1Neglected tropical diseases in the People’s Republic of China: progress towards elimination显示文摘Since the founding of the People's Republic of China in 1949,considerable progress has been made in the control and elimination of the country's initial set of 11 neglected tropical diseases.Indeed,elimination as a public health problem has been declared for lymphatic filariasis in 2007 and for trachoma in 2015.The remaining numbers of people affected by soil-transmitted helminth infection,clonorchiasis,taeniasis,and echinococcosis in 2015 were 29.1 million,6.0 million,366200,and 166100,respectively.In 2017,after more than 60 years of uninterrupted,multifaceted schistosomiasis control,has seen the number of cases dwindling from more than 10 million to 37600.Meanwhile,about 6000 dengue cases are reported,while the incidence of leishmaniasis,leprosy,and rabies are down at 600 or fewer per year.Sustained social and economic development,going hand-in-hand with improvement of water,sanitation,and hygiene provide the foundation for continued progress,while rigorous surveillance and specific public health responses will consolidate achievements and shape the elimination agenda.Targets for poverty elimination and strategic plans and intervention packages post-2020 are important opportunities for further control and elimination,when remaining challenges call for sustainable efforts.Men-Bao Qian Jin Chen Robert Bergquist Zhong-Jie Li Shi-Zhu Li Ning Xiao Jurg Utzinger Xiao-Nong Zhou 2019Infectious Diseases of Poverty2019,8,5:23
2Surveillance-response systems: the key to elimination of tropical diseases显示文摘Tropical diseases remain a major cause of morbidity and mortality in developing countries.Although combined health efforts brought about significant improvements over the past 20 years,communities in resource-constrained settings lack the means of strengthening their environment in directions that would provide less favourable conditions for pathogens.Still,the impact of infectious diseases is declining worldwide along with progress made regarding responses to basic health problems and improving health services delivery to the most vulnerable populations.The London Declaration on Neglected Tropical Diseases(NTDs),initiated by the World Health Organization’s NTD roadmap,set out the path towards control and eventual elimination of several tropical diseases by 2020,providing an impetus for local and regional disease elimination programmes.Tropical diseases are often patchy and erratic,and there are differing priorities in resources-limited and endemic countries at various levels of their public health systems.In order to identify and prioritize strategic research on elimination of tropical diseases,the‘First Forum on Surveillance-Response System Leading to Tropical Diseases Elimination’was convened in Shanghai in June 2012.Current strategies and the NTD roadmap were reviewed,followed by discussions on how to identify and critically examine prevailing challenges and opportunities,including inter-sectoral collaboration and approaches for elimination of several infectious,tropical diseases.A priority research agenda within a‘One Health-One World’frame of global health was developed,including(i)the establishment of a platform for resource-sharing and effective surveillance-response systems for Asia Pacific and Africa with an initial focus on elimination of lymphatic filariasis,malaria and schistosomiasis;(ii)development of new strategies,tools and approaches,such as improved diagnostics and antimalarial therapies;(iii)rigorous validation of surveillance-response systems;and(iv)designing pilot studies to transfer Chinese experiences of successful surveillance-response systems to endemic countries with limited resources.Ernest Tambo Lin Ai Xia Zhou Jun-Hu Chen Wei Hu Robert Bergquist Jia-Gang Guo Jürg Utzinger Marcel Tanner Xiao-Nong Zhou 2014Infectious Diseases of Poverty2014,3,1:14
3Surveillance and response systems for elimination of tropical diseases: summary of a thematic series in Infectious Diseases of Poverty显示文摘The peer-reviewed journal Infectious Diseases of Poverty provides a new platform to engage with,and disseminate in an open-access format,science outside traditional disciplinary boundaries.The current piece reviews a thematic series on surveillance-response systems for elimination of tropical diseases.Overall,22 contributions covering a broad array of diseases are featured–i.e.clonorchiasis,dengue,hepatitis,human immunodeficiency virus/acquired immune deficiency syndrome(HIV/AIDS),H7N9 avian influenza,lymphatic filariasis,malaria,Middle East respiratory syndrome(MERS),rabies,schistosomiasis and tuberculosis(TB).There are five scoping reviews,a commentary,a letter to the editor,an opinion piece and an editorial pertaining to the theme“Elimination of tropical disease through surveillance and response”.The remaining 13 articles are original contributions mainly covering(i)drug resistance;(ii)innovation and validation in the field of mathematical modelling;(iii)elimination of infectious diseases;and(iv)social media reports on disease outbreak notifications released by national health authorities.Analysis of the authors’affiliations reveals that scientists from the People’s Republic of China(P.R.China)are prominently represented.Possible explanations include the fact that the 2012 and 2014 international conferences pertaining to surveillance-response mechanisms were both hosted by the National Institute of Parasitic Diseases(NIPD)in Shanghai,coupled with P.R.China’s growing importance with regard to the control of infectious diseases.Within 4 to 22 months of publication,three of the 22 contributions were viewed more than 10000 times each.With sustained efforts focusing on relevant and strategic information towards control and elimination of infectious diseases,Infectious Diseases of Poverty has become a leading journal in the field of surveillance and response systems in infectious diseases and beyond.Xia Zhou Peiling Yap Marcel Tanner Robert Bergquist Jürg Utzinger Xiao-Nong Zhou 2016Infectious Diseases of Poverty2016,5,1:12
4三苯双脒、青蒿琥酯、蒿甲醚和吡喹酮单剂、多剂或联合用药治疗大鼠华支睾吸虫感染的实验研究显示文摘目的观察三苯双脒、青蒿琥酯、蒿甲醚、或吡喹酮单剂、多剂给药,及其伍用治疗感染华支睾吸虫大鼠的疗效。方法147只SD大鼠各感染50个华支睾吸虫囊蚴,于感染后42~44d分组治疗。各药物采用灌胃给药。①60只感染大鼠随机分为11组(每组4~5只),分别为三苯双脒150mg/kg(顿服)、75mg/(kg·d)×2d、50mg/(kg·d)×3d和25mg/kg(tid)×2d组;吡喹酮150mg/kg(顿服)、75mg/(kg·d)×2d和25mg/kg(tid)×2d;青蒿琥酯或蒿甲醚75mg/kg(顿服)和37.5mg/(kg·d)×2d组。②另87只感染大鼠随机分为15组(每组4~6只),用青蒿琥酯或蒿甲醚(30mg/kg)分别与吡喹酮(150mg/kg)、三苯双脒(50mg/kg和75mg/kg)伍用组;三苯双脒(50mg/kg)与吡喹酮(150mg/kg)伍用组;三苯双脒(75mg/kg)与吡喹酮(187.5mg/kg)伍用组,及各药的单用组。并设同批感染未治疗对照组。受治鼠于治疗后2周剖杀,收集胆管和肝组织内的残留华支睾吸虫,计算各组的平均虫数和减虫率,用非参数统计方法(Mann-Whitney秩和检验)对相应组间的平均虫数进行分析。结果感染华支睾吸虫的大鼠口服单剂三苯双脒或吡喹酮(150mg/kg)的减虫率分别为57.2%和63.8%。三苯双脒各小剂量多次给药组的减虫率稍高,达77.1%~79.4%,而吡喹酮小剂量多次给药组的减虫率则为50.6%~54.2%。但两种药物各组间的平均虫数的差异无统计学意义。青蒿琥酯和蒿甲醚各单剂给药组与小剂量多次给药组的减虫率均较高,分别为90.4%~98.5%和100%。三苯双脒小剂量(50或75mg/kg)与吡喹酮(150mg/kg或187.5mg/kg)伍用治疗,减虫率为74.9%~100%,高于其各单药组的减虫率(26.9%~79.6%)。青蒿琥酯或蒿甲醚小剂量(30mg/kg)与吡喹酮(150mg/kg)或三苯双脒(50或75mg/kg)伍用治疗,减虫率为74.9%~97.9%,亦高于其各药组的减虫率(24.8%~79.6%)。结论青蒿琥酯、蒿甲醚、吡喹酮和三苯双脒均为有效的抗华支睾吸虫药物,各药物小剂量伍用具有增效作用。肖树华 薛剑 Marcel TANNER 张永年 Jennifer KEISER Jug UTZINGER 强慧琴 刘晓云 2008中国寄生虫学与寄生虫病杂志2008,26,5:10
5中国大陆福寿螺种群遗传学研究显示文摘目的通过遗传标记分析中国大陆福寿螺种群结构特点,为研究其侵入途径和扩散模式提供基础。方法利用CO I基因扩增引物对中国大陆60个采集点的581个福寿螺标本进行遗传学分析。利用DnaSP 5.10.01进行单倍型多样性和核酸多样性分析,并通过Network 4.2.0.1进行单倍型网络分析。综合GenBank上可利用的福寿螺单倍型和本研究获得的单倍型进行进化关系分析,了解中国大陆福寿螺的进化地位。结果共获得556条有效序列,分属25个单倍型,其中6个单倍型频率较高,占整个样本的96.0%。进化分析表明我国存在Pom cea canaliculata和P.insula-rum2个种。P.insularum单倍型与国外报告类型存在较大差异。结论中国大陆福寿螺种群结构复杂,可能有多种来源和扩散模式。吕山 张仪 刘和香 胡铃 柳伟 刘琴 李石柱 胡薇 Jurg Utzinger 周晓农 2011中国血吸虫病防治杂志2011,23,2:10
6Controlling schistosomiasis with praziquantel:How much longer without a viable alternative?显示文摘The current approach of morbidity control of schistosomiasis,a helminth disease of poverty with considerable public health and socioeconomic impact,is based on preventive chemotherapy with praziquantel.There is a pressing need for new drugs against this disease whose control entirely depends on this single drug that has been widely used over the past 40 years.We argue that a broader anthelminthic approach supplementing praziquantel with new antischistosomals targeting different parasite development stages would not only increase efficacy but also reduce the risk for drug resistance.Repositioning drugs already approved for other diseases provides a shortcut to clinical trials,as it is expected that such drugs rapidly pass the regulatory authorities.The antischistosomal properties of antimalarial drugs(e.g.,semisynthetic artemisinins,synthetic trioxolanes,trioxaquines and mefloquine)and of drugs being developed or registered for other purposes(e.g.,moxidectin and miltefosin),administered alone or in combination with praziquantel,have been tested in the laboratory and clinical trials.Another avenue to follow is the continued search for new antischistosomal properties in plants.Here,we summarise recent progress made in schistosomiasis chemotherapy,placing particular emphasis on repositioning of existing drugs against schistosomiasis.Robert Bergquist Jürg Utzinger Jennifer Keiser 2017Infectious Diseases of Poverty2017,6,1:7

8Comparison of novel and standard diagnostic tools for the detection of Schistosoma mekongi infection in Lao People’s Democratic Republic and Cambodia显示文摘Background:Given the restricted distribution of Schistosoma mekongi in one province in Lao People’s Democratic Republic(Lao PDR)and two provinces in Cambodia,together with progress of the national control programmes aimed at reducing morbidity and infection prevalence,the elimination of schistosomiasis mekongi seems feasible.However,sensitive diagnostic tools will be required to determine whether elimination has been achieved.We compared several standard and novel diagnostic tools in S.mekongi-endemic areas.Methods:The prevalence and infection intensity of S.mekongi were evaluated in 377 study participants from four villages in the endemic areas in Lao PDR and Cambodia using Kato-Katz stool examination,antibody detection based on an enzyme-linked immunosorbent assay(ELISA)and schistosome circulating antigen detection by lateral-flow tests.Two highly sensitive test systems for the detection of cathodic and anodic circulating antigens(CCA,CAA)in urine and serum were utilized.Results:Stool microscopy revealed an overall prevalence of S.mekongi of 6.4%(one case in Cambodia and 23 cases in Lao PDR),while that of Opisthorchis viverrini,hookworm,Trichuris trichiura,Ascaris lumbricoides and Taenia spp.were 50.4%,28.1%,3.5%,0.3%and 1.9%,respectively.In the urine samples,the tests for CCA and CAA detected S.mekongi infections in 21.0%and 38.7%of the study participants,respectively.In the serum samples,the CAA assay revealed a prevalence of 32.4%,while a combination of the CAA assay in serum and in urine revealed a prevalence of 43.2%.There was a difference between the two study locations with a higher prevalence reached in the samples from Lao PDR.Conclusions:The CCA,CAA and ELISA results showed substantially higher prevalence estimates for S.mekongi compared to Kato-Katz thick smears.Active schistosomiasis mekongi in Lao PDR and Cambodia might thus have been considerably underestimated previously.Hence,sustained control efforts are still needed to break transmission of S.mekongi.The pivotal role of highly sensitive diagnostic assays in areas targeting elimination cannot be overemphasised.Youthanavanh Vonghachack Somphou Sayasone Virak Khieu Robert Bergquist Govert Jvan Dam Pytsje THoekstra Paul L.A.M.Corstjens Beatrice Nickel Hanspeter Marti Jürg Utzinger Sinuon Muth Peter Odermatt 2017Infectious Diseases of Poverty2017,6,1:4
9Towards a global One Health index: a potential assessment tool for One Health performance显示文摘Background:A One Health approach has been increasingly mainstreamed by the international community, as it provides for holistic thinking in recognizing the close links and inter-dependence of the health of humans, animals and the environment. However, the dearth of real-world evidence has hampered application of a One Health approach in shaping policies and practice. This study proposes the development of a potential evaluation tool for One Health performance, in order to contribute to the scientific measurement of One Health approach and the identification of gaps where One Health capacity building is most urgently needed.Methods:We describe five steps towards a global One Health index (GOHI), including (i) framework formulation;(ii) indicator selection;(iii) database building;(iv) weight determination;and (v) GOHI scores calculation. A cell-like framework for GOHI is proposed, which comprises an external drivers index (EDI), an intrinsic drivers index (IDI) and a core drivers index (CDI). We construct the indicator scheme for GOHI based on this framework after multiple rounds of panel discussions with our expert advisory committee. A fuzzy analytical hierarchy process is adopted to determine the weights for each of the indicators.Results:The weighted indicator scheme of GOHI comprises three first-level indicators, 13 second-level indicators, and 57 third-level indicators. According to the pilot analysis based on the data from more than 200 countries/territories the GOHI scores overall are far from ideal (the highest score of 65.0 out of a maximum score of 100), and we found considerable variations among different countries/territories (31.8–65.0). The results from the pilot analysis are consistent with the results from a literature review, which suggests that a GOHI as a potential tool for the assessment of One Health performance might be feasible.Conclusions:GOHI—subject to rigorous validation—would represent the world’s first evaluation tool that constructs the conceptual framework from a holistic perspective of One Health. Future application of GOHI might promote a common understanding of a strong One Health approach and provide reference for promoting effective measures to strengthen One Health capacity building. 10Towards integrated surveillance-response systems for the prevention of future pandemics显示文摘Most human pathogens originate from non-human hosts and certain pathogens persist in animal reservoirs.The transmission of such pathogens to humans may lead to self-sustaining chains of transmission.These pathogens represent the highest risk for future pandemics.For their prevention,the transmission over the species barrier—although rare—should,by all means,be avoided.In the current COVID-19 pandemic,surprisingly though,most of the current research concentrates on the control by drugs and vaccines,while comparatively little scientific inquiry focuses on future prevention.Already in 2012,the World Bank recommended to engage in a systemic One Health approach for zoonoses control,considering integrated surveillance-response and control of human and animal diseases for primarily economic reasons.First examples,like integrated West Nile virus surveillance in mosquitos,wild birds,horses and humans in Italy show evidence of financial savings from a closer cooperation of human and animal health sectors.Provided a zoonotic origin can be ascertained for the COVID-19 pandemic,integrated wildlife,domestic animal and humans disease surveillance-response may contribute to prevent future outbreaks.In conclusion,the earlier a zoonotic pathogen can be detected in the environment,in wildlife or in domestic animals;and the better human,animal and environmental surveillance communicate with each other to prevent an outbreak,the lower are the cumulative costs.Jakob Zinsstag Jürg Utzinger Nicole Probst-Hensch Lv Shan Xiao-Nong Zhou 2020Infectious Diseases of Poverty2020,9,5:3

14The impact of pastoralist mobility on tuberculosis control in Ethiopia:a systematic review and meta-synthesis显示文摘Background:Directly observed treatment,short-course(DOTS)is the current mainstay to control tuberculosis(TB)worldwide.Context-specific adaptations of DOTS have impending implications in the fight against TB.In Ethiopia,there is a national TB control programme with the goal to eliminate TB,but uneven distribution across lifestyle gradients remains a challenge.Notably,the mobile pastoralist communities in the country are disproportionately left uncovered.The aim of this study was to summarize the evidence base from published literature to guide TB control strategy for mobile pastoralist communities in Ethiopia.Main text:We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines and systematically reviewed articles in seven electronic databases:Excerptra Medical Database,African Journal Online,PubMed,Google Scholar,Centre for Agriculture and Bioscience International Direct,Cochrane Library and Web of Science.The databases were searched from inception to December 31,2018,with no language restriction.We screened 692 items of which 19 met our inclusion criteria.Using a meta-ethnographic method,we identified six themes:(i)pastoralism in Ethiopia;(ii)pastoralists’livelihood profile;(iii)pastoralists’service utilisation;(iv)pastoralists’knowledge and awareness on TB control services;(v)challenges of TB control in pastoral settings;and(vi)equity disparities affecting pastoralists.Our interpretation triangulates the results across all included studies and shows that TB control activities observed in pastoralist regions of Ethiopia are far fewer than elsewhere in the country.Conclusions:This systematic review and meta-synthesis shows that TB control in Ethiopia does not align well with the pastoralist lifestyle.Inaccessibility and lack of acceptability of TB care are the key bottlenecks to pastoralist TB service provision.Targeting these two parameters holds promise to enhance effectiveness of an intervention.Faisal Nooh Lisa Crump Abdiwahab Hashi Rea Tschopp Esther Schelling Klaus Reither Jan Hattendorf Seid M.Ali Brigit Obrist Jurg Utzinger Jakob Zinsstag 2019Infectious Diseases of Poverty2019,8,5:2
15The genetic variation of Angiostrongylus cantonensis in the People’s Republic of China显示文摘Background:The People’s Republic of China(P.R.China)is the presumptive home range of the rat lungworm Angiostrongylus cantonensis,a major aetiological agent of human eosinophilic meningitis.We present a study of the genetic variation of A.cantonensis in P.R.China.Our aim was to deepen the current knowledge pertaining to its origin and global spread from a molecular perspective.Methods:Adult A.cantonensis were collected in the frame of a national survey and identified based on morphological criteria.Polymerase chain reaction(PCR)was employed to amplify the target DNA sequences(cytochrome c oxidase subunit I(cox1),nicotinamide adenine dinucleotide dehydrogenase subunit 1(nad1)and internal transcribed spacer(ITS)).The PCR product of cox1 was directly submitted to sequencing,while clone sequencing was used for nad1 and ITS.The identity of the samples was verified by comparing the sequences to those of accepted A.cantonensis specimens.The specific composition of substitutions in each gene was analysed,and the genotypes were compared based on the complete cox1,nad1 and ITS genes.Results:We characterised the complete mitochondrial genes cox1 and nad1 of 130 specimens and obtained 357 nuclear sequences containing two complete ITS(ITS1 and ITS2)and 5.8S rRNA of the same samples.All specimens were genetically confirmed as A.cantonensis.Two major groups(i.e.I and II)were identified according to the phylogeny of cox1 sequences.Group I could be further categorised into six distinct clades.Almost half of the specimens(47.7%)belong to the clade Ia and 22.3%to the group II.The former was widely distributed across the study region.A variable number of repeat units in three microsatellites was observed,resulting in considerable length variation in ITS.Intragenomic variation of ITS sequences was found in a large proportion of the samples.Genotyping showed a striking difference between mitochondrial DNA and ITS.Conclusions:Our results demonstrate that A.cantonensis is the only rat lungworm species in P.R.China and shows high genetic diversity.Results of diversity and genotyping of A.cantonensis can be impacted by the sequencing strategy and biomarker.Although ITS may be a valuable marker for interspecific identification,it is not suitable for studying the intraspecific variation of A.cantonensis due to its high intragenomic variation and current challenges for direct sequencing.Shan Lv Yi Zhang Peter Steinmann Jurg Utzinger Xiao-Nong Zhou 2017Infectious Diseases of Poverty2017,6,1:2
