1膝关节粘连:治疗和预防研究新进展显示文摘背景:膝关节粘连严重影响了患者术后的日常生活,而传统的治疗方式疗效有限。降低膝关节粘连发生率是提高手术预期疗效的关键,也是临床医学上需要急待解决的重要问题。目的:对膝关节粘连的各类研究文献进行分析,总结近10年的研究成果,为防治膝关节粘连提供新的思路和方法。方法:由第一作者检索Pub Med数据库,查找2006至2016年的文献,英文检索词为'tissue adhesions,arthrofibrosis,joint stiffness,prevent,treatment',共检索到1 500余篇相关文献,59篇符合纳入标准。结果与结论:(1)近年来大量学者研究发现,创伤/手术引起膝关节出血、炎症反应,造成血管内皮细胞生长因子和白细胞介素6高表达,增加了成纤维细胞的活性,导致大量胶原蛋白沉积,发生粘连;(2)改进传统治疗方式和运用微创技术治疗膝关节粘连取得了一定的疗效,但仍存在再粘连风险;(3)关节腔内用药预防膝关节粘连在基础研究方面获得了显著成果,可成为预防膝关节粘连的一种有效方案,但临床可行性存在争议,需进一步深入研究,寻找疗效确切、不良反应小的药物。吴海啸 王鹏 张超 Egiazaryan KA Ratyev AP Melnichenko SY Kuzin VV Skoroglyadov AV 2017中国组织工程研究2017,21,36:12
2Biobased polyurethanes for biomedical applications显示文摘Polyurethanes(PUs)are a major family of polymers displaying a wide spectrum of physico-chemical,mechanical and structural properties for a large range of fields.They have shown suitable for biomedical applications and are used in this domain since decades.The current variety of biomass available has extended the diversity of starting materials for the elaboration of new biobased macromolecular architectures,allowing the development of biobased PUs with advanced properties such as controlled biotic and abiotic degradation.In this frame,new tunable biomedical devices have been successfully designed.PU structures with precise tissue biomimicking can be obtained and are adequate for adhesion,proliferation and differentiation of many cell’s types.Moreover,new smart shape-memory PUs with adjustable shape-recovery properties have demonstrated promising results for biomedical applications such as wound healing.The fossil-based starting materials substitution for biomedical implants is slowly improving,nonetheless better renewable contents need to be achieved for most PUs to obtain biobased certifications.After a presentation of some PU generalities and an understanding of a biomaterial structure-biocompatibility relationship,recent developments of biobased PUs for non-implantable devices as well as short-and long-term implants are described in detail in this review and compared to more conventional PU structures.Sophie Wendels Luc Av´erous 2021Bioactive Materials2021,6,4:6
3Incretin manipulation in diabetes management显示文摘Incretin-based therapies have revolutionized the medical management of type 2 diabetes mellitus(T2DM) in the 21 st century. Glucagon-like peptide-1(GLP-1) suppresses appetite and gastric motility, and has trophic effects on pancreas, cardio-protective and renal effects. GLP-1 analogues and dipeptidyl peptidase-4 inhibitors form the incretin-based therapies. Significant reduction of hemoglobin A1 c when used as monotherapy and in combination regimens, favorable effects on body weight, and low risk of hypoglycemia are their unique therapeutic benefits. Their safety and tolerability are comparable to other anti-diabetic medications. Concern about elevated risk of pancreatitis has been discarded by two recent meta-analyses. 4骨骼肌缺血再灌注损伤及发病机制初探显示文摘目的 探讨骨骼肌缺血再灌注损伤过程的微循环变化、组织学改变以及多肽含量变化和意义。方法 42只雄性大鼠随机分为留伴行神经组和去伴行神经组 ,建立标准骨骼肌缺血再灌注模型。采用激光多谱勒及显微放大分析系统、组织学方法以及凝胶电泳方法等观察缺血再灌注损伤变化。结果 缺血再灌注损伤后的骨骼肌微血管管径在 2 0分钟时恢复率基本达到高峰约 6 0 % ,此后为平台期 :90分钟最高峰 ,为 75 %。主干血管流速率亦在 2 0分钟基本达到上限。病理检查显示 :缺血的骨骼肌纤维呈空泡状 ,核形态增大 ,染色加深 ,红细胞严重聚集。凝胶蛋白电泳提示 :缺血骨骼肌中分子量2 0KD左右的多肽显著增加。结论 骨骼肌缺血再灌注损伤发生后 ,骨骼肌的微循环发生不同程度的破坏 ,及因之导致骨骼结构损伤和异常的 2 0KD多肽含量明显增加 ,以及红细胞凝聚 ,白细胞的改变 。张俐 Seaber AV Urbaniak JR 2001中国骨伤2001,14,11:4

7β-TCP/α-CSH复合材料骨支架在骨缺损治疗中的应用研究进展显示文摘骨缺损的治疗是骨科面临的世界性难题之一,目前公认的治疗方法是通过骨组织工程支架进行修复填充。理想的骨组织工程支架应具备生物相容性、骨传导性、骨诱导性及降解性,同时具有合适的孔径、孔隙率和机械强度。单一材料由于其理化性质的局限性,无法满足临床需要。β-磷酸三钙(β-TCP)/α-半水硫酸钙(α-CSH)复合材料骨支架由于其在生物相容性、可吸收性及机械强度等方面的优势,在体内和体外试验中都取得了一定的效果,但β-TCP/α-CSH复合材料骨支架的机械强度会随α-CSH的完全降解而消失,因此目前其临床研究主要集中在颌面和口腔领域。β-TCP/α-CSH复合材料骨支架修复骨缺损是目前临床上较有潜力的治疗方案,其联合干细胞移植可能是未来提高临床骨缺损修复率的新方向。吴海啸 张超 Goryaeva GL Kuzin VV Skoroglyadov AV 2016山东医药2016,56,32:3
