1The gap report of global climate change mitigation, finance, and governance after the United States declared its withdrawal from the Paris Agreement显示文摘On 1 June 2017, the US President Donald Trump officially announced the withdrawal from the Paris Agreement, thus the study on the reasons of withdrawal, the potential impacts, and coping strategies has become a focus among policy circles and of the international community. Based on the self-developed US Policy Assessment Model, this paper systematically evaluates the three potential 'major deficits' in terms of mitigation, climate finance, and global climate governance, as a result of the US withdrawal from the Paris Agreement and puts forward policy suggestions for coping with such transformations accordingly. The study shows that the United States 'withdrawal from the Paris Agreement will affect the existence and implementation of successive climate policies and result in an additional 8.8-13.4% increase in the global emissions reduction deficit.The United States' withdrawal will also deteriorate the existing climate finance mechanism. The Green Climate Fund(GCF)'s funding gap will increase by US$2 billion, while the gap of long-term climate finance will increase by about US$5 billion a year. 2Intra-abdominal aortic balloon occlusion in the management of placenta percreta显示文摘Background:Massive bleeding is the main concern for the management of placenta percreta(PP).Intra-abdominal aortic balloon occlusion(IABO)is one method for pelvic devascularization,but the efficacy of IABO is uncertain.This study aims to investigate the outcomes of IABO in PP patients.Methods:We retrospectively reviewed the clinical data of PP cases from six tertiary centers in China between January 2011 and December 2015.PP cases with/without the use of IABO were analyzed.Propensity score matching analysis was performed to reduce the effect of selection bias.Postpartum hemorrhage(PPH)and the rate of hysterectomy,as well as neonatal outcomes,were analyzed.Results:One hundred and thirty-two matched pairs of patients were included in the final analysis.Compared with the control group,maternal outcomes,including PPH(68.9%vs.87.9%,χ^(2)=13.984,P<0.001),hysterectomy(8.3%vs.65.2%,χ^(2)=91.672,P<0.001),and repeated surgery(1.5%vs.12.1%,χ^(2)=11.686,P=0.001)were significantly reduced in the IABO group.For neonatal outcomes,Apgar scores at 1 minute(8.67±1.79vs.8.53±1.68,t=-0.638,P=0.947)and 5 minutes(9.43±1.55vs.9.53±1.26,t=0.566,P=0.293)were not significantly different between the two groups.Conclusions:IABO can significantly reduce blood loss,hysterectomies,and repeated surgeries.This procedure has not shown harmful effects on neonatal outcomes.Weiran Zheng Ruochong Dou Jie Yan Xinrui Yang Xianlan Zhao Dunjin Chen Yuyan Ma Weishe Zhang Yiling Ding Ling Fan Huixia Yang 2022Chinese Medical Journal2022,,4:2
3Prim-O-glucosylcimifugin ameliorates aging-impaired endogenous tendon regeneration by rejuvenating senescent tendon stem/progenitor cells显示文摘Adult tendon stem/progenitor cells(TSPCs)are essential for tendon maintenance,regeneration,and repair,yet they become susceptible to senescence with age,impairing the self-healing capacity of tendons.In this study,we employ a recently developed deep-learning-based efficacy prediction system to screen potential stemness-promoting and senescence-inhibiting drugs from natural products using the transcriptional signatures of stemness.The top-ranked candidate,prim-O-glucosylcimifugin(POG),a saposhnikovia root extract,could ameliorate TPSC senescent phenotypes caused by long-term passage and natural aging in rats and humans,as well as restore the self-renewal and proliferative capacities and tenogenic potential of aged TSPCs.In vivo,the systematic administration of POG or the local delivery of POG nanoparticles functionally rescued endogenous tendon regeneration and repair in aged rats to levels similar to those of normal animals.Mechanistically,POG protects TSPCs against functional impairment during both passage-induced and natural aging by simultaneously suppressing nuclear factor-κB and decreasing mTOR signaling with the induction of autophagy.Thus,the strategy of pharmacological intervention with the deep learning-predicted compound POG could rejuvenate aged TSPCs and improve the regenerative capacity of aged tendons.Yu Wang Shanshan Jin Dan Luo Danqing He Min Yu Lisha Zhu Zixin Li Liyuan Chen Chengye Ding Xiaolan Wu Tianhao Wu Weiran Huang Xuelin Zhao Meng Xu Zhengwei Xie Yan Liu 2023Bone Research2023,11,4:0
4Security and Privacy Issues in Electronic Health Network显示文摘Electronic health network(EHN) is an information system providing functions involved in e-health. In this paper, we devise mechanisms covering three important security and privacy issues of EHN including trust management, privacy preserving, and data sharing. First, we propose an authenticated key agreement scheme based on hierarchical identity-based signature(HIBS). We abstract a hierarchical architecture from the social network architecture of EHN. To support large-scale scenarios, we introduce a virtual signature generation phase into traditional HIBS, thus our scheme will be efficient even the depth is quite big. Second, we propose a fast data searching scheme based on symmetric searchable encryption(SSE). To improve the searching efficiency, we introduce a two-level cache structure into the traditional SSE. Third, we propose an access control scheme based on hierarchical identitybased encryption(HIBE). To make it a fine-grained scheme, we organize the data owner's file in hierarchy and introduce a virtual key generation phase to traditional HIBE. Also, the scheme can provide delegation and revocation functions easily. Besides, our schemes guarantee known-key secrecy, forward secrecy, and antidirection secrecy and possess the resistance capability to collude-attack. 5Resonance analysis of a high-speed railway bridge using a stochastic finite element method显示文摘There is always some randomness in the material properties of a structure due to several circumstances and ignoring it increases the threat of inadequate structural safety reserves.A numerical approach is used in this study to consider the spatial variability of structural parameters.Statistical moments of the train and bridge responses were computed using the point estimation method(PEM),and the material characteristics of the bridge were set as random fields following Gaussian random distribution,which were discretized using Karhunen-Loève expansion(KLE).The following steps were carried out and the results are discussed herein.First,using the stochastic finite element method(SFEM),the mean value and standard deviation of dynamic responses of the train-bridge system(TBS)were examined.The effectiveness and accuracy of the computation were then confirmed by comparing the results to the Monte-Carlo simulation(MCS).Next,the influence of the train running speed,bridge vibration frequency,and span of the bridge on dynamic coefficient and dynamic response characteristics of resonance were discussed by using the SFEM.Finally,the lowest limit value of the vibration frequency of the simple supported bridges(SSB)with spans of 24 m,32 m,and 40 m are presented.Xiang Ping Yan Weiran Jiang Lizhong Zhou Wangbao Wei Biao Liu Xiang 2023Earthquake Engineering and Engineering Vibration2023,22,4:0

8High Risk Factors for Placenta Accreta Other than Pregnancy and Their Impact on Patient Prognosis显示文摘Objective This study aimed to determine the most pertinent factors responsible for placenta accreta spectrum disorders in patients without any history of pregnancy and evaluate their prognostic implications.Methods This retrospective cohort study included 1009 patients diagnosed with placenta accreta spectrum disorders based on standardized diagnostic criteria across 10 tertiary hospitals in China between January 1,2018,and December 31,2018;45 patients without a history of pregnancy were selected.The collected data mainly included demographic characteristics(including age,operative history,and ultrasound findings)and maternal-fetal outcomes(including any history of intraoperative bleeding,blood transfusion details,maternal-fetal complications,and fetal Apgar scores).SPSS 24.0 was used for statistical analyses.The Mann-Whitney U test and logistic regression were performed;a two-tailed P<0.050 was considered statistically significant.Results Ultrasound-based detection of placenta previa(χ^(2)=9.911,P=0.003)showed a strong association with placenta accreta spectrum types.The severity of placenta accreta spectrum was directly proportional to the likelihood of having coexistent complete placenta previa(χ^(2)=11.626,P=0.009)and being diagnosed by ultrasound(χ^(2)=5.449,P=0.047).Blood transfusion also impacted placenta accreta spectrum types in relation to maternal prognosis(χ^(2)=8.785,P=0.004).On univariate analysis,older age led to more complications(U=82.000,P=0.011),and in vitro fertilization-embryo transfer caused more intraoperative bleeding(U=91.500,P=0.007).Although the 1-and 5-minute Apgar scores were statistically significant,the rates of neonatal asphyxia did not differ(P>0.050).Endometrial damage led to lower Apgar scores on both univariate(1 minute:U=29.500,P=0.027;and 5 minutes:U=33.500,P=0.031)and multivariate(1 minute:β=−1.510,95%confidence interval,−2.639 to 0.381,P=0.010;and 5 minutes:β=−0.968,95%confidence interval,−1.779 to 0.157,P=0.021)analyses.Conclusion In patients who had no history of pregnancy,placenta previa was a strong risk factor for severe placenta accreta spectrum disorders.Endometrial damage led to lower Apgar scores;this warrants greater consideration in the clinic.Xinrui Yang Weiran Zheng Jie Yan Huixia Yang 2023Maternal-Fetal Medicine2023,5,3:0
9Retained Placenta Creta After Selective Fetal Reduction in Twin Pregnancy:A Case Report显示文摘Placenta accreta spectrum(PAS)disorders are severe obstetric complications and can cause life-threatening hemorrhage,hysterectomy,or even death.PAS disorders in one of dichorionic diamniotic twins are rare to be seen in clinical practice and lack of standard management strategies.We reported a case of retained placenta creta after selective fetal reduction in one of dichorionic diamniotic twins.A 32-year-old woman with a history of cesarean section was diagnosed with placenta increta in one of dichorionic diamniotic twins at 16 weeks of gestation.After discussion,the couple required to be hospitalized for selective fetal reduction.She received selective termination by ultrasound-guided single fetal intracardiac injection of potassium chloride.The follow-up prenatal ultrasound examinations detected the placenta of the reduced fetus did not stop invade the myometrium of the uterus but developed from placenta increta to placenta percreta.The patient was diagnosed with placental percreta by prenatal ultrasound signs and surgical findings.The patient underwent scheduled cesarean delivery at 34^(+5)gestational weeks.The outcome of maternal and fetal was satisfactory for our elaborate operation and optimal management.PAS disorders are severe obstetric complications with an increasing incidence.Combined with twin pregnancy makes PAS disorders more intractable.We should learn that selective fetal reduction probably cannot solve it thoroughly.Intensive and standard management is essential to promote maternal and fetal outcomes.Weiran Zheng Xinrui Yang Yuchun Zhu Jie Yan Huixia Yang 2021Maternal-Fetal Medicine2021,3,4:0
