| 2 | Current status of intravenous tissue plasminogen activator dosage for acute ischaemic stroke:an updated systematic review显示文摘The optimal dose of recombinant tissue plasminogen activator(rtPA)for acute ischaemic stroke(AIS)remains controversial,especially in Asian countries.We aimed to update the evidence regarding the use of low-dose versus standard-dose rtPA.We performed a systematic literature search across MEDLINE,Embase,Cochrane Central Register of Controlled Trials(CENTRAL),PsycINFO and Cumulative Index to Nursing and Allied Health Literature(CINAHL)from inception to 22 August 2016 to identify all related studies.The outcomes were death or disability(defined by modified Rankin Scale 2-6),death,and symptomatic intracerebral haemorrhage(sICH).Where possible,data were pooled for meta-analysis with ORs and corresponding 95% CIs by means of random-effects or fixed-effects meta-analysis.We included 26 observational studies and 1 randomised controlled trial with a total of 23210 patients.Variable doses of rtPA were used for thrombolysis of AIS in Asia.Meta-analysis shows that low-dose rtPA was not associated with increased risk of death or disability(OR 1.13,95% CI 0.95 to 1.33),or death(OR 0.86,95% CI 0.74 to 1.01),or decreased risk of sICH(OR 1.06,95% CI 0.65 to 1.72).The results remained consistent when sensitivity analyses were performed including only low-dose and standard-dose rtPA or only Asian studies.Our review shows small difference between the outcomes or the risk profile in the studies using low-dose and/or standard-dose rtPA for AIS.Low-dose rtPA was not associated with lower risk of death or disability,death alone,or sICH. | Xia Wang Shoujiang You Shoichiro Sato Jie Yang Cheryl Carcel Danni Zheng Sohei Yoshimura Craig S Anderson Else Charlotte Sandset Thompson Robinson John Chalmers Vijay K Sharma | 2018 | Stroke & Vascular Neurology2018，3，1： | 10 |
| 3 | Cdx2 expression and its promoter methylation during metaplasia-dysplasia-carcinoma sequence in Barrett's esophagus显示文摘AIM:To examine how the expression of caudal type homebox transcription factor 2(Cdx2) is regulated in the development of malignancy in Barrett's esophagus.METHODS:Cdx2,mucin(MUC) series(MUC2,MUC5AC and MUC6),p53 and E-cadherin expression in Barrett's esophagus and adenocarcinoma specimens were examined by immunostaining.Isolated clusters of cells from(1) MUC2 and Cdx2-positive intestinal metaplastic mucosa;(2) MUC5AC and MUC6-positive,and MUC2 and Cdx2-negative high-grade dysplasia(HD),or intramucosal adenocarcinoma(IMC);and(3) MUC5AC,MUC6 and Cdx2-positive poorly-differentiated invasive adenocarcinoma(PDA) were analyzed by methylationspecific polymerase chain reaction using sets of primers for detecting methylation status of the Cdx2 gene.RESULTS:Most of the non-neoplastic Barrett's esophageal mucosa showing intestinal-type metaplasia with or without low-grade dysplasia was positive for E-cadherin,MUC series and Cdx2,but negative for p53.A portion of the low-grade to HD was positive for E-cadherin,MUC5AC,MUC6 and p53,but negative for MUC2 and Cdx2.The definite IMC area was strongly positive for MUC5AC,MUC6 and p53,but negative for MUC2 and Cdx2.Methylation of the Cdx2 promoter was not observed in intestinal metaplasia,while hypermethylation of part of its promoter was observed in hot dipped and IMC.Hypermethylation of a large fraction of the Cdx2 promoter was observed in PDA.CONCLUSION:Cdx2 expression is restored irrespective of the methylation status of its promoter.Apparent positive immunohistochemical results can be a molecular mark for gene silencing memory. | Kenji Makita Riko Kitazawa Shuho Semba Koto Fujiishi Miku Nakagawa Ryuma Haraguchi Sohei Kitazawa | 2013 | World Journal of Gastroenterology2013，19，4： | 5 |

| 6 | Quantitative evaluation of severity of behavioral and psychological symptoms of dementia in patients with vascular dementia显示文摘To quantitatively evaluate severity of behavioral and psychological symptoms of dementia(BPSD)for vascular dementia(VD).Changes of 51 patients with VD in BPSD between the first and 24th week were assessed using the Neuropsychiatric Inventory(NPI)and the behavioral pathology in Alzheimer’s disease(BEHAVE-AD)rating scale,in detrended fluctuation analysis(DFA)represented by diurnal activity(DA),evening activity(EA),and nocturnal activity(NA),and the relationships were analyzed.The subscores of activity disturbances,diurnal rhythm disturbances,and anxieties and phobias in the BEHAVE-AD score,and that of agitation,irritability,and sleep disorder in the NPI score were significantly increased compared with the first week,as was for the changes for EA in the DFA value.A linear correlation was observed between the changes of activity disturbances plus anxieties and phobias,and those of DA,and between the development of diurnal rhythm and those of EA,the vehement and autism scores and those of DA,and the difference in sleep disorder scores and those of EA,respectively.Analysis of DA,NA,and EA may reflect the fluctuational degrees of VD-BPSD,can provide a useful assessment of VD-BPSD accompanied by clinical scores for VD. | Wei-Dong Pan Sohei Yoshida Qian Liu Chun-Lan Wu Jun Wang Jin Zhu Ding-Fang Cai | 2013 | Translational Neurodegeneration2013，2，1： | 2 |

| 19 | 初始不可切除胰腺导管腺癌转化手术的现状与展望显示文摘胰腺导管腺癌预后一直不佳,患者5年生存率〈5%,化疗领域的进展将不可切除胰腺导管腺癌中位生存时间延长。不可切除胰腺导管腺癌的治疗仍充满挑战,需多学科团队共同制订治疗方案。令人欣慰的是,近期研究结果显示:局部晚期和远处转移的胰腺导管腺癌对化疗良好的反应性及转化手术率的增高。近期几项研究结果显示:对初始不可切除胰腺导管腺癌行转化治疗后,外科手术可获得更高的切除率(20%-61%)、更高的R0切除率(27%-89%)和淋巴结阴性率(29%-86%),且多数研究中的手术死亡率和术后并发症发生率均在可控制范围内。不可切除胰腺导管腺癌患者行转化手术后中位生存时间可达24-56个月,显著优于无法行外科手术的患者。此外,关于最佳化疗方案的选择、残留肿瘤体积的测量、外科手术指征、最佳手术时间及切除范围仍是研究热点。对初始不可切除胰腺导管腺癌患者,转化手术治疗是目前改善其预后的重要方法,亦需多学科团队医师共同努力;临床尚需更多设计优良的临床研究确认转化治疗在不可切除胰腺导管腺癌中的应用价值。 | Sohei Satoi Tomohisa Yamamoto Kentaro Inoue Yoichi Matsui | 2018 | 中华消化外科杂志2018，17，4： | 1 |
