| 1 | 在结肠和直肠的癌症的淋巴节点收获: 当前的考虑显示文摘 The prognostic significance of identifying lymph node(LN) metastases following surgical resection for colon and rectal cancer is well recognized and is reflected in accurate staging of the disease.An established body of evidence exists,demonstrating an association between a higher total LN count and improved survival,particularly for node negative colon cancer.In node positive disease,however,the lymph node ratios may represent a better prognostic indicator,although the impact of this on clinical treatment has yet to be universally established.By extension,strategies to increase surgical node harvest and/or laboratory methods to increase LN yield seem logical and might improve cancer staging.However,debate prevails as to whether or not these extrapolations are clinically relevant,particularly when very high LN counts are sought.Current guidelines recommend a minimum of 12 nodes harvested as the standard of care,yet the evidence for such is questionable as it is unclear whether an increasing the LN count results in improved survival.Findings from modern treatments,including down-staging in rectal cancer using pre-operative chemoradiotherapy,paradoxically suggest that lower LN count,or indeed complete absence of LNs,are associated with improved survival;implying that using a specific number of LNs harvested as a measure of surgical quality is not always appropriate.The pursuit of a sufficient LN harvest represents good clinical practice;however,recent evidence shows that the exhaustive searching for very high LN yields may be unnecessary and has little in? uence on modern approaches to treatment. | James R McDonald Andrew G Renehan Sarah T O’Dwyer Najib Y Haboubi | 2012 | World Journal of Gastrointestinal Surgery2012，4，1： | 8 |

| 3 | Correlation of endoscopic disease severity with pediatric ulcerative colitis activity index score in children and young adults with ulcerative colitis显示文摘AIM To investigate of pediatric ulcerative colitis activity index(PUCAI) in ulcerative colitis correlate with mucosal inflammation and endoscopic assessment of disease activity(Mayo endoscopic score).METHODS We reviewed charts from ulcerative colitis patients who had undergone both colonoscopy over 3 years. Clinical assessment of disease severity within 35 d(either before or after) the colonoscopy were included. Patients were excluded if they had significant therapeutic interventions(such as the start of corticosteroids or immunosuppressive agents) between the colonoscopy and the clinical assessment. Mayoendoscopic score of the rectum and sigmoid were done by two gastroenterologists. Inter-observer variability in Mayo score was assessed.RESULTS We identified 99 patients(53% female, 74% pancolitis) that met inclusion criteria. The indications for colonoscopy included ongoing disease activity(62%), consideration of medication change(10%), assessment of medication efficacy(14%), and cancer screening(14%). Based on PUCAI scores, 33% of patients were in remission, 39% had mild disease, 23% had moderate disease, and 4% had severe disease. There was 'moderate-substantial' agreement between the two reviewers in assessing rectal Mayo scores(kappa = 0.54, 95%CI: 0.41-0.68). CONCLUSION Endoscopic disease severity(Mayo score) assessed by reviewing photographs of pediatric colonoscopy has moderate inter-rater reliability, and agreement was less robust in assessing patients with mild disease activity. Endoscopic disease severity generally correlates with clinical disease severity as measured by PUCAI score. However, children with inflamed colons can have significant variation in their reported clinical symptoms. | 4 | A randomized open-label trial of on-demand rabeprazole vs ranitidine for patients with non-erosive reflux disease显示文摘AIM:To compare the efficacy of the proton-pump inhibitor,rabeprazole,with that of the H 2-receptor antagonist,ranitidine,as on-demand therapy for relieving symptoms associated with non-erosive reflux disease(NERD).METHODS:This is a single center,prospective,randomized,open-label trial of on-demand therapy with rabeprazole(group A) vs ranitidine(group B) for 4 wk.Eighty-three patients who presented to the American University of Beirut Medical Center with persistent gastroesophageal reflux disease(GERD) symptoms and a normal upper gastrointestinal endoscopy were eligible for the study.Patients in group A(n = 44) were allowed a maximum rabeprazole dose of 20 mg twice daily,while those in group B(n = 39) were allowed a maximum ranitidine dose of 300 mg twice daily.Efficacy was assessed by patient evaluation of global symptom relief,scores of the SF-36 quality of life(QoL) questionnaires,total number of pills used,and number of medication-free days.RESULTS:Among the 83 patients who were enrolled in the study,76 patients(40 in the rabeprazole group and 36 in the ranitidine group) completed the 4-wk trial.Baseline characteristics were comparable between both groups.After 4 wk,there was no significant difference in the subjective global symptom relief between the rabeprazole and the ranitidine groups(71.4% vs 65.4%,respectively;P = 0.9).There were no statistically significant differences between mean cumulative scores of the SF-36 QoL questionnaire for the two study groups(rabeprazole 22.40 ± 27.53 vs ranitidine 17.28 ± 37.06;P = 0.582).There was no significant difference in the mean number of pills used(rabeprazole 35.70 ± 29.75 vs ranitidine 32.86 ± 26.98;P = 0.66).There was also no statistically significant difference in the mean number of medication-free days between both groups.CONCLUSION:Rabeprazole has a comparable efficacy compared to ranitidine when given on-demand for the treatment of NERD.Both medications were associated with improved quality of life. | Abdallah A Kobeissy Jana G Hashash Faek R Jamali Assaad M Skoury Reham Haddad Sarah El-Samad Rami Ladki Rola Aswad Assaad M Soweid | 2012 | World Journal of Gastroenterology2012，18，19： | 4 |
| 5 | Perioperative glucose management and outcomes in liver transplant recipients: A qualitative systematic review显示文摘AIM To investigate the relationship between post-liver transplantation(LT) glycemic control and LT outcomes. METHODS A qualitative systematic review on relevant prospective interventions designed to control glucose levels including insulin protocols. Studies investigating an association between glycemic control and post-LT outcomes such as mortality, graft rejection, and infection rate were reviewed. Pub Med, EMBASE, and other databases were searched through October 2016. RESULTS Three thousands, six hundreds and ninety-two patients from 14 studies were included. Higher mortality rate was seen when blood glucose(BG) ≥ 150 mg/dL(P = 0.05). BG ≥ 150 mg/dL also led to higher rates of infection. Higher rates of graft rejection were seen at BG > 200 mg/dL(P < 0.001). Mean BG ≥ 200 mg/dL was associated with more infections(P = 0.002).Nurse-initiated protocols and early screening strategies have shown a reduction in negative post-LT outcomes.CONCLUSION Hyperglycemia in the perioperative period is associated with poor post-LT outcomes.
