| 1 | Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures. | Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena | 2011 | World Journal of Gastroenterology2011,17,11: | 28 |

| 3 | Resveratrol inhibits matrix metalloproteinases to attenuate neuronal damage in cerebral ischemia:a molecular docking study exploring possible neuroprotection显示文摘The main pathophysiology of cerebral ischemia is the structural alteration in the neurovascular unit, coinciding with neurovascular matrix degradation. Resveratrol has been reported to be one of the most potent chemopreventive agents that can inhibit cellular processes associated with ischemic stroke. Matrix metalloproteinases(MMPs) has been considered as a potential drug target for the treatment of cerebral ischemia. To explore this, we tried to investigate the interaction of resveratrol with MMPs through molecular docking studies. At 30 minutes before and 2 hours after cerebral ischemia/reperfusion induced by occlusion of the middle cerebral artery, 40 mg/kg resveratrol was intraperitoneally administered. After resveratrol administration, neurological function and brain edema were significantly alleviated, cerebral infarct volume was significantly reduced, and nitrite and malondialdehyde levels in the cortical and striatal regions were significantly decreased. The molecular docking study of resveratrol and MMPs revealed that resveratrol occupied the active site of MMP-2 and MMP-9. The binding energy of the complexes was –37.848672 k J/mol and –36.6345 k J/mol for MMP-2 and MMP-9, respectively. In case of MMP-2, Leu 164, Ala 165 and Thr 227 were engaged in H-Bonding with resveratrol and in case of MMP-9, H-bonding was found with Glu 402, Ala 417 and Arg 424 residues. | 4 | Effect of molasses binder on the physical and mechanical properties显示文摘Molasses was used as an alternative binder to the bentonite binder. The change in moisture absorption by pellets prepared with different iron ores and different molasses contents were investigated. Iron ore properties exerted the major effect on pellet behavior and final pellet quality. The absorbed moisture content of pellets prepared without binder, bentonite-added pellets, and molasses-added pellets were in the range of 7.72%–9.95%, 9.62%–10.84%, and 6.14%-6.69%, respectively. The wet pellet compressive strength of molasses-added pellets(43–230 N/pellet) was superior to that of bentonite-added pellets(9.47–11.92 N/pellet). | 8 | Laparoscopic liver resection:Wedge resections to living donor hepatectomy, are we heading in the right direction?显示文摘Despite inception over 15 years ago and over 3000 completed procedures, laparoscopic liver resection has remained mainly in the domain of selected centers and enthusiasts. Requirement of extensive open liver resection(OLR) experience, in-depth understanding of anatomy and considerable laparoscopic technical expertise may have delayed wide application. However healthy scepticism of its actual benefits and presence of a potential publication bias; concern about its safety and technical learning curve, are probably equally responsible. Given that a large proportion of our work, at least in transplantation is still OLR, we have attempted to provide an entirely unbiased, mature opinion of its pros and cons in the current invited review. We have dividedthis review into two sections as we believe they merit separate attention on technical and ethical grounds. The first part deals with laparoscopic liver resection(LLR) in patients who present with benign or malignant liver pathology, wherein we have discussed its overall outcomes; its feasibility based on type of pathology and type of resection and included a small section on application of LLR in special scenarios like cirrhosis. The second part deals with the laparoscopic living donor hepatectomy(LDH) experience to date, including its potential impact on transplantation in general. Donor safety, graft outcomes after LDH and criterion to select ideal donors for LLR are discussed. The recurrence of landslides, floods, and seismic activities makes it more susceptible to deformational activities. The tectonic analysis using geomorphic indices and morphometric parameters will help in determining the hazard-prone area of the river basin. Geomorphic indices and morphometric parameters are calculated to investigate the role of neotectonic activities, as it acts as a controlling factor in the development of landforms in the tectonically active terrains. Neotectonic activities influence the terrain topography, which significantly affects the drainage system and geomorphological setup of the area. In this study, the assessment of active tectonics of study area was determined using Advanced Spaceborne Thermal Emission and Reflection Radiometer(ASTER) Global Digital Elevation Model(GDEM) based on Geomorphic Indices(Stream Length Gradient index, Hypsometric integral, Asymmetry factor, Basin shape, Valley floor width to Valley height ratio, Mountain front sinuosity index) cumulatively with Linear, Areal and Relief morphometric parameters on 27 delineated basins of the study area. The combined classification of Relative Tectonic Activity Index(Iat) and morphometric parameters of 27 basins categorized all the zones into four different classes:Class 1 – Very High(<1.97;410 km^2);Class 2 – High(1.97 – 2.05;275 km^2);Class 3 – Moderate(2.05 – 2.21;273 km^2),and Class 4 – Low(>2.21;299 km^2). The basins with tectonic activities have a consistent relationship with structural disturbances, basin geometry, and field studies. Despite its importance as a major foodborne pathogen,our understanding of the molecular mechanisms underlying C. jejuni stress survival and pathogenesis is limited. Inorganic polyphosphate(poly P) has been shown to play significant roles in bacterial resistance to stress and virulence in many pathogenic bacteria. C. jejuni contains the complete repertoire of enzymes required for poly P metabolism. Recent work in our laboratory and others have demonstrated that poly P controls a plethora of C. jejuni properties that impact its ability to survive in the environment as well as to colonize/infect mammalian hosts. This review article summarizes the current literature on the role of poly P in C. jejuni stress survival and virulence and discusses on how poly P-related enzymes can be exploited for therapeutic/prevention purposes. | 20 | Evaluation of functional outcome and stability of sutureless scleral tunnel fixated IOLs in children with ectopia lentis显示文摘AIM:To evaluate functional outcome of sutureless scleral tunnel intraocular lens(SSTIOL)in children with crystalline lens subluxation of more than 7 clock hours.METHODS:A prospective interventional study was conducted consisting of 45 eyes of 44 children in age group 6-18 y having>7 clock hours of lens subluxation who underwent lensectomy-vitrectomy followed by SSTIOL implantation.Primary outcome was improvement in best corrected visual acuity(BCVA)and secondary outcomes were assessment of intraocular lens(IOL)tilt using ultrasound biomicroscopy(UBM),mean change in astigmatism at last follow-up of 1 y and associated complications.RESULTS:The mean preoperative and postoperative BCVA was 1.05±0.28 and 0.64±0.45(log MAR)respectively(P=0.001)at last follow-up.The mean astigmatism preoperatively and postoperatively was-4.17±2.69 D and-1.86±1.25 D respectively(P=0.011).Significant IOL tilt(>5 degrees)was present in 5 cases.The mean percentage endothelial loss was 3.65%±1.92%.The most serious complication encountered was retinal detachment seen in 2 cases.CONCLUSION:SSTIOL implantation provides efficient visual rehabilitation in children provided there is stringent case selection.We recommend caution in children having white-to-white distance>12 mm and presence of peripheral retinal degenerations. | Anju Rastogi Prateek Kumar Shweta Dhiman Manisha Mishra Kamlesh AnAND Ankita Bhardwaj | 2020 | International Journal of Ophthalmology(English edition)2020,13,1: | 2 |
