| 2 | Micro RNAs potential utility in colon cancer: Early detection, prognosis, and chemosensitivity显示文摘Over the past decade, research has shown that aberrant expression of micro RNA(mi RNA) is involved in colorectal cancer development and progression. Micro RNAs are small sequences of non-coding RNA that regulate expression of genes involved in important cellular functions, such as cell differentiation, multiplication, and apoptosis. A specific mi RNA may display the effects of a tumor suppressor or oncogene. Altered mi RNA expression is found in colorectal cancer(CRC) and patterns of mi RNA expression correlate with CRC detection and outcome. Studies also have examined the use of circulating serum mi RNA and fecal mi RNA expression as non-invasive markers for early detection. | 6 | Role of Joshi's external stabilization system with percutaneous screw fixation in high-energy tibial condylar fractures associated with severe soft tissue injuries显示文摘Purpose: The treatment of high-energy tibial condylar fractures which are associated with severe soft tissue injuries remains contentious and challenging. In this study, we assessed the results of Joshi's external stabilization system (JESS) by using the principle of ligamentotaxis and percutaneous screw fixation for managing high-energy tibial condylar fractures associated with severe soft tissue injuries. Methods: Between June 2008 and June 2010, 25 consecutive patients who were 17-71 years (mean, 39.7), underwent the JESS fixation for high-energy tibial condylar fractures associated with severe soft tissue injuries. Out of 25 patients, 2 were lost during follow-up and in 1 case early removal of frame was done, leaving 22 cases for final follow-up. Among them, 11 had poor skin condition with abrasions and blisters and 2 were open injuries (Gustilo-Anderson grade I & 11). The injury mechanisms were motor vehicle accidents (n - 19), fall from a height (n = 2) and assault (n - 1). The fractures were classified according to Schatzker classification system. Results: There were 7 type-V, 14 type-Vl and 1 type-IV Schatzker's tibial plateau fractures. The average interval between the injury and surgery was 6.8 days (range 2-13). The average hospital stay was 13 days (range, 7-22). The average interval between the surgery and full weight bearing was 13.6 weeks (range 11-20). The average range of knee flexion was 121° (range 105°-135°). The normal extension of the knee was observed in 20 patients, and an extensor lag of 5°-8° was noted in 2 patients. The complications included superficial pin tract infections (n 4) with no knee stiffness. Conclusion: JESS with lag screw fixation combines the benefit of traction, external fixation, and limited internal fixation, at the same time as allowing the ease of access to the soft tissue for wound checks, pin care, dressing changes, measurement of compartment pressure, and the monitoring of the neurovascular status. The uses of such catalysts have gained importance worldwide. This article describes some of the important silicated catalysts, namely, heteropolyacids, polyphosphoric acid, perchloric acid, fluoroboric acid, and silicated sulphuric acid. These catalysts have been used in a number of organic reactions to yield compounds that are important in the chemical and pharmaceutical industries. We summarize the beneficial effects of these catalysts and the reports that have been published on them in the past several years. In the present review, the description of the catalysts are introduced followed by a recent research history, and a comparison between the silica supported catalysts and other(polymer) supported catalysts. | 11 | Investigation of rockfall-prone road cut slope near Lengpui Airport,Mizoram,India显示文摘Rockfall is one of severe natural hazards that are frequently reported in northeast region of India. It carries rock block falling from the cliff with high velocities and energies which can result in damages to vehicles, disruption to transportation, injuries and fatalities. The massive rockfall event which occurred in April 2017 on the highway NH-44 A, near Lengpui Airport, blocked the traffic for 1 d, and fortunately,no casualties were reported as the event occurred in the night. This is the only highway connecting the Aizawl city to the airport and the region is highly prone to rockfall events. Hence assessment of rockfall along this highway is necessary. In the current study, rockfall hazard assessment has been carried out on three locations by rockfall hazard rating system(RHRS). During pre-failure analysis, the result shows that most hazardous slopes have RHRS score of 639. The slopes were found to be vulnerable and later on the rockfall activity occurred. Three-dimensional(3 D) stability analysis has been carried out using 3 DEC software package to analyze the failure behavior and to decide the rockfall-prone zone(unstable blocks)for slope. The total displacement of 2.24 cm and velocity of 2,25 mm/s of the failed block have been observed in the numerical analysis. Further, the rockfall vulnerable zone(unstable blocks) is considered to determine the parameters such as run-out distance, bounce height and energies of the falling rock blocks. | 17 | Ultrasound curriculum taught by first-year medical students: A four-year experience in Tanzania显示文摘BACKGROUND: Diagnostic imaging is an integral aspect of care that is often insufficient, if not altogether absent, in rural and remote regions of low to middle income countries(LMICs) such as Tanzania. The introduction of ultrasound can significantly impact treatment in these countries due to its portability, low cost, safety, and usefulness in various medical assessments. This study reviews the implementation of a four-week ultrasound course administered annually from 2013–2016 in a healthcare professional school in Mwanza, Tanzania by first-year allopathic US medical students.METHODS: Participants(n=582, over 4 years) were recruited from the Tandabui Institute of Health Sciences and Technology to take the ultrasound course. Subjects were predominantly clinical officer students, but other participants included other healthcare professional students, practicing healthcare professionals, and school employees. Data collected includes pre-course examination scores, post-course examination scores, course quiz scores, demographic surveys, and postcourse feedback surveys. Data was analyzed using two-tailed t-tests and the single factor analysis of variance(ANOVA).RESULTS: For all participants who completed both the pre-and post-course examinations(n=229, 39.1% of the total recruited), there was a significant mean improvement in their ultrasound knowledge of 42.5%, P<0.01.CONCLUSION: Our data suggests that trained first-year medical students can effectively teach a point of care ultrasound course to healthcare professional students within four weeks in Tanzania. Future investigation into the level of long-term knowledge retention, impact of ultrasound training on knowledge of human anatomy and diagnostic capabilities, and how expansion of an ultrasound curriculum has impacted access to care in rural Tanzania is warranted. | Scan P. Owing to a lack of consensus regarding what constitutes a smart city,diverse smart city architectures have been proposed. However,it is observed that adequate consideration is not given to the most important element of a smart city i.e. its people. In our opinion,energy efficient technologically driven city does not necessarily lead to a smart city. Ethics,tradition and law form essential ingredients of complex social palette that cannot be ignored. In this work we propose Ethics-Aware Object-Oriented Smart City Architecture(EOSCA)that has two distinguishing features. Firstly,we propose an object oriented layered architecture where an object represents an abstraction of a real world thing along with requisite security and ethics parameters. Secondly,we propose to integrate socio-cultural and ethical aspects within the smart city architecture by dedicating a separate ethics layer.
