Developmental dysplasia of the hip in the newborn: A systematic review显示文摘Developmental dysplasia of the hip(DDH) denotes a wide spectrum of conditions ranging from subtle acetabular dysplasia to irreducible hip dislocations. Clinical diagnostic tests complement ultrasound imaging in allowing diagnosis, classification and monitoring of this condition. Classification systems relate to the alpha and beta angles in addition to the dynamic coverage index(DCI). Screening programmes for DDH show considerable geographic variation; certain risk factors have been identified which necessitate ultrasound assessment of the newborn. The treatment of DDH has undergone significant evolution, but the current gold standard is still the Pavlik harness. Duration of Pavlik harness treatment has been reported to range from 3 to 9.3 mo. The beta angle, DCI and the superior/lateral femoral head displacement can be assessed via ultrasound to estimate the likelihood of success. Success rates of between 7% and 99% have been reported when using the harness to treat DDH. Avascular necrosis remains the most devastating complication of harness usage with a reported rate of between 0% and 28%. Alternative non-surgical treatment methods used for DDH include devices proposed by LeD amany, Frejka, Lorenz and Ortolani. The Rosen splint and Wagner stocking have also been used for DDH treatment. Surgical treatment for DDH comprises open reduction alongside a combination of femoral or pelvic osteotomies. Femoral osteotomies are carried out in cases of excessive anteversion or valgus deformity of the femoral neck. The two principal pelvic osteotomies most commonly performed are the Salter osteotomy and Pemberton acetabuloplasty. Serious surgical complications include epiphyseal damage, sciatic nerve damage and femoral neck fracture.Vivek Gulati Kelechi Eseonu Junaid Sayani Nizar Ismail Chika Uzoigwe Muhammed Zaki Choudhury Pooja Gulati Adeel Aqil Saket Tibrewal 2013World Journal of Orthopedics2013,4,2:29
Role of nitric oxide in hepatic ischemia-reperfusion injury显示文摘Hepatic ischemia-reperfusion injury (IRI) occurs upon restoration of hepatic blood flow after a period of ischemia. Decreased endogenous nitric oxide (NO) production resulting in capillary luminal narrowing is central in the pathogenesis of IRI. Exogenous NO has emerged as a potential therapy for IRI based on its role in decreasing oxidative stress, cytokine release, leukocyte endothelial-adhesion and hepatic apoptosis. This review will highlight the influence of endogenous NO on hepatic IRI,role of inhaled NO in ameliorating IRI,modes of delivery, donor drugs and potential side effects of exogenous NO.Arunotai Siriussawakul Ahmed Zaky John D Lang 2010World Journal of Gastroenterology2010,16,48:14
Octreotide ameliorates gastric lesions in chronically mild stressed rats显示文摘AIM:To evaluate the effect of chronic mild stress(CMS) on the emergence of gastric ulcers and possible modulation by octreotide,a synthetic somatostatin analogue. METHODS:Adult male Wistar rats were subjected to nine different unpredictable random stress procedures for 21 d,a multifactorial interactional animal model for CMS.Octreotide was administered daily for 21 d at two dose levels(50 and 90μg/kg)before exposure to stress procedure.Macro-and microscopical assessments were made,in addition to quantification of plasma corticosterone and gastric mucosal inflammatory, oxidative stress, and apoptotic biomarkers. RESULTS:Exposure to CMS elevated plasma corticosterone(28.3±0.6μg/dL,P=0.002),an event that was accompanied by gastric lesions(6.4±0.16 mm,P=0.01) and confirmed histopathologically. Moreover, the insult elevated gastric mucosal lipid peroxides(13±0.5 nmol/g tissue,P=0.001),tumor necrosis factor-α(3008.6±78.18 pg/g tissue,P<0.001),prostaglandin E2(117.1 ±4.31 pg/g tissue,P=0.002),and caspase-3 activity (2.4±0.14 OD/mg protein,P=0.002).Conversely,CMS mitigated interleukin-10(627.9±12.82 pg/g tissue,P= 0.001).Furthermore,in animals exposed to CMS,octreotide restored plasma corticosterone(61%and 71%from CMS,P=0.002)at both dose levels. These beneficial effects were associated with a remarkable suppression of gastric lesions(38%and 9%from CMS,P=0.01)and reversal of derangements in gastric mucosa. CONCLUSION:The current investigation provides evidence that exposure to CMS induces gastric ulceration, which was alleviated by administration of octreotide possibly possessing antioxidant,anti-inflammatory,and anti-apoptotic actions.Noha N Nassar Mona F Schaalan Hala F Zaki Dalaal M Abdallah 2011World Journal of Gastroenterology2011,17,9:6
Safety and efficacy of photodynamic therapy using BCECF-AM compared to mitomycin C in controlling post-operative fibrosis in a rabbit model of subscleral trabeculectomy显示文摘AIM:To evaluate the safety and efficacy of cellular photoablation using BCECF-AM[2',7'-bis-(2-carboxyethyl)-5-(and-6)-carboxyfluorescein,acetoxymethyl ester mixed isomers]as a method to control postoperative fibrosis in subscleral trabeculectomy(SST) compared to mitomycin C(MMC) in a rabbit model.· METHODS:A comparative prospective case-control animal study was conducted.Fourteen rabbits were subjected to SST with intraoperative use of wound modulating agents(MMC or BCECF-AM) of the right eye(study groups Ⅰ and Ⅱ respectively) and SST without use of intraoperative wound modulating agents for the left eye(control group Ⅱ).Two rabbits 4 eyes were considered as control group Ⅰ with no surgical intervention.BCECF-AM was injected subconjunctivally30 min before surgery followed by intraoperative illumination with diffuse blue light for 10 min.Antifibrotic efficacy was established by clinical response and histological examination.Clinical response was assessed by measuring intraocular pressure(IOP) at day 1,3,5,7,14,21 postoperatively.Success was defined by >20.0%reduction in IOP from the preoperative values without anti-glaucoma medications.· RESULTS:The mean percentage of reduction was35.0%in the study group Ⅰ with only one eye(14.3%) had12.5%reduction.The mean percentage of reduction was28.0%in the study group Ⅱ with two eyes(28.6%) in study group Ⅱ had 14.2%reduction each.Regarding the control group Ⅱ,the mean percentage of reduction was14.3%with 64.3%eyes had <20.0%reduction.There was a highly statistically significant difference between each of the study groups(right eyes) and the corresponding control group Ⅱ(left eyes) as regards the mean postoperative IOP values started from day 5 in both study groups and this highly significant difference remained so till the end of the follow up period.Histologically,MMC treated blebs showed thinning of conjunctival epithelium with marked reduction of the goblet cells relative to control.Marked sub-epithelial edema was seen along with variable collagen dispersion.Mild cellularity was noted in sub-epithelial tissue.BCECF-AM treated blebs showed normal conjunctival epithelial thickness with abundant goblet cells.Mild subepithelial edema was noted along with moderate collagen dispersion. No histological abnormality was noted in the ciliary body or the cornea in any of the studied groups.· CONCLUSION:Cellular photoablation using BCECF-AM is a safe and effective wound modulating agent to control postoperative fibrosis in trabeculectomy. However MMC considered as a more potent adjuvant to trabeculectomy than BCECF-AM in promoting IOP reduction.Azza Mohamed Ahmed Said Rania Gamal Eldin Zaki Thanaa Helmy Mohamed Manal Ibraheem Salman 2016International Journal of Ophthalmology(English edition)2016,9,3:5
MatSciBERT:A materials domain language model for text mining and information extraction显示文摘A large amount of materials science knowledge is generated and stored as text published in peer-reviewed scientific literature. While recent developments in natural language processing, such as Bidirectional Encoder Representations from Transformers(BERT)models, provide promising information extraction tools, these models may yield suboptimal results when applied on materials domain since they are not trained in materials science specific notations and jargons. Here,we present a materials-aware language model, namely,MatSciBERT,trained on a large corpus of peer-reviewed materials science publications. We show that MatSciBERT outperforms SciBERT,a language model trained on science corpus, and establish state-of-the-art results on three downstream tasks, named entity recognition, relation classification, and abstract classification. We make the pre-trained weights of MatSciBERT publicly accessible for accelerated materials discovery and information extraction from materials science texts.Tanishq Gupta Mohd Zaki N.M.Anoop Krishnan Mausam 2022npj Computational Materials2022,,1:4
Nafamostat mesylate attenuates the pathophysiologic sequelae of neurovascular ischemia显示文摘Nafamostat mesylate,an apparent soi-disant panacea of sorts,is widely used to anticoagulate patients undergoing hemodialysis or cardiopulmonary bypass, mitigate the inflammatory response in patients diagnosed with acute pancreatitis, and reverse the coagulopathy of patients experiencing the commonly preterminal disseminated intravascular coagulation in the Far East. The serine protease inhibitor nafamostat mesylate exhibits significant neuroprotective effects in the setting of neurovascular ischemia.Nafamostat mesylate generates neuroprotective effects by attenuating the enzymatic activity of serine proteases, neuroinflammatory signaling cascades, and the endoplasmic reticulum stress responses, downregulating excitotoxic transient receptor membrane channel subfamily 7 cationic currents, modulating the activity of intracellular signal transduction pathways, and supporting neuronal survival brain-derived neurotrophic factor/TrkB/ERK1/2/CREB,nuclear factor kappa B.The effects collectively reduce neuronal necrosis and apoptosis and prevent ischemia mediated disruption of blood-brain barrier microarchitecture.Investigational clinical applications of these compounds may mitigate ischemic reperfusion injury in patients undergoing cardiac, hepatic, renal,or intestinal transplant, preventing allograft rejection, and treating solid organ malignancies.Neuroprotective effects mediated by nafamostat mesylate support the wise conduct of randomized prospective controlled trials in Western countries to evaluate the clinical utility of this compound.George Zaki Ghali Michael George Zaki Ghali 2020Neural Regeneration Research2020,15,12:4
Evaluation of DNA damage profile in obese women and its association to risk of metabolic syndrome, polycystic ovary syndrome and recurrent preeclampsia显示文摘Metabolic syndrome(MS)is a cluster of metabolic abnormalities.Obesity and MS are always accompanied by elevated oxidative stress which might affect cellular biomolecules such as DNA.The aim of the present study is to investigate DNA damage profile in obese premenopausal women and its relation to the risk of MS,polycystic ovary syndrome(PCOS)and history of recurrent pre-eclampsia.The study included 90 obese women included cases with MS(nZ30),PCOS(n Z30)and previous history of recurrent preeclampsia(nZ30)and,age-matched healthy non-obese control women(n Z 50).The assessment of leukocyte DNA damage was done by comet assay for all cases and controls.Anthropometry and biochemical parameters have been measured. Results showed that mean percent of DNA damage was significantly higher in MS,PCOS as well as in women with the recurrent preeclampsia as compared to healthy controls. The high level of mean DNA damage frequency in obese women was significantly associated with the increased number of metabolic syndrome components.Cases with 2,3 and 3e5 components showed significantly higher levels of DNA damage than controls. Moreover, cases with 3e5 MS components showed significant higher DNA compared to those with the two components.Regarding PCOS,significant positive association between the mean frequency of DNA damage and waist circumference was observed. The study suggests that metabolic abnormalities,PCOS and recurrent pre-eclampsia might be contributed in development of DNA damage in obese women.DNA damage can serve as an early marker for obesity complications in premenopausal women.Moushira Zaki Walaa Basha Hala T.El-Bassyouni Safinaz El-Toukhy Tamer Hussein 2018Genes & Diseases2018,5,4:4
Association of vitamin D receptor gene polymorphism(VDR)with vitamin D deficiency, metabolic and inflammatory markers in Egyptian obese women显示文摘Vitamin D deficiency might contribute to the pathogenesis of metabolic syndrome and could cause immune disturbance. The aim of this study is to analyze the associations between Vitamin D receptor(VDR)gene polymorphism, serum 25-hydroxy vitamin D,metabolic and inflammatory biomarkers in Egyptian obese women. The study included 201 obese women with vitamin D deficiency and 249 obese matched age healthy controls with sufficient vitamin D levels.Their age ranged between 25 and 35 years.Inflammatory biomarkers(interleukin-6 and C-reactive protein)and serum 25(OH)D were measured by enzyme-linked immunosorbent assay.Insulin resistance(IR)was determined by the homeostasis model assessment of insulin resistance(HOMA-IR).Vitamin D receptor(VDR)gene polymorphisms of FokI,ApaI,and TaqI were studied by PCR using the restriction fragment length polymorphism(RFLP)technique.Obese women with vitamin D deficiency had significant higher values of inflammatory and metabolic parameters compared to controls.Multivariable-logistic regression showed associations between 25(OH)D deficiency and metabolic components when comparing cases with controls. Moreover, cases carrying polymorphic alleles showed significant lower levels of serum 25(OH)D and higher HOMA-IR,blood pressure levels and lipid parameters compared to those with the wild type homozygote in obese cases with vitamin D deficiency.Vitamin D deficiency in Egyptian obese women with vitamin D deficiency is associated with abnormal metabolic components and abnormal inflammatory biomarkers. Moreover,VDR polymorphisms play important role in immune and inflammation status.Moushira Zaki Sanaa Kamal Walaa ABasha Eman Youness Wafaa Ezzat Hala El-Bassyouni Khalda Amr 2017Genes & Diseases2017,4,3:3
Recurrence of lymphangioleiomyomatosis: Nine years after a bilateral lung transplantation显示文摘Lymphangioleiomyomatosis(LAM) is a rare, slowly progressive lethal lung disease primary afflicting young women. LAM is characterized by proliferation of abnormal smooth muscle cells that target the lungs, causing cystic destruction and eventual respiratory failure leading to death. Recent ten year mortality due to end stage LAM has been reported to be approximately 10%-20%, but may vary. The decline in lung function in LAM is gradual, occurring at a rate of about 3% to 15% per year but can vary from patient to patient. But recently therapy with mammalian target of rapamycin(m TOR) inhibitors such as sirolimus has shown promising results in the stabilization of lung function and reduction of chylous effusions in LAM. Lung transplantation is a viable option for patients who continue to have decline in lung function despite m TOR therapy. Unique issues that may occur post-transplant in a recipient with LAM include development of chylous effusion and a risk of recurrence. We describe a case of LAM recurrence in a bilateral lung transplant recipient who developed histological findings of LAM nine years after transplantation.Khawaja S Zaki Zahra Aryan Atul C Mehta Olufemi Akindipe Marie Budev 2016World Journal of Transplantation2016,6,1:3
Epalrestat improves motor symptoms by reducing oxidative stress and inflammation in the reserpine induced mouse model of Parkinson’s disease显示文摘Background:Parkinson's disease(PD)is a progressive neurodegenerative disorder affecting a large number of elderly people worldwide. The current therapies for PD are symptom-based;they do not provide a cure but improve the quality of life.Muscular dysfunction is the hallmark clinical feature of PD and oxidative stress and inflammation play a critical role in its pathogenesis.Epalrestat is used for the treatment of diabetic neuropathy and is known to improve antioxidative defense mechanisms in the CNS.Therefore,in this study,we investigated the role of Epalrestat in the reserpine induced mouse model of PD.Method:We used Swiss Albino mice for the PD model and tested for akinesia/bradykinesia,muscular rigidity, palpebral ptosis, and tremor,as well as conducting swim and open field tests.Brain samples were used to determine oxidative stress parameters and infiltration of immune cells. Results:Epalrestat treatment significantly improved akinesia and bradykinesia, muscular dysfunctions, tremor level, and gait functions compared to the reserpine group. It also improved the latency in the swim test.Eplarestat significantly reduced lipid peroxidation and NO concentration in different brain tissues and increased the activity of antioxidative enzymes, glutathione, catalase, and superoxide dismutase.Furthermore,Epalrestat reduced neuroinflammation by reducing the number of infiltrating immune cells. Conclusion:Eplarestat improves muscular dysfunction in PD by reducing oxidative stress and inflammation.Md.Mahbubur Rahman Rupali Rani Chakraborti Md.Abdullah Potol Ariful Haque Abir Ozayra Sharmin Mahabub Alam Md.Fazlur Rahman Khan Rownock Afrin Humayra Jannat Rasiqh Wadud Zaki Farhad Habib 2020Animal Models and Experimental Medicine2020,3,1:3
Soybean Root System Architecture Trait Study through Genotypic,Phenotypic, and Shape-Based Clusters显示文摘We report a root system architecture(RSA)traits examination of a larger scale soybean accession set to study trait genetic diversity.Suffering from the limitation of scale, scope, and susceptibility to measurement variation,RSA traits are tedious to phenotype.Combining 35,448 SNPs with an imaging phenotyping platform,292 accessions(replications=14)were studied for RSA traits to decipher the genetic diversity.Based on literature search for root shape and morphology parameters,we used an ideotypebased approach to develop informative root(iRoot)categories using root traits. The RSA traits displayed genetic variability for root shape, length, number, mass, and angle.Soybean accessions clustered into eight genotype-and phenotype-based clusters and displayed similarity.Genotype-based clusters correlated with geographical origins.SNP profiles indicated that much of US origin genotypes lack genetic diversity for RSA traits, while diverse accession could infuse useful genetic variation for these traits.Shape-based clusters were created by integrating convolution neural net and Fourier transformation methods, enabling trait cataloging for breeding and research applications. The combination of genetic and phenotypic analyses in conjunction with machine learning and mathematical models provides opportunities for targeted root trait breeding efforts to maximize the beneficial genetic diversity for future genetic gains.Kevin G.Falk Talukder Zaki Jubery Jamie A.O’Rourke Arti Singh Soumik Sarkar Baskar Ganapathysubramanian Asheesh K.Singh 2020Plant Phenomics2020,2,1:3
Modeling bending behavior of shape memory alloy wire-reinforced composites: Semi-analytical model and finite element analysis显示文摘In this study,we propose a novel and simple exact semi-analytical model for superelastic Shape Memory Alloy(SMA)wire reinforced composites subjected to bending loads. In order to study the mechanical response of the composite during loading/unloading,a Representative Volume Element(RVE)is extracted to examine the bending response of the composite.Analytical moment–curvature, and shear force-shear strain relations are derived based on a 3-Dimensional(3 D)thermomechanical SMA model and Timoshenko beam theory. The composite Simpson’s rule is adopted to numerically solve the exact analytical moment–curvature and shear force-shear strain relationships. Results including the moment–curvature response, axial stress distribution along the vertical and longitudinal directions, martensite volume fraction, and the tip deflection are reported and validated against 3 D finite element simulations. The influence of temperature, martensite volume fraction distribution, and matrix stiffness on the mechanical performance of the composite is also investigated. In particular, the composite is found to behave superelastically under certain conditions of temperature,SMA volume fraction, and elastic stiffness of the matrix.Such behavior is advantageous in applications requiring large recoverable strains or high energy dissipation density.Nguyen Van VIET Wael ZAKI 2021Chinese Journal of Aeronautics2021,34,8:3
Improving the reconstruction efficiency of sparsity adaptive matching pursuit based on the Wilkinson matrix显示文摘Sparsity adaptive matching pursuit(SAMP)is a greedy reconstruction algorithm for compressive sensing signals.SAMP reconstructs signals without prior information of sparsity and presents better reconstruction performance for noisy signals compared to other greedy algorithms. However,SAMP still suffers from relatively poor reconstruction quality especially at high compression ratios. In the proposed research, the Wilkinson matrix is used as a sensing matrix to improve the reconstruction quality and to increase the compression ratio of the SAMP technique.Furthermore, the idea of block compressive sensing(BCS)is combined with the SAMP technique to improve the performance of the SAMP technique.Numerous simulations have been conducted to evaluate the proposed BCS-SAMP technique and to compare its results with those of several compressed sensing techniques.Simulation results show that the proposed BCS-SAMP technique improves the reconstruction quality by up to six decibels(d B)relative to the conventional SAMP technique. In addition, the reconstruction quality of the proposed BCS-SAMP is highly comparable to that of iterative techniques. Moreover, the computation time of the proposed BCS-SAMP is less than that of the iterative techniques, especially at lower measurement fractions.Rasha SHOITAN Zaki NOSSAIR I.I.IBRAHIM Ahmed TOBAL 2018Frontiers of Information Technology & Electronic Engineering2018,19,4:3
Liver transplantation as a management of hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC) is one of the most common cancers worldwide and has a poor prognosis if untreated. It is ranked the third among the causes of cancer-related death. There are multiple etiologic factors that can lead to HCC. Screening for early HCC is challenging due to the lack of well specific biomarkers. However, early diagnosis through successful screening is very important to provide cure rate. Liver transplantation(LT) did not gain wide acceptance until the mid-1980 s,after the effective immunosuppression withcyclosporine became available. Orthotopic LT is the best therapeutic option for early, unresectable HCC. It is limited by both, graft shortage and the need for appropriate patient selection. It provides both, the removal of tumor and the remaining cirrhotic liver. In Milan,a prospective cohort study defined restrictive selection criteria known as Milan criteria(MC) that led to superior survival for transplant patients in comparison with any other previous experience with transplantation or other options for HCC. When transplantation occurs within the established MC,the outcomes are similar to those for nonmalignant liver disease after transplantation. The shortage of organs from deceased donors has led to the problems of long waiting times and dropouts. This has led to the adoption of extended criteria by many centers. Several measures have been taken to solve these problems including prioritization of patients with HCC,use of pretransplant adjuvant treatment, and living donor LT.Ayman Zaki Azzam 2015World Journal of Hepatology2015,7,10:3

Sensitivity analysis of using diethanolamine instead of methyldiethanolamine solution for GASCO’S Habshan acid gases removal plant显示文摘Sweeting natural gas processes are mainly focused on removing carbon dioxide (CO2) and hydrogen sulfide (H2S). The high-energy requirements and opera-tional limitations make amine absorption process sensitive to any change in conditions. This paper presented a steady- state simulation using Hysys to reasonably predict removal amounts of carbon dioxide and hydrogen sulfide from natural gas with the diethanolamine (DEA) solvent. The product specifications are taken from the real plant (GASCO5S Habshan) which uses the methyldiethanola-mine (MDEA) solvent, while this simulation uses DEA under the same operation conditions. First, the simulation validation has been checked with the data of the real plant. The results show accurate prediction for CO2 slippage and accepted agreement for H2S content compared with the data of the plant. A parametric analysis has been performed to test all possible parameters that affect the performance of the acid gases removal plant. The effects of operational parameters are examined in terms of carbon dioxide and hydrogen sulfide contents in clean gas and reboiler duty.Samah Zaki NAJI Ammar Ali ABD 2019Frontiers in Energy2019,13,2:2

Role of macrophages and monocytes in hepatitis C virus infections显示文摘A number of studies conducted over many years have shown that hepatitis C virus(HCV)can infect a variety of cell types. In vivo infection of monocytes, macrophages, and dendritic cells by HCV has been frequently shown by a number of researchers. These studies have demonstrated replication of HCV by detecting the presence of both negative genomic strands and a variety of non-structural HCV proteins in infected cells. In addition, analyses of genome sequences have also shown that different cell types can harbor different HCV variants.Investigators have also done preliminary studies of which cellular genes are affected by HCV infection, but there have not yet been a sufficient number of these studies to understand the effects of infection on these cells.Analyses of in vitro HCV replication have shown that monocytes, macrophages and dendritic cells can be infected by HCV from patient sera or plasma. These studies suggest that entry and cellular locations may vary between different cell types. Some studies suggest that macrophages may preferentially allow HCV genotype 1 to replicate, but macrophages do not appear to select particular hypervariable regions.Overall, these studies agree with a model where monocytes and macrophages act as an amplification system,in which these cells are infected and show few cytopathic effects, but continuously produce HCV.This allows them to produce virus over an extended time and allows its spread to other cell types.Dennis Revie Syed Zaki Salahuddin 2014World Journal of Gastroenterology2014,20,11:2
Long non-coding RNA(lncRNA):A potential therapeutic target in acute lung injury显示文摘Acute Lung Injury(ALI)and its severe form Acute Respiratory Distress Syndrome(ARDS)are the major cause of ICU death worldwide.ALI/ARDS is characterized by severe hypoxemia and inflammation that leads to poor lung compliance.Despite many advances in understanding and management,ALI/ARDS is still causing significant morbidity and mortality.Long non-coding RNA(lncRNA)is a fast-growing topic in lung inflammation and injury.lncRNA is a class of non-coding RNA having a length of more than 200 nucleotides. It has been a center of research for understanding the pathophysiology of various diseases in the past few years.Multiple studies have shown that lncRNAs are abundant in acute lung injury/injuries in mouse models and cell lines.By targeting these long non-coding RNAs,many investigators have demonstrated the alleviation of ALI in various mouse models. Therefore, lncRNAs show great promise as a therapeutic target in ALI.This review provides the current state of knowledge about the relationship between lncRNAs in various biological processes in acute lung injury and its use as a potential therapeutic target.Almaz Zaki M.Shadab Ali Vijay Hadda Syed Mansoor Ali Anita Chopra Tasneem Fatma 2022Genes & Diseases2022,9,5:2
The role of graft reperfusion sequence in the development of non-anastomotic biliary strictures following orthotopic liver transplantation: A meta-analysis显示文摘Background: Liver transplant is a potential cure for liver failure and hepatic malignancy but there are many techniques which have been described for vascular reconstruction. This study was to compare the prevalence of non-anastomotic biliary stricture and other surgical complications based on Clavien-Dindo scoring system, in initial portal reperfusion(sequential) versus simultaneous or initial artery reperfusion. Data sources: Meta-analysis of published studies comparing the outcomes of both techniques was carried out. Data search was conducted across the major databases and studies were selected under the guidance of the Cochrane guidelines for systematic reviews and meta-analysis. Results: Seven studies were included to address the primary and the secondary outcomes. No statistical difference was found in the incidence of non-anastomotic biliary strictures(OR = 0.40; P = 0.14), regardless of reperfusion technique. The pooled estimate of the Clavien-Dindo grading of complications was not significantly different between the techniques, though Clavien-Dindo II complications were higher in the simultaneous or initial artery reperfusion group than the initial portal reperfusion group(OR = 2.73; P = 0.01). Similarly, there was no difference in the operative time, hospital stay and other outcomes addressed in this report. Conclusions: The available evidence suggests that there is no significant difference demonstrated in the rate of non-anastomotic biliary strictures or other complications, between the two techniques, except for Clavien-Dindo II complications.Mohamed Bekheit Matteo Catanzano Stuart Shand Irfan Ahmed ELSaid ELKayal Gihan Mohamed Shehata Adel Zaki 2019Hepatobiliary & Pancreatic Diseases International2019,18,1:2
