1Effect of sedative agent selection on morbidity, mortality and length of stay in patients with increase in intracranial pressure显示文摘BACKGROUND: To identify the effects of sedative agent selection on morbidity, mortality, and length of stay in patients with suspected increase in intracranial pressure. Recent trends and developments have resulted in changes to medications that were previously utilized as pharmacological adjuncts in the sedation and intubation of patients with suspected increases in intracranial pressure. Medications that were previously considered contraindicated are now being used with increasing regularity without demonstrated safety and effectiveness. The primary objective of this study is to evaluate and compare the use of Ketamine as an induction agent for patients with increased intracranial pressure. The secondary objective was to evaluate and compare the use of Etomidate, Midazolam, and Ketamine in patients with increased intracranial pressure. METHODS: We conducted a retrospective chart review of patients transported to our facility with evidence of intracranial hypertension that were intubated before trauma center arrival. Patients were identifi ed during a 22-month period from January 2014 to October 2015. Goals were to evaluate the impact of sedative agent selection on morbidity, mortality, and length of stay.RESULTS: During the review 148 patients were identifi ed as meeting inclusion criteria, 52 were excluded due to incomplete data. Of those the patients primarily received; Etomidate, Ketamine, and Midazolam. Patients in the Ketamine group were found to have a lower mortality rate after injury stratifi cation. 4Ground cover management and farmyard manure effects on soil nitrogen dynamics, productivity and economics of organically grown lettuce (Lactuca sativa L. subsp, secalina)显示文摘Ground cover management and farmyard manure(FYM) management have important roles in organic lettuce production.However,there is not enough information about their combined effects.In order to assess the effects of individual and combined ground cover and FYM management on soil mineral N(NH_4-N and NO_3-N) dynamics,lettuce yield and economics in organic farming,a two-year field experiment was conducted on a certified organic farm in Kisac(Vojvodina,Serbia).The experiment had a two-factorial split-plot completely randomized block design.FYM was applied on the half of the experimental field as a whole-plot factor,while ground cover management included the following four treatments:(Ⅰ)control,without ground cover management(φ);(Ⅱ) hoeing(H);(Ⅲ) agrotextile cover;(Ⅳ) straw mulch(SM),which were applied on split plots.The applied SM and AT increased soil moisture by 12.83 and 3.73%,respectively,compared to the control treatment.FYM increased soil mineral N concentration,the lettuce fresh matter(FM) yield and nitrate concentration in lettuce.However,nitrate concentration in lettuce in all treatments was below the limit required by the European Commission(EC)(2001).The highest lettuce yield was obtained by AT(39122 kg),followed by SM(33 925 kg),and it was higher with FYM application by 16.85%.Hoeing did not positively affect the yield,probably due to its negative effect on soil moisture.Additional profit was higher with FYM,showing the following decreasing order:AT>H>SM>0.However,value/cost ratio(VCR) was in the order(?)>H>SM>AT on FYM fertilized plots.FYM application led to higher lettuce yield and higher profit and VCR.In case a farm does not have FYM,SM is the best alternative due to its positive effect on the yield and low investment costs.Manojlovic Maja Cabilovski Ranko Nikolic Ljiljana Dzigurski Dejana Seremesic Srdan Bavec Martina 2017Journal of Integrative Agriculture2017,16,4:1

6Simultaneous endoscopic and video-assisted retroperitoneal debridement in walled-off pancreatic necrosis using a laparoscopic access platform:Two case reports显示文摘BACKGROUND Infected walled-off necrosis is a potentially life-threatening complication of necrotizing pancreatitis.While some patients can be treated by drainage alone,many patients also need evacuation of the infected debris.Central necroses in relation to the pancreatic bed are easily reached via an endoscopic transluminal approach,whereas necroses that involve the paracolic gutters and the pelvis are most efficiently treated via a percutaneous approach.Large and complex necroses may need a combination of the two methods.CASE SUMMARY Transluminal and percutaneous drainage followed by simultaneous endoscopic and modified video-assisted retroperitoneal debridement was carried out in two patients with very large(32-38 cm),infected walled-off necroses using a laparoscopic access platform.After 34 d and 86 d and a total of 9 and 14 procedures,respectively,complete regression of the walled-off necroses was achieved.The laparoscopic access platform improved both access to the cavities as well as the overview.Simultaneous transluminal and percutaneous necrosectomy are feasible with the laparoscopic access platform serving as a useful adjunctive.CONCLUSION This approach may be necessary to control infection and achieve regression in some patients with complex collections.Lars Lindgaard Morten Laksáfoss Lauritsen Srdan Novovic Erik Feldager Hansen John Gásdal Karstensen Palle Nordblad Schmidt 2022World Journal of Gastroenterology2022,28,5:1
