1Landscape of DILI-related adverse drug reaction in China Mainland显示文摘Drug-induced liver injury(DILI)is a type of bizarre adverse drug reaction(ADR)damaging liver(L-ADR)which may lead to substantial hospitalizations and mortality.Due to the general low incidence,detection of L-ADR remains an unsolved public health challenge.Therefore,we used the data of 6.673 million of ADR reports from January 1st,2012 to December 31st,2016 in China National ADR Monitoring System to establish a new database of L-ADR reports for future investigation.Results showed that totally 114,357 ADR reports were retrieved by keywords searching of liver-related injuries from the original heterogeneous system.By cleaning and standardizing the data fields by the dictionary of synonyms and English translation,we resulted 94,593 ADR records reported to liver injury and then created a new database ready for computer mining.The reporting status of L-ADR showed a persistent 1.62-fold change over the past five years.The national population-adjusted reporting numbers of L-ADR manifested an upward trend with age increasing and more evident in men.The annual reporting rate of L-ADR in age group over 80 years old strikingly exceeded the annual DILI incidence rate in general population,despite known underreporting situation in spontaneous ADR reporting system.The percentage of herbal and traditional medicines(H/TM)L-ADR reports in the whole number was 4.5%,while 80.60%of the H/TM reports were new findings.There was great geographical disparity of reported agents,i.e.more cardiovascular and antineoplastic agents were reported in higher socio-demographic index(SDI)regions and more antimicrobials,especially antitubercular agents,were reported in lower SDI regions.In conclusion,this study presented a large-scale,unbiased,unified,and computer-minable L-ADR database for further investigation.Age-,sex-and SDI-related risks of L-ADR incidence warrant to emphasize the precise pharmacovigilance policies within China or other regions in the world.Jiabo Wang Haibo Song Feilin Ge Peng Xiong Jing Jing Tingting He Yuming Guo Zhuo Shi Chao Zhou Zixin Han Yanzhong Han Ming Niu Zhaofang Bai Guangbin Luo Chuanyong Shen Xiaohe Xiao 2022Acta Pharmaceutica Sinica B2022,12,12:12
2Strategy for efficient cloning of biosynthetic gene clusters from fungi显示文摘Filamentous fungi are excellent sources for the production of a group of bioactive small molecules which are often called secondary metabolites(SMs). The advanced genome sequencing technology combined with bioinformatics analysis reveals a large number of unexplored biosynthetic gene clusters(BGCs) in the fungal genomes. To unlock this fungal SM treasure, many approaches including heterologous expression are being developed and efficient cloning of the BGCs is a crucial step to do this.Here, we present an efficient strategy for the direct cloning of fungal BGCs. This strategy consisted of Splicing by Overlapping Extension(SOE)-PCR and yeast assembly in vivo. By testing 14 BGCs DNA fragments ranging from 7 kb to 52 kb, the average positive rate was over 80%. The maximal insertion size for fungal BGC assembly was 52 kb. Those constructs could be used conveniently for the heterologous expression leading to the discovery of novel natural products. 3Impact of age and tumor size on the development of the Kasabach-Merritt phenomenon in patients with kaposiform hemangioendothelioma:a retrospective cohort study显示文摘Introduction The Kasabach–Merritt phenomenon(KMP)is a severe complication of kaposiform hemangioendothelioma(KHE).The risk factors for KMP need further investigation.Methods The medical records of patients with KHE were reviewed.Univariate and multivariate logistic regression models were used for the risk factors for KMP,and the area under the receiver operator characteristic(ROC)curve was used to assess the predictive power of risk factors.Results A total of 338 patients with KHE were enrolled.The incidence of KMP was 45.9%.Age of onset(P<0.001,odds ratio[OR]0.939;95%confidence interval[CI]0.914–0.966),lesion size(P<0.001,OR 1.944;95%CI 1.646–2.296),mixed type(P=0.030,OR 2.428;95%CI 1.092–5.397),deep type(P=0.010,OR 4.006;95%CI 1.389–11.556),and mediastinal or retroperitoneal lesion location(P=0.019,OR 11.864;95%CI 1.497–94.003)were correlated with KMP occurrence through multivariate logistic regression.ROC curve analysis revealed that the optimal cutoffs were 4.75 months for the age of onset(P<0.001,OR 7.206,95%CI 4.073–12.749)and a lesion diameter of 5.35 cm(P<0.001,OR 11.817,95%CI 7.084–19.714).Bounded by a lesion size of 5.35 cm,we found significant differences in tumor morphology,age of onset,treatments,and hematological parameters.Using an onset age of 4.75 months as a cutoff,we found significant differences in tumor morphology,lesion size,hematological parameters,and prognosis.Conclusion For KHE patients with an onset age<4.75 months and/or lesion diameter>5.35 cm,clinicians should be wary of the occurrence of KMP.Active management is recommended to improve the prognosis.Jiangyuan Zhou Yuru Lan Tong Qiu Xue Gong Zixin Zhang Chunshui He Qiang Peng Fan Hu Xuepeng Zhang Guoyan Lu Liqing Qiu Feiteng Kong Yongbo Zhang Siyuan Chen Yi Ji 2023Precision Clinical Medicine2023,6,2:0

5Design and comparative analysis of self-propelling drill bit applied to deep-sea stratum drilling robot显示文摘Robotic subsea stratum drilling robot is a method for new subsea stratigraphic geological investigation and resource exploration.Resistance at the front end is the main source of resistance to the robot’s motion in the strata.Since there is no continuous and strong downward drilling force as in conventional drilling rigs,robot movement relies heavily on the drill bit to reduce the drilling resistance.In this study we propose a self-propelling drill bit that can discharge soil debris to provide propulsive force and reduce the resistance.The key parameter of the drill bit design,the spiral blade lead angle,was determined by theoretical analysis of the drill bit’s soil discharging effect.To verify the structural advantages of the self-propelling drill bit in reducing resistance,a comparative analysis with a conventional conical drill bit was conducted.The drilling process of both bits was simulated using finite element simulation at various rotation speeds,the penetration force and torque data of both drill bits were obtained,and tests prepared accordingly in subsea soil were conducted.The simulations and tests verified that the penetration force of the self-propelling drill bit was lower than that of the conventional conical drill bit.The self-propelling drill bit can reduce the resistance effectively,and may play an important role in the stratum movement of drilling robots.Peihao ZHANG Xingshuang LIN Hao WANG Jiawang CHEN Zhenwei TIAN Zixin WENG Ziqiang REN Peng ZHOU 2023Journal of Zhejiang University-Science A(Applied Physics & Engineering)2023,24,10:0
6Relationship Between Gene-Phenotype and Clinical Manifestations of Chromosomal Copy Number Variations Indicated by Non-Invasive Prenatal Testing显示文摘Objective:To analyze the clinical value of non-invasive prenatal testing(NIPT)in detecting chromosomal copy number variations(CNVs)and to explore the relationship between gene expression and clinical manifestations of chromosomal copy number variations.Methods:3551 naturally conceived singleton pregnant women who underwent NIPT were included in this study.The NIPT revealed abnormalities other than sex chromosome abnormalities and trisomy 13,18,and 21.Pregnant women with chromosome copy number variations underwent genetic counseling and prenatal ultrasound examination.Interventional prenatal diagnosis and chromosome microarray analysis(CMA)were performed.The clinical phenotypes and pregnancy outcomes of different prenatal diagnoses were analyzed.Additionally,a follow-up was conducted by telephone to track fetal development after birth,at six months,and one year post-birth.Results:A total of 53 cases among 3551 cases showed chromosomal copy number variation.Interventional prenatal diagnosis was performed in 36 cases:27 cases were negative and 8 were consistent with the NIPT test results.This indicates that NIPT’s positive predictive value(PPV)in CNVs is 22.22%.Conclusion:NIPT has certain clinical significance in screening chromosome copy number variations and is expected to become a routine screening for chromosomal microdeletions and microduplications.However,further interventional prenatal diagnosis is still needed to identify fetal CNVs.Zixin Pi Xiaoyan Duan Jing Peng Yanhui Liu 2024Journal of Clinical and Nursing Research2024,8,1:0
7Surveillance and examination of microbial contamination in ice cream in China显示文摘This study investigated the microbial contamination of ice cream in China.A total of 2887 ice cream samples were collected from different regions of China.Contamination by the aerobic plate count(APC),coliforms,and three foodborne pathogens,Listeria monocytogenes(L.monocytogenes),Staphylococcus aureus(S.aureus),and Salmonella spp.,was detected in the samples.L.monocytogenes isolates were further analyzed for antibiotic susceptibility and multi-locus sequence typing(MLST).The results showed that APC was more than 10^(5) colony forming units(CFU)/g in 6.10%(176/2887)and coliform was more than 10^(2) CFU/g in 15.69%(453/2887)of all samples.The positive rates of S.aureus,L.monocytogenes,and Salmonella spp.were 0.66%,0.62%,and 0.10%,respectively.Among these,S.aureus contamination was more than 102 CFU/g in two samples,and L.monocytogenes in the positive sample was in the range of 0.3-240 most probable number(MPN)/g.with a median of 4.3 MPN/g.The hygienic status of the packaged samples was much better than that of the bulk samples(P<0.05).Catering samples were more frequently and heavily contaminated than samples from retail and wholesale outlets(P<0.05).No significant differences were observed in samples bought from urban and rural areas(P>0.05).For 18 L.monocytogenes strains isolated from ice cream,the resistance rate of nine antibiotics was 5.56%(1/18).By MLST,the L.monocytogenes strains were classified into nine sequence types(STs),of which ST8 was the most common(six isolates).These results indicate that a potential health risk to the public may be caused by ice cream,particularly in susceptiblepopulations.Lin Yan Xiaoyan Pei Junjie Miao Ying Li Shuran Yang Zixin Peng Xiaorong Yang Lingling Mei Qingwen Yang Huan Ren Dajin Yang Hongmei Shi 2022Food Quality and Safety2022,6,3:0
8Well-structured 3D channels within GO-based membranes enable ultrafast wastewater treatment显示文摘Graphene oxide(GO)-based membranes have been widely studied for realizing efficient wastewater treatment,due to their easily functionalizeable surfaces and tunable interlayer structures.However,the irregular structure of water channels within GO-based membrane has largely confined water permeance and prevented the simultaneously improvement of purification performance.Herein,we purposely construct the well-structured three-dimensional(3D)water channels featuring regular and negatively-charged properties in the GO/SiO_(2)composite membrane via in situ close-packing assembly of SiO_(2)nanoparticles onto GO nanosheets.Such regular 3D channels can improve the water permeance to a record-high value of 33,431.5±559.9 L·m^(−2)·h−1(LMH)bar−1,which is several-fold higher than those of current state-of-the-art GO-based membranes.We further demonstrate that benefiting from negative charges on both GO and SiO2,these negatively-charged 3D channels enable the charge selectivity well toward dye in wastewater where the rejection for positive-charged and negative-charged dye molecules is 99.6%vs.7.2%,respectively.The 3D channels can also accelerate oil/water(O/W)separation process,in which the O/W permeance and oil rejection can reach 19,589.2±1,189.7 LMH bar−1 and 98.2%,respectively.The present work unveils the positive role of well-structured 3D channels on synchronizing the remarkable improvement of both water permeance and purification performance for highly efficient wastewater treatment.Huaqiang Fu Zhe Wang Peng Li Wei Qian Zixin Zhang Xin Zhao Hao Feng Zhugen Yang Zongkui Kou Daping He 2023Nano Research2023,16,2:0

10Dwell time and bloodstream infection incidence of umbilical venous catheterization in China显示文摘Importance:Central line-associated bloodstream infection(CLABSI)is one of the most serious complications of central venous access devices.Reducing the risk of CLABSI is of utmost significance in efforts to improve neonatal mortality rates and enhance long-term prognosis.Objective:To determine the dwell time and incidence of CLABSI of umbilical venous catheterization(UVC)for preterm infants in China.Methods:Preterm infants with UVC admitted to 44 tertiary neonatal intensive care units in 24 provinces in China were enrolled.Study period was from November 2019 to August 2021.The end point of observations was 48 h after umbilical venous(UV)catheter removal.The primary outcomes were dwell time of UV catheter and UVC-associated CLABSI.Data between infants with UV catheter dwell time≤7 days and>7 days,and with birth weight(BW)≤1000 g and>1000 g were compared.Results:In total,2172 neonates were enrolled(gestational age 30.0±2.4 weeks,BW 1258.5±392.8 g).The median UV catheter dwell time was 7(6–10)days.The incidence of UVC-associated CLABSI was 3.03/1000 UV catheter days.For infants with UV catheter dwell time≤7 days and>7 days,the UVC-associated CLABSI incidence was 3.71 and 2.65 per 1000 UV catheter days,respectively,P=0.23.For infants with UVC dwell times of 3–6,7–12,and 13–15 days,the UVC-associated CLABSI rates were 0.14%,0.68%,and 2.48%(P<0.01).The Kaplan–Meier plot of UV catheter dwell time to CLABSI showed no difference between infants with BW≤1000 g and>1000 g(P=0.60).Interpretation:The median dwell time of UV catheter was 7 days,and the incidence of UVC-associated CLABSI was 3.03/1000 catheter days in China.The daily risk of UVC-associated CLABSI and other complications increased with the dwell time.Xu Zheng Dan He Zixin Yang Lu Chen Min Jiang Yujie Qi Fei Qin Jie Yu Yaguang Peng Ling Liu Mingyan Hei 2023Pediatric Investigation2023,7,4:0
