| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 2 | Huangqi Guizhi Wuwu Decoction for treating diabetic peripheral neuropathy:a meta-analysis of 16 randomized controlled trials显示文摘OBJECTIVE:This meta-analysis was performed to systematically assess the efficacy and safety of the Chinese herbal medicine Huangqi Guizhi Wuwu Decoction(HGWWD) for treating diabetic peripheral neuropathy.DATA SOURCES:Six electronic databases,including the Cochrane Library,MEDLINE database,Chinese Biomedical Database,Chinese National Knowledge Infrastructure Database,Chinese Science and Technique Journals Database,and the Wanfang Database,were search ed on the internet for randomized controlled trials published up until 1 December 2015.The search terms included 'Chinese herbal medicine','diabetic peripheral neuropathy' and 'randomized controlled trials' in Chinese and in English.DATA SELECTION:We included randomized controlled trials using HGWWD/modified HGWWD for the treatment group,without restriction for the control group.We assessed literature quality in accordance with the Cochrane Review Handbook.A random or a fixed effects model was used to analyze outcomes using Rev Man 5.2 software.OUTCOME MEASURES:The primary outcomes were changes in symptoms and nerve conduction velocities.The secondary outcomeswere fasting blood glucose and hemorheological indexes.RESULTS:Sixteen randomized controlled trials,with a total of 1,173 patients,were included.Meta-analysis revealed that the efficacy of HGWWD for diabetic peripheral neuropathy was significantly superior compared with the control treatment(i.e.,control group)(risk ratio = 0.36,95% confidence interval(CI):0.29–0.46,Z =8.33,P < 0.00001) Compared with the control group,there was an increase in median motor nerve conduction velocity(mean difference(MD) = 3.46,95%CI:1.88–5.04,Z = 4.30,P < 0.01) and median sensory nerve conduction velocity(MD = 3.30,95%CI:2.04–4.56,Z = 5.14,P < 0.01).There was also an increase in peroneal motor nerve conduction velocity(MD = 3.22,95%CI:2.45–3.98,Z = 8.21,P < 0.01) and peroneal sensory nerve conduction velocity(MD = 3.05,95%CI:2.01–4.09,Z = 5.75,P < 0.01) in the treatment groups.No significant difference in fasting blood glucose was found between the treatment groups and the control groups(MD =-0.12,95%CI:-0.42–0.19,Z = 0.76,P = 0.45).Plasma viscosity was significantly decreased after treatment(MD =-0.11,95%CI:-0.21 to-0.02,Z = 2.30,P = 0.02).No significant difference in fibrinogen was detectable(MD =-0.53,95%CI:-1.28–0.22,Z = 1.38,P = 0.17).Four trials reported that treatment groups experienced no adverse reactions.Adverse events were not mentioned in the other 12 trials.No trial reported the incidence of complications,quality of life outcomes,or health economics.CONCLUSION:HGWWD treatment improves diabetic neurologic symptoms and ameliorates nerve conduction velocities.Our study suggests that HGWWD may have significant therapeutic efficacy for the treatment of diabetic peripheral neuropathy.However,the methodological quality of the randomized controlled trials was generally low.Larger and better-designed randomized controlled trials are required to more reliably assess the clinical effectiveness of HGWWD. | Bing Pang Tian-yu Zhao Lin-hua Zhao Fang Wan Ru Ye Qiang Zhou Feng Tian Xiao-lin Tong | 2016 | Neural Regeneration Research2016,11,8: | 32 |
| 3 | Deformation mechanism of roadways in deep soft rock at Hegang Xing’an Coal Mine显示文摘Engineering geomechanics characteristics of roadways in deep soft rock at Hegang Xing'an Coal Mine were studied and the nature of clay minerals of roadway surrounding rock was analyzed. This paper is to solve the technical problems of high stress and the difficulty in supporting the coal mine, and provide a rule for the support design. Results show that mechanical deformation mechanisms of deep soft rock roadway at Xing'an Coal Mine is of ⅠABⅡABCⅢABCD type, consisting of molecular water absorption (the ⅠAB -type), the tectonic stress type + gravity deformation type + hydraulic type (the ⅡABC -type), and the ⅢABCD -type with fault, weak intercalation and bedding formation. According to the compound mechanical deformation mechanisms, the corresponding mechanical control measures and conversion technologies were proposed, and these technologies have been successfully applied in roadway supporting practice in deep soft rock at Xing'an Coal Mine with good effect. Xing'an Coal Mine has the deepest burial depth in China, with its overburden ranging from Mesozoic Jurassic coal-forming to now. | 5 | Precise nanomedicine for intelligent therapy of cancer显示文摘Precise nanomedicine has been extensively explored for efficient cancer imaging and targeted cancer therapy, as evidenced by a few breakthroughs in their preclinical and clinical explorations. Here, we demonstrate the recent advances of intelligent cancer nanomedicine, and discuss the comprehensive understanding of their structure-function relationship for smart and efficient cancer nanomedicine including various imaging and therapeutic applications, as well as nanotoxicity. In particular, a few emerging strategies that have advanced cancer nanomedicine are also highlighted as the emerging focus such as tumor imprisonment, supramolecular chemotherapy, and DNA nanorobot. The challenge and outlook of some scientific and engineering issues are also discussed in future development. | 6 | Geothermal data analysis at the high-temperature hydrothermal area in Western Sichuan显示文摘The western Sichuan hydrothermal area is located at the northeastern margin of the eastern syntaxis of the Qinghai-Tibet Plateau, which is also the eastern end of the Mediterranean-Himalayan geothermal activity zone. There are 248 warm or hot springs in this area, and 11 have temperatures beyond the local boiling temperature. Most of these hot springs are distributed along the Jinshajiang, Dege-Xiangcheng, Ganzi-Litang, and Xianshuihe faults, forming a NW-SE hydrothermal belt. A geothermal analysis of this high-temperature hydrothermal area is an important basis for understanding the deep geodynamic process of the eastern syntaxis of the Qinghai-Tibet Plateau. In addition, this study offers an a priori view to utilize geothermal resources, which is important in both scientific research and application. We use gravity, magnetic, seismic, and helium isotope data to analyze the crust-mantle heat flow ratio and deep geothermal structure. The results show that the background terrestrial heat flow descends from southwest to northeast. The crustal heat ratio is not more than 60%. The high temperature hydrothermal active is related to crustal dynamics processes. Along the Batang-Litang-Kangding line, the Moho depth increases eastward, which is consistent with the changing Qc/Qm(crustal/mantle heat flow) ratio trend. The geoid in the hydrothermal zone is 4–6 km higher than the surroundings, forming a local 'platform'. The NW-SE striking local tensile stress zone and uplift structure in the upper and middle crust corresponds with the surface hydrothermal active zone. There is an average Curie Point Depth(CPD) of 19.5–22.5 km in Batang, Litang, and Kangding. The local shear-wave(S-wave) velocity is relatively low in the middle and lower crust. The S-wave shows a low velocity trap(Vs<3.2 km s.1) at 15–30 km, which is considered a high-temperature partial melting magma, the crustal source of the hydrothermal active zone. We conclude that the hydrothermal system in this area can be divided into Batang-type and Kangding-type, both of which rely on a crustal heating cycle of atmospheric precipitation and surface water along the fracture zone. | 7 | Clinical Observation of Transepithelial Corneal Collagen Cross-linking by Iontophoresis of Riboflavin in Treatment of Keratoconus显示文摘Purpose: To evaluate the efficacy and safety of transepithelial collagen cross-linking by iontophoretic delivery of riboflavin in treatment of progressive keratoconus.Methods:.Eleven patients(15 eyes) with progressive keratoconus were enrolled. After 0.1% riboflavin-distilled water solution was deliveried via transepithelial iontophpresis for 5 min with 1 m A current, and ultraviolet radiation(370 nm,.3 m W /cm2) was performed at a 1.5 cm distance for 30 min. The follow up were 6 months in all eyes. The uncorrected visual acuity, corrected visual acuity,endothelial cell counting, corneal thickness,.intraocular pressure, corneal curvature, corneal topography,.OCT and corneal opacity before and 6-month after surgery were analyzed.Results: At 6 month postoperatively, mean uncorrected visual acuity and corrected visual acuity changed from 0.36 to 0.30 and from 0.42 to 0.57 without statistical significance..The mean value of each index of corneal curvature declined without statistical significance.Kmax value dereased from 60.91 to59.91, and the astigmatism declined from 3.86 to 3.19. Central corneal thickness decreased from 460.93 μm to 455.40μm,.and thinnest corneal thickness declined from 450.87 μm to 440.60 μm with no statistical significance..Intraocular pressure was significantly elevated from 10.85 mm Hg to 12.62 mm Hg. Endothelial cell count did not change significantly. No corneal haze occurred. | 8 | Combination of symptoms, syndrome and disease: Treatment of refractory diabetic gastroparesis显示文摘AIM:To assess effect of combination of symptoms,syndrome and disease on treatment of diabetic gastroparesis with severe nausea and vomiting.METHODS:Professor Tong Xiaolin’s clinical electronic medical records of patients who were treated between January 1,2006 and October 1,2012 were used as a database.Patients who met the inclusion criteria were enrolled.General information(name,sex and age),symptoms and blood glucose levels were obtained from the clinic electronic medical record,which was supplemented by a telephone interview.The patient-rated Gastroparesis Cardinal Symptom Index(GCSI)was used to evaluate the severity of the symptoms of gastroparesis.The effects of the treatment were assessed by the change in the severity of the symptoms of gastroparesis and the change in blood glucose between the baseline levels and the post-treatment levels at 1,2,4,8 and 12 wk.RESULTS:Forty-five patients had a mean GCSI nausea and vomiting severity score of 4.21±0.67 and a total GCSI score of 2.77±0.63 before treatment.There was a significant improvement in the nausea and vomiting score at every return visit compared with the baseline score(1 wk:3.02±1.04 vs 4.18±0.71,P<0.001;2wk:2.32±1.25 vs 4.16±0.73,P<0.001;4 wk:2.12±1.26 vs 4.12±0.73,P<0.001;8 wk:1.79±1.09vs 4.24±0.77,P<0.001;12 wk:0.69±0.92 vs 4.25±0.70,P<0.001).Twenty-five of the 45 patients had complete resolution of vomiting during the observation period(mean time to resolution was 37.9±27.3 d).The postprandial fullness and early satiety subscale,bloating subscale and total GCSI scores were also improved.Finally,the blood glucose levels improved after treatment,although the change was not significant.CONCLUSION:Use of the combination of symptoms,syndrome and disease to treat diabetic gastroparesis with refractory nausea and vomiting may be a new treatment option. | Jun-Ling Li Min Li Bing Pang Qiang Zhou Jia-Xing Tian Hong-Xing Liu Xi-Yan Zhao Xiao-Lin Tong | 2014 | World Journal of Gastroenterology2014,20,26: | 13 |
| 9 | Value of early diagnosis of sepsis complicated with acute kidney injury by renal contrast-enhanced ultrasound显示文摘BACKGROUND The incidence of acute kidney injury(AKI)in patients with sepsis is high,and the prognosis of patients with septic AKI is poor.The early diagnosis and treatment of septic AKI is of great significance in improving the prognosis of patients with sepsis.AIM To explore the value of contrast-enhanced ultrasound(CEUS),serum creatinine(Scr),and other indicators in the early diagnosis of septic AKI.METHODS Ninety patients with sepsis during hospitalization at Tongji Hospital of Tongji University were recruited as subjects.Each patient was recorded with relevant basic data,clinical indicators,and CEUS results.The patients were divided into AKI group and non-AKI group according to the results of renal function diagnosis after 48 h.On the 7th day,the renal function of the non-AKI group was re-evaluated and the patients were further divided into AKI subgroup and non-AKI subgroup.The differences of the indicators in different groups were compared,and the diagnostic value of each indicator and their combination for septic AKI was analyzed.RESULTS Systemic inflammatory response score(2.58±0.75),blood lactic acid(3.01±1.33 mmol/L),Scr(141.82±27.19μmol/L),blood urea nitrogen(4.41±0.81mmol/L),and rise time(10.23±2.63 s)in the AKI group were higher than those in the non-AKI group.Peak intensity(PI)(10.78±3.98 dB)and wash in slope(WIS)(1.07±0.53 dB/s)were lower than those in the non-AKI group.The differences were statistically significant(P<0.05).The PI(12.83±3.77 dB)and WIS(1.22±0.68 dB/s)in the AKI subgroup were lower than those in the non-AKI subgroup,and the differences were statistically significant(P<0.05).The area under curve(AUC)of Scr for the diagnosis of septic AKI was 0.825 with a sensitivity of 56.76% and a specificity of 100%.The AUCs of WIS and PI(0.928 and 0.912)were higher than those of Scr.Their sensitivities were 100%,but the specificities were 71.70% and 75.47%.The AUC of the combination of three indicators for the diagnosis of septic AKI was 0.943,which was significantly higher than the AUC diagnosed by each single indicator.The sensitivity was 94.59%,and the specificity was 81.13%.CONCLUSION The combination of Scr,PI,and WIS can improve the diagnostic accuracy of septic AKI.PI and WIS are expected to predict the occurrence of early septic AKI. | Xiu-Yan Wang Yan-Ping Pang Tian Jiang Shuo Wang Jiang-Tao Li Bao-Min Shi Chen Yu | 2019 | World Journal of Clinical Cases2019,7,23: | 10 |
| 10 | Main progress and problems in research on Ordovician hydrocarbon accumulation in the Tarim Basin显示文摘The Tarim Basin is the largest petroliferous basin in the northwest of China, and is composed of a Paleozoic marine craton basin and a Meso-Cenozoic continental foreland basin. It is of great significance in exploration of Ordovician. In over 50 years of exploration, oil and gas totaling over 1.6 billion tonnes oil-equivalent has been discovered in the Ordovician carbonate formation. The accumulation mechanisms and distribution rules are quite complicated because of the burial depth more than 3,500 m, multi-source, and multi-stage accumulation, adjustment, reconstruction and re-enrichment in Ordovician. In this paper, we summarized four major advances in the hydrocarbon accumulation mechanisms of Ordovician carbonate reservoirs. First, oil came from Cambrian and Ordovician source rocks separately and as a mixture, while natural gas was mainly cracked gas generated from the Cambrian-Lower Ordovician crude oil. Second, most hydrocarbon migrated along unconformities and faults, with different directions in different regions. Third, hydrocarbon migration and accumulation had four periods: Caledonian, early Hercynian, late Hercynian and Himalayan, and the latter two were the most important for oil and gas exploration. Fourth, hydrocarbon accumulation and evolution can be generally divided into four stages: Caledonian (the period of hydrocarbon accumulation), early Hercynian (the period of destruction), late Hercynian (the period of hydrocarbon reconstruction and re-accumulation), and Himalayan (the period of hydrocarbon adjustment and re-accumulation). Source rocks (S), combinations of reservoir-seal (C), paleo-uplifts (M), structure balance belt (B) matched in the same time (T) control the hydrocarbon accumulation and distribution in the Ordovician formations. Reservoir adjustment and reconstruction can be classified into two modes of physical adjustment and variation of chemical compositions and five mechanisms. These mechanisms are occurrence displacement, biodegradation, multi-source mixing, high-temperature cracking and late gas invasion. Late hydrocarbon accumulation effects controlled the distribution of current hydrocarbon. The T-BCMS model is a basic geological model to help understanding the control of reservoirs. | 11 | Control of Formation of Lithological Reservoirs by Surrounding Mudstone显示文摘Taking the Jiyang depression as an example, this paper discusses the control of the formation of lithological reservoir by surrounding rocks by integrated application of geological analysis, physical simulation, and the analysis of oil & gas accumulation mechanism. Geological statistical shows that the major burial depth and interval of lithological reservoirs in the Jiyang depression are related to the hydrocarbon generation in and expulsion from the Lower Tertiary source rocks and the time of the formation of most lithological reservoirs coincides with the peak of hydrocarbon generation and expulsion. The lithological traps located in the center of effective source rocks are propitious to high oil saturation than those located on the margin of effective source rocks. The hydrocarbon charge degree of the lithological reservoir has a positive correlation with the intensity of hydrocarbon expulsion from surrounding source rocks. Geological analyses and NMR experiments also show that the oil saturation of surrounding source rocks control the hydrocarbon potential of lithological traps, and a critical value for oil saturation of surrounding mudstone is required, that is, when the oil saturation of surrounding mudstone is lower than this critical value, no oil and gas accumulate in the lithological trap. A kind of double-cell ozonesonde has been developed at the Institute of Atmospheric Physics(IAP), Chinese Academy of Sciences(named the 'IAP ozonesonde') based on previous experience over the past 20 years of developing the singlecell GPSO3 ozonesonde. The IAP ozonesonde is of the Electrochemical Concentration Cell(ECC) type. A detailed description of the IAP ozonesonde is firstly provided in the present paper, followed by a presentation of results from a series of launches carried out to evaluate its performance. The analysis involved comparing its observations with measurements from the GPSO3 and ECC ozonesondes(Model type ENSCI-Z) as well as a Brewer spectrophotometer. The results showed that the IAP ozonesonde is a vast improvement over the GPSO3 ozonesonde, able to capture vertical ozone structures very well and in good agreement with ECC ozonesonde measurements. The average difference in the ozone partial pressure between the IAP and ECC ozonesondes was 0.3 mPa from the surface to 2.5 km, close to zero from 2.5 to 9 km and generally less than 1 mPa for layers higher than 9 km. The apparent deviation is likely caused by a decreasing pump flow rate in the IAP ozonesonde which needs further improvement. The total ozone amounts measured by the IAP ozonesonde profiles were highly comparable with the Brewer data with a relative difference of 6%. | 20 | Replication potentials of HIV-1/HSIV in PBMCs from northern pigtailed macaque (Macaca leonina)显示文摘The northern pig-tailed macaque(Macaca leonina) has been identified as an independent species of Old World monkey, and we previously found that PBMCs from M. leonina were susceptible to human immunodeficiency virus type 1(HIV-1), which may be due to the absence of a TRIM5 protein restricting HIV-1 replication. Here we investigated the infection potentials of six laboratory adapted HIV-1 strains and three primary HIV-1 isolates in PBMCs from M. leonina. The results indicate that these strains are characterized by various but low replication levels, and among which, HIV-1NL4-3 shows the highest replication ability. Based on the abundant evidence of species-specific interactions between restriction factors APOBEC3 and HIV/SIV-derived Vif protein, we subsequently examined the replication potentials of vif-substituted HIV-1(HSIV) in M. leonina PBMCs. Notably, HSIV-vifmac and stHIV-1SV chimeras, two HIV-1NL4-3-derived viruses encoding the viral infectivity factor(Vif) protein from SIVmac239, replicated robustly in cells from M. leonina, which suggests that HSIV could effectively antagonize the antiviral activity of APOBEC3 proteins expressed in cells of M. leonina.
