| 7 | Outcomes of gout in patients with cirrhosis:A national inpatient sample-based study显示文摘BACKGROUND Hyperuricemia is a prerequisite for the development of gout.Elevated serum uric acid(UA)levels result from either overproduction or decreased excretion.A positive correlation between serum UA levels,cirrhosis-related complications and the incidence of nonalcoholic fatty liver disease has been established,but it is unknown whether hyperuricemia results in worsening cirrhosis outcomes.We hypothesize that patients with cirrhosis will have poorer gout outcomes.AIM To explore the link between cirrhosis and the incidence of gout-related complications.METHODS This was a cross-sectional study.The national inpatient sample was used to identify patients hospitalized with gout,stratified based on a history of cirrhosis,from 2001 to 2013 via the International Classification of Diseases,Ninth Revision,Clinical Modification codes.Primary outcomes were mortality,gout complications and joint interventions.Theχ^(2)test and independent t-test were performed to assess categorical and continuous data,respectively.Multiple logistic regression was used to control for confounding variables.RESULTS Patients without cirrhosis were older(70.37±13.53 years vs 66.21±12.325 years;P<0.05).Most patients were male(74.63%in the cirrhosis group vs 66.83%;adjusted P<0.05).Patients with cirrhosis had greater rates of mortality(5.49%vs 2.03%;adjusted P<0.05),gout flare(2.89%vs 2.77%;adjusted P<0.05)and tophi(0.97%vs 0.75%;adjusted P=0.677).Patients without cirrhosis had higher rates of arthrocentesis(2.45%vs 2.21%;adjusted P<0.05)and joint injections(0.72%vs 0.52%;adjusted P<0.05).CONCLUSION Gout complications were more common in cirrhosis.Those without cirrhosis had higher rates of interventions,possibly due to hesitancy with performing these interventions given the higher complication risk in cirrhosis. | Ayham Khrais Aaron Kahlam Ali Tahir Amjad Shaikh Sushil Ahlawat | 2023 | World Journal of Hepatology2023,15,2: | 0 |
| 8 | Experimental Study on Volume Change Indices of Bentonite Soils显示文摘This paper reports experimental results regarding statically compacted clay specimens to study the volume change behavior of bentonites. The volume change indices such as the coefficients of compressibility, volume compressibility, and consolidation (i.e. av, mv and cv respectively) and the saturated coefficient of permeability k at different surcharge pressures were determined with the commonly adopted procedures. The swell potentials, swelling pressures, different phases of the swollen specimens were analyzed for the volume change behavior during compression. Experimental results revealed that the swell potential is dependent on the initial dry density, the initial water content and the vertical pressure at which the clay specimens were allowed to swell. The swelling pressure was found to be similar for the specimens with varying water content, showing strong dependency on the initial void ratio. The compression indices (viz. mv and av) of saturated specimens decreased with an increase in the vertical pressure. About 80% to 90% of the volume change occurred in the primary compression phase under any given vertical pressure. | 9 | A cost effective approach to design of energy efficient residential buildings显示文摘The objective of this study is to identify cost-optimal efficiency packages at several levels of building energy savings.A two-story residential building located in Jordan is selected as a case study.DesignBuilder software is used to predict the annual energy usage of a twostory residence in Irbid,Jordan.Real-time experimental data from a single isolated controlled room was used to verify the proposed model.In addition to energy analysis,the economic,environmental,and social benefits of the proposed design have been investigated.The sequential search optimization approach is used to estimate the minimum cost of the building while considering various design scenarios.In addition,the impact of various energy conservation techniques on residential buildings is assessed,and the payback period for each program is calculated.Ultimately,the optimal combination of design to achieve energy efficiency measures has been identified in several climate regions.The simulations results predict that the annual electricity consumption can be reduced up to 50%if the proper combinations of energy conservation measures are selected at the lowest cost.The payback period is 9.3 years.Finally,energy efficiency measures can lead to a total of 9470 jobs/year job opportunities.The study provide practical framework to link between energy performance criteria and economic goals of building.Linking the energy performance requirements to economic targets provides guidelines for homeowners,contractors,and policymakers for making a suitable decision regarding the retrofitting of existing residential buildings.The study focuses on developing new methodologies that support minimizing costs during a building’s lifecycle while maximizing environmental benefits which can not be identified by a series of parametric analyses using individual energy-efficient measures. | Khaled Bataineh Ayham Al Rabee | 2022 | Frontiers of Architectural Research2022,11,2: | 0 |
