1Multisciplinary management of patients with liver metastasis from colorectal cancer显示文摘Colorectal cancer(CRC) is one of the leading causes of cancer-related death. Surgery, radiotherapy and chemotherapy have been till now the main therapeutic strategies for disease control and improvement of the overall survival. Twenty-five per cent(25%) of CRC patients have clinically detectable liver metastases at the initial diagnosis and approximately 50% develop liver metastases during their disease course. Twentythirty per cent(20%-30%) are CRC patients with metastases confined to the liver. Some years ago various studies showed a curative potential for liver metastases resection. For this reason some authors proposed the conversion of unresectable liver metastases to resectable to achieve cure. Since those results were published, a lot of regimens have been studied for resectability potential. Better results could be obtained by the combination of chemotherapy with targeted drugs, such as anti-VEGF and antiEGFR monoclonal antibodies. However an accurate selection for patients to treat with these regimens and to operate for liver metastases is mandatory to reduce the risk of complications. A multidisciplinary team approach represents the best way for a proper patient management. The team needs to include surgeons, oncologists, diagnostic and interventional radiologists with expertise in hepatobiliary disease, molecular pathologists, and clinical nurse specialists. 2源于近端股骨间充质干细胞免疫抑制作用的研究显示文摘目的探讨源于成人近端股骨的间充质干细胞(MSC)是否具有免疫抑制功能,为临床移植物抗宿主病的防治寻找理想的细胞储备库。方法从全髋关节置换术患者的近端股骨中分离并培养MSC,将不同数量的细胞直接或以Transwell法加入混合淋巴细胞反应(MLR)中,检测MSC对外周血淋巴细胞(PBL)增殖水平的影响。以ELISA法及抗体中和法探讨细胞因子TGFβ1在MSC免疫调节中的作用。结果3×103、1×104、3×104、1×105个源于近端股骨的MSC对PBL增殖的抑制率分别为62%±18%、38%±4%、31%±16%、28%±12%,与采自髂骨骨髓穿刺物MSC的抑制作用相比差异无统计学意义(均P>0.05),后者对PBL的增殖抑制率为58%±12%、37%±8%、28%±6%、27%±6%;以0.2μm的微孔膜对MSC与MLR进行分隔,3×104及1×104个MSC对PBL的增殖抑制率为36%±8%和53%±12%;在ELISA实验中,MSC单独培养,MSC与MLR直接接触,或经Transwell共培养的这3种上清中转化生长因子(TGF)β1的表达水平差异无统计学意义,加入浓度递增的TGFβ1抗体不能逆转MSC对PBL增殖的抑制作用。结论源于近端股骨的MSC具有一定的免疫抑制功能,除TGFβ1外其他可溶性因子可能介导这一作用。张翼鷟 Loic Fouillard Alain Chapel Morad Bensidhoum Christelle Mazurier Aish Nasef Sandrine Bouchet Manuel Lopez Dominique Thierry Norbert Cclaucle Gorin 达万明 2005中华医学杂志2005,85,39:11

4Fifteen years of preclinical and clinical experiences about biotherapy treatment of lesions induced by accidental irradiation and radiotherapy显示文摘High dose radiation exposures involving medical treatments or accidental irradiation may lead to extended damage to the irradiated tissue. Alleviation or even eradication of irradiation induced adverse events is therefore crucial. Because developments in cell therapy have brought some hope for the treatment of tissues damages induced by irradiation, the Institute for Radiation and Nuclear Safety contributed to establish the clinical guidelines for the management of accidentally irradiated victims and to provide the best supportive care to patients all over the world. In the past 15 years, we contributed to develop and test cell therapy for protection against radiation side effects in several animalmodels, and we proposed mechanisms to explain the benefit brought by this new therapeutic approach. We established the proof of concept that mesenchymal stem cells (MSCs) migrate to damaged tissues in the nonobese diabetic/severe combined immunodeficiency immunotolerant mice model and in non-human primate after radiation exposure. We showed that the intravenous injection of MSCs sustains hematopoiesis after total body irradiation, improves wound healing after radiodermatitis and protects gut function from irradiation damages. Thanks to a tight collaboration with clinicians from several French hospitals, we report successful treatments of therapeutic/accidental radiation damages in several victims with MSC infusions for hematopoiesis correction, radio-induced burns, gastrointestinal disorders and protection homeostatic functions of gut management after radio-therapy.Alain Chapel Sabine Francois Luc Douay Marc Benderitter Jan Voswinkel 2013World Journal of Stem Cells2013,5,3:5
5Prediction of delayed graft function using different scoring algorithms: A single-center experience显示文摘AIM To compare the performance of 3 published delayed graftfunction(DGF) calculators that compute the theoretical risk of DGF for each patient.METHODS This single-center,retrospective study included 247 consecutive kidney transplants from a deceased donor.These kidney transplantations were performed at our institution between January 2003 and December 2012.We compared the occurrence of observed DGF in our cohort with the predicted DGF according to three different published calculators. The accuracy of the calculators was evaluated by means of the c-index(receiver operating characteristic curve).RESULTS DGF occurred in 15.3% of the transplants under study.The c index of the Irish calculator provided an area under the curve(AUC) of 0.69 indicating an acceptable level of prediction,in contrast to the poor performance of the Jeldres nomogram(AUC = 0.54) and the Chapal nomogram(AUC = 0.51). With the Irish algorithm the predicted DGF risk and the observed DGF probabilities were close. The mean calculated DGF risk was significantly different between DGF-positive and DGF-negative subjects(P < 0.0001). However,at the level of the individual patient the calculated risk of DGF overlapped very widely with ranges from 10% to 51% for recipients with DGF and from 4% to 56% for those without DGF.The sensitivity,specificity and positive predictive value of a calculated DGF risk ≥ 30% with the Irish nomogram were 32%,91% and 38%. 9New insights for pelvic radiation disease treatment:Multipotent stromal cell is a promise mainstay treatment for the restoration of abdominopelvic severe chronic damages induced by radiotherapy显示文摘Radiotherapy may induce irreversible damage on healthy tissues surrounding the tumor. It has been reported that the majority of patients receiving pelvic radiation therapy show early or late tissue reactions of graded severity as radiotherapy affects not only the targeted tumor cells but also the surrounding healthy tissues. The late adverse effects of pelvic radiotherapy concern 5% to 10% of them, which could be life threatening. However, a clear medical consensus concerning the clinical management of such healthy tissue sequelae does not exist. Although no pharmacologic interventions have yet been proven to efficiently mitigate radiotherapy severe side effects, few preclinical researches show the potential of combined and sequential pharmacological treatments to prevent the onset of tissue damage. Our group has demonstrated in preclinical animal models that systemic mesenchymal stromal cell(MSC) injection is a promising approach for the medical management of gastrointestinal disorder after irradiation. We have shown that MSCs migrate to damaged tissues and restore gut functions after irradiation.We carefully studied side effects of stem cell injection for further application in patients. 12Role of the complement cascade in cerebral aneurysm formation,growth,and rupture显示文摘Rupture of intracranial aneurysms is the most common cause of nontraumatic subarachnoid hemorrhage,but the intricate neuroinflammatory processes which contribute to aneurysm pathophysiology are not well‑understood.Mounting evidence has implicated the complement cascade in the progression of aneurysms from their formation to rupture.In this article,we identify and review studies that have sought to determine the role of the complement system in the aneurysm pathogenesis.The studies were generally conducted by immunhistological analyses on aneurysm tissue collected intraoperatively,and multiple components of the complement cascade and its modulators were identified in specific regions of the aneurysm wall.The results of the studies suggest that the complement cascade is locally upregulated and disinhibited in the perianeurysmal environment,and that it contributes to chronic as well as acute immunological damage to the aneurysm wall.In the future,understanding the mechanisms at work in complement‑mediated damage is necessary to leading the development of novel therapies.Blake E.S.Taylor Geoff Appelboom Robert Zilinyi Ariana Goodman David Chapel Melissa LoPresti Edward Sander Connolly Jr. 2015Neuroimmunology and Neuroinflammation2015,2,1:2
