 3 | Influence of ship emissions on NOx, SO2, O3 and PM concentrations in a North-Sea harbor in France显示文摘The influence of in-port ship emissions on gases and PM_(10) concentrations has been estimated in the port city of Calais, northern France, one of the busiest harbor in Europe,with numerous rotations of ferries or roll-on/roll-off cargo in average per day. NO_x, SO_2, O_3 and PM_(10) concentrations were continuously measured over a three-month period, as well as real-time particle size distribution. A rural site located at Cape Gris-Nez, 20km from Calais, was considered to deduce intrinsic contribution of ship emissions at the harbor city.The average concentrations of the studied species as well as the pattern of the conditional bivariate probability function at the two sites evidenced that in-port shipping, especially during the maneuvering operations, has an important influence on the NO_xand SO_2 concentrations. The impact of shipping in the harbor of Calais on average concentrations was estimated to 51% for SO_2, 35% for NO, 15% for NO_2 and 2% for PM_(10) in the studied period.Concentration peaks of SO_2 and NO_x associated with an O_3 depletion appeared synchronized with departures and arrivals of ferries. For winds blowing from the harbor,when compared to the background level, the number of particles appeared 10 times higher,with the highest differences in the 30–67nm and the 109–167 nm size ranges. The average impact of in-port ships on PM310 concentrations was estimated to + 28.9 μg/mand concerned mainly the PM1 size fraction(40%).
