18组织工程气管临床移植显示文摘背景失去正常气道会造成灾难性的后果,而大气道置换的各种尝试均遇到严重问题,组织工程气管置换的先决条件是该组织工程材料必须具备合适的基质、细胞、理想的机械性能而且无抗原性。本文中,作者旨在利用组织工程的原理和技术,通过生物工程气管基质的临床应用,评估该技术在一例患有终末期气道疾病患者中的应用情况。方法通过特定的技术去除供体一段气管的细胞和主要组织相容性复合体(MHC)抗原,将来源于受体(一名30岁患有终末期支气管软化的女性患者)的上皮细胞和间充质干细胞源性软骨细胞经细胞培养后种植于该段移植物基质中,然后用该移植物置换受体的左主支气管。结果术后该移植物立即建立了功能性气道,患者的生活质量得到了改善,在术后4个月时该段气管外观和机械性能保持正常,患者未产生抗供体的抗体,也未接受任何抗排异药物治疗。结论结果表明,该生物组织工程气管具有良好的机械性能,具备气管的正常功能,并且无免疫排异反应。这一结果提示自体细胞与适当生物材料结合的移植物可成功治疗某些严重的临床疾病。Paolo Macchiarini Philipp Jungebluth Tetsuhiko Go M Adelaide Asnaghi Louisa E Rees Tristan A Cogan Amanda Dodson Jaume Martorell Silvia Bellini Pier Paola Pamigotto Sally C Dickinson Anthony P Hollander Sara Mantero Maria Teresa Conconi Martin A Birchall 哈肖别克·卡斯木(译) 2009世界临床医学2009,3,2:0
