| 1 | Portopulmonary hypertension and serum endothelin levels in hospitalized patients with cirrhosis显示文摘BACKGROUND:Cirrhosis is associated with several extrahepatic manifestations including portopulmonary hypertension (PPHT).Recent data suggest that endothelins (ETs) are related to the pathophysiology of PPHT.The study aimed to measure serum ET levels in hospitalized cirrhotic patients and to determine their association with PPHT and patient outcome.METHODS:Fifty-seven cirrhotic patients [43 males;median age 58 (28-87) years] underwent Doppler echocardiography.Patients with systolic pulmonary arterial pressure ≥40 mmHg and pulmonary acceleration time <100 ms were deemed to have PPHT.ET-1,2,and 3 serum levels were measured with an ELISA assay.All-cause mortality was recorded over a median period of 24 months.RESULTS:Nine out of 57 patients (15.8%) had PPHT.Among various clinical variables,only autoimmune hepatitis was associated with PPHT (OR=11.5;95% CI,1.58-83.4;P=0.01).ET-1 levels [9.1 (1.6-20.7) vs 2.5 (1.4-9.2) pg/mL,P=0.02] and the ET-1/ET-3 ratio [4.73 (0.9-22.4) vs 1.6 (0.3-10.7),P=0.02] were significantly higher in patients with PPHT than in those without.ET-2 and ET-3 levels did not differ between the two groups.There was no difference in survival between the two groups,although ET-1 levels were associated with an adverse outcome in Cox regression analysis (HR=1.11;95% CI,1.02-1.22;P=0.02 per unit increase in ET-1).CONCLUSION:Our data suggest that ET-1 and the ET-1/ET-3 ratio are elevated in patients with PPHT and that ET-1 is associated with a poor outcome irrespective of PPHT. | Aristotelis Tsiakalos Gregorios Hatzis Ioannis Moyssakis Aggelos Karatzaferis Panayiotis D Ziakas George E Tzelepis | 2011 | Hepatobiliary & Pancreatic Diseases International2011，10，4： | 7 |
| 2 | Hepatitis B virus reactivation in hepatitis B virus surface antigen negative patients receiving immunosuppression: A hidden threat显示文摘AIM: To present the characteristics and the course of a series of anti- hepatitis B virus core antibody (HBc) antibody positive patients, who experienced hepatitis B virus (HBV) reactivation after immunosuppression. METHODS: We retrospectively evaluated in our tertiary centers the medical records of hepatitis B virus surface antigen (HBsAg) negative patients who suffered from HBV reactivation after chemotherapy or immunosuppression during a 3-year period (2009-2011). Accordingly, the clinical, laboratory and virological characteristics of 10 anti-HBc (+) anti-HBs (-)/HBsAg (-) and 4 anti-HBc (+)/antiHBs (+)/HBsAg (-) patients, who developed HBV reactivation after the initiation of chemotherapy or immunosuppressive treatment were analyzed. Quantitative determination of HBV DNA during reactivation was performed in all cases by a quantitative real time polymerase chain reaction kit (COBAS Taqman HBV Test; cut-off of detection: 6 IU/mL). RESULTS: Twelve out of 14 patients were males; median age 74.5 years. In 71.4% of them the primary diagnosis was hematologic malignancy; 78.6% had received rituximab (R) as part of the immunosuppressive regimen. The median time from last chemotherapy schedule till HBV reactivation for 10 out of 11 patients who received R was 3 (range 2-17) mo. Three patients (21.4%) deteriorated, manifesting ascites and hepatic encephalopathy and 2 (14.3%) of them died due to liver failure. | 3 | Single-stage laparoscopic management of acute gallstone pancreatitis:outcomes at different timings显示文摘BACKGROUND:Definitive therapy for gallstone pancreatitis requires eradication of gallstones with cholecystectomy and common bile duct(CBD) clearance.Current guidelines recommend this be done within the same admission and preferably by laparoscopic cholecystectomy and CBD exploration.We report our experience of laparoscopic single-stage management with cholecystectomy and intraoperative cholangiogram followed by laparoscopic bile duct exploration(LBDE) when necessary performed at three different stages.METHODS:From January 1998 to December 2012,134 patients(100 females and 34 males) underwent single-stage laparoscopic management of gallstone pancreatitis.Patients were classified according to the timing of surgery:'A',≤7 days from symptom onset(n=27); 'B',8 to 30 days(n=58) and 'C',>30 days(n=49).RESULTS:LBDE was performed in 30 patients with a success rate of 100%.CBD stones were found in 25 patients(A:22.2%,B:22.4%,C:12.2%).CBD stones were more common in patients undergoing surgery within 30 days of presentation than after this time point(P=0.35).Multiple choledocholithiasis was more frequent in patients treated within 7 days(P=0.04).The 30-day mortality after surgery was 0,with no conversion to an open approach.Overall complication rate was 11.9%,which did not differ significantly between patients treated within 7 days or after this time point(P=0.83).CONCLUSIONS:This study demonstrated the feasibility and reproducibility of single-stage laparoscopic management of acute gallstone pancreatitis,which has a low complication rate at any stage.Patients undergoing early treatment have a higher incidence of choledocholithiasis and multiple stones than those treated after 30 days,supporting the passage of stones with time. | Antonio Navarro-Sanchez Hutan Ashrafian Aggelos Laliotis Kamran Qurashi Alberto Martinez-Isla | 2016 | Hepatobiliary & Pancreatic Diseases International2016，15，3： | 5 |

| 6 | Association of breastfeeding with tidal breathing analysis in infants with bronchiolitis显示文摘BACKGROUND Tidal breathing flow-volume(TBFV)analysis provides important information about lung mechanics in infants.AIM To assess the effects of breastfeeding on the TBFV measurements of infants who recover from acute bronchiolitis.METHODS In this cross-sectional study,TBFV analysis was performed in infants with bronchiolitis prior to hospital discharge.The ratio of time to peak expiratory flow to total expiratory time(tPEF/tE)at baseline and after the administration of 400 mcg salbutamol was evaluated.RESULTS A total of 56 infants(35 boys),aged 7.4±2.8 mo,were included.Of them,12.5%were exposed to tobacco smoke and 41.1%were breastfed less than 2 mo.There were no differences in baseline TBFV measurements between the breastfeeding groups;however,those who breastfed longer than 2 mo had a greater change in tPEF/tE after bronchodilation(12%±10.4%vs 0.9%±7.1%;P<0.001).Moreover,there was a clear dose-response relationship between tPEF/tE reversibility and duration of breastfeeding(P<0.001).In multivariate regression analysis,infants who breastfed less(regression coefficient-0.335,P=0.010)or were exposed to cigarette smoke(regression coefficient 0.353,P=0.007)showed a greater change in tPEF/tE after bronchodilation,independent of sex,prematurity,and family history of asthma or atopy.CONCLUSION Infants who recover from bronchiolitis and have a shorter duration of breastfeeding or are exposed to cigarette smoke,have TBFV measurements indicative of obstructive lung disease. | Evanthia Perikleous Sotirios Fouzas Athina Karageorgiou Paschalis Steiropoulos Evangelia Nena Athanasios Chatzimichael Aggelos Tsalkidis Emmanouil Paraskakis | 2021 | World Journal of Clinical Pediatrics2021，10，6： | 2 |
