| 1 | Cytokine production in patients with cirrhosis and TLR4 polymorphisms显示文摘AIM:To analyze the cytokine production by peripheral blood cells from cirrhotic patients with and without TLR4 D299G and/or T399I polymorphisms.METHODS:The study included nine patients with cirrhosis and TLR4 D299G and/or T399I polymorphisms,and 10 wild-type patients matched for age,sex and degree of liver failure.TLR4 polymorphisms were determined by sequence-based genotyping.Cytokine production by peripheral blood cells was assessed spontaneously and also after lipopolysaccharide(LPS)and lipoteichoic acid(LTA)stimulation.RESULTS:Patients with TLR4 polymorphisms had a higher incidence of previous hepatic encephalopathy than wild-type patients(78%vs 20%,P=0.02).Spontaneous production of interleukin(IL)-6 and IL-10 was lower in patients with TLR4 polymorphisms than in wild-type patients[IL-6:888.7(172.0-2119.3)pg/m L vs 5540.4(1159.2-26053.9)pg/m L,P<0.001;IL-10:28.7(6.5-177.1)pg/m L vs 117.8(6.5-318.1)pg/m L,P=0.02].However,the production of tumor necrosis factor-α,IL-6 and IL-10 after LPS and LTA stimulation was similar in the two groups.CONCLUSION:TLR4 polymorphisms were associated with a distinctive pattern of cytokine production in cirrhotic patients,suggesting that they play a role in the development of cirrhosis complications. | Juan Camilo Nieto Elisabet Sánchez Eva Román Silvia Vidal Laia Oliva Carlos Guarner-Argente Maria Poca Xavier Torras Cándido Juárez Carlos Guarner German Soriano | 2014 | World Journal of Gastroenterology2014，20，46： | 5 |
| 2 | rate of adverse events of gastroduodenal snare polypectomy for non-flat polyp is low: A prospective and multicenter study显示文摘AIM To evaluate the rate of adverse events(AEs) during consecutive gastric and duodenal polypectomies in several Spanish centers. METHODS Polypectomies of protruded gastric or duodenal polyps ≥ 5 mm using hot snare were prospectively included. Prophylactic measures of hemorrhage were allowed in predefined cases. AEs were defined and graded according to the lexicon recommended by the American Society for Gastrointestinal Endoscopy. Patients were followed for 48 h, one week and 1 mo after theprocedure. RESULTS308 patients were included and a single polypectomy was performed in 205. Only 36(11.7%) were on prior anticoagulant therapy. Mean polyp size was 15 ± 8.9 mm(5-60) and in 294 cases(95.4%) were located in the stomach. Hemorrhage prophylaxis was performed in 219(71.1%) patients. Nine patients presented AEs(2.9%), and 6 of them were bleeding(n = 6, 1.9%)(in 5 out of 6 AE, different types of endoscopic treatment were performed). Other 24 hemorrhagic episodes could be managed without any change in the outcome of the endoscopy and, consequently, were considered incidents. | 4 | The potential leaching and mobilization of trace elements from FGD-gypsum of a coal-fired power plant under water re-circulation conditions显示文摘Experimental and geochemical modelling studies were carried out to identify mineral and solid phases containing major, minor, and trace elements and the mechanism of the retention of these elements in Flue Gas Desulphurisation(FGD)-gypsum samples from a coal-fired power plant under filtered water recirculation to the scrubber and forced oxidation conditions. The role of the p H and related environmental factors on the mobility of Li, Ni, Zn, As, Se, Mo, and U from FGD-gypsums for a comprehensive assessment of element leaching behaviour were also carried out. Results show that the extraction rate of the studied elements generally increases with decreasing the p H value of the FGD-gypsum leachates. The increase of the mobility of elements such as U, Se, and As in the FGD-gypsum entails the modification of their aqueous speciation in the leachates; UO2SO4, H2 Se, and HAs O2 are the aqueous complexes with the highest activities under acidic conditions.The speciation of Zn, Li, and Ni is not affected in spite of p H changes; these elements occur as free cations and associated to SO42 in the FGD-gypsum leachates. The mobility of Cu and Mo decreases by decreasing the p H of the FGD-gypsum leachates, which might be associated to the precipitation of Cu Se2 and Mo Se2, respectively. | 5 | Adalimumab induction therapy for ulcerative colitis with intolerance or lost response to infliximab: An open-label study显示文摘AIM: To evaluate the efficacy of adalimumab induction therapy in patients with ulcerative colitis who previously responded to infliximab and then lost response or became intolerant. METHODS: Ten patients with ulcerative colitis were enrolled in a 4-wk open-label trial. The patients received a loading dose of 160 mg adalimumab at wk 0 followed by 80 mg at wk 2. The primary efficacy measure was clinical improvement at wk 4, as defined by a decrease in clinical activity index (CAI) of more than 4. RESULTS: Four of 10 patients (40%) benefited from subsequent adalimumab therapy; one patient achieved remission (CAI < 4) and 3 had clinical improvement at wk 4. 6 patients had no response (60%); 2 of 6 (33.3%) subsequently underwent colectomy. This was accompanied by a decrease in median CRP concentration from 16.8 mg/mL at baseline to 3.85 mg/mL at wk 4, excluding two patients who underwent colectomy after two infusions of adalimumab. Among the 6 patients with severe colitis (CAI > 12) at baseline, none achieved remission and only one patient had clinical improvement at wk 4.
