| 1 | Autoantibody to MOG suggests two distinct clinical subtypes of NMOSD显示文摘We characterized a unique group of patients with neuromyelitis optica spectrum disorder(NMOSD) who carried autoantibodies of aquaporin-4(AQP4) and myelin-oligodendrocyte glycoprotein(MOG). Among the 125 NMOSD patients, 10(8.0%) were AQP4- and MOG-ab double positive, and 14(11.2%) were MOG-ab single positive. The double-positive patients had a multiphase disease course with a high annual relapse rate(P=0.0431), and severe residual disability(P<0.0001). Of the double-positive patients, 70% had MS-like brain lesions, more severe edematous, multifocal regions on spinal magnetic resonance imaging(MRI), pronounced decreases of retinal nerve fiber layer thickness and atrophy of optic nerves. In contrast, patients with only MOG-ab had a higher ratio of monophasic disease course and mild residual disability. Spinal cord MRI illustrated multifocal cord lesions with mild edema, and brain MRIs showed more lesions around lateral ventricles. NMOSD patients carrying both autoantibodies to AQP4 and MOG existed and exhibited combined features of prototypic NMO and relapsing-remitting form of MS, whereas NMOSD with antibodies to MOG only exhibited an 'intermediate' phenotype between NMOSD and MS. | 3 | Estimating emissions of HCFC-22 and CFC-11 in China by atmospheric observations and inverse modeling显示文摘This paper presents the estimation of Chinese emissions of HCFC-22 and CFC-11 in 2009 by an inverse modeling method based on in-situ measurement data from the Shangdianzi Global Atmosphere Watch (GAW) Regional Station (SDZ) and atmospheric transport simulations. After inversion (a-posteriori) estimates of the Chinese emissions in 2009 increased by 6.6% for HCFC-22 from 91.7 (± 83.6) to 98.3 (± 47.4) kt/yr and by 22.5% for CFC-11 from 13 (±12.6) to 15.8 (±7.2) kt/yr compared to an a-priori emission. While the model simulation with a-priori emissions already captured the main features of the observed variability at the measurement site, the model performance (in terms of correlation and mean-square-error) improved using a-posteriori emissions. | 6 | Intraductal papillary mucinous neoplasm and the pancreatic incidentaloma显示文摘Asymptomatic pancreatic lesions(APL) are a commonly encountered problem in today's pancreatic surgical practices.Current literature regarding etiologies and incidence of APLs,particularly intraductal papillary mucinous neoplasm(IPMN),is presented.APLs constitute a wide spectrum of pathology(solid/cystic,benign/premalignant/malignant) but,overall,IPMN is now the most common diagnosis.The Sendai Guidelines and their function as a basis for risk stratification in branch duct IPMN are presented.The importance of traditionally analyzed cyst characteristics including size,presence of mucin or nodules and cyst fluid aspirate as indicators of malignancy is emphasized,noting also the potential correlation of main duct dilatation,thickened septae and elevated cyst fluid CEA with increased risk of malignancy.Current complication rates after resection of APLs are reviewed and found to be generally equivalent to those for symptomatic resections.A potential multidisciplinary treatment strategy is offered considering the costs of surgery versus repeated imaging or follow up endoscopy for these lesions.The decision for intervention is ultimately based on the Sendai Guidelines in the context of the individual patient. | Tara S Kent Charles M Vollmer Jr Mark P Callery | 2010 | World Journal of Gastrointestinal Surgery2010，2，10： | 3 |
