| 1 | Primary endoscopic approximation suture under cap-assisted endoscopy of an ERCP-induced duodenal perforation显示文摘Duodenal perforation during endoscopic retrograde cholangiopancreatography(ERCP) is a rare complication,but it has a relatively high mortality risk.Early diagnosis and prompt management are key factors for the successful treatment of ERCP-related perforation.The management of perforation can initially be conservative in cases resulting from sphincterotomy or guide wire trauma.However,the current standard treatment for duodenal free wall perforation is surgical repair.Recently,several case reports of endoscopic closure techniques using endoclips,endoloops,or fully covered metal stents have been described.We describe four cases of iatrogenic duodenal bulb or lateral wall perforation caused by the scope tip that occurred during ERCP in tertiary referral centers.All the cases were simply managed by endoclips under transparent capassisted endoscopy.Based on the available evidence and our experience,endoscopic closure was a safe and feasible method even for duodenoscope-induced perforations.Our results suggest that endoscopists may be more willing to use this treatment. | Tae Hoon Lee Byoung Wook Bang Jee In Jeong Hyung Gil Kim Seok Jeong Seon Mee Park Don Haeng Lee Sang-Heum Park Sun-Joo Kim | 2010 | World Journal of Gastroenterology2010,16,18: | 19 |
| 2 | Does the bile duct angulation affect recurrence of choledocholithiasis?显示文摘AIM:To investigate whether bile duct angulation and T-tube choledochostomy influence the recurrence of choledocholithiasis.METHODS:We conducted a retrospective study including 259 patients who underwent endoscopic sphincterotomy and cholecystectomy for choledocholithiasis between 2000 and 2007.The imaginary line was drawn along the center of the bile duct and each internal angle was measured at the two angulation sites ofthe bile duct respectively.The values of both angles were added together.We then tested our hypothesis by examining whether T-tube choledochostomy was performed and stone recurrence occurred by reviewing each subject's medical records.RESULTS:The overall recurrence rate was 9.3% (24 of 259 patients).The mean value of sums of angles in the recurrence group was 268.3°± 29.6°,while that in the non-recurrence group was 314.8°± 19.9° (P < 0.05).Recurrence rate of the T-tube group was 15.9% (17 of 107),while that of the non T-tube group was 4.6% (7 of 152) (P < 0.05).Mean value of sums of angles after T-tube drainage was 262.5°± 24.6° and that before T-tube drainage was 298.0°± 23.9° in 22 patients (P < 0.05).CONCLUSION:The bile duct angulation and T-tube choledochostomy may be risk factors of recurrence of bile duct stones. | Dong Beom Seo Byoung Wook Bang Seok Jeong Don Haeng Lee Shin Goo Park Yong Sun Jeon Jung Il Lee Jin-Woo Lee | 2011 | World Journal of Gastroenterology2011,17,36: | 15 |
| 3 | Risk factors for local recurrence after en bloc endoscopic submucosal dissection for early gastric cancer显示文摘AIM: To investigate factors related to recurrence following en bloc resection using endoscopic submucosal dissection(ESD) in patients with early gastric cancer(EGC). METHODS: A total of 1121 patients(1215 lesions) who had undergone ESD for gastric neoplasia between April 2003 and May 2010 were retrospectively reviewed. Data from 401 patients(415 lesions) were analyzed, following the exclusion of those who underwent piecemeal resection, with deep resection margin invasion or lateral margin infiltration, and diagnosed with benign lesions. RESULTS: Local recurrence after en bloc ESD was found in 36 cases(8.7%). Unclear resection margins, long procedure times, and narrow safety margins were identified as risk factors for recurrence. Lesions located in the upper third of the stomach showed a higher rate of recurrence than those located in the lower third of the stomach(OR = 2.9, P = 0.03). The probability of no recurrence for up to 24 mo was 79.9% in those with a safety resection margin ≤ 1 mm and 89.5% in those with a margin > 1 mm(log-rank test, P = 0.03). CONCLUSION: Even in cases in which en bloc ESD is performed for EGC, local recurrence still occurs. To reduce local recurrences, more careful assessment will be needed prior to the implementation of ESD in casesin which the tumor is located in the upper third of the stomach. | 4 | Efficacy of cap-assisted colonoscopy according to lesion location and endoscopist training level显示文摘AIM: To evaluate the efficacy of cap-assisted colonoscopy(CAC) for detection of colorectal polyps and adenomas according to the lesion location and endoscopist training level.METHODS: Patients 20 years or older, who underwent their first screening colonoscopy in a single tertiary center from May 2011 to December 2012 were enrolled in this study. All patients underwent either CAC or standard colonoscopy(SC), and all of the procedures were performed by 11 endoscopists(8 trainees and 3 experts). All procedures were performed with highdefinition colonoscopes and narrow band imaging. The eight trainees had experiences of performing 150 to 500 colonoscopies, and the three experts had experiences of performing more than 3000 colonoscopies. A 4-mmlong transparent cap was attached to the end of a colonoscope in the CAC group. We retrospectively evaluated the number of polyps and adenomas, polyp detection rate(PDR), and the number of adenomas and adenoma detection rate(ADR) according to the lesion location and endoscopist training level between CAC and SC. We also evaluated the number of polyps and adenomas according to their size between CAC and SC.RESULTS: Overall, PDR and ADR using CAC were significantly higher than those using SC for both whole colon(48.5% vs 40.7%, P = 0.012; 35.7% vs 28.3%, P = 0.012) and right-side colon(35.3% vs 26.6%, P = 0.002; 27.0% vs 16.9%, P < 0.001). The number of polyps and adenomas per patient using CAC was significantly higher than that using SC for both the whole colon(1.07 ± 1.59 vs 0.82 ± 1.31, P = 0.008; 0.72 ± 1.32 vs 0.50 ± 1.01, P = 0.003) and right-side colon(0.66 ± 1.18 vs 0.41 ± 0.83, P < 0.001; 0.46 ± 0.97 vs 0.25 ± 0.67, P < 0.001). In the trainee group, the PDR and ADR using CAC were significantly higher than those using SC for both the whole colon(46.7% vs 39.7%, P = 0.040; 33.9% vs 26.0%, P =0.012) and right-side colon(34.2% vs 26.5%, P = 0.015; 25.3% vs 15.9%, P = 0.001). | 6 | Giant choledocholithiasis treated by mechanical lithotripsy using a gastric bezoar basket显示文摘Mechanical lithotripsy(ML) is usually considered as a standard treatment option for large bile duct stones.However,it is impossible to retrieve oversized stones because the conventional lithotripsy basket may not be able to grasp the stone.However,there is no established endoscopic extraction method for such giant stone removal.We describe a case of successful extraction of a 4-cm large stone using a gastric bezoar basket.A 78-year-old woman had suffered from upper abdominal pain for 20 d.Contrast-enhanced computed tomogram revealed a 4-cm single stone in the distal common bile duct(CBD).Endoscopic stone retraction was decided upon and endoscopic papillary balloon dilation was performed using a large balloon.An attempt to capture the stone using a standard lithotripsy basket failed due to the large stone size.Subsequently,we used a gastric bezoar basket to successfully capturethe stone.The stone was fragmented into small pieces and extracted.The stone was completely removed after two sessions of endoscopic retrograde cholangiopancreatography;each of which took 30 min.No complications occurred during or after the procedure.The patient was fully recovered and discharged on day 11 of hospitalization.ML using a gastric bezoar basket is a safe and effective retrieval method in select cases,and is considered as an alternative nonoperative option for the management of difficult CBD stones. | Hyun Jung Chung Seok Jeong Don Haeng Lee Jung Il Lee Jin-Woo Lee Byoung Wook Bang Kye Sook Kwon Hyung Kil Kim Yong Woon Shin Young Soo Kim | 2012 | World Journal of Gastroenterology2012,18,25: | 7 |
| 7 | Effectiveness of contrast-enhanced harmonic endoscopic ultrasound for the evaluation of solid pancreatic masses显示文摘AIM:To evaluate the usefulness of contrast-enhanced harmonic endoscopic ultrasound(CH-EUS)in differentiating between pancreatic adenocarcinomas and other pancreatic disease.METHODS:This retrospective cohort study evaluated90 patients who were seen between November 2010and May 2013.All these patients had solid pancreatic masses that had a hypoechoic appearance on EUS.All patients underwent CH-EUS to evaluate this diagnostic method’s usefulness.The mass lesions observed on CH-EUS were classified into three categories based on their echo intensity:hypoenhanced,isoenhanced,and hyperenhanced lesions.We adjusted the sensitivity and the specificity of each category for detecting malignancies.We also estimated the accuracy of CH-EUS by comparing it to a pathological diagnosis.RESULTS:Of the 90 patients,62 had a pancreatic adenocarcinoma.Fifty-seven out of 62 pancreatic adenocarcinomas showed a hypoenhanced pattern on CHEUS.The sensitivity was 92%,the specificity 68%and the accuracy approximately 82%.The area under the curve of the receiver operating characteristic analysis for CH-EUS was 0.799.There is a significant association between the hypoenhanced pattern on CH-EUS and pancreatic duct adenocarcinoma(χ2=35.264,P<0.001).In pathological examinations,the number of specimens for EUS-fine needle aspiration(EUS-FNA)was considered insufficient for diagnosis in three patients,and in two patients,the results were reported to be negative for malignancy.Pancreatic masses in all five patients revealed a hypoenhanced pattern with CH-EUS.Three patients were diagnosed with pancreatic adenocarcinoma based on the pathology results of a biopsy,and the remaining two patients were clinically diagnosed with malignancy.CONCLUSION:CH-EUS is useful for distinguishing between pancreatic adenocarcinoma and other pancreatic disease.When a pancreatic mass shows a hypoenhanced pattern on CH-EUS but involves either insufficient samples or negative results with EUS-FNA,clinicians might consider performing another pathologic diagnosis on the basis of an EUS-FNA sample or a biopsy. | Jin-Seok Park Hyung Kil Kim Byoung Wook Bang Sang Gu Kim Seok Jeong Don Haeng Lee | 2014 | World Journal of Gastroenterology2014,20,2: | 7 |
| 8 | Asymptomatic lymphangioma involving the spleen and retroperitoneum in adults显示文摘Lymphangioma,a benign neoplasm of the lymphatic system,is common in children but rare in adults.Its clinical manifestations include abdominal pain,nausea, vomiting and a palpable mass.However,abdominal sonography or abdominal computed tomography(CT) scan can also incidentally reveal lymphangioma.A larger or symptomatic lymphangioma is treated with total resection to prevent recurrence,infection,torsion and enlargement.Although lymphangioma rarely becomes malignant,its prognosis is generally good. | 10 | 4-1BB signaling activates glucose and fatty acid metabolism to enhance CD8^(+) T cell proliferation显示文摘4-1BB(CD137)is a strong enhancer of the proliferation of CD8^(+)T cells.Since these cells require increased production of energy and biomass to support their proliferation,we hypothesized that 4-1BB signaling activated glucose and fatty acid metabolism.We found that treatment with agonistic anti-4-1BB mAb promoted the proliferation of CD8^(+)T cells in vitro,increasing their size and granularity.Studies with a glycolysis inhibitor and a fatty acid oxidation inhibitor revealed that CD8^(+)T cell proliferation required both glucose and fatty acid metabolism.Anti-4-1BB treatment increased glucose transporter 1 expression and activated the liver kinase B1(LKB1)-AMP-activated protein kinase(AMPK)-acetyl-CoA carboxylase(ACC)signaling pathway,which may be responsible for activating the metabolism of glucose and fatty acids.We also examined whether blocking glucose or fatty acid metabolism affected cell cycle progression and the anti-apoptotic effect of 4-1BB signaling.The increase of anti-apoptotic factors and cyclins in response to anti-4-1BB treatment was completely prevented by treating CD8^(+)T cells with the fatty acid oxidation inhibitor,etomoxir,but not with the glycolysis inhibitor,2-deoxy-D-glucose.We conclude that anti-4-1BB treatment activates glucose and fatty acid metabolism thus supporting the increased demand for energy and biomass,and that fatty acid metabolism plays a crucial role in enhancing the cell cycle progression of anti-CD3-activated CD8^(+)T cells in vitro and the anti-apoptotic effects of 4-1BB signaling on these cells. | Beom K Choi Do Y Lee Don G Lee Young H Kim Seon-Hee Kim Ho S Oh Chungyong Han Byoung S Kwon | 2017 | Cellular & Molecular Immunology2017,14,9: | 6 |
| 11 | Cinnamon extract suppresses experimental colitis through modulation of antigen-presenting cells显示文摘AIM:To investigate the anti-inflammatory effects of cinnamon extract and elucidate its mechanisms for targeting the function of antigen presenting cells.METHODS:Cinnamon extract was used to treat murine macrophage cell line(Raw 264.7),mouse primary antigen-presenting cells(APCs,MHCII+) and CD11c+dendritic cells to analyze the effects of cinnamon extract on APC function.The mechanisms of action of cinnamon extract on APCs were investigated by analyzing cytokine production,and expression of MHC antigens and co-stimulatory molecules by quantitative real-time PCR and flow cytometry.In addition,the effect of cinnamon extract on antigen presentation capacity and APC-dependent T-cell differentiation were analyzed by [H3]-thymidine incorporation and cytokine analysis,respectively.To confirm the anti-inflammatory effects of cinnamon extract in vivo,cinnamon or PBS was orally administered to mice for 20 d followed by induction of experimental colitis with 2,4,6 trinitrobenzenesulfonic acid.The protective effects of cinnamon extract against experimental colitis were measured by checking clinical symptoms,histological analysis and cytokine expression prof iles in inflamed tissue.RESULTS:Treatment with cinnamon extract inhibited maturation of MHCII+ APCs or CD11c+ dendritic cells(DCs) by suppressing expression of co-stimulatory molecules(B7.1,B7.2,ICOS-L),MHCII and cyclooxygenase(COX)-2.Cinnamon extract induced regulatory DCs(rDCs) that produce low levels of pro-inflammatory cytokines [interleukin(IL)-1β,IL-6,IL-12,interferon(IFN)-γ and tumor necrosis factor(TNF)-α] while expressing high levels of immunoregulatory cytokines(IL-10 and transforming growth factor-β).In addition,rDCs generated by cinnamon extract inhibited APC-dependent T-cell proliferation,and converted CD4+ T cells into IL-10high CD4+ T cells.Furthermore,oral administration of cinnamon extract inhibited development and progression of intestinal colitis by inhibiting expression of COX-2 and pro-inflammatory cytokines(IL-1β,IFN-γ and TNF-α),while enhancing IL-10 levels.CONCLUSION:Our study suggests the potential of cinnamon extract as an anti-inflammatory agent by targeting the generation of regulatory APCs and IL-10+ regulatory T cells. | Ho-Keun Kwon Ji-Sun Hwang Choong-Gu Lee Jae-Seon So Anupama Sahoo Chang-Rok Im Won Kyung Jeon Byoung Seob Ko Sung Haeng Lee Zee Yong Park Sin-Hyeog Im | 2011 | World Journal of Gastroenterology2011,17,8: | 6 |

| 14 | Therapeutic effect of DA-9601 on chronic reflux gastritis induced by sodium taurocholate in rats显示文摘AIM: To investigate the therapeutic effects of DA-9601 on sodium taurocholate (TCA)-induced chronic reflux gastritis in SD rats.METHODS: In this study, we have investigated the therapeutic effects of DA-9601 on chronic erosive and atrophic gastritis induced by 6 mo of TCA administration (5 mmol/L in drinking water) in SD rats. RESULTS: Four weeks of DA-9601 administration (0.065%, 0.216% in rat chow), following the withdrawal of TCA treatment, resulted in a significant decrease in total length of erosions in rats in a dose-dependent manner. Furthermore, the indicators of atrophic gastritis, such as reduced mucosal thickness and reduction in the number of parietal cells, were improved by the administration of DA-9601 in a dose-related manner. DA-9601 also attenuated inflammatory cell infiltration and the proliferation of collagenous fiber in the gastric mucosa. The improvement in the reduction of the gastric mucus was observed in the rats receiving a high dose of DA-9601 (0.216%). The therapeutic effect of DA-9601 on experimental chronic erosive gastritis was superior to that of rebamipide (1.08% in rat chow). Biochemical analyses showed increased mucosal prostaglandin E2 and reduced glutathione levels by DA-9601 treatment. CONCLUSION: We suggest that DA-9601 is apromising agent for the treatment of chronic erosive and atrophic gastritis with an etiological factor of bile reflux. | 17 | Mitochondria as therapeutic targets for cancer stem cells显示文摘Cancer stem cells(CSCs) are maintained by theirsomatic stem cells and are responsible for tumor initiation, chemoresistance, and metastasis. Evidence for the CSCs existence has been reported for a number of human cancers. The CSC mitochondria have been shown recently to be an important target for cancer treatment, but clinical significance of CSCs and their mitochondria properties remain unclear. Mitochondriatargeted agents are considerably more effective compared to other agents in triggering apoptosis of CSCs, as well as general cancer cells, via mitochondrial dysfunction. Mitochondrial metabolism is altered in cancer cells because of their reliance on glycolytic intermediates, which are normally destined for oxidative phosphorylation. Therefore, inhibiting cancer-specific modifications in mitochondrial metabolism, increasing reactive oxygen species production, or stimulating mitochondrial permeabilization transition could be promising new therapeutic strategies to activate cell death in CSCs as well, as in general cancer cells. This review analyzed mitochondrial function and its potential as a therapeutic target to induce cell death in CSCs. | 20 | Plasma Levels of Monocyte Chemotactic Protein 3 and Beta-Nerve Growth Factor Increase with Amnestic Mild Cognitive Impairment显示文摘A number of studies have investigated peripheral inflammatory indices, including plasma cytokines and related molecules according to subtypes of dementia, but not in mild cognitive impairment (MCI). In this study, we used multiplex cytokine assay to assess the plasma levels of 22 cytokines in patients with MCI subtyped as amnestic and non-amnestic, according to cognitive features. When comparing the levels of plasma growth factors, chemokines and cytokines, plasma levels of monocyte chemotactic protein 3 (MCP-3), and beta-nerve growth factor (β-NGF) in these two groups, they were found to be significantly higher in amnestic MCI patients than in non-amnestic MCI patients, after adjusting for age and gender.
