| 4 | Ampullary cancer of intestinal origin and duodenal cancer-A logical clinical and therapeutic subgroup in periampullary cancer显示文摘Periampullary cancers include pancreatic, ampullary, biliary and duodenal cancers. At presentation, the majority of periampullary tumours have grown to involve the pancreas, bile duct, ampulla and duodenum. This can result in difficulty in defining the primary site of origin in all but the smallest tumors due to anatomical proximity and architectural distortion. This has led to variation in the reported proportions of resected periampullary cancers. Pancreatic cancer is the most common cancer resected with a pancreaticoduodenectomy followed by ampullary(16%-50%), bile duct(5%-39%), and duodenal cancer(3%-17%). Patients with resected duodenal and ampullary cancers have a better reported median survival(29-47 mo and 22-54 mo) compared to pancreatic cancer(13-19 mo). The poorer survival with pancreatic cancer relates to differences in tumour characteristics such as a higher incidence of nodal, neural and vascular invasion. While small ampullary cancers can present early with biliary obstruction, pancreatic cancers need to reach a certain size before biliary obstruction ensues. This larger size at presentation contributes to a higher incidence of resection margin involvement in pancreatic cancer. Ampullary cancers can be subdivided into intestinal or pancreatobiliary subtype cancers with histomolecular staining. This avoids relying on histomorphology alone, as even some poorly differentiated cancers preserve the histomolecular profile of their mucosa of origin. Histomolecular profiling is superior to anatomic location in prognosticating survival. Ampullary cancers of intestinal subtype and duodenal cancers are similar in their intestinal origin and form a logical clinical and therapeutic subgroup of periampullary cancers. They respond to 5-FU based chemotherapeutic regimens such as capecitabine-oxaliplatin. Unlike pancreatic cancers, KRAS mutation occurs in only approximately a third of ampullary and duodenal cancers. Future clinical trials should group ampullary cancers of intestinal origin and duodenal cancers together given their similarities and their response to fluoropyrimidine therapy in combination with oxaliplatin. | 5 | Distinctive Precursory Air–Sea Signals between Regular and Super El Ni os显示文摘Statistically different precursory air–sea signals between a super and a regular El Ni no group are investigated, using observed SST and rainfall data, and oceanic and atmospheric reanalysis data. The El Ni no events during 1958–2008 are first separated into two groups: a super El Ni no group(S-group) and a regular El Ni no group(R-group). Composite analysis shows that a significantly larger SST anomaly(SSTA) tendency appears in S-group than in R-group during the onset phase[April–May(0)], when the positive SSTA is very small. A mixed-layer heat budget analysis indicates that the tendency difference arises primarily from the difference in zonal advective feedback and the associated zonal current anomaly(u).This is attributed to the difference in the thermocline depth anomaly(D) over the off-equatorial western Pacific prior to the onset phase, as revealed by three ocean assimilation products. | 11 | Another Record: Ocean Warming Continues through 2021 despite La Nina Conditions显示文摘The increased concentration of greenhouse gases in the atmosphere from human activities traps heat within the climate system and increases ocean heat content(OHC). Here, we provide the first analysis of recent OHC changes through 2021 from two international groups. The world ocean, in 2021, was the hottest ever recorded by humans, and the 2021 annual OHC value is even higher than last year’s record value by 14 ± 11 ZJ(1 zetta J = 1021 J) using the IAP/CAS dataset and by16 ± 10 ZJ using NCEI/NOAA dataset. The long-term ocean warming is larger in the Atlantic and Southern Oceans than in other regions and is mainly attributed, via climate model simulations, to an increase in anthropogenic greenhouse gas concentrations. The year-to-year variation of OHC is primarily tied to the El Nino-Southern Oscillation(ENSO). In the seven maritime domains of the Indian, Tropical Atlantic, North Atlantic, Northwest Pacific, North Pacific, Southern oceans,and the Mediterranean Sea, robust warming is observed but with distinct inter-annual to decadal variability. Four out of seven domains showed record-high heat content in 2021. | 16 | Asymmetry of Atmospheric Responses to Two-Type El Nino and La Nina over Northwest Pacific显示文摘The mechanism for asymmetric atmospheric responses to the central Pacific(CP) El Ni?o and La Ni?a over the western North Pacific(WNP) is studied in this paper. The negative anomalies of rainfall over the key region of WNP are explained by diagnosing the column-integrated equations of moisture and moist static energy(MSE). It is revealed that the nonlinear advection of moist enthalpy is critical to introduce negative rainfall anomalies over the region. The anomalous easterly(westerly) in La Ni?a(CP El Ni?o) causes negative advection of anomalous moist enthalpy, inducing negative heating anomaly and an anticyclone anomaly in the WNP, which weakens(strengthens) the cyclone(anticyclone) in La Ni?a(CP El Ni?o). The MSE budget analysis shows a larger nonlinear term in CP El Ni?o than in eastern Pacific(EP) El Ni?o, inconsistent with the amplitudes of sea surface temperature anomalies. The reason is that the nonlinear term transforms to positive above 700 h Pa in EP El Ni?o, offsetting the negative advection below 700 h Pa and thus making the nonlinear term smaller. The nonlinear term is negative at low levels in CP El Ni?o, resulting in a larger nonlinear term. The stronger precipitation anomalies in the WNP during EP El Ni?o can be attributed to the linear moist enthalpy advection.
