| 1 | How important is donor age in liver transplantation?显示文摘The age of liver donors has been increasing in the past several years because of a donor shortage. In the United States, 33% of donors are age 50 years or older, as are more than 50% in some European countries. The impact of donor age on liver transplantation(LT) has been analyzed in several studies with contradictory conclusions. Nevertheless, recent analyses of the largest databases demonstrate that having an older donor is a risk factor for graft failure. Donor age is included as a risk factor in the more relevant graft survival scores, such as the Donor Risk Index, donor age and Model for End-stage Liver Disease, Survival Outcomes Following Liver Transplantation, and the Balance of Risk. The use of old donors is related to an increased rate of biliary complications and hepatitis C virus-related graft failure. Although liver function does not seem to be significantly affected by age, the incidence of several liver diseases increases with age, and the capacity of the liver to manage or overcome liver diseases or external injuries decreases. | 3 | Essential role of small bowel capsule endoscopy in reclassification of colonic inflammatory bowel disease type unclassified显示文摘AIM To evaluate the role of small bowel capsule endoscopy(SBCE) on the reclassification of colonic inflammatory bowel disease type unclassified(IBDU).METHODS We performed a multicenter, retrospective study including patients with IBDU undergoing SBCE, between 2002 and 2014. SBCE studies were reviewed and the inflammatory activity was evaluated by determining the Lewis score(LS). Inflammatory activity was considered significant and consistent with Crohn's disease(CD) when the LS ≥ 135. The definitive diagnosis during follow-up(minimum 12 mo following SBCE) was based on the combination of clinical, analytical, imaging, endoscopic and histological elements.RESULTS Thirty-six patients were included, 21 females(58%) with mean age at diagnosis of 33 ± 13(15-64) years. The mean follow-up time after the SBCE was 52 ± 41(12-156) mo. The SBCE revealed findings consistent with significant inflammatory activity in the small bowel(LS ≥ 135) in 9 patients(25%); in all of them the diagnosis of CD was confirmed during follow-up. In 27 patients(75%), the SBCE revealed no significant inflammatory activity(LS < 135); among these patients, the diagnosis of Ulcerative Colitis(UC) was established in 16 cases(59.3%), CD in 1 case(3.7%) and 10 patients(37%) maintained a diagnosis of IBDU during follow-up. A LS ≥ 135 at SBCE had a sensitivity = 90%, specificity = 100%, positive predictive value = 100% and negative predictive value = 94% for the diagnosis of CD.CONCLUSION SBCE proved to be fundamental in the reclassification of patients with IBDU. | 4 | PillCam~ SB3 capsule: Does the increased frame rate eliminate the risk of missing lesions?显示文摘Since its emergence in 2000, small bowel capsule endoscopy(SBCE) has assumed a pivotal role as an investigation method for small bowel diseases. The PillCam SB2-ex offers 12 h of battery time, 4 more than the previous version(SB2). Rahman et al recently found that the PillCam SB2-ex has a significantly increased completion rate, although without higher diagnostic yield, compared with the SB2. We would like to discuss these somewhat surprising results and the new potentialities of the PillCam SB3 regarding the diagnostic yield of small bowel studies. PillCam SB3 offers improved image resolution and faster adaptable frame rate over previous versions of SBCE. We recently compared the major duodenal papilla detection rate obtained with PillCam SB3 and SB2 as a surrogate indicator of diagnostic yield in the proximal small bowel. The PillCam SB3 had a significantly higher major duodenal papilla detection rate than the PillCam SB2(42.7% vs 24%, P = 0.015). | 5 | Capsule endoscopy capture rate: Has 4 frames-per-second any impact over 2frames-per-second?显示文摘AIM: To compare the current capsule and a new prototype at 2 and 4 frames-per-second, respectively, in terms of clinical and therapeutic impact. METHODS: One hundred patients with an indication for capsule endoscopy were included in the study. All procedures were performed with the new device(SB24). After an exhaustive evaluation of the SB24 videos, they were then converted to 'SB2-like' videos for their evaluation. Findings, frames per finding, and clinical and therapeutic impact derived from video visualization were analyzed. Kappa index for interobserver agreement and χ2 and Student's t tests for qualitative/quantitative variables, respectively, were used. Values of P under 0.05 were considered statistically significant.RESULTS: Eighty-nine out of 100 cases included in the study were ultimately included in the analysis. The SB24 videos detected the anatomical landmarks(Zline and duodenal papilla) and lesions in more patients than the 'SB2-like' videos. On the other hand, the SB24 videos detected more frames per landmark/lesion than the 'SB2-like' videos. However, these differences were not statistically significant(P > 0.05). Both clinical and therapeutic impacts were similar between SB24 and 'SB2-like' videos(K = 0.954). The time spent by readers was significantly higher for SB24 videos visualization(P < 0.05) than for 'SB2-like' videos when all images captured by the capsule were considered. | 6 | Impact of coronary angioplasty in elderly patients with non-ST-segment elevation myocardial infarction显示文摘Background As treatment of coronary artery disease improved over the last years,management of elder patients remained a matter of debate since this age group has been underrepresented in most trials.The study aimed to evaluate a population of patients with≥85 years old with non-ST-segment elevation myocardial infarction(NSTEMI)and compare the prognosis according to coronary revascularization execution.Methods We retrospectively studied 324 patients included in a national multicenter registry between October 2010 and October 2018,who underwent coronary angiography and had at least one stenosis≥50%.Results In this population,73.1%of the patients underwent percutaneous coronary intervention(PCI)and 26.9%of the patients underwent optimized medical treatment(OMT).The OMT group had more past history of diabetes,stroke and dementia.On coronary angiography,the PCI group used more often the femoral artery access and single-vessel lesions were also more common.Three-vessel disease was more common in the OMT group.During hospitalization,there were more major bleeding events and death in the PCI group.During the one-year follow-up,there were no significant differences in all-cause mortality rate.Conclusions Very old patients with NSTEMI submitted to OMT had more comorbidities and more three-vessel disease,factors that could have influenced the therapeutic decision.Patients undergoing PCI had more in-hospital major bleeding events and mortality,with no significant differences after one year. | Fernando F.Goncalves José P.Guimaraes Sara C.Borges Pedro S.Mateus José I.Moreira | 2020 | Journal of Geriatric Cardiology2020,17,8: | 2 |

| 8 | Esophageal stenosis with sloughing esophagitis:A curious manifestation of graft-vs-host disease显示文摘We report a case of a 56-year-old woman with a history of allogenic bone marrow transplantation for two years,complaining with dysphagia and weight loss. Upper endoscopy revealed esophageal stenosis and extensive mucosa sloughing. Biopsies confirmed the diagnosis of graft-vs-host disease(GVHD). Balloon dilation,corticosteroids and cyclosporin resulted in marked clinical improvement. Gastrointestinal tract is involved in the majority of patients with chronic GVHD. Esophageal manifestations are rare and include vesiculobullous disease,ulceration,esophageal webs,casts or strictures.
