| 3 | Psychometrics of the chronic liver disease questionnaire for Southern Chinese patients with chronic hepatitis B virus infection显示文摘AIM:To test the psychometric properties of a Chinese [(Hong Kong) HK] translation of the chronic liver disease questionnaire (CLDQ).METHODS:A Chinese (HK) translation of the CLDQ was developed by iterative translation and cognitive debriefing.It was then administered to 72 uncomplicated and 78 complicated chronic hepatitis B (CHB) patients in Hong Kong together with a structured questionnaire on service utilization,and the Chinese (HK) SF-36 Health Survey Version 2 (SF-36v2).RESULTS:Scaling success was ≥ 80% for all but three items.A new factor assessing sleep was found and items of two (Fatigue and Systemic Symptoms) subscales tended to load on the same factor.Internal consistency and test-retest reliabilities ranged from 0.58-0.90 for different subscales.Construct validity was confirmed by the expected correlations between the SF-36v2 Health Survey and CLDQ scores.Mean scores of CLDQ were significantly lower in complicated compared with uncomplicated CHB,supporting sensitivity in detecting differences between groups.CONCLUSION:The Chinese (HK) CLDQ is valid,reliable and sensitive for patients with CHB.Some modif ications to the scaling structure might further improve its psychometric properties. | Elegance Ting Pui Lam Cindy Lo Kuen Lam Ching Lung Lai Man Fung Yuen Daniel Yee Tak Fong | 2009 | World Journal of Gastroenterology2009，15，26： | 16 |
| 4 | A nuclear import inhibitory peptide ameliorates the severity of cholecystokinin-induced acute pancreatitis显示文摘AIM: To assess the effect of our novel cell-permeable nuclear factor-kappaB (NF-κB) inhibitor peptide PN50 in an experimental model of acute pancreatitis. PN50 was produced by conjugating the cell-penetrating penetratin peptide with the nuclear localization signal of the NF-κB p50 subunit.METHODS: Pancreatitis was induced in male Wistar rats by administering 2×100 μg/kg body weight of cholecystokininoctapeptide (CCK) intraperitoneally (IP) at an interval of 1 h. PN50-treated animals received 1 mg/kg of PN50 IP 30 min before or after the CCK injections. | 10 | Outcome of 1355 consecutive transabdominal chorionic villus samplings in 1351 patients显示文摘Background The true risk of choronic villus sampling (C VS) is poorly defined. The objective of this study was to review the clinical o utcome of transabdominal CVS performed in a university teaching unit, with an em phasis on the complication rate. Methods A comprehensive audit database was maintained for 1351 pregnant women, including 17 sets of twin pregnancies, who had a CVS. Details a nd outcome of all CVSs made in the unit between May 1996 and May 2004 were revie wed. All CVSs were performed by one of 5 operators using the identical techniqu es. Results All procedures were performed transabdominally. A tota l of 1355 CVSs were performed because there were 4 dichorionic twin pregnancies which required 2 punctures. The mean gestation at CVS was (11.8±0.7) weeks, and 97.3% of the procedures were performed between 11 and 13 completed weeks. The majority (96.2%) required only 1 puncture to achieve correct needle placeme nt. The procedure failed to obtain an adequate sample in 4 subjects (0.30%). A total of 1351 chromosomal studies were requested and there was 1 case (0.07%) of culture failure. The results of chromosomal studies were available within 1 4 days in 36.7% of the cases and within 21 days in 94.0%. Overall, 77 chromos omal abnormalities (5.7%) and 5 cases of thalassemia major were detected. Preg nancy outcome was unknown in only 13 singleton subjects (0.96%). In the remain ing 1355 fetuses, there were 76 pregnancy terminations (5.56%), 10 fetal losses with obvious obstetric causes (0.73%), and 21 potentially procedure-relat ed fetal losses (1.54%). In the last group, the majority had one or more co-e xisting obstetric complications. The background fetal loss rate for pregnancies at similar gestational age in the unit was about 0.8%. Therefore, the procedu re-related fetal loss rate was estimated to be at the maximum of 0.74%. Conclusions In experienced hands, first trimester transabdomina l CVS is an accurate and safe invasive prenatal diagnostic procedure. | 14 | Therapeutic proteasome inhibition in experimental acute pancreatitis显示文摘AIM: To establish the therapeutic potential of proteasome inhibition, we examined the therapeutic effects of MG132 (Z-Leu-Leu-Leu-aldehyde) in an experimental model of acute pancreatitis.METHODS: Pancreatitis was induced in rats by two hourly intraperitoneal (ip) injections of cholecystokinin octapeptide (CCK; 2 x 100 μg/kg) and the proteasome inhibitor MG132 (10 mg/kg ip) was administered 30 min after the second CCK injection. Animals were sacrificed 4 h after the first injection of CCK.RESULTS: Administering the proteasome inhibitor MG132 (at a dose of 10 mg/kg, ip) 90 min after the onset of pancreatic inflammation induced the expression of cell-protective 72 kDa heat shock protein (HSP72) and decreased DNA-binding of nuclear factor-kB (NF-kB). Despite their great adoption in the user communities, RS are still vulnerable to unscrupulous producers who try to promote their products by shilling the systems. With the advent of social networks new sources of information have been made available which can potentially render RS more resistant to attacks. In this paper we explore the information provided in the form of social links with clustering for diminishing the impact of attacks. We propose two algorithms, CluTr and WCluTr, to combine clustering with 'trust' among users. | 19 | CMOS-compatible high-index doped silica waveguide with an embedded silicon-nanocrystal strip for all-optical analog-to-digital conversion显示文摘Passive all-optical signal processors that overcome the electronic bottleneck can potentially be the enabling components for the next-generation high-speed and lower power consumption systems. Here, we propose and experimentally demonstrate a CMOS-compatible waveguide and its application to the all-optical analog-to-digital converter(ADC) under the nonlinear spectral splitting and filtering scheme. As the key component of the proposed ADC, a 50 cm long high-index doped silica glass spiral waveguide is composed of a thin silicon-nanocrystal(Si-nc) layer embedded in the core center for enhanced nonlinearity. The device simultaneously possesses low loss(0.16 dB/cm at 1550 nm), large nonlinearity(305 W^-1∕km at 1550 nm), and negligible nonlinear absorption.A 2-bit ADC basic unit is achieved when pumped by the proposed waveguide structure at the telecom band and without any additional amplification. Simulation results that are consistent with the experimental ones are also demonstrated, which further confirm the feasibility of the proposed scheme for larger quantization resolution.This demonstrated approach enables a fully monolithic solution for all-optical ADC in the future, which can digitize broadband optical signals directly at low power consumption. Macroscopically, a gastric IHP resembles a subepithelial tumor(SET); as a result, accurately diagnosing gastric IHP is difficult. This issue has clinical significance because gastric IHP can be misdiagnosed as SET or as malignant neoplasm In addition, adenocarcinoma can accompany benign gastric IHP. Although in most cases, gastric IHPs are asymptomatic and are found incidentally, these polyps may cause anemia secondary to chronic bleeding.
