2Comparative evaluation of microscopy,OptiMAL® and 18S rRNA gene based multiplex PCR for detection of Plasmodium falciparum & Plasmodium vivax from field isolates of Bikaner,India显示文摘Objective:To evaluate microscopy,OptiMAL? and multiplex PCR for the identification of Plasmodium falciparum(P.falciparum) and Plasmodium vivax(P.vivax) from the field isolates of Bikaner,Rajasthan(Northwest India).Methods:In this study,a multiplex PCR(P.falciparum and P.vivax) was further developed with the incorporation of Plasmodium malariae(P.malariae) specific primer and also a positive control.The performance of microscopy,plasmodium lactate dehydrogenase(pLDH) based malaria rapid diagnostic test OptiMAL? and 18S rRNA gene based multiplex PCR for the diagnosis of P.falciparum and P.vivax was compared.Results:The three species multiplex PCR if.falciparum,P.vivax and P.malariae) with an inbuilt positive control was developed and evaluated.In comparison with multiplex PCR,which showed the sensitivity and specificity of 99.36%(95%CI,98.11%-100.00%) and 100.00%(95%CI,100.00%-100.00%),the sensitivity and specificity of microscopy was 90.44%(95%CI,88.849-95.04%) and 99.22%(95% CI,97.71%-100.00%),and OptiMAL? was 93.58%(95%CI,89.75%-97.42%) and 97.69%(95%CI, 95.10%-100.00%).The efficiencies were 99.65%,95.10%and 95.45%for multiplex PCK.microscopy and OptiMAL?.respectively.Conclusions:Our results raise concerns over the overall sensitivities of microscopy and OptiMAL?,when compared to the multiplex PCR and thus stress the need for new molecular interventions in the accurate detection of the malarial parasites.This further highlights the fact that further developments are needed to improve the performance of rapid diagnostic tests at field level.Deepak Pakalapati Shilpi Garg Sheetal Middha Abhishek Kochar Amit Kumar Subudhi Boopathi Pon Arunachalam Sanjay Kumar Kochar Vishal Saxena Pareek RP Jyoti Acharya Dhanpat Kumar Kochar Ashis Das 2013Asian Pacific Journal of Tropical Medicine2013,6,5:3

7NARX neural network approach for the monthly prediction of groundwater levels in Sylhet Sadar, Bangladesh显示文摘Groundwater is important for managing the water supply in agricultural countries like Bangladesh. Therefore, the ability to predict the changes of groundwater level is necessary for jointly planning the uses of groundwater resources. In this study, a new nonlinear autoregressive with exogenous inputs(NARX) network has been applied to simulate monthly groundwater levels in a well of Sylhet Sadar at a local scale. The Levenberg-Marquardt(LM) and Bayesian Regularization(BR) algorithms were used to train the NARX network, and the results were compared to determine the best architecture for predicting monthly groundwater levels over time. The comparison between LM and BR showed that NARX-BR has advantages over predicting monthly levels based on the Mean Squared Error(MSE), coefficient of determination(R^2), and Nash-Sutcliffe coefficient of efficiency(NSE). The results show that BR is the most accurate method for predicting groundwater levels with an error of ± 0.35 m. 8Challenges in managing ST elevation myocardial infarction during the COVID-19 pandemic显示文摘BACKGROUND Coronavirus disease 2019(COVID-19)may contribute to delayed presentations of acute myocardial infarction.Delayed presentation with late reperfusion is often associated with an increased risk of mechanical complications and adverse outcomes.Inherent delays are possible as every patient who is acutely sick is being considered a potential case or a career of COVID-19.Also,standardized personal protective equipment precautions are established for all members of the team,regardless of pending COVID-19 testing which might further add to delays.AIM To compare performance measures and clinical outcomes of all patients who presented to our facility with ST elevation myocardial infarction(STEMI)during the COVID-19 pandemic to same time cohort from 2019.METHODS All patients who presented to our facility with STEMI during the pandemic were compared to a matched cohort during the same time period in 2019.STEMI with unknown time of symptom onset and inpatient STEMI patients were excluded.Primary outcome was major adverse cardiac events(MACE)in-hospital and up to 14 d after STEMI,including death,myocardial infarction,cardiac arrest,or stroke.Significant differences among groups for continuous variables were tested through ANOVA,using SYSTAT,version 13.Chi-square tests of association were used to compare patient characteristics among groups using SYSTAT.Relative risk scores and associated tests for significance were calculated for discrete variables using MedCalc(MedCalc Software,Ostend,Belgium).RESULTS There was a significantly longer time interval from symptom onset to first medical contact(FMC)in the COVID-19 group(P<0.02).Time to first electrocardiogram,door-to-balloon time,and FMC to balloon time were not significantly affected.The right coronary artery was the most common culprit for STEMI in both the cohorts.Over 60%of patients had one or more obstructive(>50%)lesion(s)remote from the culprit site.In-hospital and 14 d MACE were more prevalent in the COVID-19 group(P<0.01 and P<0.001).CONCLUSION This single academic center study in the United States suggests that there is a delay in patients with STEMI seeking medical attention during the COVID-19 pandemic which could be translating into worse clinical outcomes.Megan Smith Aniruddha Singh Douglas McElroy Shilpi Mittal Richard Pham 2021World Journal of Cardiology2021,13,4:1

14Human islet amyloid polypeptide:A therapeutic target for the management of type 2 diabetes mellitus显示文摘Type 2 diabetes mellitus(T2DM)and other metabolic disorders are often silent and go unnoticed in patients because of the lack of suitable prognostic and diagnostic markers.The current therapeutic regimens available for managing T2DM do not reverse diabetes;instead,they delay the progression of diabetes.Their efficacy(in principle)may be significantly improved if implemented at earlier stages.The misfolding and aggregation of human islet amyloid polypeptide(hIAPP)or amylin has been associated with a gradual decrease in pancreatic b-cell function and mass in patients with T2DM.Hence,hIAPP has been recognized as a therapeutic target for managing T2DM.This review summarizes hIAPP's role in mediating dysfunction and apoptosis in pancreatic b-cells via induction of endoplasmic reticulum stress,oxidative stress,mitochondrial dysfunction,inflammatory cytokine secretion,autophagy blockade,etc.Furthermore,it explores the possibility of using intermediates of the hIAPP aggregation pathway as potential drug targets for T2DM management.Finally,the effects of common antidiabetic molecules and repurposed drugs;other hIAPP mimetics and peptides;small organic molecules and natural compounds;nanoparticles,nanobodies,and quantum dots;metals and metal complexes;and chaperones that have demonstrated potential to inhibit and/or reverse hIAPP aggregation and can,therefore,be further developed for managing T2DM have been discussed.Pratiksha H.Roham Shreyada N.Save Shilpy Sharma 2022Journal of Pharmaceutical Analysis2022,12,4:1
