| 1 | 不同年龄小儿穿孔性阑尾炎流行病学及临床表现差异分析显示文摘目的分析不同年龄段儿童急性穿孔性阑尾炎流行病学和临床表现的差异,为改进急性穿孔性阑尾炎的早期诊断提供理论依据。方法收集近20年本院收治的急性穿孔性阑尾炎患儿的临床资料。根据年龄分为新生儿及婴幼儿组(1d至3岁)、学龄前组(4~7岁)、小学年龄组(8~12岁)、青春期组(13~16岁)。统计穿孔性阑尾炎的性别比例和年龄分布。计算比较不同年龄段患儿的穿孔率。比较不同月份不同年龄段患儿穿孔性阑尾炎的发病情况。分析不同年龄段患儿穿孔性阑尾炎主要临床症状、体征、实验室及影像检查的差异。结果本院近20年共收治急性阑尾炎5859例,其中穿孔性阑尾炎1446例,男936例,女510例,男女比1.84:1,总穿孔率为24.68%。不同年龄段患儿阑尾炎穿孔率分别为:新生儿及婴幼儿组28.65%,学龄前组28.63%,小学年龄组22.64%,青春期组13.35%。穿孔性阑尾炎发病呈季节性分布,其中夏季7、8、9三个月发病率最高,而2、3、4三个月发病率最低。腹痛是各年龄组最常见症状,各年龄组发生率无明显差异(P〉0.05)。转移性右下腹痛仅出现于15.70%的患儿中,且随年龄增长发生率逐渐增加。发热、呕吐、厌食、腹泻等症状发生率随年龄增长逐渐降低,差异有统计学意义(P〈0.05)。而转移性右下腹痛和腹部压痛及肌紧张的发生率随年龄增长逐渐升高,差异有统计学意义(P〈0.01)。中性粒细胞比例新生儿及婴幼儿组低于小学年龄组和青春期组,C反应蛋白水平新生儿及婴幼儿组高于小学年龄组和青春期组,差异有统计学意义(P〈0.05)。腹部平片和B型超声显示青春期组肠梗阻征和肠壁增厚伴蠕动减少的发生率高于其他各组,B型超声和CT显示小学年龄组和青春期组阑尾粪石出现率高于新生儿及婴幼儿组和学龄前组,差异有统计学意义(P〈0.05)。结论阑尾炎穿孔率随年龄的变化而不同。穿孔性阑尾炎发病呈季节性分布。不同症状、体征在不同年龄组的发生率有显著差异。不同年龄组患儿中性粒细胞比例和C反应蛋白不同。部分影像学表现在不同年龄组患儿中出现率不等。 | 孙松 Bal Mukunda Basnet 郑珊 徐梦华 陈功 秦宏 | 2013 | 中华小儿外科杂志2013,34,11: | 35 |

| 3 | 全结肠巨结肠:十年诊疗经验与随访显示文摘目的对近10年来的全结肠巨结肠患儿进行回顾性研究,并对术前、术后并发症及随访情况进行评估。方法1996~2005年间我院共治疗25例全结肠巨结肠患儿,女8例,男17例。观察指标:年龄、临床表现、诊断方法、手术时年龄、回肠受累程度、全静脉营养时间、手术前后并发症及电话问卷调查随访。结果25例患儿中16例(64.0%)在新生儿期即表现为腹胀、呕吐及胎粪延迟排出。术前腹部平片或钡灌肠检查均不能提供确诊的特异性征象。25例均经手术探查、术中病理检查诊断为全结肠巨结肠,23例行回肠造瘘术,2例行Ⅰ期拖出根治手术。18例接受根治手术治疗(12例为改良Martin术;4例Ikeda-soper术;1例Rehbein,1例Soave拖出术);2例回肠造瘘术后因惊厥和败血症死亡;5例失随访。手术前后并发症包括:小肠结肠炎(术前44.4%,术后61.6%),肛周糜烂(77.8%),电解质失衡(50.0%),吻合口瘘(16.7%)。全静脉营养平均时间术前(17.77±12.54)d;术后(10.27±5.23)d。18例根治手术后10例获随访,平均随访时间(27.60±35.39)个月。7例患儿每天排便1~3次,2例每天5~6次,1例每天大便次数大于10次。所有患儿身高体重发育良好。结论全结肠巨结肠诊断较为困难、并发症多,随着对该病的进一步认识及积极治疗,疗效已有明显改善,长期预后良好。 | Basnet Anupama 赵瑞 郑珊 肖现民 | 2007 | 中华小儿外科杂志2007,28,3: | 13 |
| 4 | Cancer situation in China:what does the China cancer map indicate fromthe first national death survey to the latest cancer registration?显示文摘Background:Over the past four decades,the Chinese government has conducted three surveys on the distribution of causes of death and built cancer registration.In order to shine a new light on better cancer prevention strategies in China,we evaluated the profile of cancer mortality over the forty years and analyzed the policies that have been implemented.Methods:We described spatial and temporal changes in both cancer mortality and the ranking of major cancer types in China based on the data collected from three national surveys during 1973-1975,1990-1992,2004-2005,and the latest cancer registration data published by National Central Cancer Registry of China.The mortality data were compared after conversion to age-standardized mortality rates based on the world standard population(Segi’s population).The geographical distribution characteristics were explored by marking hot spots of different cancers on the map of China.Results:From 1973 to 2016,China witnessed an evident decrease in mortality rate of stomach,esophageal,and cervical cancer,while a gradual increase was recorded in lung,colorectal,and female breast cancer.A slight decrease of mortality rate has been observed in liver cancer since 2004.Lung and liver cancer,however,have become the top two leading causes of cancer death for the last twenty years.From the three national surveys,similar profiles of leading causes of cancer death were observed among both urban and rural areas.Lowermortality rates from esophageal and stomach cancer,however,have been demonstrated in urban than in rural areas.Rural areas had similar mortality rates of the five leading causes of cancer death with the small urban areas in 1973-1975.Additionally,rural areas in 2016 also had approximate mortality rates of the five leading causes with urban areas in 2004-2005.Moreover,stomach,esophageal,and liver cancer showed specific geographical distributions.Althoughmortality rates have decreased atmost of the hotspots of these cancers,they were still higher than the national average levels during the same time periods.Conclusions:Building up a strong primary public health system especially among rural areas may be one critical step to reduce cancer burden in China. | Ruimei Feng Qingling Su Xiaoyin Huang Til Basnet Xin Xu Weimin Ye | 2023 | Cancer Communications2023,43,1: | 10 |

| 7 | Genome-wide transcriptome analysis reveals molecular pathways involved in leafy head formation of Chinese cabbage(Brassica rapa)显示文摘Chinese cabbage plants go through seedling and rosette stages before forming their leafy head.Chinese cabbage plants resemble pak-choi plants at their seedling stage,but in their rosette stage the leaves of Chinese cabbage differentiate,as they increase in size with shorter petioles.In order to understand the molecular pathways that play a role in leafy head formation,transcript abundance of young emerging leaves was profiled during development of two Chinese cabbage genotypes and a single pak-choi genotype.The two Chinese cabbages differed in many aspects,among others earliness,leaf size and shape,leaf numbers,and leafy head shape.Genome-wide transcriptome analysis clearly separated the seedling stages of all three genotypes together with the later stages from pak-choi,from the later developmental stages of both Chinese cabbages(rosette,folding,and heading).Weighted correlation network analysis and hierarchical clustering using Euclidean distances resulted in gene clusters with transcript abundance patterns distinguishing the two Chinese cabbages from pak-choi.Three clusters included genes with transcript abundance affected by both genotype and developmental stage,whereas two clusters showed only genotype effects.This included a genotype by developmental stage cluster highly enriched with the MapMan category photosynthesis,with high expression during rosette and folding in Chinese cabbages and low expression in the heading inner leaves that are not exposed to light.The other clusters contained many genes in the MapMan categories Cell,showing again differences between pak-choi and both Chinese cabbages.We discuss how this relates to the differences in leaf blade growth between Chinese cabbage and pak-choi,especially at the rosette stage.Overall,comparison of the transcriptome between leaves of two very different Chinese cabbages with pak-choi during plant development allowed the identification of specific gene categories associated with leafy head formation. | XiaoXue Sun Ram Kumar Basnet Zhichun Yan Johan Bucher Chengcheng Cai Jianjun Zhao Guusje Bonnema | 2019 | Horticulture Research2019,6,1: | 4 |
| 8 | Progress Towards Genetics and Breeding for Minor Genes Based Resistance to Ug99 and Other Rusts in CIMMYT High-Yielding Spring Wheat显示文摘Wheat rusts continue to cause signif icant losses worldwide despite major efforts given to their genetic control.This is due to frequent evolution and selection of virulence in pathogen overcoming the deployed race-specific resistance genes.Although the life of effective race-specif ic resistance genes can be prolonged by using gene combinations,an alternative approach being implemented at CIMMYT is to deploy varieties that posses adult plant resistance(APR)based on combinations of minor,slow rusting genes.When present alone,the APR genes do not confer adequate resistance especially under high disease pressure;however,combinations of 4 or 5 minor genes usually result in'near-immunity'or a high level of resistance.Although only a few APR genes are catalogued,various APR QTLs are now known and could lead to further characterization of additional genes.Four characterized genes have pleiotropic effects in conferring partial APR to all 3 rusts and powdery mildew,thus simplifying the task of breeding wheat varieties that are resistant to multiple diseases.Significant progress was made recently in developing high-yielding wheat germplasm that possesses high levels of APR to all three rusts by implementing a MexicoKenya shuttle breeding scheme.Parents with APR to Ug99 were hybridized with high-yielding parents that had adequate to high levels of APR to leaf rust and yellow rust.Segregating populations and advanced lines from these crosses were selected under high rust pressures in Mexico(leaf rust and yellow rust)and Kenya(Ug99 stem rust and yellow rust)to identify highyielding progenies that possess high to adequate APR to all three rusts.International distribution of these high-yielding wheats is underway through CIMMYT international yield trials and screening nurseries.It is expected that several wheat varieties with APR to three rusts will be released and grown in various countries in the near-future that will allow determining the durability of resistance. | Ravi Prakash Singh Sybil Herrera-Foessel Julio Huerta-Espino Sukhwinder Singh Sridhar Bhavani Caixia Lan Bhoja Raj Basnet | 2014 | Journal of Integrative Agriculture2014,13,2: | 3 |
| 9 | 术前轻度认知功能障碍对早期术后认知功能障碍的预测价值显示文摘目的评估非神经、非心脏手术术前存在轻度认知功能障碍(MCI)的患者早期术后认知功能障碍(POCD)的发病率,明确MCI对术后发生早期POCD的预测价值。方法回顾性分析2018年9月至2021年6月在同济大学附属同济医院行非心脏及非神经外科手术的106例患者(>65岁)的病例资料。所有入组患者于手术前1 d和术后7 d进行神经心理学评估。排除4例简易智力状态检查量表(MMSE)评分<23分患者,将余研究对象随机分为建模组(n=87)和验证组(n=15)。根据MCI诊断标准,将建模组患者进一步分为MCI组和非MCI组,比较两亚组术后早期POCD的发病率。对POCD的危险因素进行logistic回归分析,构建发生早期POCD的预测模型。用验证组人群对预测模型进行外部验证,进而评估该预测模型的诊断价值。结果建模人群中,MCI组28例(32.2%),发生早期POCD者6例(21.4%);非MCI组59例(67.8%),发生早期POCD者16例(27.1%)。MCI组与非MCI组患者早期POCD的发生率差异无统计学意义。Logistic回归分析显示,数字符号替代测试(DSST)评分是该类患者发生早期POCD的独立危险因素(P=0.03)。建模组预测模型的曲线下面积(AUC)为0.883(95%CI 0.807~0.959),验证组预测模型的AUC为0.840(98%CI 0.583~1.000);校准曲线提示该预测模型具有较好的稳定性。结论对于接受非神经及非心脏手术的患者,术前存在MCI不影响早期POCD,但DSST评分降低时,应警惕POCD的发生。 | 张静亚 杜雪 Basnet Diksa 刘苏 刘健慧 | 2022 | 中国临床医学2022,29,1: | 2 |
