Intraoperative blood loss in orthotopic liver transplantation:The predictive factors显示文摘Liver transplantation has been associated with massive blood loss and considerable transfusion requirements. Bleeding in orthotopic liver transplantation is multifactorial. Technical difficulties inherent to this complex surgical procedure and pre operative derangements of the primary and secondary coagulation system are thought to be the principal causes of perioperative hemorrhage. Intraoperative practices such as massive fluid resuscitation and resulting hypothermia and hypocalcemia secondary to citrate toxicity further aggravate the preexisting coagulopathy and worsen the perioperative bleeding. Excessive blood loss and transfusion during orthotopic liver transplant are correlated with diminished graft survival and increased septic episodes and prolonged ICU stay. With improvements in surgical skills, anesthetic technique, graft preservation, use of intraoperative cell savers and overall perioperative management, orthotopic liver transplant is now associated with decreased intra operative blood losses. The purpose of this review is to discuss the risk factors predictive of increased intra operative bleeding in patients undergoing orthotopic liver transplant. | Chandra Kant Pandey Anshuman Singh Kamal Kajal Mandeep Dhankhar Manish Tandon Vijay Kant Pandey Sunaina Tejpal Karna | 2015 | World Journal of Gastrointestinal Surgery2015,7,6: | 8
在有经历非肝的外科的肝疾病的病人的 Perioperative 风险因素显示文摘 The patients with liver disease present for various surgical interventions. Surgery may lead to complications in a significant proportion of these patients. These complications may result in considerable morbidity and mortality. Preoperative assessment can predict survival to some extent in patients with liver disease undergoing surgical procedures. A review of literature suggests nature and the type of surgery in these patients determines the peri-operative morbidity and mortality. Optimization of premorbid factors may help to reduce perioperative mortality and morbidity. The purpose of this review is to discuss the effect of liver disease on perioperative outcome; to understand various risk scoring systems and their prognostic significance; to delineate different preoperative variables implicated in postoperative complications and morbidity; to establish the effect of nature and type of surgery on postoperative outcome in patients with liver disease and to discuss optimal anaesthesia strategy in patients with liver disease. | Chandra Kant Pandey Sunaina Tejpal Karna Vijay Kant Pandey Manish Tandon Amit Singhal Vivek Mangla | 2012 | World Journal of Gastrointestinal Surgery2012,4,12: | 4

Genetic variability for yield and fiber related traits in genetically modified cotton显示文摘Background:Cotton(Gossypium hirsutum L.)is grown for fiber and oil purposes in tropical and sub-tropical areas of the world.Pakistan is the 4th largest producer of cotton. It has a significant contribution in the GDP of Pakistan. Therefore, the present study was performed to assess the genetic variations and genetic diversity of yield and fiber quality traits in cotton and to analyze the associations present among them. Results:Analysis of variance exhibited significant variation for all studied traits except total number of nodes and the height to node ratio. The phenotypic coefficient of variation was higher than the genotypic coefficient of variation for all studied traits.Plant height, monopodial branches, total number of bolls, lint index, seed index, and seed cotton yield displayed high heritabilities in a broad sense with maximum genetic advanee.Correlation analysis revealed that seed cotton yield had a significant positive association with plant height, the nu mber of monopodial branches, the nu mber of sympodia I branches, ginning outturn(GOT),the number of bolls, seed per boll, seed index, uniformity index, the number of sympodial branches, reflectance, and seed index at the genotypic level while a significant positive relationship was observed with plant height, the number of sympodial branches, boll number, and GOT.Plant height, monopodial branches,GOT,boll weight, seeds per boll, and short fiber index exerted direct positive effects on seed cotton yield. The first 6 principal component analysis(PCs)out of the total fourteen PCs displayed eigenvalues(>1)and had maximum share to total variability(82.79%).The attributes that had maximum share to total divergence in eluded plant height, uniformity index, the nu mber of sympodial branches, seed per boll,GOT,seed cotton yield, and short fiber index. Conclusion:The genotype AA-802,IUB-13,FH-159,FH-458,and CIM-595 were genetically diverse for most of the yield and fiber quality traits and could be utilized for the selection of better performing genotypes for further improvement. | SAHAR Adeela ZAFAR Muhammad Mubashar RAZZAQ Abdul MANAN Abdul HAROON Muhammad SAJID Sunaina REHMAN Abdul MO Huijuan ASHRAF Muhammad REN Maozhi SHAKEEL Amir YUAN Youlu | 2021 | Journal of Cotton Research2021,4,3: | 1

Treatment Outcomes of Germ Cell Tumors of Ovary:Single Institutional Study显示文摘5%of all ovarian tumours are accounted to germ cell tumours(GCT’s).Affecting mostly young women, the highest incidence is seen in second and third decade of life. They are highly malignant but chemosensitive and more curable than their epithelial counterparts.Treating these tumors with effective surgery and combination chemotherapy survival rates have dramatically improved in recent decades. We present our experience of ovarian germ cell tumours in the department of Surgical Oncology,Rajendra Institute of Medical Sciences(RIMS),Ranchi with special emphasis on treatment outcomes.A retrospective review of hospital medical records of patients with ovarian germ cell tumours diagnosed and treated at RIMS from June 2019 to August 2020,was performed.Clinical profile and treatment outcome of patients were recorded.A total of 19 patients met criteria. The median age at diagnosis was 20 years(range 11-42 years)and all had good performance status. All except two patients underwent surgery,70.6%and 29.4%in upfront and interval debulking surgery(IDS)setting respectively.Fertility preserving surgery was done in 75%patients in the primary surgery group and 60%undergoing IDS.83.3%patients received BEP as adjuvant chemotherapy whereas 80%as neo-adjuvant chemotherapy.Majority(31.5%)patients had dysgerminoma as final histology, followed by mixed histology(26.3%),yolk sac tumour(15.7%),immature teratoma(15.7%)and choriocarcinoma(10.5%).47.3%patients were in Stage I at the time of diagnosis.78.9%patients were alive without disease,10.5%recurred,and 10.5%were lost to follow up. | Rohit Kumar Jha Ajit Kumar Kushwaha Mukunda Kumar Sunaina Wadhwa Sumedha Gargy | 2021 | Journal of Oncology Research2021,3,1: | 0
Diagnostic and therapeutic challenge of heart failure after liver transplant:Case series显示文摘Heart failure(HF) following liver transplant(LT) surgery is a distinct clinical entity with high mortality. It is known to occur in absence of obvious risk factors. No preoperative workup including electrocardiogram, echocardiography at rest and on stress, reasonably prognosticates the risk. In patients of chronic liver disease, cirrhotic cardiomyopathy, alcoholic cardiomyopathy, and stress induced cardiomyopathy have each been implicated as a cause for HF after LT. However distinguishing one etiology from another not only is difficult, several etiologies may possibly coexist in a given patient. Diagnostic dilemma is further compounded by the fact that presentation and management of HF irrespective of the possible underlying cause, remains the same. In this case series, 6 cases are presented and in the light of existing literature modification in the preoperative workup are suggested. | Manish Tandon Sunaina Tejpal Karna Chandra Kant Pandey Ravindra Chaturvedi | 2017 | World Journal of Hepatology2017,9,33: | 0
