1Osteoporosis in primary biliary cholangitis显示文摘Primary biliary cholangitis(PBC) is an autoimmune cholestatic liver disease with multiple debilitating complications. Osteoporosis is a common complication of PBC resulting in frequent fractures and leading to significant morbidity in this population, yet evidence for effective therapy is lacking. We sought to summarize our current understanding of the pathophysiology of osteoporosis in PBC, as well as current and emerging therapies in order to guide future research directions. A complete search with a comprehensive literature review was performed with studies from Pub Med, EMBASE, Web of Science, Cochrane database, and the Countway Library. Osteoporosis in PBC is driven primarily by decreased bone formation, which differs from the increased bone resorption seen in postmenopausal osteoporosis. Despite this fundamental difference, current treatment recommendations are based primarily on experience with postmenopausal osteoporosis. Trials specific to PBC-related osteoporosis are small and have not consistently demonstrated a benefit in this population. As it stands, prevention of osteoporosis in PBC relies on the mitigation of risk factors such as smoking and alcohol use, as well as encouraging a healthy diet and weight-bearing exercise. The primary medical intervention for the treatment of osteoporosis in PBC remains bisphosphonates though a benefit in terms of fracture reduction has never been shown. 2Albumin as a prognostic marker for ulcerative colitis显示文摘AIM To evaluate the role of albumin at the time of ulcerative colitis(UC) diagnosis in predicting the clinical course of disease.METHODS Nationwide cohort of patients with newly diagnosed UC in the Veterans Affairs health care system was identified and divided into two categories: hypoalbuminemia(i.e.,≤ 3.5 gm/dl) or normal albumin levels(i.e.,> 3.5 gm/dl) at the time of UC diagnosis. The exposure of interest was presence of hypoalbuminemia defined asalbumin level ≤ 3.5 g/dl at the time of UC diagnosis. Patients were then followed over time to identify the use of ≥ 2 courses of corticosteroids(CS),thiopurines,anti-TNF medications and requirement of colectomy for UC management. RESULTS The eligible study cohort included 802 patients,but 92(11.4%) patients did not have their albumin levels checked at the time of UC diagnosis,and they were excluded. A total of 710 patients,who had albumin levels checked at time of UC diagnosis,were included in our study. Amongst them,536 patients had a normal albumin level and 174 patients had hypoalbuminemia. Patients with hypoalbuminemia at diagnosis had a higher likelihood of ≥ 2 courses of CS use(adjusted HR = 1.7,95%CI: 1.3-2.3),higher likelihood of thiopurine or anti-TNF use(adjusted HR = 1.72,95%CI: 1.23-2.40) than patients with normal albumin level at diagnosis. 4Loss of the vitamin D receptor in human breast and prostate cancers strongly induces cell apoptosis through downregulation of Wnt/β-catenin signaling显示文摘Vitamin D co-regulates cell proliferation, differentiation and apoptosis in numerous tissues, including cancers. The known anti-proliferative and pro-apoptotic actions of the active metabolite of vitamin D,1,25-dihydroxy-vitamin D [1,25(OH)2 D] are mediated through binding to the vitamin D receptor(VDR). Here,we report on the unexpected finding that stable knockdown of VDR expression in the human breast and prostate cancer cell lines, MDA-MB-231 and PC3, strongly induces cell apoptosis and inhibits cell proliferation in vitro. Implantation of these VDR knockdown cells into the mammary fat pad(MDA-MB-231),subcutaneously(PC3) or intra-tibially(both cell lines) in immune-incompetent nude mice resulted in reduced tumor growth associated with increased apoptosis and reduced cell proliferation compared with controls.These growth-retarding effects of VDR knockdown occur in the presence and absence of vitamin D and are independent of whether cells were grown in bone or soft tissues. Transcriptome analysis of VDR knockdown and non-target control cell lines demonstrated that loss of the VDR was associated with significant attenuation in the Wnt/β-catenin signaling pathway. In particular, cytoplasmic and nuclear β-catenin protein levels were reduced with a corresponding downregulation of downstream genes such as Axin2, Cyclin D1,interleukin-6(IL-6), and IL-8. Stabilization of β-catenin using the GSK-3β inhibitor BIO partly reversed the growth-retarding effects of VDR knockdown. Our results indicate that the unliganded VDR possesses hitherto unknown functions to promote breast and prostate cancer growth, which appear to be operational not only within but also outside the bone environment. 5Experimental investigations of transient pressure variations in a high head model Francis turbine during start-up and shutdown显示文摘Penetration of the power generated using wind and solar energy to electrical grid network causing several incidents of the grid tripping, power outage, and frequency drooping. This has increased restart(star-stop) cycles of the hydroelectric turbines significantly since grid connected hydroelectric turbines are widely used to manage critical conditions of the grid. Each cycle induces significant stresses due to unsteady pressure loading on the runner blades. The presented work investigates the pressure loading to a high head( H P=377 m, DP =1.78 m) Francis turbine during start-stop. The measurements were carried out on a scaled model turbine( H M=12.5 m, DM =0.349 m). Total four operating points were considered. At each operating point, three schemes of guide vanes opening and three schemes of guide vanes closing were investigated. The results show that total head variation is up to 9% during start-stop of the turbine. On the runner blade, the maximum pressure amplitudes are about 14 kPa and 16 kPa from the instantaneous mean value of 121 kPa during rapid start-up and shutdown, respectively, which are about 1.5 times larger than that of the slow start-up and shutdown. 62,3-Diaryl-3H-imidazo[4,5-b]pyridine derivatives as potential anticancer and anti-inflammatory agents显示文摘In this study we examined the suitability of the 3H-imidazo[4,5-b]pyridine ring system in developing novel anticancer and anti-inflammatory agents incorporating a diaryl pharmacophore. Eight 2,3-diaryl-3H-imidazo[4,5-b]pyridine derivatives retrieved from our in-house database were evaluated for their cytotoxic activity against nine cancer cell lines. The results indicated that the compounds showed moderate cytotoxic activity against MCF-7, MDA-MB-468, K562 and SaOS2 cells, with K562 being the most sensitive among the four cancer cell lines. The eight 2,3-diaryl-3H-imidazo[4,5-b]pyridine derivatives were also evaluated for their COX-1 and COX-2 inhibitory activity in vitro. The results showed that compound 3f exhibited 2-fold selectivity with IC_(50) values of 9.2 and 21.8 mmol/L against COX-2 and COX-1, respectively. It is more prevalent in male gender, and with increasing age, obesity, and insulin resistance. Besides weight loss, there are limited treatment options. The use of anti-diabetic medications has been studied with mixed results. In this review, we discuss the use of anti-diabetic medications in the management of NAFLD with a specific focus on sodium-glucose cotransporter 2 inhibitors. 9Effect of Punica granatum peel extract on learning and memory in rats显示文摘Objective:To evaluate potential memory enhancing effect of Punica granatum peel extract on rats.Methods:Healthy adult male albino rats of Wistar strain were used.Each group of 6 rats were administered either distilled water or 50 mg/kg of extract or 100 mg/kg of extract for 15 days and subjected to passive avoidance test or T-maze test.In the next phase rats were administered distilled water or 100 mg/kg of extract for 15 days and the rats were given injection diazepam before subjecting them to the tests.Results:The overall performance was better in test groups compared to control groups.Among the test groups,100 mg/kg rats performed better than 50 mg/kg.The effect on spatial learning parameters like mean number of alternations and mean percentage bias was more marked compared to retention testing parameters like latency. 100 mg/kg Punica extract treated group also improved performance of diazepam treated rats. 11Biloma: An unusual complication in a patient with pancreatic cancer显示文摘The term biloma describes an encapsulated collection of bile within the abdomen, usually secondary to bile duct disruption. The commonest causes reported in the literature are iatrogenic (secondary to hepatobiliary surgery), trauma or complications due to choledocholithiasis. A few cases have been reported as complications of cholangiocarcinoma or acute cholecystitis. We report the case of a 64-year-old man initially diagnosed with a non-obstructive malignancy of the pancreas, who developed a spontaneous intrahepatic biloma 8 mo later. This was identifi ed following a 1-wk history of fever, rigors and icterus. The biloma was identifi ed on computed tomography and subsequently drained under ultrasound guidance. Forty-eight hours later, a stent was inserted endoscopically into his common bile duct and he made an uneventful in-hospital recovery. 12Nanoscale deformation of multiaxially forged ultrafine-grained Mg-2Zn-2Gd alloy with high strength-high ductility combination and comparison with the coarse-grained counterpart显示文摘Cold processing of magnesium(Mg) alloys is a challenge because Mg has a hexagonal close-packed(HCP)lattice with limited slip systems, which makes it difficult to plastically deform at low temperature. To address this challenge, a combination of annealing of as-cast alloy and multi-axial forging was adopted to obtain isotropic ultrafine-grained(UFG) structure in a lean Mg-2Zn-2Gd alloy with high strength(yield strength: ~227 MPa)-high ductility(% elongation: ~30%) combination. This combination of strength and ductility is excellent for the lean alloy, enabling an understanding of deformation processes in a formable high strength Mg-rare earth alloy. The nanoscale deformation behavior was studied via nanoindentation and electron microscopy, and the behavior was compared with its low strength(yield strength: ~46 MPa)-low ductility(% elongation: ~7%) coarse-grained(CG) counterpart. In the UFG alloy, extensive dislocation slip was an active deformation mechanism, while in the CG alloy, mechanical twinning occurred.The differences in the deformation mechanisms of UFG and CG alloys were reflected in the discrete burst in the load-displacement plots. The deformation of Mg-2Zn-2Gd alloys was significantly influenced by the grain structure, such that there was change in the deformation mechanism from dislocation slip(non-basal slip) to nanoscale twins in the CG structure. 14Computing in-situ strength of rock masses based upon RQD and modified joint factor: Using pressure and damage sensitive constitutive relationship显示文摘In this study, a new model was presented for computing strength of rock masses based upon in-situ observations of RQD popularly known as rock quality designation. This model links up the rock mass parameters from in-situ investigations with the strength parameters of jointed rocks obtained from laboratory scale experimental observations. Using the constitutive relation, the author derived a pressure and damage sensitive plastic parameter to determine strength of rock masses for varied extents of discontinuity and pressure induced damage. The test results show that plasticity characterized by hardening and softening inclusive of damage invariably depends upon mean pressure and extent of deformations already experienced by rock masses. The present work explores the test data that reveal the dependence of in-situ strength on incremental joint parameters obtained from the joint number,joint orientation, joint roughness, gouge parameters and water pressure. Substituting the relationship between the RQD and modified joint factor with that between modulus ratio and strength ratio, the model shows successfully that using damage inclusive plastic parameter and RQD provides a relationship for estimating the strength of rock masses. 15Design and development of guide vane cascade for a low speed number Francis turbine显示文摘Guide vane cascade of a low speed number Francis turbine is developed for the experimental investigations.The test setup is able to produce similar velocity distributions at the runner inlet as that of a reference prototype turbine.Standard analytical methods are used to design the reference turbine.Periodic walls of flow channel between guide vanes are identified as the starting profile for the boundary of the cascade.Two alternative designs with three guide vanes and two guide vanes,without runner,are studied.A new approach,for the hydraulic design and optimization of the cascade test setup layout,is proposed and investigated in details.CFD based optimization methods are used to define the final layout of the test setup.The optimum design is developed as a test setup and experimental validation is done with PIV methods.The optimized design of cascade with one guide vane between two flow channels is found to produce similar flow conditions to that in the runner inlet of a low speed number Francis turbine.Biraj Singh THAPA Chirag TRIVEDI Ole Gunnar DAHLHAUG 2016Journal of Hydrodynamics2016,28,4:2

19Implications of multigene testing for hereditary breast cancer in primary care显示文摘Approximately 1 in 8 women will develop breast cancer during their lifetime and the risk factors include age, family history, and reproductive factors. In women with a family history of breast cancer, there is a proportion in which a gene mutation can be the cause of the predisposition for breast cancer. A careful assessment of family and clinical history should be performed in these women in order to determine if a genetic counseling referral is indicated. In cases of hereditary breast cancer, genetic testing with a multigene panel can identify specific genetic mutations in over 100 genes. The most common genes mutated in hereditary breast cancer are the high-penetrance BRCA1 and BRCA2 genes. In addition, other mutations in high-penetrance genes in familial cancer syndromes and mutations in DNA repair genes can cause hereditary breast cancer. Mutations in low-penetrance genes and variants of uncertain significance may play a role in breast cancer development, but the magnitude and scope of risk in these cases remain unclear, thus the clinical utility of testing for these mutations is uncertain. 20The thermal and storage stability of bovine haemoglobin by ultraviolet-visible and circular dichroism spectroscopies显示文摘The effects of temperature,pH and long-term storage on the secondary structure and conformation changes of bovine haemoglobin(bHb) were studied using circular dichroism(CD) and ultraviolet-visible(UV-vis) spectroscopies.Neural network software was used to deconvolute the CD data to obtain the fractional content of the five secondary structures.The storage stability of bHb solutions in pH 6,7 and8 buffers was significantly higher at 4 ℃ than at 23 ℃ for the first 3 days.A complete denaturation of bHb was observed after 40 days irrespective of storage temperature or pH.The bHb solutions were also exposed to heating and cooling cycles between 25 and 65 ℃ and structural changes were followed by UVvis and CD spectroscopies.These experiments demonstrated that α-helix content of bHb decreased steadily with the increasing temperature above 35 ℃ at all pH values.The loss in a-helicity and gain in random coil conformations was pH-dependent and the greatest under alkaline conditions.Furthermore,there was minimal recovery of the secondary structure content upon cooling to 25 ℃.The use of bHb as a model drug is very common and this study elucidates the significance of storage and processing conditions on its stability.Ruchir Bhomia Vivek Trivedi Nichola J.Coleman John C.Mitchell 2016Journal of Pharmaceutical Analysis2016,6,4:2
