2Endoscopic ultrasonography for gastric submucosal lesions显示文摘Gastric submucosal tumors(SMTs) are a rather frequent finding,occurring in about 0.36%of routine upper GIendoscopies.Endoscopic ultrasonography(EUS) has emerged as a reliable investigative procedure for evaluation of these lesions.Diagnostic EUS has the ability to differentiate intramural tumors from extraluminal compressions and can also show the layer of origin of gastric SMTs.Tumors can be further characterized by their layer of origin,echo pattern and margin.EUS-risk criteria of their malignant potential are presented,although the emergence of EUS-FNA has opened new indications for transmural tissue diagnosis and expanded the possibilities of EUS in SMTs of the stomach.Tissue diagnosis should address whether the SMT is a Gastrointestinal stromal tumour(GIST) or another tumor type and evaluate the malignant potential of a given GIST.However,there seems to be a lack of data on the optimal strategy in SMTs suspected to be GISTs with a negative EUS-FNA tissue diagnosis.The current management strategies,as well as open questions regarding their treatment are also presented.Ioannis S Papanikolaou Konstantinos Triantafyllou Anastasia Kourikou Thomas Rsch 2011World Journal of Gastrointestinal Endoscopy2011,3,5:44

5Long-term outcome and prognostic factors of patients with hilar cholangiocarcinoma显示文摘AIM: To evaluate the long-term outcome and prognostic factors of patients with hilar cholangiocarinoma. METHODS: Ninety-six consecutive patients underwent treatment for malignant hilar bile duct tumors during 1995–2005. Of the 96 patients, 20 were initially treated with surgery (n = 2 R0 / n = 18 R1). In non-operated patients, data analysis was performed retrospectively. RESULTS: Among the 96 patients, 76 were treated with endoscopic transpapillary (ERC, n = 45) and/or percutaneous transhepatic biliary drainage (PTBD, n = 31). The mean survival time of these 76 patients undergoing palliative endoscopic and/or percutaneous drainage was 359 ± 296 d. The mean survival time of patients with initial bilirubin levels > 10 mg/dL was significantly lower (P < 0.001) than patients with bilirubin levels < 10 mg/dL. The mean survival time of patients with Bismuth stage Ⅱ (n = 8), Ⅲ (n = 28) and Ⅳ (n = 40) was 496 ± 300 d, 441 ± 385 d and 274 ± 218 d, respectively. Thus, patients with advanced Bismuth stage showed a reduced mean survival time, but the difference was not significant. The type of biliary drainage had no significant beneficial effect on the mean survival time (ERC vs PTBD, P = 0.806). CONCLUSION: Initial bilirubin level is a significant prognostic factor for survival of patients. In contrast, age, tumor stage according to the Bismuth-Corlette classification, and types of intervention are not significant prognostic parameters for survival. 8中国能否应对指南挑战?显示文摘中国的卫生保健费用日益增加,但缺乏对卫生保健质量和经费支出的严格监控。基于现有最佳证据,以患者为中心的指南有助于提高卫生保健质量和促进从证据到实践的转化。本文阐述了指南如何推动中国医疗卫生体制改革(简称'医改'),并特别总结了目前指南制定的现状及相关挑战,同时结合中国医改大背景,对可能的解决方案进行了初步探索。中国目前尚缺乏由某个专门的权威机构来制定和传播循证指南的能力。大多数指南使用者信赖专家小组制定的推荐意见,但这些推荐意见缺乏透明性(包括利益冲突的处理过程及证据检索、评价与合成过程的透明性),质量也无法保证。此外,人们对指南的作用存在认识误区,认为指南的作用只是为临床工作者提供决策信息而非必须遵守的准则;另外也有观点认为无法制定传统中医药的循证指南。所以,如果能够建立一个为指南制定提供或整合所有证据、指导指南制定工作及评价指南的核心机构,并与国际相关组织合作,不仅可以加强中国循证指南的制定能力,而且能够应对循证指南制定面临的挑战。杨克虎 陈耀龙 李幼平 Holger J Schünemann 2013中国循证医学杂志2013,13,6:18
9Current biomarkers for hepatocellular carcinoma: Surveillance, diagnosis and prediction of prognosis显示文摘Biomarkers for surveillance, diagnosis and prediction of prognosis in patients with hepatocellular carcinoma(HCC) are currently not ready for introduction into clinical practice because of limited sensitivity and specificity. Especially for the early detection of small HCC novel biomarkers are needed to improve the current effectiveness of screening performed byultrasound. The use of high-throughput technologies in hepatocellular research allows to identify molecules involved in the complex pathways in hepatocarcinogenesis. Several invasive and non-invasive biomarkers have been identified already and have been evaluated in different clinical settings. Gene signatures with prognostic potential have been identified by gene expression profiling from tumor tissue. However, a single 'all-in-one' biomarker that fits all-surveillance, diagnosis, prediction of prognosis-has not been found so far. The future of biomarkers most probably lies in a combination of non-invasive biomarkers, imaging and clinical parameters in a surveillance setting. Molecular profiling of tumorous and non-tumorous liver tissue may allow a prediction of prognosis for the individual patient and hopefully clear the way for individual treatment approaches. 10Value of three-dimensional reconstructions in pancreatic carcinoma using multidetector CT:Initial results显示文摘AIM:To evaluate the use of three-dimensional imaging of pancreatic carcinoma using multidetector computed tomography(CT)in a prospective study.METHODS:Ten patients with suspected pancreatic tumors were examined prospectively using multidetec-tor CT(Somatom Sensation 16,Siemens,Erlangen,Germany).The images were evaluated for the pres-ence of a pancreatic carcinoma and invasion of the peripancreatic vessels and surrounding organs.Using the isotropic CT data sets,a three-dimensional image was created with automatic vascular analysis and semi-automatic segmentation of the organs and pancreatic tumor by a radiologist.The CT examinations and the three-dimensional images were presented to the sur-geon directly before and during the patient's operation using the Medical Imaging Interaction Toolkit-based software 'ReLiver'.Immediately after surgery,the value of the two images was judged by the surgeon.The operation and the histological results served as the gold standard.RESULTS:Nine patients had a pancreatic carcinoma(all pT3),and one patient had a serous cystadenoma.One tumor inf iltrated the superior mesenteric vein.The inf iltration was correctly evaluated.All carcinomas were resectable.In comparison to the CT image with axial and coronal reconstructions,the three-dimensional image was judged by the surgeons as better for operation planning and consistently described as useful.CONCLUSION:A 3D-image of the pancreas repre-sents an invaluable aid to the surgeon.However,the 3D-software must be further developed in order to be integrated into daily clinical routine.Miriam Klauβ Max Schbinger Ivo Wolf Jens Werner Hans-Peter Meinzer Hans-Ulrich Kauczor Lars Grenacher 2009World Journal of Gastroenterology2009,15,46:16
11Remains of the day:Biliary complications related to single-port laparoscopic cholecystectomy显示文摘AIM:To assesse the rate of bile duct injuries(BDI)and overall biliary complications during single-port laparoscopic cholecystectomy(SPLC)compared to conventional laparoscopic cholecystectomy(CLC).METHODS:SPLC has recently been proposed as an innovative surgical approach for gallbladder surgery.So far,its safety with respect to bile duct injuries has not been specifically evaluated.A systematic review of the literature published between January 1990 and November 2012 was performed.Randomized controlled trials(RCT)comparing SPLC versus CLC reporting BDI rate and overall biliary complications were included.The quality of RCT was assessed using the Jadad score.Analysis was made by performing a meta-analysis,using Review Manager 5.2.This study was based on the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines.A retrospective study including all retrospective reports on SPLC was also performed alongside.RESULTS:From 496 publications,11 RCT including 898patients were selected for meta-analysis.No studies were rated as high quality(Jadad score≥4).Operative indications included benign gallbladder disease operated in an elective setting in all studies,excluding all emergency cases and acute cholecystitis.The median follow-up was 1 mo(range 0.03-18 mo).The incidence of BDI was 0.4%for SPLC and 0%for CLC;the difference was not statistically different(P=0.36).The incidence of overall biliary complication was 1.6%for SPLC and 0.5%for CLC,the difference did not reached statistically significance(P=0.21,95%CI:0.66-15).Sixty non-randomized trials including 3599 patients were also analysed.The incidence of BDI reported then was 0.7%.CONCLUSION:The safety of SPLC cannot be assumed,based on the current evidence.Hence,this new technology cannot be recommended as standard technique for laparoscopic cholecystectomy.Pierre Allemann Nicolas Demartines Markus Schfer 2014World Journal of Gastroenterology2014,20,3:14
12Grazing effects on herbage nutritive values depend on precipitation and growing season in Inner Mongolian grassland显示文摘Aims Grasslands are widely used for production of livestock,which depend on the nutritive value of herbage species.However,there are still large uncertainties as to how grazing,precipitation and growing season interactively affect herbage nutritive value.Methods Here,based on a grazing experiment with seven grazing intensities in an Inner Mongolian grassland since the year 2005,we analysed nutritive value of four dominant grass species from June to September in both a relatively wetter year(2008)and a drier year(2010).Herbage with high nutritive value is characterized by high concentration of crude protein,high concentration of cellulase digestible organic matter and low concentration of neutral detergent fibre.Important Findings We found that(i)grazing significantly increased the nutritive value of Leymus chinensis,Agropyron michnoi and Cleistogenes squarrosa but had minor effects on the nutritive value of Stipa grandis.(ii)For all species,nutritive values were greater in the wetter year than in the drier year and were greatest in the early growing season(June)and lowest at the end of the growing season(September).Inter-annual and inter-seasonal variations in nutritive value were much greater for L.chinensis and A.michnoi than for C.squarrosa and S.grandis,suggesting higher water use efficiency for the latter two species.(iii)Grazing significantly decreased the drought resistance of three species,but not of S.grandis.(iv)Grazing significantly increased inter-month variation in nutritive value for L.chinensis and A.michnoi but had relatively minor or no effects on that of C.squarrosa and S.grandis.Therefore,grazing effects on species nutritive values showed strong inter-annual and seasonal patterns,and species-specific responses might be related to species traits(i.e.water use efficiency,digestibility).Our results have important implications for ecosystem management of arid and semiarid grasslands under intense grazing and global climatic change.Haiyan Ren Guodong Han Zhichun Lan Hongwei Wan Philipp Schönbach Martin Gierus Friedhelm Taube 2016Journal of Plant Ecology2016,9,6:14

14Assessing the USLE Crop and Management Factor C for Soil Erosion Modeling in a Large Mountainous Watershed in Central China显示文摘Due to the impoundment of the Yangtze River, the Three Gorges Dam in China fosters high land-use dynamics. Soil erosion is expected to increase dramatically. One of the key factors in soil erosion control is the vegetation cover and crop type. However, determining these factors adequately for the use in soil erosion modeling is very time-consuming especially for large mountainous areas, such as the Xiangxi (香溪) catchment in the Three Gorges area. In our study, the crop and management factor C was calculated using the fractional vegetation cover (CFVC) based on Landsat-TM images from 2005, 2006, and 2007 and on literature studies (CLIT). In 2007, the values of CFVC range between 0.001 and 0.98 in the Xiangxi catchment. The mean CFVC value is 0.05. CLIT values are distinctly higher, ranging from 0.08 to 0.46 with a mean value of 0.32 in the Xiangxi catchment. The mean potential soil loss amounts to 120.62 t/ha/a in the Xiangxi catchment when using CLIT for modeling. Based on CFVC, the predicted mean soil loss in the Xiangxi catchment is 11.50 t/ha/a. 16Transforming growth factor-β1 induces intestinal myofibroblast differentiation and modulates their migration显示文摘AIM:To investigate the effects of transforming growth factorβ1(TGF-β1)on the differentiation of colonic lamina propria fibroblasts(CLPF)into myofibroblasts in vitro. METHODS:Primary CLPF cultures were incubated with TGF-β1 and analyzed for production ofα-smooth muscle actin(α-SMA),fibronectin(FN)and FN isoforms.Migration assays were performed in a modified 48-well Boyden chamber.Levels of total and phosphorylated focal adhesion kinase(FAK)in CLPF were analyzed after induction of migration.RESULTS:Incubation of CLPF with TGF-β1 for 2 d did not changeα-SMA levels,while TGF-β1 treatment for 6 d significantly increasedα-SMA production. Short term incubation(6 h)with TGF-β1 enhanced CLPF migration,while long term treatment(6 d)of CLPF with TGF-β1 reduced migration to 15%-37% compared to untreated cells.FN and FN isoform mRNA expression were increased after short term incubation with TGF-β1(2 d)in contrast to long term incubation with TGF-β1 for 6 d.After induction of migration, TGF-β1-preincubated CLPF showed higher amounts of FN and its isoforms and lower levels of total and phosphorylated FAK than untreated cells. 17Deep-learning based denoising and reconstruction of super-resolution structured illumination microscopy images显示文摘Super-resolution structured illumination microscopy(SR-SIM) provides an up to twofold enhanced spatial resolution of fluorescently labeled samples. The reconstruction of high-quality SR-SIM images critically depends on patterned illumination with high modulation contrast. Noisy raw image data(e.g., as a result of low excitation power or low exposure time), result in reconstruction artifacts. Here, we demonstrate deep-learning based SR-SIM image denoising that results in high-quality reconstructed images. A residual encoding–decoding convolutional neural network(RED-Net) was used to successfully denoise computationally reconstructed noisy SR-SIM images.We also demonstrate the end-to-end deep-learning based denoising and reconstruction of raw SIM images into high-resolution SR-SIM images. Both image reconstruction methods prove to be very robust against image reconstruction artifacts and generalize very well across various noise levels. 20Yangtze Three Gorges Reservoir, China: A holistic assessment of organic pollution, mutagenic effects of sediments and genotoxic impacts on fish显示文摘Besides obvious benefits,the Three Gorges Dam's construction resulted in new pollution scenarios with the potentials to threaten the Three Gorges Reservoir(TGR) ecosystem.In order to record organic contamination,to find links to ecotoxicological impacts and to serve as reference for ensuing monitoring,several sites in the TGR area were screened applying the triad approach with additional lines-of-evidence as a holistic assessment method.Sediments and the benthic fish species Pelteobagrus vachellii were sampled in 2011 and 2012 to determine organic pollution levels,mutagenic potentials and genotoxic impacts.Two regional hot-spots near the cities of Chongqing and Kaixian were identified and further investigated in 2013.Only polycyclic aromatic hydrocarbons(PAHs) could be detected in sediments in 2011(165–1653 ng/g),emphasizing their roles as key pollutants of the area.Their ubiquity was confirmed at Chongqing(150–433 ng/g) and Kaixian(127–590 ng/g) in2013.Concentrations were comparable to other major Chinese and German rivers.However,the immense sediment influx suggested a deposition of 216–636 kg PAH/day(0.2–0.6 mg PAH/(m2·day)),indicating an ecotoxicological risk.PAH source analysis highlighted primary impacts of combustion sources on the more industrialized upper TGR section,whereas petrogenic sources dominated the mid-low section.Furthermore,sediment extracts from several sites exhibited significant activities of frameshift promutagens in the Ames fluctuation assay.Additionally,significant genotoxic impairments in erythrocytes of P.vachellii were detected(Chongqing/Kaixian),demonstrating the relevance of genotoxicity as animportant mode of action in the TGR's fish.PAHs,their derivatives and non-target compounds are considered as main causative agents.Tilman Floehr Bj?rn Scholz-Starke Hongxia Xiao Josef Koch Lingling Wu Junli Hou Anja Wolf Axel Bergmann Kerstin Bluhm Xingzhong Yuan Martina Ro?-Nickoll Andreas Sch?ffer Henner Hollert 2015Journal of Environmental Sciences2015,27,12:9
