3T1ρ/T2 mapping and histopathology of degenerative cartilage in advanced knee osteoarthritis显示文摘AIM To investigate whether normal thickness cartilage in osteoarthritic knees demonstrate depletion of proteoglycan or collagen content compared to healthy knees.METHODS Magnetic resonance(MR) images were acquired from5 subjects scheduled for total knee arthroplasty(TKA)(mean age 70 years) and 20 young healthy control subjects without knee pain(mean age 28.9 years). MR images of T1ρ mapping, T2 mapping, and fat suppressed proton-density weighted sequences were obtained.Following TKA each condyle was divided into 4 parts(distal medial, posterior medial, distal lateral, posterior lateral) for cartilage analysis. Twenty specimens(bone and cartilage blocks) were examined. For each joint,the degree and extent of cartilage destruction was determined using the Osteoarthritis Research Society International cartilage histopathology assessment system.In magnetic resonance imaging(MRI) analysis, 2 readers performed cartilage segmentation for T1ρ/T2 values and cartilage thickness measurement.RESULTS Eleven areas in MRI including normal or near normal cartilage thickness were selected. The corresponding histopathological sections demonstrated mild to moderate osteoarthritis(OA). There was no significant difference in cartilage thickness in MRI between control and advanced OA samples [medial distal condyle, P = 0.461;medial posterior condyle(MPC), P = 0.352; lateral distal condyle, P = 0.654; lateral posterior condyle, P = 0.550],suggesting arthritic specimens were morphologically similar to normal or early staged degenerative cartilage.Cartilage T2 and T1ρ values from the MPC were significantly higher among the patients with advanced OA(P= 0.043). For remaining condylar samples there was no statistical difference in T2 and T1ρ values between cases and controls but there was a trend towards higher values in advanced OA patients. CONCLUSION Though cartilage is morphologically normal or near normal, degenerative changes exist in advanced OA patients. 4Antibody markers in the diagnosis of inflammatory boweldisease显示文摘Inflammatory bowel disease(IBD), including Crohn's disease and ulcerative colitis, is a chronic intestinal inflammation of unknown etiology. The diagnosis of IBD is based on endoscopic, radiologic and histopathologic criteria. Recently, the search for a noninvasive marker that could augment or replace part of this diagnostic process has become a focus of IBD research. In this review, antibody markers, including microbial antibodies, autoantibodies and peptide antibodies, will be described, focusing on their common features. At present, no single marker with qualities that are satisfactory for the diagnosis and treatment of IBD has been identified, although panels of some antibodies are being evaluated with keen interest. The discovery of novel IBD-specific and sensitive markers is anticipated. 5study to determine guidelines for pediatric colonoscopy显示文摘AIM To investigated characteristics, diagnosis, bowelcleansing preparation, sedation, and colonoscope length and diameter in Japanese pediatric patients receiving total colonoscopy.METHODS The present study evaluated consecutive patients aged ≤ 15 years who had undergone their first colonoscopy in Kurume University between January 2007 and February 2015. Data were retrospectively analyzed. We identified 110 pediatric patients who had undergone colonoscopy that had reached the cecum, allowing the observation of the total colon.RESULTS Hematochezia, abdominal pain, and diarrhea were the most common symptoms. For bowel-cleansing preparation, pediatric patients aged ≤ 12 years were treated with magnesium citrate, and patients aged 13-15 years were treated with polyethylene glycol 4000. For sedation, thiamylal with pentazocine, which has an analgesic effect, was used in patients aged ≤ 6 years, and midazolam with pentazocine was used in patients aged ≥ 7 years. Regarding the choice of endoscope, short and thin endoscopes were selected for younger patients, particularly patients aged ≤ 3 years. Positive diagnoses were made in 78 patients(70.9%). Inflammatory bowel disease(n = 49, 44.5%), including ulcerative colitis(n = 37, 33.6%) and Crohn's disease(n = 12, 10.9%), was the most common diagnosis. CONCLUSION Colonoscopy offers a high diagnostic capability for pediatric patients with gastrointestinal symptoms. 7Factors correlating with acoustic radiation force impulse elastography in chronic hepatitis C显示文摘AIM: To investigate the factors other than fibrosis stage correlating with acoustic radiation force impulse(ARFI) elastograpy in chronic hepatitis C. METHODS: ARFI elastograpy was performed in 108 consecutive patients with chronic hepatitis C who underwent a liver biopsy. The proportion of fibrosis area in the biopsy specimens was measured by computerassisted morphometric image analysis. RESULTS: ARFI correlated significantly with fibrosis stage(β = 0.1865, P < 0.0001) and hyaluronic acid levels(β = 0.0008, P = 0.0039) in all patients by multiple regression analysis. Fibrosis area correlated significantly with ARFI by Spearman's rank correlation test but not by multiple regression analysis. ARFI correlated significantly with body mass index(BMI)(β =-0.0334, P = 0.0001) in F 0 or F 1, with γ-glutamyltranspeptidase levels(β = 0.0048, P = 0.0012) in F 2, and with fibrosis stage(β = 0.2921, P = 0.0044) and hyaluronic acid levels(β = 0.0012, P = 0.0025) in F 3 or F 4. The ARFI cutoff value was 1.28 m/s for F ≥ 2, 1.44 m/s for F ≥ 3, and 1.73 m/s for F 4. CONCLUSION: ARFI correlated with fibrosis stage and hyaluronic acid but not with inflammation. 8A new index for non-invasive assessment of liver fibrosis显示文摘AIM:To construct and evaluate a new non-invasive fibrosis index for assessment of the stage of liver f ibrosis. METHODS:A new f ibrosis index (Fibro-Stiffness index) was developed in 165 of 285 patients with chronic hepatitis C, and was validated in the other 120 patients where liver biopsy was performed. Its usefulness was compared with liver stiffness (LS) measured by FibroScan, the aminotransferase-to-platelet ratio index, the Forns index and the FibroIndex. RESULTS: The Fibro-Stiffness index consists of LS,platelet count and prothrombin time. The values of the Fibro-Stiffness index differed signif icantly between neighboring f ibrosis stages except F0-F1. The area under the receiver operating characteristics curves of the Fibro-Stiffness index for prediction of F≥2 (0.90), F≥ 3 (0.90) and F= 4(0.92) in the estimation group and those for F≥ 3 (0.93) and F =4 (0.97) in the validation group were the highest among the 5 methods examined. The accuracy of the Fibro-Stiffness index had highest values for F≥2, F≥3 and F=4 in both the estimation and validation groups. The diagnostic performance for F= 4 was improved by a combination of the Fibro-Stiffness index with serum hyaluronic acid level. CONCLUSION: The Fibro-Stiffness index was constructed and validated. 10Selective decrease in colonic CD56^+ T and CD161^+ T cells in the inflamed mucosa of patients with ulcerative colitis显示文摘AIM: To investigate the role of local colonic mucosal NK receptor-positive T (NKR+ T) cells in the regulation of intestinal inflammation, we analyzed the population and function of these cells in ulcerative colitis (UC). METHODS: Colonic mucosal tissues were obtained from colonoscopic biopsies of the descending colon from 96 patients with UC (51 endoscopically uninfl amed, 45 inflamed) and 18 normal controls. Endoscopic appearance and histologic score at the biopsied site were determined by Matts’ classification. A single cell suspension was prepared from each biopsy by collagenase digestion. Two NKR+ T cell subsets, CD56+ (CD56+CD3+) T cells and CD161+ (CD161+CD3+) T cells, were detected by flow cytometric analysis. Intracellular cytokine analysis for anti-in? ammatory cytokine interleukin-10 (IL-10) was performed by in vitro stimulation with phorbol-myristate- acetate (PMA) and ionomycin. RESULTS: CD56+ T cells and CD161+ T cells are present in the normal human colon and account for 6.7% and 21.3% of all mononuclear cells, respectively. The populations of both CD56+ T cells and CD161+ T cells were decreased significantly in the inflamed mucosa of UC. In contrast, the frequency of conventional T cells (CD56-CD3+ cells and CD161-CD3+ cells) was similar among the patient and control groups. The populations of NKR+ T cells were correlated inversely with the severity of infl ammation, which was classif ied according to the endoscopic and histologic Matts’ criteria. 11H pylori seropositivity and cytokine gene polymorphisms显示文摘AIM: To investigate whether the pro- and anti-inflammatory cytokine gene polymorphisms, IL1B-511C/T, IL1B-31C/T, IL6-634C/G, TNF-1031T/C, TNF-857C/T, and IL10-1082A/G, interact with smoking and drinking habits to influence infection with H pylori.METHODS: The subjects were 410 Japanese transit company employees. C-reactive protein and conventional cardiovascular risk factors were evaluated. Serum anti-H pylori antibodies were measured. The genotypes of IL1B-511C/T, IL1B-31C/T, IL6-634C/G, TNF-1031T/C, TNF-857C/T, and IL10-1082A/G polymorphisms were determined by allelic discrimination using fluorogenic probes and a 5′nuclease assay.RESULTS: In gender- and age-adjusted logistic analyses, the subjects with TNF-857T/T had a significantly lower odds ratio (OR) for H pylori seropositivity (reference -857C/C; OR = 0.15, 95% CI: 0.03-0.59, P = 0.007). After stratification according to smoking and drinking status, among never-smokers, the subjects with IL1B-511C/T had a significantly lower OR (reference -511C/C; OR = 0.30, 95% CI: 0.10-0.90, P = 0.032). Among drinkers in the 1-5 times/wk category, the subjects with IL1B-511T/T had a significantly lower OR (reference C/C; OR = 0.38, 95% CI: 0.16-0.95, P = 0.039), and the subjects with IL1B-31C/T and T/T had a significantly higher OR (reference C/C; C/T: OR = 2.59, 95% CI, P = 0.042: 1.04-6.47; C/C: OR = 3.17, 95% CI: 1.23-8.14, P = 0.017). Among current smokers, the subjects withIL6-634C/G had a significantly higher OR (reference C/C; OR = 2.28, 95% CI: 1.13-4.58, P = 0.021). However, the interactions terms between the aforementioned genotypes and lifestyles were not statistically significant.CONCLUSION: Contrary to previous findings, the results herein suggest that the TNF-857T/T genotype may be protective against chronic infection with H pylori. Drinking and smoking habits may influence the effect of cytokine gene polymorphisms. 13Disruption of Dhcr7 and Insig1/2 in cholesterol metabolism causes defects in bone formation and homeostasis through primary cilium formation显示文摘Human linkage studies suggest that craniofacial deformities result from either genetic mutations related to cholesterol metabolism or high-cholesterol maternal diets. However, little is known about the precise roles of intracellular cholesterol metabolism in the development of craniofacial bones, the majority of which are formed through intramembranous ossification. Here, we show that an altered cholesterol metabolic status results in abnormal osteogenesis through dysregulation of primary cilium formation during bone formation. We found that cholesterol metabolic aberrations, induced through disruption of either Dhcr7(which encodes an enzyme involved in cholesterol synthesis) or Insig1 and Insig2(which provide a negative feedback mechanism for cholesterol biosynthesis), result in osteoblast differentiation abnormalities. Notably, the primary cilia responsible for sensing extracellular cues were altered in number and length through dysregulated ciliary vesicle fusion in Dhcr7 and Insig1/2 mutant osteoblasts. As a consequence, WNT/β-catenin and hedgehog signaling activities were altered through dysregulated primary cilium formation.Strikingly, the normalization of defective cholesterol metabolism by simvastatin, a drug used in the treatment of cholesterol metabolic aberrations, rescued the abnormalities in both ciliogenesis and osteogenesis in vitro and in vivo. 14Mutations in carboxy-terminal part of E2 including PKR/eIF2αphosphorylation homology domain and interferon sensitivity determining region of nonstructural 5A of hepatitis C virus 1b:Their correlation with response to interferon monotherapy and viral load显示文摘AIM: To study the amino acid substitutions in the carboxy (C)-terminal part of E2 protein and in the interferon (IFN) sensitivity determining region (ISDR) and their correlation with response to IFN and viral load in 85 hepatitis C virus (HCV)-lb-infected patients treated with IFN. METHODS: The C-terminal part of E2 (codons 617-711) including PKR/eIF2a phosphorylation homology domain (PePHD) and ISDR was sequenced in 85 HCV-1b-infected patients treated by IFN monotherapy. RESULTS: The amino acid substitutions in PePHD detected only in 4 of 85 patients were not correlated either with response to IFN or with viral load. The presence of substitutions in a N-terminal variable region (codons 617-641) in the C-terminal part of E2 was significantly correlated with both small viral load (33.9% vs 13.8%, P = 0.0394) and sustained response to IFN (25.0% vs 6.9 %, P = 0.0429). Four or more substitutions in ISDR were significantly correlated with both small viral load (78.6% vs 16.2%, P < 0.0001) and sustained response to IFN (85.7% vs 2.9%, P < 0.0001). In multivariate analysis, ISDR in nonstructural (NS) 5A (OR = 0.39, P < 0.0001) and N-terminal variable region (OR = 0.51, P = 0.039) was selected as the independent predictors for small viral load, and ISDR (OR = 39.0, P < 0.0001) was selected as the only independent predictor for sustained response. 15Roles of sphingosine-1-phosphate signaling in angiogenesis显示文摘Sphingosine-1-phosphate (S1P) is a blood-borne lipid mediator with pleiotropic biological activities. S1P acts via the specific cell surface G-protein-coupled receptors, S1P1-5. S1P1 and S1P2 were originally identified from vascular endothelial cells (ECs) and smooth muscle cells, respectively. Emerging evidence shows that S1P plays crucial roles in the regulation of vascular functions, including vascular formation, barrier protection and vascular tone via S1P1, S1P2 and S1P3. In particular, S1P regulates vascular formation through multiple mechanisms; S1P exerts both positive and negative effects on angiogenesis and vascular maturation. The positive and negative effects of S1P are mediated by S1P1 and S1P2, respectively. These effects of S1P1 and S1P2 are probably mediated by the S1P receptors expressed in multiple cell types including ECs and bone-marrow-derived cells. 16Effects on coagulation factor production following primary hepatomitogen-induced direct hyperplasia显示文摘AIM:To investigate the molecular mechanisms involved in coagulation factor expression and/or function during direct hyperplasia(DH) -mediated liver regeneration.METHODS:Direct hyperplasia-mediated liver regeneration was induced in female C57BL/6 mice by administering 1,4-bis[2-(3,5-dichloropyridyloxy) ] benzene(TCPOBOP) ,a representative hepatomitogen.Mice were weighed and sacrificed at various time points [Day 0(D0:prior to injection) ,3 h,D1,D2,D3,and D10] after TCPOBOP administration to obtain liver and blood samples.Using the RNA samples extracted from the liver,a comprehensive analysis was performed on the hepatic gene expression profiling of coagulation-related factors by real-time RT-PCR(fibrinogen,prothrombin,factors Ⅴ,Ⅶ,Ⅷ,Ⅸ,Ⅹ,Ⅺ,Ⅻ,XⅢβ,plasminogen,antithrombin,protein C,protein S,ADAMTS13,and VWF) .The corresponding plasma levels of coagulation factors(fibrinogen,prothrombin,factors Ⅴ,Ⅶ,Ⅷ,Ⅸ,Ⅹ,Ⅺ,Ⅻ,XⅢ,and VWF) were also analyzed and compared with their mRNA levels.RESULTS:Gavage administration of TCPOBOP(3 mg/kg body weight) resulted in a marked and gradual increase in the weight of the mouse livers relative to the total body weight to 220% by D10 relative to the D0(control) ratios.At the peak of liver regeneration(D1 and D2) ,the gene expression levels for most of the coagulationrelated factors(fibrinogen,prothrombin,factors Ⅴ,Ⅶ,Ⅷ,Ⅸ,Ⅺ,Ⅻ,XⅢβ,plasminogen,antithrombin,protein C,ADAMTS13,VWF) were found to be downregulated in a time-dependent manner,and gradually recovered by D10 to the basal levels.Only mRNA levels of factor Ⅹ and protein S failed to show any decrease during the regenerative phase.As for the plasma levels,5 clotting factors(prothrombin,factors Ⅷ,Ⅸ,Ⅺ,and Ⅻ) demonstrated a significant decrease(P < 0.05) during the regeneration phase compared with D0.Among these 5 factors,factor Ⅸ and factor Ⅺ showed the most dramatic decline in their activities by about 50% at D2 compared to the basal levels,and these reductions in plasma activity for both factors were consistent with our RT-PCR findings.In contrast,the plasma activities of the other coagulation factors(fibrinogen,factors Ⅴ,Ⅶ,XⅢ,and VWF) were not significantly reduced,despite the reduction in the liver mRNA levels.Unlike the other factors,FX showed a temporal increase in its plasma activity,with significant increases(P < 0.05) detected at D1.CONCLUSION:Investigating the coagulation cascade protein profiles during liver regeneration by DH may help to better understand the basic biology of the liver under normal and pathological conditions.Kohei Tatsumi Kazuo Ohashi Sanae Taminishi Soichi Takagi Rie Utoh Akira Yoshioka Midori Shima Teruo Okano 2009World Journal of Gastroenterology2009,15,42:5
17ADCP application for long-term monitoring of coastal water显示文摘Three kind of application of ADCP is reported for long-term monitoring in coastal sea.(1)The rourine monitoring of water qualities.The water quality and ADCP echo data (600 kHz) observed in the long-term are analgzed at MT (Marine Tower) Station of Kansai International Airport in the Osaka Bay, Japan. The correlation between the turbidity and echo intensity in the surface layer is not good because air bubbles generated by breaking wave are not detected by the turbidity meter, but detected well by ADCP. When estimating the turbidity consists of plankton population from echo intensity, the effect ofbubbles have to be eliminated. (2) Monitoring stirring up of bottom sediment. The special observation was carded out by using following two ADCP in the Osaka Bay, One ADCP was installed upward on the sea. The other ADCP was hanged downward at the gate type stand about 3 m above from the bottom. At the spring tide, high echo intensities indicating the stirring up of bottom sediment were observed. (3) The monitoring for the boundary condition of water mixing at an estuary. In summer season, the ADCP was set at the mouth of Tanabe Bay in Wakayama Prefecture, Japan.During the observation, water temperature near the bottom showed remarkable falls with interval of about 5~7 d. When the bottom temperature fell, the inflow current with low echo intensity water appears at the bottom layer in the ADCP record. 18Study on Targeting and in vitro Anti-oxidation of Baicalin Solid Lipid Nanoparticles显示文摘Objective To prepare liver-targeted baicalin solid lipid nanoparticles(BSLNs) and to study their in vitro anti-oxidative activity.Methods BSLNs were prepared by emulsification ultrasonic dispersion method and characterized by transmission electron microscopy and laser particle size distribution;The tissue in vivo distribution was detected by pharmacokinetics;In vitro anti-superoxide dismutase(SOD) activity and reduction capacity of BSLNs were determined;The ability of removing hydroxyl radical was determined by phenanthroline-Fe2+ oxidation.Results The best prescription was baicalin-soybean lecithin-glyceryl monostearate-poloxamer 188(1:5:15:30);The encapsulation efficiency and drug loading were 84.7% and 5.65%,respectively,mean size of particles was(68.6 ± 8) nm,Zeta potential was 22.13 mV;The in vitro anti-oxidant results showed that BSLNs had a significant inhibitory effect on SOD and a strong reducing capacity as well as a removing hydroxide radical ability.The targeting rate of BSLNs was 6.931 for liver.Conclusion The results demonstrate that BSLNs could enhance the liver targeting ability and in vitro anti-oxidative activity significantly.MIYAMURA Mitsuhiko YOKOTA Junko YOSHIOKA Saburo 2012Chinese Herbal Medicines2012,4,4:4
19Prediction of colorectal tumor grade and invasion depth through narrow-band imaging scoring显示文摘AIM To determine the usefulness of assigning narrow-band imaging(NBI) scores for predicting tumor grade and invasion depth in colorectal tumors.METHODS A total of 161 colorectal lesions were analyzed from138 patients who underwent endoscopic or surgical resection after conventional colonoscopy and magnifying endoscopy with NBI. The relationships between the surface and vascular patterns of the lesions, as visualized with NBI, and the tumor grade and depth of submucosa(SM) invasion were determined histopathologically.Scores were assigned to distinct features of the surface microstructures of tubular and papillary-type lesions.Using a multivariate analysis, a model was developed for predicting the tumor grade and depth of invasion based on NBI-finding scores.RESULTS NBI findings that correlated with a high tumor grade were associated with the 'regular/irregular'(P < 0.0001)surface patterns and the 'avascular area' pattern(P =0.0600). The vascular patterns of 'disrupted vessels'(P = 0.0714) and 'thick vessels'(P = 0.0133) but none of the surface patterns were associated with a depth of invasion of ≥ 1000 μm. In our model, a total NBIfinding score ≥ 1 was indicative of a high tumor grade(sensitivity: 0.97; specificity: 0.24), and a total NBIfinding score ≥ 9(sensitivity: 0.56; specificity: 1.0) was predictive of a SM invasion depth ≥ 1000 μm. Scores less than these cutoff values signified adenomas and a SM invasion depth < 1000 μm, respectively. 20Endoscopic submucosal dissection in early gastric cancer in elderly patients and comorbid conditions显示文摘The prognosis of early gastric cancer(EGC) is good if there is no concomitant lymph node metastasis. Therefore, the early detection of EGC is important to improve the prognosis of patients with gastric cancer. In Japan, 40% to 50% of all gastric cancers are EGC, and endoscopic submucosal dissection(ESD) is widely accepted as a local treatment for these lesions, particularly for large lesions that at one time were an indication for gastrectomy because of the difficulty of en-bloc resection. Consequently, this procedure can preserve the entire stomach and the patient's postoperative quality of life. ESD has become a general technique with improved procedures and devices, and has become the preferred treatment for EGC rather than gastrectomy. Therefore, ESD may demonstrate many advantages in patients who have several comorbidities, particularly elderly population, patients taking antithrombotic agents, or patients with chronic kidney disease, or liver cirrhosis. However, it is not yet clear whether patients with both EGC and comorbidities are feasible candidates for ESD and whether they would consequently be able to achieve a survival benefit after ESD.
