| 1 | Intussusception in adults:Clinical characteristics,diagnosis and operative strategies显示文摘AIM:To evaluate 20 adults with intussusception and to clarify the cause, clinical features, diagnosis, and management of this uncommon entity.METHODS: A retrospective review of patients aged >18 years with a diagnosis of intestinal intussusception between 2000 and 2008. Patients with rectal prolapse, prolapse of or around an ostomy and gastroenterostomy intussusception were excluded. RESULTS: There were 20 cases of adult intussusception. Mean age was 47.7 years. Abdominal pain, nausea, and vomiting were the most common symptoms. The majority of intussusceptions were in the small intestine (85%). There were three (15%) cases of colonic intussusception. Enteric intussusception consisted of five jejunojejunal cases, nine ileoileal, and four cases of ileocecal invagination. Among enteric intussusceptions, 14 were secondary to a benign process, and in one of these, the malignant cause was secondary to metastatic lung adenocarcinoma. All colonic lesions were malignant. All cases were treated surgically. CONCLUSION: Adult intussusception is an unusual and challenging condition and is a preoperative diagnostic problem. Treatment usually requires resection of the involved bowel segment. | 2 | Effects of geoclimatic factors on the variability in Pinus pinea cone, seed, and seedling traits in Turkey native habitats显示文摘Background:Stone pine(Pinus pinea L.)is harvested for its edible kernels.It is an important forest tree species,and also plays an important role in afforestation.Successful afforestation activities strongly depend on the quality of seeds and seedlings.Five eastern native populations of stone pine were analyzed,in terms of their geographical and climatic parameters,in order to identify their cone,seed,pine nut,and seedling traits.With this regard,we aimed to contribute to the reforestation,yield,and breeding of this species.Results:All native populations occur in the sub-humid bioclimatic zone,according to Emberger’s bioclimatic classification,ranging 74.7-63.2 of Emberger’s Q Index value(Q),which synthesizes humidity and temperature,with lower values indicating more arid conditions.With regard to geographical factors,altitude was found to have the greatest effect on cone diameter and seed weight,while there was a weak correlation of cone,seed,and pine nut dimension with longitude and latitude.In terms of climatic factors,Q and mean of the maximum temperature of June,July,and August(ME)were found to be the main drivers behind significant effects in cone,seed,and pine nut morphometric traits.The most negative effect on seed number cone−1(r:−0.897),pine nut number cone−1(r:−0.923),pine nut yield(r:−0.903),and pine nut weight(r:−0.878)was Q,whereas cone diameter,seed length,and seed weight were positively correlated with the ME(r:0.939,r:0.889,and r:0.866,respectively).Cone and seed diameter and seed weight increased with decreasing summer drought index(r:−0.806,r:−0.846,and r:−0.866,respectively).Rainfall in the June of the third year prior to harvest time(RJ)had a negative effect on the percentage of sound seeds per cone.Annual rainfall in the third year prior to harvest time(AR)significantly affected pine nut weight(r:0.889),although rainfall during summer had a negative effect.The empty seed ratio of the different populations ranged from 20 to 68%,while considering all populations,the empty seed ratio was 46%.Conclusions:The study suggests that the Q Index and ME should be taken into account to determine cone to pine nut yield.With this regard,plantations of stone pine should be established on sub-humid areas and Q values should not exceed 75. | Safa Balekoglu Servet Caliskan Hüseyin Dirik | 2020 | Ecological Processes2020，9，1： | 4 |

| 5 | Reaching proficiency in laparoscopic splenectomy显示文摘AIM:To investigate the proficiency level reached in laparoscopic splenectomy using the learning curve method.METHODS:All patients in need of splenectomy for benign causes in whom laparoscopic splenectomy was attempted by a single surgeon during a time period of 6 years were included in the study (n=33). Besides demographics, operation-related variables and the response to surgery were recorded. The patients were allocated to groups of five, ranked according to the date of the operation. Operation duration, complications, postoperative length of stay, conversion to laparotomy and splenic weight were then compared between these groups.RESULTS: There was a significant difference regarding operation times between the groups (P = 0.001). An improvement was observed after the first 5 cases. The learning curve was flat up to the 25th case. Following the 25th case the operation times decreased still further. There was no difference between the groups regarding the other parameters.CONCLUSION: Unlike the widely accepted 'L' shape, the learning curve for laparoscopic splenectomy is a horizontal lazy 'S' with two distinct slopes. Privileges may be granted after the first 5 cases. | 6 | Isolation and diagnosis of Helicobacter pylori by a new method: Microcapillary culture显示文摘AIM:To investigate the performance of the microcapillary culture method(MCM) in Helicobacter pylori(H.pylori) isolation and diagnosis.METHODS:Microcapillary culture(MC),classical culture(CC),rapid urease(CLO) test,and histopathologic examination(HE) were performed with biopsy samples.Homogenized biopsy samples were loaded into capillary tubes and incubated for 48 h at 37 ℃ without providing a microaerophilic environment.Additionally,three or four loops of the homogenized sample were inoculated in a ready-to-use selective medium(Becton Dickinson,Helicobacter Agar,Modified) specific for the isolation of H.pylori and incubated at 37 ℃ in a microaerophilic atmosphere provided by Campy Gen(Becton Dickinson,Gas Pack).Bacteria reproducing in microcapillary tubes were evaluated in an inverted microscope and also were evaluated after performing a CC with the content.Results obtained by CC,CLO test,and HE were compared with those of MC.The diagnostic performances of the methods used in this study were evaluated for specificity,sensitivity,positive predictive value(PPV),negative predictive value(NPV),and CI.RESULTS:H.pylori was found positive by CLO test +HE and/or CC culture in 26 patient antrum and corpus biopsy samples.In 25(25/26) patient biopsy samples,H.pylori was isolated by MCM,whereas in only 14(14/26) patient biopsy samples,H.pylori was isolatedby CC.CLO test and HE were found positive in 17(17/26) patient biopsy samples.Comparing the results of the isolation of H.pylori by MCM,CC,CLO test,and HE,the sensitivity of the MCM was found as 96%,the specificity as 80%,the PPV as 83%,the NPV as 95%,and the 95%CI as 0.76(χ2 =31.51,P < 0.01) whereas the sensitivity of the CC was found as 54%(χ2 =19.15,P < 0.01),and the sensitivity of the CLO test and HE were found as 65%(χ2 =25.26,P < 0.01).CONCLUSION:This new microcapillary cultivation method for H.pylori has high diagnostic sensitivity compared with CC,HE,and CLO tests. | Adil M Allahverdiyev Melahat Bagirova Reyhan Caliskan Hrisi Bahar Tokman Hayat Aliyeva Gokce Unal Olga Nehir Oztel Emrah Sefik Abamor Hilal Toptas Pelin Yuksel Fatma Kalayci Mustafa Aslan Yusuf Erzin Kadir Bal Bekir S Kocazeybek | 2015 | World Journal of Gastroenterology2015，21，9： | 2 |
