| 1 | A phase Ⅱ study of gemcitabine and carboplatin combination chemotherapy in gallbladder carcinoma显示文摘BACKGROUND: Patients with carcinoma of the gallbladder have advanced, unresectable tumor at the time of presentation and face a dismal prognosis in the absence of a standard palliative chemotherapy regimen. This study was undertaken to evaluate the efficacy and safety of combined chemotherapy of gemcitabine and carboplatin in 20 patients with advanced gallbladder carcinoma. METHODS: The criteria of eligibility included chemonaive patients with unresectable gallbladder cancer, bidimensionaIly measurable disease, Zubrod’s performance status≤2, and adequate major organ function. The patients received gemcitabine (1000 mg/m^2) on days 1 and 8, and carboplatin (target AUC of 5.0 mg/ml) on day 1, in a 21-day cycle. CT was used for response assessment. RESULTS: In this group of 20 patients with advanced gallbladder carcinoma 6 were men and 14 women, with a median age of 55 years. The stage of the tumor at presentation was IVB in 14 patients (70%), IVA in 3 (15%) and Ⅲ in 3 (15%). Four patients (21%) achieved a complete response, and 3 (15.7%), a partial response; an overall response rate was 36.7%. The median time to progression of the tumor was 33.8 weeks, and 1-year survival rate of the patients was 43.3%. Anemia of WHO grade Ⅲ or Ⅳ was seen in 2 patients (10%) and 1 patient (5%), respectively. Grade Ⅲ neutropenia and thrombocytopenia were observed in 2 patients (10%) and 1 patient (5%), respectively. | 2 | Helicobacter pylori eradication in patients with chronic immune thrombocytopenic purpura显示文摘AIM:To assess the effect of Helicobacter pylori(H.pylori)eradication on platelet counts in patients with chronic immune thrombocytopenic purpura(cITP).METHODS:A total of 36 cITP patients were included in the study.The diagnosis of H.pylori was done by rapid urease test and Giemsa staining of the gastric biopsy specimen.All H.pylori positive patients received standard triple therapy for 14 d and were subjected for repeat endoscopy at 6 wk.Patients who continued to be positive for H.pylori on second endoscopyreceived second line salvage therapy.All the patients were assessed for platelet response at 6 wk,3rd and 6th months.RESULTS:Of the 36 patients,17 were positive for H.pylori infection and eradication was achieved in16 patients.The mean baseline platelet count in the eradicated patients was 88615.38±30117.93/mm3and platelet count after eradication at 6 wk,3 mo and6 mo was 143230.77±52437.51/mm3(P=0.003),152562.50±52892.3/mm3(P=0.0001),150187.50±41796.68/mm3(P=0.0001)respectively and in the negative patients,the mean baseline count was71000.00±33216.46/mm3 and at 6 wk,3rd and 6th month follow up was 137631.58±74364.13/mm3(P=0.001),125578.95±71472.1/mm3(P=0.005),77210.53±56892.28/mm3(P=0.684)respectively.CONCLUSION:Eradication of H.pylori leads to increase in platelet counts in patients with cITP and can be recommended as a complementary treatment with conventional therapy. | Ravinder Naik Noonavath Chandrasekharan Padma Lakshmi Tarun Kumar Dutta Vikram Kate | 2014 | World Journal of Gastroenterology2014,20,22: | 6 |

| 4 | An evaluation of the emission profile for two-wheelers at a traffic junction显示文摘Two-wheeler vehicles are an important mode of transportation in developing countries.However,the emissions from two-wheeler vehicles are significant.Urban two-wheeler vehicles with gasoline-fueled engines produce NO,and particulate matter emissions that affect urban air quality.During traffic light stops and programmed stops,for instance,pollutants are emitted and are dangerous to human health.In this experimental study,two-wheeler vehicles with different makes,technologies and engine capacities were tested for exhaust emissions including gravimetric and online measurements at different engine speeds and a no load condition at a si mutated traffic junction.Gravimetric measurements were performed by collecting the particulate mass(at two engine speeds:1500 and 2500 rpm) from a diluted engine-out exhaust on quartz filter paper.Next,these collected particulates were used to determine the presence of metals,as well as the benzene soluble organic fraction(BSOF).The total particulate mass,BSOF and trace elements were slightly higher at a higher engine speed(2500 rpm).Online measurements were performed by sampling the engine exhaust(at four engine speeds:1500.2000,2500,and 3000 rpm)and using online instruments to determine the particle number and size distribution,the particle-bound polyaromatic hydrocarbons(PAHs),the gaseous emissions and the smoke opacity.Engines with higher cubic capacity emitted a higher concentration of nano-particles.The particle-bound PAH concentration increased as the engine speed increased,but this concentration was notably low for the highest engine speed tested(3000 rpm).The regulated gaseous emissions increased as the engine speed increased for all vehicles. | Avinash Kumar Agarwal Prakhar Bothra Tarun Gupta Pravesh Chandra Shukla | 2015 | Particuology2015,13,1: | 4 |
| 5 | Emission profiling of diesel and gasoline cars at a city traffic junction显示文摘In congested urban roads,cars must stop at intersections because of city traffic lights.As a result,pedestrians and traffic police personnel are exposed to pollutants emanating from the tailpipe of various vehicles at such city traffic junctions.In this study,various gasoline- and diesel-fueled cars complying with different emission standards were tested for their emissions in simulated city traffic junction conditions.The engine exhaust from these cars was subjected to physicochemical characterization at different engine speeds under no-load conditions.These engine conditions were chosen because the cars idle at different engine speeds at a city traffic junction.Gravimetric and real-time measurements were performed for the tailpipe exhaust sampled from these vehicles.Exhaust particles were collected on 47 mm diameter quartz filter papers and subjected to gravimetric analysis for determining the total particulate mass(TPM)and trace metals while the engines were operated at two different engine idling speeds,1500 rpm(representing low idling) and 2500 rpm(representing high idling).At similar engine operating conditions,TPM and trace metals were lower for the exhaust from gasoline engines compared to the exhaust from diesel engines.Real-time measurements were performed for particle-bound poly-aromatic hydrocarbons(PAHs),particle number and size distribution,regulated gaseous emissions and smoke opacity of the exhaust at four different engine speeds,1500,2000,2500,and 3000 rpm.Particle-bound PAHs showed a decreasing trend for the vehicles that complied with stricter vehicular emission standards.Higher particle peak number concentrations were observed for diesel exhausts compared to the results for gasoline exhaust.Regulated gaseous emissions were also compared. | Avinash Kumar Agarwal Tarun Gupta Prakhar Bothra Pravesh Chandra Shukla | 2015 | Particuology2015,13,1: | 4 |
| 6 | Assessment of biomass and net primary productivity of a dry tropical forest using geospatial technology显示文摘This study quantifies biomass, aboveground and belowground net productivity, along with additional environmental factors over a 2-3 year period in Barnawapara Sanctuary of Chhattisgarh, India through satellite remotesensing and GIS techniques. Ten sampling quadrates20×20, 5×5 and 1×1 m were randomly laid for overstorey (OS), understorey (US) and ground vegetation(GS), respectively. Girth of trees was measured at breast height and collar diameters of shrubs and herbs at 0.1 m height. Biomass was estimated using allometric regression equations and herb biomass by harvesting. Net primary productivity (NPP) was determined by Ssumming biomass increment and litter crop values. Aspect and slope influenced the vegetation types, biomass and NPP in different forests. Standing biomass and NPP varied from 18.6 to101.5 Mg ha^(-1) and 5.3 to 12.7 Mg ha^(-1) a^(-1), respectively,in different forest types. The highest biomass was found in dense mixed forest, while net production recoded in Teak forests. Both were lowest in degraded mixed forests of different forest types. OS, US and GS contributed 90.4, 8.7and 0.7%, respectively, for the total mean standing biomass in different forests.
