10汉化版切尔西物理功能评估量表在ICU成年人患者中的应用显示文摘目的:探索中文版切尔西物理功能评估量表(CPAx-Chi)诊断ICU获得性衰弱(ICU-AW)的阈值及评估诊断价值。方法:学习CPAx-Chi量表使用细节和注意事项,由2名研究者同时采用CPAx-Chi量表和医学研究委员会肌力评分(MRC-Score)量表对兰州大学第一医院综合ICU的200例患者进行独立评估。以MRC-Score总分≤48分为标准,通过计算受试者工作特征曲线下面积(ROC AUC)、尤登指数(Youden's index,YI)及一致性检验,确定CPAx-Chi量表在诊断ICU-AW的阈值及价值。结果:以MRC-Score总分≤48分为标准,A组ROC AUC=0.899(95%CI 0.862~1.025);B组ROC AUC=0.874(95%CI 0.824~0.925);CPAx-Chi量表诊断ICU-AW的临界阈值为31.5分;A组最大YI=0.643,敏感度为87%,特异度为77%;B组最大YI=0.62,敏感度为75%,特异度为87%。CPAx-Chi量表诊断ICU-AW的临界阈值为30.5分,B组最大YI=0.62,敏感度为79%,特异度为83%。以CPAx-Chi≤31阈值,ICU-AW组和非ICU-AW组的CPAx-Chi评分差异有统计学意义;CPAx-Chi≤31分和MRC-Score≤48分诊断ICU-AW的一致性高,A组Kappa=0.845(P=0.02);B组数据Kappa=0.839(P=0.04),且有统计学意义。结论:CPAx-Chi量表评分≤31分是诊断ICU-AW的临界值,且具有良好的灵敏度和特异度。CPAx-Chi量表可以在我国重症医学领域推广应用。吴雨晨 王国强 丁楠楠 Corner EJ 姜变通 岳伟岗 张志刚 魏花萍 2021中国实用护理杂志2021,37,26:5

12The Yin and Yang of Wnt/Ryk axon guidance in development and regeneration显示文摘In the developing embryo,nascent axons navigate towards their specific targets to establish the intricate network of axonal connections linking neurons within the mature nervous system.Molecular navigational systems comprising repulsive and attractive guidance cues form chemotactic gradients along the pathway of the exploring growth cone.Axon-bound receptors detect these gradients and determine the trajectory of the migrating growth cone.In contrast to their benevolent role in the developing nervous system,repulsive guidance receptors are detrimental to the axon’s ability to regenerate after injury in the adult.In this review we explore the essential and beneficial role played by the chemorepulsive Wnt receptor,Ryk/Derailed in axon navigation in the embryonic nervous system(the Yin function).Specifically,we focus on the role of Wnt5a/Rykmediated guidance in the establishment of two major axon tracts in the mammalian central nervous system,the corticospinal tract and the corpus callosum.Recent studies have also identified Ryk as a major suppressor of axonal regeneration after spinal cord injury.Thus,we also discuss this opposing aspect of Ryk function in axonal regeneration where its activity is a major impediment to axon regrowth(the Yang function).CLARK Charlotte EJ LIU Yaobo COOPER Helen M 2014Science China(Life Sciences)2014,57,4:4

14Natural orifice transluminal endoscopic surgeryvs laparoscopic ovariectomy:Complications and inflammatory response显示文摘AIM:To compare natural orifice transluminal endoscopic surgery (NOTES) vs standard laparoscopic ovariectomy in mini pigs with respect to technical aspects,complications and parameters of systemic inflammatory response.METHODS:This was a randomized,experimental,survival study.Ten female mini pigs underwent NOTES transgastric ovariectomy (NOTES group) and ten female mini pigs underwent laparoscopic ovariectomy (LAP group).A 'percutaneous endoscopic gastrostomy' approach with guidewire and sphincterotome was used for gastrotomy creation.The ovary was resected using standard biopsy forceps and a snare.The access site was closed using a 'KING' closure with a single endoloop and several clips.In the laparoscopic group,a three-port laparoscopy and an ovariectomy were performed with the use of standard laparoscopic devices.C-reactive protein (CRP),white blood count and interleukin (IL)-6 plasma levels were used as indicators of systemic inflammatory response.All animals were euthanized 28 d after surgery.RESULTS:All animals survived without complications.The mean procedure time was 41.3 min ± 17.6 min (NOTES group) and 25.7 min ± 5.25 min (LAP group,P < 0.02).Postmortem examinations demonstrated that 50% and 70% of animals were free of any complications in the NOTES and LAP groups,respectively.The remaining animals developed minor complications (adhesions) in a comparable frequency between the two groups.In the NOTES group,one animal developed a small intramural gastric abscess close to the gastrotomy site.A minor serous exudate that was present in 50% and 40% of the animals in the NOTES and laparoscopy groups,respectively,was not considered a complication.In both groups CRP levels increased significantly on the 2nd and 7th postoperative days (POD) and returned to normal after 28 d.On POD 2,an increase of CRP level was significantly higher in the NOTES group compared to the LAP group.Values of IL-6 did not differ from baseline values in either of the groups postoperatively.Interestingly,the platelet count decreased significantly on POD 2,but returned close to baseline values on POD 7 and PODs 28-30.CONCLUSION:Both NOTES and laparoscopic ovariectomies had a similar frequency of minor complications.However,the NOTES technique produced an increased systemic inflammatory response on POD 2.Jan Martínek Ondej Ryska Tereza Filípková Radek Dolezel Stefan Juhas Jan Motlík Monika Holubová Vladimír Nosek Barbora Rotnáglová Miroslav Zavoral Miroslav Ryska 2012World Journal of Gastroenterology2012,18,27:4
15Association between High Fat-low Carbohydrate Diet Score and Newly Diagnosed Type 2 Diabetes in Chinese Population显示文摘Objective To study the association between high fat-low carbohydrate diet score and newly diagnosed type 2 diabetes in Chinese population. Methods Data about 20 717 subjects aged 45-59 years from the cross-sectional 2002 China National Nutrition and Health Survey were analyzed. High fat-low carbohydrate diet was scored according to the energy of carbohydrate, fat, and protein. Results Of the 20 717 subjects, 1 332 were diagnosed with hyperglycemia and 662 were newly diagnosed with type 2 diabetes. Multivariate adjusted analysis showed that the highest score of type 2 diabetes patients was 2.75 (95% CI: 2.09-3.61). The score of type 2 diabetes patients was 1.87 (95% CI: 1.35-2.58) after further adjustment for their socioeconomic status and physical activity. No significant difference was found in the odds ratio after further adjustment for BMI, blood pressure, lipid level, and energy intake. No evidence was observed for the relation between high fat-low carbohydrate-diet score in type 2 diabetes patients due to high family income, less education, physical activity, overweight, hypertension, high TG, or low HDL level. 16Multiple haploids, triploids, and tetraploids found in modern-day “living fossil” Ginkgo biloba显示文摘Ginkgo biloba,the last extant representative of a lineage of Mesozoic gymnosperms,is one of the few seed plants with an exceptionally long(~300 Myr)evolutionary history free of genome-wide duplications(polyploidy).Despite this genome conservatism,we have recently found a viable spontaneous tetraploid Ginkgo sapling during routine screening of several plants,demonstrating that natural polyploidy is possible in Ginkgo.Here we provide a much wider flow cytometry survey of ploidy in some European Ginkgo collections,and own seedlings(>2200 individuals and~200 cultivars).We found a surprisingly high level of ploidy variation in modern-day Ginkgo and documented altogether 13 haploid,3 triploid,and 10 tetraploid Ginkgo plants or cultivars,most of them being morphologically distinct from common diploids.Haploids frequently produced polyploid(dihaploid)buds or branches.Tetraploids showed some genome size variation.The surveyed plants provide a unique resource for future Ginkgo research and breeding,and they might be used to accelerate the modern diversification of this nearly extinct plant lineage.PetrŠmarda Lucie Horová Ondřej Knápek Heidi Dieck Martin Dieck Katarína Ražná Pavel Hrubík Laszlo Orlóci Laszlo Papp Kristýna Veselá Pavel Veselý Petr Bureš 2018Horticulture Research2018,5,1:4
