| 1 | Double guidewire technique vs transpancreatic precut sphincterotomy in difficult biliary cannulation显示文摘AIM:To compare the outcomes between doubleguidewire technique(DGT) and transpancreatic precut sphincterotomy(TPS) in patients with difficult biliary cannulation.METHODS:This was a prospective,randomized study conducted in single tertiary referral hospital in Korea.Between January 2005 and September 2010.A total of 71 patients,who bile duct cannulation was not possible and selective pancreatic duct cannulation was achieved,were randomized into DGT(n = 34) and TPS(n = 37) groups.DGT or TPS was done for selective biliary cannulation.We measured the technical success rates of biliary cannulation,median cannulation time,and procedure related complications.RESULTS:The distribution of patients after randomization was balanced,and both groups were comparable in baseline characteristics,except the higher percentage of endoscopic nasobiliary drainage in the DGT group(55.9% vs 13.5%,P < 0.001).Successful cannulation rate and mean cannulation times in DGT and TPS groups were 91.2% vs 91.9% and 14.1 ± 13.2 min vs 15.4 ± 17.9 min,P = 0.732,respectively.There was no significant difference between the two groups.The overall incidence of post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis was 38.2% vs 10.8%,P < 0.011 in the DGT group and the TPS group;post-procedure pancreatitis was significantly higher in the DGT group.But the overall incidence of post-ERCP hyperamylasemia was no significant difference between the two groups;DGT group vs TPS group:14.7% vs 16.2%,P < 1.0.CONCLUSION:When free bile duct cannulation was difficult and selective pancreatic duct cannulation was achieved,DGT and TPS facilitated biliary cannulation and showed similar success rates.However,post-procedure pancreatitis was significantly higher in the DGT group. | Young Wook Yoo Sang-Woo Cha Woong Cheul Lee Sae Hee Kim Anna Kim Young Deok Cho | 2013 | World Journal of Gastroenterology2013，19，1： | 25 |

| 3 | Changing strategies for target therapy in gastric cancer显示文摘In spite of a worldwide decrease in the incidence of gastric cancer, this malignancy still remains one of the leading causes of cancer mortality. Great efforts have been made to improve treatment outcomes in patients with metastatic gastric cancer, and the introduction of trastuzumab has greatly improved the overall survival. The trastuzumab treatment took its first step in opening the era of molecular targeted therapy, however several issues still need to be resolved to increase the efficacy of targeted therapy. Firstly, many patients with metastatic gastric cancer who receive trastuzumab in combination with chemotherapeutic agents develop resistance to the targeted therapy. Secondly, many clinical trials testing novel molecular targeted agents with demonstrated efficacy in other malignancies have failed to show benefit in patients with metastatic gastric cancer, suggesting the importance of the selection of appropriate indications according to molecular characteristics in application of targeted agents. | 4 | Efficacy of Saam acupuncture treatment on improvement of immune cell numbers in cancer patients:a pilot study显示文摘OBJECTIVE: To collect preliminary data on the effects of Saam acupuncture with regard to the immunity in cancer patients.METHODS: Ten cancer patients were analyzed for improvements in immunity. Acupuncture was applied at the 5 acupuncture points, Jingqu(LU 8), Zutonggu(BL 66), Yanggu(SI 5), Yangchi(TE 4), and Zhongwan(CV 12) for 2 weeks with 4 sessions. We assessed the effect of Korean Saam acupuncture on the immune system in cancer patients by measuring particular blood cell subsets, including CD3 +,CD4+, CD8+, CD19+, and CD56+ cells, as well as total white blood cell count, absolute neutrophil count, and fatigue score. The measurement was performed before and after acupuncture and at a 2-week follow-up.RESULTS: There was a statistically significant increase in the number of CD3+(P=0.023) and CD8+ cells(P<0.001) and T-cell subsets, as well as a decrease in the fatigue severity scale(FSS) score(P=0.001) after Saam acupuncture using the 5 acupoints.CONCLUSION: Acupuncture may improve the immune system by increasing the counts of a few immune cells and relieve fatigue in cancer patients by decreasing FSS scores. | 5 | Expression of vascular endothelial growth factor and its receptors VEGFR-1 and 2 in gastrointestinal stromal tumors,leiomyomas and schwannomas显示文摘AIM: To investigate the role of vascular endothelial growth factor (VEGF) and its receptors VEGFR-1 and 2 in the growth and differentiation of gastrointestinal stromal tumors (GISTs). METHODS: Thirty-three GISTs, 15 leiomyomas and 6 schwannomas were examined by immunohistochemistry in this study. RESULTS: VEGF protein was expressed in the cytoplasm of tumor cells, and VEGFR-1 and 2 were expressed both in the cytoplasm and on the membrane of all tumors. Immunohistochemical staining revealed that 26 GISTs (78.8%), 9 leiomyomas (60.0%) and 3 schwannomas (50.0%) were positive for VEGF; 24 GISTs (72.7%), 12 leiomyomas (80.0%) and 4 schwannomas (66.7%) were positive for VEGFR-1; 30 GISTs (90.9%), 5 leiomyomas (33.3%) and 4 schwannomas (66.7%) were positive for VEGFR-2. VEGFR-2 expression was statistically different between GISTs and leiomyomas (p < 0.0001). However, there was no correlation between the expression of VEGF pathway componenets and the clinical risk categories. | 6 | Effect of Nucleases on the Cellular Internalization of Fluorescent Labeled DNA-Functionalized Single-Walled Carbon Nanotubes显示文摘Nuclease effects on the cell internalization of single-walled carbon nanotubes(SWNTs)functionalized with fluorescent-labeled DNA in serum containing cell growth media were examined.When Cy3-labeled DNA-functionalized SWNT conjugates(Cy3DNA-SWNTs)were incubated with HeLa cells in a fatal bovine serum(FBS)medium,a high fl uorescence intensity was obtained from the cells,indicative for the high level inclusion of Cy3DNA-SWNTs.However,the fluorescence intensity was remarkably reduced if Cy3DNA-SWNTs were incubated with cells in the FBS-free medium.Further systematic control experiments revealed that Cy3 dye molecules were released from Cy3DNA-SWNT conjugates by nuclease,and the free Cy3 dyes penetrate into HeLa cell with high efficiency.Although the actual amounts of SWNTs internalized in the cells were almost identical for both cells incubated in the FBS-present and FBS-absent media according to the Raman measurements,one should be cautious to determine the degree of SWNT internalization based on the fl uorescence intensities especially when the coloring dye molecules were linked to oligonucleotides in nuclease containing media. | Hye Kyung Moon Chan Il Chang Dong-Ki Lee Hee Cheul Choi | 2008 | Nano Research2008，1，4： | 7 |
| 7 | Electrical Stimulation of Auricular Acupressure for Dry Eye:A Randomized Controlled-Clinical Trial显示文摘Objective: To evaluate the clinical efficacy of electrical stimulation(ES) of auricular acupressure on reducing the ocular symptoms and signs before and after treatment for dry eye. Methods: The inclusion criteria were the tear film break-up time(TFBUT) below 5 s and a Schirmer test-Ⅰ below 5 mm in dry eyes with ocular symptoms for at least 6 months. Subjects were randomized into a treatment group(50 cases) with continuous low frequency ES under auricular acupressure at acupoints and a no ES under auricular acupressure(no-ES, control group, 50 cases) on the same acupoints. Auricular acupressure were stimulated with ES at 4 master points of both ears, which were performed twice a week for 4 weeks at each point for 30 s. The ocular symptoms, the TFBUT, and Schirmer test-Ⅰ were evaluated before and after this procedure. Results: There were significantly better scores in TFBUT(P=0.032), the Schirmer test-Ⅰ(P=0.044) and ocular symptoms(P=0.029) at 3 months post-treatment in the treatment group than in the control group. The total effective rate in the treatment group was accomplished in 41(82%) of the 50 cases of dry eye. | 8 | Age-related changes of the corticospinal tract in the human brain A diffusion tensor imaging study显示文摘The corticospinal tract(CST) is one of the most important neural tracts for motor function in the human brain.Little is known about age-related changes of the CST.In this study,we tried to evaluate age-related changes of the CST using diffusion tensor imaging in 60 healthy subjects.The diffusion tensor imaging result revealed that the tract number and fractional anisotropy value were decreased,and the apparent diffusion coefficient(ADC) value was increased with aging.The distribution showed a semilog pattern for tract number,fractional anisotropy and ADC of the CST,and the pattern of each graph was near-linear.When compared with the diffusion tensor imaging parameters of subjects in the 20 s age group,tract number and fractional anisotropy values were significantly decreased in the 50 s-70 s age groups.Likewise,the ADC value was significantly higher in the 50 s-70 s age groups.The CST in the brain of normal subjects degenerated continuously from the 20 s to the 70 s,with a near-linear pattern,and degeneration of the CST began to manifest significantly in the subjects in their 50 s,compared with the subjects in their 20 s. | Sung Ho Jang Sang-Hyun Cho Mi Young Lee Yong Hyun Kwon Min Cheul Chang | 2011 | Neural Regeneration Research2011，6，4： | 2 |
