1Strategies for managing the insufficiency of the proximal landing zone during endovascular thoracic aortic repair显示文摘Background The insufficiency of the proximal landing zone (PLZ) is a frequent factor challenging the applicability and efficacy of endovascular repair (EVR) for thoracic aortic disorders. This study discusses two strategies for conquering this challenge.Methods Ten patients underwent EVR for thoracic aortic diseases during a one-year period ending June 30, 2004. Nine patients had DeBakey type Ⅲ dissecting aortic aneurysm (DAA), and one had descending thoracic aortic aneurysm (DTAA). The PLZ, defined as the distance from the origin of the left subclavian artery (LSA) to the primary entry tear of the dissection or to the proximal aspect of DTAA, was less than 15 mm in all instances. EVR with intentional coverage of the LSA without any supportive bypass was employed in 6 patients with DAA, and the preliminary right-left carotid and left carotid-subclavian bypass combined with EVR in the DTAA and other 3 DAA cases. Results Technical success was achieved in all the patients. The patient with DTAA died from hemispheric cerebral infarction and subsequent multiple system organ failure following an uneventful recovery from the cervical reconstruction performed 1 week previously. In cases receiving the EVR with intentional coverage of the LSA, in two patients dizziness occurred, which noticeably resolved after intravenous administration of mannitol for 4 to 5 days, and a drop in blood pressure of the left arm was noted in all the cases, but remained clinically silent. No neurological deficits or limb ischaemia developed perioperatively or during the followup, ranging from 3 to 12 months, and complete thrombosis of the thoracic aortic false lumen was revealed on CT at 3 months in the 9 patients with DAA.Conclusions Both the intentional bypass absent coverage of the LSA and the adjunctive surgical bypass appear to be feasible and effective in managing the insufficiency of the PLZ during the endovascular thoracic aortic repair.FUWei-guo DONGZhi-hui WANGYu-qi GUODa-qiao XUXin CHENBin JIANGJun-hao YANGJue SHIZheng-yu 2005Chinese Medical Journal2005,,13:20
2Preoperative diagnosis and operative effect of intracranial aneurysm with three-dimensional computed tomography angiography显示文摘BACKGROUND: Digital subtraction angiography (DSA) is always regarded as the golden standard for diagnosis of intracranial aneurysm; however, the procedure is complex, traumatic, expensive and easy to induce vascular complication. Three-dimensional computed tomography angiography (3D-CTA) can make up deficiencies of DSA; therefore, it is used in clinical therapy wider and wider. OBJECTIVE: To evaluate the clinical effect of 3D-CTA on disruption and hemorrhage of intracranial aneurysm pre- and post-operation and compare with the effect of DSA.DESIGN: Auto-control contrast observation.SETTING: Department of Neurosurgery, Shengjing Hospital of China Medical University.PARTICIPANTS: A number of 106 patients with disruption and hemorrhage of intracranial aneurysm were selected from the Department of Neurosurgery, Shengjing Hospital of China Medical University from January 2003 to April 2006. All patients were diagnosed with cranial operation and consent. There were 47 males and 59 females aged from 3-76 years with the mean age of (47±13) years. Among them, 82 patients had extensive subarachnoid hemorrhage (SAH), 7 had hemorrhage at longitudinal fissure, and 17 had hemorrhage at ambiens cisterna and lateral fissure. Moreover, intraventricular hematocele was accompanied on 13 patients and hematom on 9 patients. METHODS: ① 3D-CTA examination: Siemens SOMATOM Sensation 64 CT was used in this study. The thickness was 1 mm and interval of reconstruction was 0.8 mm. Localizing section was plainly scanned as the standard of canthus line. Scan ranged from 30 mm below sella to 50 mm above sella. Non-ion contrast medium of Omnipaque 350 (concentration of iodine was 350 g/L) was inserted into anterior vein of elbow with 18G trochar retained with high-pressured injectoc pum. The speed was 4.5 mL/s and the total volume was 80-100 mL with the means of 90 mL. Scan started at 10-20 s after injection of contrast medium. Original image was dealt with Leonardo workstation and retreated with Syngo software. Volume rendering and maximum intensity projection were used to reconstructed images. ② All 106 patients suffered from occlusion of aneurysm clamp. Before operation, 3D-CTA was undertaken and DSA was followed. After operation, patients were rechecked with 3D-CTA. MAIN OUTCOME MEASURES: Comparisons between 3D-CTA and DSA.RESULTS: All 106 patients were involved in the final analysis. ① Examination of 3D-CTA and DSA: Among 118 patients with aneurysm, 110 were checked with 3D-CTA and the detected rate was 93.2% (110/118). Among other 8 cases, 3 were negative and checked again with DSA; 1 had pericallosal aneurysm, 1 ophthalmic aneurysm, and 1 anterior choroidal artery of aneurysm. 3D-CTA results of other 5 cases were suspicious, and then, they were regarded as having aneurysm with DSA. Before operation, correlation among site, body, neck of aneurysm and peripheral anatomic structure were shown sufficiently. After operation, 82 patients were rechecked with 3D-CTA, which was complete occlusion, precise, unobvious constriction, emphraxis or remains as compared with 3D-CTA those pre-operation. ② Characteristics of 3D-CTA: With multiple vessels and angles, 3D-CTA observed the relationship between aneurysm neck and carried artery and showed thrombosis in cavity of aneurysm, calcification of aneurysm wall and peripheral structure of vessel at the same time. However, DSA could not detect the reactions mentioned above. It could delete image of cranium, simulate image of operative route, eliminate artifact induced by metal, but not distinguish blood stream direction. Meanwhile, posterior communicating artery was always poor during circle of Willis artery showing. CONCLUSION: ① 3D-CTA is characterized by simple operation and non-invasive showing vascular stereo structure and correlation. 3Multiscale MRF-based Texture Segmentation of SAR Image显示文摘We propose a multiscale Bayesian segmentation algorithm for SAR image in this paper. A hierarchical two-level Markov random field (MRF) is applied to represent both texture and region label over the wavelet lattice. The high level uses an isotropic Multi-level logistic (MLL) random field to characterize the blob-like region formation process at each scale and the interscale dependencies over the corresponding multiresolution region. At lower level a novel Causal Gaussian autoregressive (CGAR) process is proposed to describe the fill-in of multiresolution region. Once the multiscale double MRFs model is established, in term of Sequential maximum a posteriori (SMAP), model parameter estimate and region segmentation are performed alternately from coarse to fine scale. 5Reversible histone acetylation/ deacetylation modification by p300 and HDAC3 is involved in the regulation of IL-18 promoter activity显示文摘Interleukin-18 (IL-18) is a pleiotropic cytokine involved in the development of T helper type 1 (Thl) cells, and it plays important roles in regulation of both the innate and acquired immune responses. The aim of this study was to elucidate whether the reversible histone acetylation/ deacetylation modification participates in the regulation of IL-18 transcription expression. The transcription coactivator p300 containing the histone acetyltransferase (HAT) activity, and the histone deacetylase 3 (HDAC3) were used in this study to analyze the effect of this modification in the regula-tion of mouse IL-18 gene. The results demonstrate that transfection of p300-expression plasmid promotes the en-dogenous IL-18 mRNA synthesis in J774 cells, and stimulates the activation of IL-18 promoter. It has been found that this stimulating effect of p300 was reversed by HDAC3, indicat-ing the involvement of the reversible histone acetyla-tion/deacetylation modification in IL-18 regulation. Fur-thermore, the data show that the HAT activity of p300 was essential to its function in activating IL-18 promoter. In ad-dition, p300 was shown to be able to work synergistically with the transcription factor c-Fos on activation of IL-18 promoter and this effect could also be impaired by HDAC3. 9Quantitative trait locus mapping of yield and plant height in autotetraploid alfalfa(Medicago sativa L.)显示文摘Alfalfa(Medicago sativa L.)is the most widely grown forage legume crop worldwide.Yield and plant height are important agronomic traits influenced by genetic and environmental factors.The objective of this study was to identify quantitative trait loci(QTL)and molecular markers associated with alfalfa yield and plant height.To understand the genetic basis of these traits,a full-sib F1 population composed of 392 individuals was developed by crossing a low-yielding precocious alfalfa genotype(male parent)with a high-yielding latematuring alfalfa cultivar(female parent).The linkage maps were constructed with 3818 single-nucleotide polymorphism(SNP)markers on 64 linkage groups.QTL for yield and plant height were mapped using phenotypic data for three years.Sixteen QTL associated with yield and plant height were identified on chromosomes 1 to 8.Six QTL explained more than 10%of phenotypic variation,representing major loci controlling yield and plant height.One locus on chromosome 1 controlling yield traits had not been identified in previous studies.Three QTL co-located with other QTL(qyield-1 and qheight-7,qheight-5 and qyield-4,qheight-6,and qyield-6).With further validation,the markers closely linked with these QTL may be used for marker-assisted selection in breeding new alfalfa varieties with high yield.Fei He Ruicai Long Tiejun Zhang Fan Zhang Zhen Wang Xijiang Yang Xueqian Jiang Changfu Yang Xuxin Zhi Mingna Li Longxi Yu Junmei Kang Qingchuan Yang 2020The Crop Journal2020,8,5:1
