3New tools for optimizing fluid resuscitation in acute pancreatitis显示文摘Acute pancreatitis(AP)is a frequent disease with degrees of increasing severity responsible for high morbidity.Despite continuous improvement in care,mortality remains significant.Because hypovolemia,together with microcirculatory dysfunction lead to poor outcome,fluid therapy remains a cornerstone of the supportive treatment.However,poor clinical evidence actually support the aggressive fluid therapy recommended in recent guidelines since available data are controversial.Fluid management remains unclear and leads to current heterogeneous practice.Different strategies may help to improve fluid resuscitation in AP.On one hand,integration of fluid therapy in a global hemodynamic resuscitation has been demonstrated to improve outcomein surgical or septic patients.Tailored fluid administration after early identification of patients with high-risk of poor outcome presenting inadequate tissue oxygenation is a major part of this strategy.On the other hand,new decision parameters have been developed recently to improve safety and efficiency of fluid therapy in critically ill patients.In this review,we propose a personalized strategy integrating these new concepts in the early fluid management of AP.This new approach paves the way to a wide range of clinical studies in the field of AP.Perrine Bortolotti Fabienne Saulnier Delphine Colling Alban Redheuil Sebastien Preau 2014World Journal of Gastroenterology2014,20,43:5
4Metabolic syndrome and non-alcoholic fatty liver disease in liver surgery: The new scourges?显示文摘The aim of this topic highlight is to review relevant evidence regarding the influence of the metabolic syndrome(MS) and its associated liver manifestation, non-alcoholic fatty liver disease(NAFLD), on the development of liver cancer as well as their impact on the results of major liver surgery. MS and NAFLD, whose incidences are significantly increasing in Western countries, are leading to a changing profile of the patients undergoing liver surgery. A MEDLINE search was performed for relevant articles using the key words 'metabolic syndrome', 'liver resection', 'liver transplantation', 'non alcoholic fatty liver disease', 'non-alcoholic steatohepatitis' and 'liver cancer'. On one hand, the MS favors the development of primary liver malignancies(hepatocellular carcinoma and cholangiocarcinoma)either through NAFLD liver parenchymal alterations(steatosis, steatohepatitis, fibrosis) or in the absence of significant underlying liver parenchyma changes. Also, the existence of NAFLD may have a specific impact on colorectal liver metastases recurrence. On the other hand, the postoperative period following partial liver resection and liver transplantation is at increased risk of both postoperative complications and mortality. These deleterious effects seem to be related to the existence of liver specific complications but also higher cardio-vascular sensitivity in a setting of MS/NAFLD. Finally, the long-term prognosis after curative surgery joins that of patients operated on with other types of underlying liver diseases. An increased rate of patients with MS/NAFLD referred to hepatobiliary units has to be expected. 5Combining Human and Rodent Genetics to Identify New Analgesics显示文摘Most attempts at rational development of new analgesics have failed, in part because chronic pain involves multiple processes that remain poorly understood.To improve translational success, one strategy is to select novel targets for which there is proof of clinical relevance,either genetically through heritable traits, or pharmacologically. Such an approach by definition yields targets with high clinical validity. The biology of these targets can be elucidated in animal models before returning to the patients with a refined therapeutic. For optimal treatment, having biomarkers of drug action available is also a plus. Here we describe a case study in rational drug design: the use of controlled inhibition of peripheral tetrahydrobiopterin(BH4) synthesis to reduce abnormal chronic pain states without altering nociceptive-protective pain. Initially identified in a population of patients with low back pain, the association between BH4 production and chronic pain has been confirmed in more than 12 independent cohorts,through a common haplotype(present in 25% of Caucasians) of the rate-limiting enzyme for BH4 synthesis,GTP cyclohydrolase 1(GCH1). Genetic tools in mice have demonstrated that both injured sensory neurons and activated macrophages engage increased BH4 synthesis to cause chronic pain. GCH1 is an obligate enzyme for de novo BH4 production. Therefore, inhibiting GCH1 activity eliminates all BH4 production, affecting the synthesis of multiple neurotransmitters and signaling molecules and interfering with physiological function. In contrast, targeting the last enzyme of the BH4 synthesis pathway,sepiapterin reductase(SPR), allows reduction of pathological BH4 production without completely blocking physiological BH4 synthesis. Systemic SPR inhibition in mice has not revealed any safety concerns to date, and available genetic and pharmacologic data suggest similar responses in humans. Finally, because it is present in vivo only when SPR is inhibited, sepiapterin serves as a reliable biomarker of target engagement, allowing potential quantification of drug efficacy. 7Glycyrrhiza uralensis flavonoids inhibit brain microglial cell TNF-α secretion,p-IκB expression,and increase brain-derived neurotropic factor(BDNF)secretion显示文摘Objective:Asthma sufferers exhibit high prevalence of anxiety/depression.Elevated tumor-necrosis factor-alpha(TNF-α)levels in peripheral system and central nervous system(CNS)are associated with anxiety/depression,whereas brain-derived neurotropic factor(BDNF)has anti-depressant effects.An anti-asthma herbal medicine intervention ASHMI inhibits peripheral TNF-αsecretion in an animal model of asthma.We hypothesize that ASHMI and its compounds may have modulatory effects on CNS TNF-αand BDNF production.We sought to determine the effect of ASHMI and individual herb constituents on brain microglial cell TNF-αproduction,and identify the active compounds that suppress TNF-αand increase BDNF.Methods:BV-2 mouse microglial cells were pre-treated with ASHMI or extracts of Ganoderma lucidum(G.lucidum),Sophora flavescens Ait(S.flavescens),and Glycyrrhiza uralensis Fischer(G.uralensis),the herbal constituents in ASHMI,or individual compounds isolated from G.uralensis at different concentrations and then stimulated with LPS.TNF-αlevels in culture supernatants were measured by ELISA.The effect of active compounds on NFkB signaling pathway and on BDNF production were determined by western blotting and ELISA,respectively.Results:ASHMI produced dose-dependent inhibition of TNF-αsecretion by cultured-mousemicroglia BV2 cells.Of the three herb extracts in ASHMI,only G.uralensis significantly and dosedependently inhibited TNF-αproduction.Among the 5 flavonoids isolated from G.uralensis,isoliquiritigeninwas themost effective.Isoliquiritigenin suppression of TNF-αproduction was associated with attenuation of p-NF-kB expression,andwas accompanied by increased BDNF secretion.Conclusion:ASHMI and its effective flavonoid,isoliquiritigenin,inhibited TNF-αproduction by LPS stimulatedmicroglial cells and elevated BDNF levels,which may prove to have anti-CNS inflammatory and anti-anxiety effects.Sangita P.Patil Changda Liu Joseph Alban Nan Yang Xiu-Min Li 2014Journal of Traditional Chinese Medical Sciences2014,1,1:3
8Liver resection and metabolic disorders: An undescribed mechanism leading to postoperative mortality显示文摘AIM: To investigate the mechanism leading to perioperative mortality in patients undergoing major liver resection and presenting with metabolic disorders.METHODS: The link between Metabolic Syndrome and non-alcoholic fatty liver disease is currently demonstrated. Various metabolic disorders and the Metabolic Syndrome(the association of ≥ 3 metabolic disorders) have been recently described as a risk factor of perioperative mortality in major liver resection. Patients who passed away during perioperative course of major liver resection and presenting with the association of ≥ 2 metabolic disorders without any other known cause of liver disorders were reviewed.RESULTS: From January 2001 to May 2010 in a tertiary centre, ten patients presenting with ≥ 2 metabolic disorders without any other known cause of liver disorders died during perioperative course of major liver resection. The same four-consecutive-steps sequence of events occurred, including jaundice. The analysis of this series suggested a rapidly deteriorating congestive liver resulting in an increased portal hypertension leading to hepatorenal syndrome and lately to multiorgan failure(mimicking septic collapse) as the mechanism leading to exitus. The acute portal hypertension is mainly related to the surgical procedure. The chronic portal hypertension is indeterminate. Patients with ≥ 2 metabolic disorders should be considered as potentially presenting with portal hypertension possibly evolving towards hepatorenal syndrome; thus, they should be considered as having a high perioperative risk and should be carefully evaluated before undergoing major liver resection.CONCLUSION: As fibrosis was not present or marginal in liver specimens, the real cause of portal hypertension in patients with multiple metabolic disorders should be investigated with further studies.Alban Zarzavadjian Le Bian Renato Costi Mohamed Said Sbai-Idrissi Claude Smadja 2014World Journal of Gastroenterology2014,20,39:2

11Flood susceptibility modelling using advanced ensemble machine learning models显示文摘Floods are one of nature's most destructive disasters because of the immense damage to land,buildings,and human fatalities.It is difficult to forecast the areas that are vulnerable to flash flooding due to the dynamic and complex nature of the flash floods.Therefore,earlier identification of flash flood susceptible sites can be performed using advanced machine learning models for managing flood disasters.In this study,we applied and assessed two new hybrid ensemble models,namely Dagging and Random Subspace(RS)coupled with Artificial Neural Network(ANN),Random Forest(RF),and Support Vector Machine(SVM)which are the other three state-of-the-art machine learning models for modelling flood susceptibility maps at the Teesta River basin,the northern region of Bangladesh.The application of these models includes twelve flood influencing factors with 413 current and former flooding points,which were transferred in a GIS environment.The information gain ratio,the multicollinearity diagnostics tests were employed to determine the association between the occurrences and flood influential factors.For the validation and the comparison of these models,for the ability to predict the statistical appraisal measures such as Freidman,Wilcoxon signed-rank,and t-paired tests and Receiver Operating Characteristic Curve(ROC)were employed.The value of the Area Under the Curve(AUC)of ROC was above 0.80 for all models.For flood susceptibility modelling,the Dagging model performs superior,followed by RF,the ANN,the SVM,and the RS,then the several benchmark models.The approach and solution-oriented outcomes outlined in this paper will assist state and local authorities as well as policy makers in reducing flood-related threats and will also assist in the implementation of effective mitigation strategies to mitigate future damage.Abu Reza Md Towfiqul Islam Swapan Talukdar Susanta Mahato Sonali Kundu Kutub Uddin Eibek Quoc Bao Pham Alban Kuriqi Nguyen Thi Thuy Linh 2021Geoscience Frontiers2021,12,3:2
12在男吸烟者之中的白血球 DNA methylation 和 colorectal 癌症显示文摘 AIM: To explore the association between methylation in leukocyte DNA and colorectal cancer (CRC) risk in male smokers using the α-tocopherol, β-carotene cancer prevention study. METHODS: About 221 incident CRC cases, and 219controls, frequency-matched on age and smoking intensity were included. DNA methylation of 1505 CpG sites selected from 807 genes were evaluated using Il-lumina GoldenGate Methylation Cancer Panel I in prediagnostic blood leukocytes of study subjects. Tertiles of methylation level classified according to the distribution in controls for each CpG site were used to analyze the association between methylation level and CRC risk with logistic regression. The time between blood draw to cancer diagnosis (classifying cases according to latency) was incorporated in further analyses using proportional odds regression. RESULTS: We found that methylation changes of 31 CpG sites were associated with CRC risk at P<0.01 level. Though none of these 31 sites remained statistically significant after Bonferroni correction, the most statistically significant CpG site associated with CRC risk achieved a Pvalue of 1.0×10-4 . The CpG site is located in DSP gene, and the risk estimate was 1.52 (95% CI: 0.91-2.53) and 2.62 (95% CI: 1.65-4.17) for the second and third tertile comparing with the lowest tertile respectively. Taking the latency information into account strengthened some associations, suggesting that the methylation levels of corresponding sites might change over time with tumor progression. CONCLUSION: The results suggest that the methylation level of some genes were associated with cancer susceptibility and some were related to tumor development over time.
